Distinct Brain Drivers and Shared Cerebello-Cortical Input in ADCY5 and SGCE Hyperkinetic Movements.

BACKGROUND: Hyperkinetic movement disorders arise from dysfunction within cortico-basal ganglia-cerebellar loops. They frequently involve psychiatric and cognitive symptoms, reflecting impairment of both motor and non-motor domains within these loops. ADCY5 (MxMD-ADCY5) and SGCE (MYC/DYT-SGCE) related movement disorders are childhood-onset monogenic hyperkinetic conditions, both characterized by myoclonus, dystonia, and frequent psychiatric manifestations. Previous evidence suggests predominant basal ganglia involvement in MxMD-ADCY5 and cerebellar involvement in MYC/DYT-SGCE. OBJECTIVES: The

Multiscale multimodal graph convolutional networks for identifying essential tremor and dystonic tremor.

BACKGROUND: Essential tremor (ET) and Dystonic tremor (DT) share overlapping motor symptoms, leading to frequent misdiagnoses. Multimodal Graph Convolutional Network (GCNs) with transformer architecture hold promise for early diagnosis and pathogenesis exploration. OBJECTIVES: To develop a multiscale multimodal GCN (MM-GCN) using structural and functional connectivity features, and identify imaging biomarkers of ET and DT. METHODS: We collected rs-fMRI, DTI, and sMRI data and constructed eight inter-regional similarity matrices (GM, FA, MD, RD, AD, ReHo, DC, and fALFF) for each subject using

Correction of the molecular phenotype of X-linked Dystonia-Parkinsonism reveals a non-canonical function of BRD4.

Transcription and mRNA processing are tightly coupled regulatory layers on gene expression, and their perturbations underly human disorders. X-linked Dystonia-Parkinsonism (XDP) is a unique example of a human disease connecting aberrant mRNA processing and the basal transcription machinery. XDP is a rare, monogenic fatal neurodegenerative disorder, and a limited understanding of the underlying molecular mechanisms hinders the development of effective therapies. In this study, we show that depletion of BRD4, a chromatin reader known for its role in transcriptional pausing, rescues the XDP molec

Mitochondrial toxins cause widespread downregulation of pathways in X-linked dystonia-parkinsonism patient-derived neurons.

The genetic mechanism underlying the neurodegenerative movement disorder X-linked dystonia-parkinsonism (XDP) involves a retrotransposon insertion within the TAF1 gene. TAF1 encodes the TATA-box binding protein-associated factor 1, the largest subunit of the basal transcription factor TFIID, which connects transcription activation to the assembly of the RNA polymerase II preinitiation complex at the core promoter of genes. This study investigated how the TAF1 mutation affects the transcriptomes of XDP patient-derived neurons under basal conditions and in response to mitochondrial toxins. Gene

Functional impact of the ATP1A3-p.A813V variant: insights into a calcium-driven hyperexcitability cascade in rapid-onset dystonia-Parkinsonism.

BACKGROUND: Mutations in the neuronal Na+/K+-ATPase subunit ATP1A3 are linked to a spectrum of neurological disorders, including rapid-onset dystonia-parkinsonism (RDP), yet their pathogenic mechanisms remain incompletely understood. We describe the complex clinical characteristics of a patient with early-onset movement disorders and a likely pathogenic de novo variant in ATP1A3(c·2438C>T, p.A813V). METHODS: We identified a de novo heterozygous ATP1A3 p.A813V variant in a patient with clinically confirmed RDP and employed an integrative approach combining molecular dynamics (MD) simulations,

Genotype-dependent response to combined levodopa/benserazide and trihexyphenidyl in pediatric genetic dystonia.

BACKGROUND: Genetic etiologies underlie a substantial proportion of pediatric dystonia, but whether treatment response to combined dopaminergic and anticholinergic therapy varies by genotype remains unknown. METHODS: This retrospective cohort study analyzed 36 children with genetically confirmed dystonia, classified into three subgroups: PANK2 (n = 7), channelopathies (SCN1A/SCN2A/SCN3A/SCN1B, KCNQ2; n = 6), and other monogenic etiologies (n = 23; 19 distinct genes). All received levodopa/benserazide and trihexyphenidyl combination therapy for 16 weeks. Outcomes included Barry-Albright Dyston

Tremor and Craniofacial Dystonia Preceding Ataxia in SCA43: Expanding the Phenotypic Spectrum of MME-related Ataxia.

Spinocerebellar ataxia type 43 (SCA43) is a rare autosomal dominant ataxia caused by pathogenic variants in the MME gene and typically presents with late-onset gait ataxia and sensory neuropathy. Hyperkinetic movement disorders are uncommon, and prominent craniofacial orobuccolingual dystonia-dominant presentations have not been documented. We report a 63-year-old woman who developed progressive orobuccolingual dystonia and stereotypical choreiform movements of the right hand several years before the onset of gait ataxia. These involuntary movements significantly interfered with feeding and sp

BACKGROUND: Dopa-responsive dystonia (DRD) is an underdiagnosed inherited movement disorder characterized by childhood-onset progressive dystonia, diurnal symptom luctuation, and exquisite responsiveness to low-dose levodopa. Misdiagnosis as epilepsy, cerebral palsy, or juvenile Parkinsonism is common due to overlapping phenotypes and non-specific auxiliary examinations. CASE PRESENTATION: A 22-year-old Chinese female presented with an 18-year history of right-sided limb and perioral involuntary movements, slurred speech, and progressive hypertonia. She was misdiagnosed with epilepsy for over

Social cognition in idiopathic focal dystonia: preliminary evidence on its relationship with cognitive and affective-behavioral functioning.

INTRODUCTION: Although motor symptoms are the defining feature of dystonia, increasing evidence indicates that non-motor symptoms substantially affect patients' quality of life. Among these, social cognition remains insufficiently explored, particularly in relation to global cognitive functioning and affective-behavioral dimensions. METHODS: In this study, 25 patients diagnosed with Idiopathic Focal Dystonia (IFD) and 25 healthy controls (HCs) underwent assessments of social cognition (cognitive and affective Theory of Mind [ToM] and empathy), global cognitive functioning (MoCA), depression (

Bilateral pallidotomy for DYT-28 generalized primary dystonia in a child: a case report and review of literature.

BACKGROUND: KMT2B-related dystonia (DYT-28) is a recently recognized cause of early-onset generalized dystonia. Pallidal deep brain stimulation (GPi-DBS) is considered the preferred surgical treatment; however, access, cost, and hardware-related concerns may limit its feasibility, especially in children from resource-constrained settings. Ablative procedures have been rarely reported in this genetic subtype. CASE DESCRIPTION: We report a 9-year-old boy with genetically confirmed KMT2B-related generalized dystonia who underwent simultaneous bilateral radiofrequency pallidotomy. The child had s

Optogenetic Inhibition of Striatal Parvalbuminergic Interneurons Unmasks Impaired GABA and Adenosine Signaling in DYT1 Knock-In Mice.

Neurochemical imbalances in the striatum are thought to contribute to the pathophysiology of DYT1 dystonia (TOR1A), a severe movement disorder. Parvalbumin-positive GABAergic fast-spiking interneurons (PV+ FSI) exert a powerful inhibition within the striatal microcircuitry. To elucidate the impact of PV+ FSI on striatal neurotransmitter dynamics in a DYT1 knock-in (KI) mouse model, we combined optogenetic inhibition of PV+ FSI with in vivo microdialysis (optodialysis) and LC-MS/MS analysis. Dialysates were collected across baseline (light off), stimulation (light on, 595 nm), and post-stimulat

Misclassifying Ego-Syntonic Violence as Obsessive Compulsive Disorder.

The Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, Text Revision (DSM-5-TR) defines obsessions in obsessive-compulsive disorder (OCD) as recurrent thoughts, urges, or images that are intrusive, unwanted, and distressing. Because the manual does not include an explicit ego-dystonicity requirement, its definition risks conflating anxiety-driven intrusions with gratification-driven impulses. This ambiguity carries significant forensic implications. When violent ideation is emotionally gratifying, it may still be described as distressing or unwanted, in a practical sense. In

Is faciobrachial dystonic seizure a misnomer? A case report of atypical leg-predominant tonic-dystonic attacks in LGI1-associated encephalitis.

BACKGROUND: Antibody‑mediated encephalitis due to leucine‑rich glioma‑inactivated 1 (LGI1) is a frequent cause of immune‑responsive paroxysmal motor phenomena in older adults. The disorder is commonly associated with brief, stereotyped involuntary movements traditionally labeled faciobrachial dystonic seizures (FBDS). However, the "faciobrachial" descriptor may underrepresent the true phenotypic spectrum. We report a case of LGI1‑associated autoimmune encephalitis presenting with leg‑predominant tonic-dystonic attacks, expanding the recognized clinical presentation and highlighting variability

Adult-onset dystonia associated with CHD8 truncating variants: case series and targeted literature review.

The CHD8 gene encodes a chromatin-remodeling protein critical for neural development and transcriptional regulation. Although CHD8 mutations are classically associated with autism spectrum disorder, macrocephaly, and intellectual disability, recent reports suggest that dystonia may also be part of this spectrum. Two unrelated female patients presenting with progressive dystonia were evaluated through detailed clinical, neurological, and neuroimaging assessments. Whole exome sequencing (WES) was performed using the Twist Human Core Exome capture kit and variants were classified following Americ

Motivators for help-seeking among people with opioid use disorder: An interpretative phenomenological analysis.

INTRODUCTION: A substantial proportion of individuals with opioid use disorder (OUD) never engage with specialist substance use treatment, despite strong evidence that treatment participation reduces overdose and mortality risk. Understanding the subjective experiences that drive or inhibit help-seeking may inform approaches that increase service engagement. METHODS: Using Interpretative Phenomenological Analysis (IPA), in-depth semi-structured interviews were conducted with six men currently attending a community drug treatment service in Bristol, UK. Interviews explored lived experiences of

Long-term response to deep brain stimulation of the globus pallidus internus in a patient of African origin with a homozygous DYT-AOPEP variant.

INTRODUCTION: Zech-Boesch syndrome (DYT31 or DYT-AOPEP) is a rare autosomal recessive form of dystonia caused by bi-allelic loss-of-function variants in the AOPEP gene. It typically presents as early-onset, progressive, and frequently drug-resistant dystonia. Although Deep Brain Stimulation (DBS) of the Globus Pallidus internus (GPi) has demonstrated encouraging outcomes in other monogenic dystonias, evidence in DYT-AOPEP remains limited. METHODS: We report the case of a patient with DYT-AOPEP, providing detailed clinical, genetic, and therapeutic characterization, along with long-term follow
