The dystonia-associated Torsin1A sustains CLCC1 function in membrane fusion of the nuclear envelope for NPC biogenesis.

DYT1 dystonia is an incurable movement disorder caused by a loss-of-function mutation in Torsin1A, an endoplasmic reticulum (ER)-resident AAA+ ATPase. Here, we use Drosophila and human cells to shed light on Torsins' mode of action. Fly germ cells lacking dTorsin arrest in development with defects in nuclear pore complex (NPC) biogenesis due to impaired nuclear envelope membrane fusion. We identify the conserved membrane protein Chloride Channel CLIC-like protein 1 (CLCC1) as a Torsin1A interaction partner whose absence phenocopies membrane fusion defects caused by Torsin deletion. CLCC1 is en

Facio-Brachial Dystonic Seizures as an Atypical Presentation of Anti-IgLON5 Disease.

Anti-IgLON5 disease is a rare neurological disorder with overlapping autoimmune and neurodegenerative mechanisms and a strong HLA haplotype association. It typically presents with sleep disturbances, though bulbar and neuropsychiatric symptoms contribute to clinical heterogeneity. The definitive diagnostic hallmark is the detection of anti-IgLON5 IgG antibodies. A 74-year-old female was initially admitted with pneumonia and persistent encephalopathy. Continuous video EEG monitoring captured multiple brief episodes of facial contraction with dystonic posturing of the left upper extremity, consi

Three-Day Bi-Hemispheric tDCS With Motor Training Shows No Improvement in Pianist's With Musician's Dystonia: A Pilot Study.

Focal hand dystonia in musicians is a task-specific movement disorder that can impair professional performance, and effective rehabilitative treatments remain limited. Prior work suggested that bi-hemispheric transcranial direct current stimulation (tDCS) paired with bimanual mirror-movement training may transiently improve fine motor control in affected pianists. This case-control pilot study tested whether extending this approach to three consecutive days would produce detectable and sustained benefit. Ten professional pianists with unilateral musician's dystonia and 10 matched healthy piani

Abnormal head movements in neurological conditions: a knowledge-based dataset with application to cervical dystonia.

Abnormal head movements (AHMs) manifest across a broad spectrum of neurological disorders, however the absence of a multi-condition resource integrating kinematic measurements, clinical severity scores, and patient demographics constitutes a persistent barrier to the development of AI-driven diagnostic tools. To address this gap, this study introduces NeuroPose-AHM, a knowledge-based dataset of neurologically induced AHMs constructed through a multi-LLM extraction framework applied to 1,430 peer-reviewed publications. The dataset contains 2,763 patient-group-level records spanning 57 neurologi

Emergency Department Presentation of Duloxetine-induced Acute Extrapyramidal Symptoms: A Case Report.

INTRODUCTION: Duloxetine, a serotonin-norepinephrine reuptake inhibitor, has been associated with extrapyramidal symptoms and tardive syndromes; however, such adverse reactions are rare and remain sparsely documented, particularly in the setting of non-psychiatric use. CASE REPORT: A young, healthy, active-duty military service member developed acute extrapyramidal symptoms-restlessness, dystonic movements, and acute dystonia-shortly after initiating duloxetine for postsurgical neuropathic pain. CONCLUSION: This case highlights the need for vigilance regarding movement disorders in patients

[Genetic Dystonias].

Dystonia is a movement disorder characterized by abnormal, sustained or intermittent movements and/or postures, which are often exacerbated by voluntary action and associated with overflow phenomena. The current classification is structured along two axes: a clinical axis, which considers features such as age at onset and pattern of distribution, and an etiological axis, which integrates genetic, acquired and anatomical causes. Genetic dystonias constitute a broad and heterogeneous group, frequently beginning in childhood or adolescence, and may present as isolated, combined or complex forms.

Schizo-obsessive disorder in a South Asian context: A case report highlighting religious obsessions and psychosis.

Background The co-occurrence of schizophrenia and obsessive-compulsive disorder has attracted increasing clinical and research attention, yet it is not recognised as a distinct diagnostic entity in current psychiatric classification systems. This overlap is associated with greater symptom severity, increased suicide risk, and poorer treatment outcomes. Despite its clinical relevance, reports from South Asia remain scarce. This case contributes regional data and supports the conceptualisation of a schizo-obsessive subtype, challenging existing nosological boundaries. Case Presentation We report

Dystonia as an early and prominent feature in a patient with CYP2U1 gene mutation: expanding the phenotype of SPG56-a case report.

Hereditary Spastic Paraplegias (HSPs) are a heterogeneous group of neurodegenerative disorders primarily characterized by progressive lower limb spasticity and weakness. Among them, Spastic Paraplegia Type 56 (SPG56) is a rare autosomal recessive form caused by CYP2U1 mutations. While spasticity is the hallmark of SPG56, recent reports have shown expanding phenotypic variability, including dystonia. We report an 8-month-old Arab male who presented with generalized tonic seizures and abnormal head movements, later identified as dystonic in nature. MRI revealed cerebral atrophy and delayed myeli

Primary Familial Brain Calcification (Fahr Disease) Due to a Novel Mutation in Solute Carrier 20 A2 Gene.

Bilateral symmetrical calcification of striatum and pallidum with or without involvement of other brain structures is a rare radiological finding. Genetic causes predominates once hypoparathyroidism is ruled out. Clinical features include a variable combination of neuropsychiatric and motor symptoms, including dystonia, Parkinsonism, ataxia, psychosis, dementia, chorea, and frontal-subcortical cognitive dysfunction. One third of primary familial brain calcification is asymptomatic. Solute carrier 20 A2 gene mutation accounts for the majority of primary familial brain calcification. Here we des

Resilience Mitigates the Link between Adverse Childhood Experiences and Musician's Dystonia: A Neuroendocrine and Psychological Perspective.

BACKGROUND: Musician's dystonia (MD) is a task-specific movement disorder affecting up to 1% of all professional musicians. Although adverse childhood experiences (ACEs) have been proposed as risk factors, the etiology of MD is not fully understood, and protective factors remain largely unexplored. OBJECTIVE: This study investigated a possible protective role of psychological resilience on the association between MD and adverse childhood experiences and examined the influence of dystonia, resilience and ACEs on the stress reactivity of musicians. METHODS: Forty participants with MD were comp

Reclaiming obsessionality: Beyond the obsessive-compulsive spectrum.

The concept of the obsessive-compulsive spectrum has exerted a major influence on contemporary psychiatry and contributed substantially to the establishment of the DSM-5 category of obsessive-compulsive and related disorders. However, the notion of obsessionality employed within this framework does not necessarily correspond to the classical psychopathological concept developed in European descriptive psychopathology. This paper reexamines the concept of obsessionality through a psychopathological comparison with neighboring phenomena, including impulsivity, addiction, autochthonous thinking,

Sick sinus syndrome: A harbinger of anti-LGI1 encephalitis.

Autoimmune encephalitis usually presents with short-duration neuropsychiatric syndromes, mainly behavioral with cognitive dysfunction, recurrent seizures, and movement disorders. Among them, anti-LGI1 encephalitis has faciobrachial dystonic seizure as a pathognomic feature. The two cases reported here exemplify another peculiar feature of anti-LGI1 encephalitis, which has rarely been reported and is therefore not typically emphasized. Both cases were initially treated for cardiac symptoms until the full-blown features of encephalitis became evident. The key to preventing adverse outcomes is ea

Paroxysmal Hemidystonia as the Presenting Manifestation of Previously Undiagnosed Diabetes Mellitus.

BACKGROUND: Hyperglycemia-related movement disorders classically present as hemichorea-hemiballismus, whereas dystonia is far less common and may occasionally precede the recognition of diabetes mellitus. CASE REPORT: A 72-year-old man presented with a 3-month history of recurrent, stereotyped dystonic spells involving the right side of the face, arm, and leg. These episodes occurred approximately twice per minute and lasted about 5 seconds each. Initial laboratory testing revealed a plasma glucose level of 494 mg/dL and an HbA1c of 16.1%, consistent with previously unrecognized diabetes mell

Motor coordination and behavioural deficits in a mouse model of KMT2B-related dystonia.

INTRODUCTION: Pathogenic variants in KMT2B cause early-onset dystonia, but a mouse model that has undergone comprehensive, dystonia-oriented phenotyping is lacking. METHODS: We conducted detailed phenotyping on heterozygous Kmt2b constitutive knockout mice and wild-type littermates, assessing growth, neurobehavioural traits, motor coordination, sensorimotor gating, social behaviour and metabolic parameters, combined with striatal RNA sequencing. RESULTS: Kmt2b knockout mice of both sexes were viable but significantly smaller and lighter than littermate controls. Knockouts were hyperlocomotiv

Pseudoathetotic Pseudodystonia as a Manifestation of Isolated Medullary Demyelination in Neuromyelitis Optica Spectrum Disorder.

BACKGROUND: Pseudoathetosis and pseudodystonia are rare sensory-driven hyperkinetic movement disorders that may mimic primary dystonia. Although these manifestations have been reported in neuromyelitis optica spectrum disorder (NMOSD), they are typically associated with cervical spinal cord lesions, and occurrence due to isolated medullary demyelination is extremely uncommon. CASE REPORT: A 32-year-old woman with aquaporin-4-positive NMOSD developed pseudoathetotic movements with dystonic-appearing posturing of both hands following an isolated demyelinating lesion of the medulla with normal s

Adaptive reweighting of synaptic and intrinsic excitability by deep brain stimulation in a dystonia model.

PURPOSE: Deep brain stimulation (DBS) is a standard treatment for movement disorders like dystonia or Parkinson's Disease. Although its clinical effectiveness is established, the mechanisms by which DBS influences neural motor networks are not fully understood. This study explores the development of adaptive network mechanisms. METHODS: We compared functional impacts of short-term and long-term DBS on a. excitability of medium spiny neurons (MSNs) and b. synaptic transmission in the striatum in the dtsz hamster model, an in vivo model exhibiting dystonic episodes, and used mathematical modell

Deutetrabenazine in Hyperkinetic Movement Disorders: VMAT2 Modulation From Choreiform Disorders to Hyperkinetic Dystonic Syndromes.

Hyperkinetic movement disorders, including dystonia and choreiform syndromes, are pathophysiologically linked to dysregulated dopamine metabolism within the basal ganglia-thalamo-cortical (BGTC) circuitry. Mounting research highlights dopaminergic hyperactivity as a driver of aberrant signaling and motor symptoms. Vesicular monoamine transporters (VMATs) package monoamines into synaptic vesicles, yet they remain pharmacologically underexplored compared to plasma membrane transporters. Recent advances in VMAT2 inhibitors offer a promising therapeutic avenue for hyperkinetic movement disorders.
