Clinical characteristics of THAP1 related dystonia: a case report with phenotypic review.

Dystonia is a movement disorder characterized by sustained or intermittent muscle contractions resulting in abnormal movements and postures. DYT-THAP1 is one of the most frequent genetic causes of generalized dystonia and may present with characteristic clinical features that help guide diagnostic suspicion before molecular confirmation. We report a 58-year-old man who developed dysphagia and dysarthria at 22 years of age, followed by progressive dystonic posturing involving the upper limbs, face, neck, and lower limbs. Family history revealed a similar condition in his father. Neurological ex

Expanding the TBL1XR1 Disease Spectrum: Generalized Dystonia Associated with a New Genetic Variant.

BACKGROUND: A growing number of identified genes increasingly reveal genetic overlaps between neurodevelopmental disorders and combined dystonia syndromes. CASE REPORT: We report a 61-year-old man with a neurodevelopmental disorder, mild ataxic signs and generalized dystonia who had been misdiagnosed with cerebral palsy for 40 years. Whole-exome sequencing identified a novel heterozygous pathogenic frameshift variant in TBL1XR1. DISCUSSION: TBL1XR1 variants are classically associated with Pierpont syndrome and autism spectrum disorder. Although movement disorders have been reported, this cas

Are we missing obsessive-compulsive disorder in children with severe intellectual disability?

Obsessive-compulsive disorder (OCD) is being missed in children with severe intellectual disability. Their compulsive behaviours are routinely attributed to autism spectrum disorder or dismissed as challenging behaviour, without systematic consideration of OCD. I argue that this misattribution is predictable, consequential and correctable. The key diagnostic tool is the ego-dystonic/ego-syntonic distinction: OCD rituals are distressing to the child and a change from their baseline, whereas autistic repetitive behaviours are typically comforting. When this distinction is applied, and OCD is tre

Globus pallidus internus versus subthalamic nucleus deep brain stimulation for Meige syndrome: a retrospective study on short- and long-term efficacy.

OBJECTIVE: Meige syndrome is a complex movement disorder characterized by blepharospasm and oromandibular dystonia that is often resistant to conventional pharmacological and botulinum toxin treatments. Deep brain stimulation (DBS) offers a promising alternative, but the relative efficacy of globus pallidus internus (GPi) versus subthalamic nucleus (STN) targeting remains underexplored. Therefore, the aim of this study was to evaluate and compare the short- and long-term efficacy of GPi-DBS and STN-DBS in managing Meige syndrome. METHODS: This retrospective study analyzed patients with primar

Beyond Improvement of Motor Symptoms: Central Effects of Botulinum Toxin on Anxiety and Depression in Focal Dystonia, Hemifacial Spasm, and Blepharospasm.

Cervical dystonia (CD), blepharospasm (BSP), and idiopathic hemifacial spasm (HFS) are focal hyperkinetic movement disorders with distinct underlying mechanisms. While CD and BSP involve central network dysfunctions within the basal ganglia-thalamo-cortical and cerebellar circuits, HFS primarily results from peripheral facial nerve hyperexcitability. Still, people living with all three conditions often struggle with mood issues like depression and anxiety, which can originate from both the burden of illness and changes in brain biology. We studied 61 patients (CD, n = 30; BSP, n = 9; HFS, n =

Shared and Specific Patterns of Brain Functional Network Abnormalities in Patients With Idiopathic Dystonia and Across Subtypes.

AIMS: Accumulating neuroimaging evidence showed that idiopathic dystonia is a large-scale network disorder, but whether the various clinical sub-types shared imaging abnormalities remains controversial. We aimed to determine whether various forms of idiopathic dystonia have common topological changes or are distinct entities. METHODS: Resting-state functional magnetic resonance imaging and clinical data were obtained from 215 patients with various forms of dystonia and 160 healthy controls (HCs). Whole-brain functional networks were constructed, and the topological parameters were calculated

[Tremor].

Tremor is one of the most common movement disorders encountered in pediatric clinical practice and encompasses a broad spectrum of clinical presentations, etiologies, and outcomes. It is defined as an involuntary, rhythmic, oscillatory movement, and its evaluation in childhood is challenging due to phenotypic variability, limited standardization of complementary studies, and reliance on clinical observation. These recommendations from the Ibero-American Academy of Pediatric Neurology (AINP) aim to provide a practical and updated approach to the classification, diagnostic evaluation, treatment,

CACNA1C -Related Channelopathy Presenting With Adult-Onset Combined Dystonia-Parkinsonism: A Novel Neurological Presentation.

Pathogenic variants in CACNA1C have been associated with cardiac arrhythmias and neurodevelopmental disorders, occasionally accompanied by movement disorders. Here we report the case of a 66-year-old female patient with combined dystonia-parkinsonism carrying a pathogenic CACNA1C variant. Neurological examination revealed painful tremulous cervical dystonia, blepharospasm, divergent strabismus, hypomimia, and mild dystonic posturing of left hand, action tremor, and bradykinesia of the upper limbs. Botulinum toxin type A injections provided only partial relief of cervical dystonia, so the patie

Reviving Hope: Overcoming Deep Brain Stimulation Failure With Partial Cranial Nerve Radiofrequency Ablation in Primary Meige Syndrome.

INTRODUCTION: Deep brain stimulation (DBS) is a standard treatment for refractory primary Meige syndrome (PMS), yet 30%-50% of patients experience suboptimal outcomes. Management strategies for these "DBS failures" remain undefined. This study evaluates the efficacy of partial cranial nerve radiofrequency ablation (RFA) as a salvage therapy for this challenging population. METHODS: We report a retrospective series of three PMS patients classified as "DBS non-responders" based on clinical refractory status or ineligibility for lead revision. Patients underwent imaging-guided partial RFA of the

Neurophysiological Basis of Tremor Disorders: Parkinson's Disease, Essential Tremor, Tremor in Dystonia, and Holmes Tremor.

Tremor disorders encompass heterogeneous syndromes with distinct clinical, electrophysiological, and pathophysiological characteristics. Parkinson's disease tremor typically presents as a unilateral 4 to 6 Hz rest tremor, with re-emergent postural components or pure postural in some, driven by abnormal pallido-thalamo-cortical (PTC) oscillations and modulated by cerebellar inputs. Essential tremor is characterized by bilateral 4 to 8 Hz kinetic >postural tremor with regular, rhythmic EMG bursts, reflecting dysfunction within the cerebello-thalamo-cortical (CTC) pathway. Tremor in dystonia is f

Diagnosis and pathogenesis of dystonia: clinical heterogeneity, shared mechanisms, and neurodevelopmental origins.

The diagnosis of dystonia is often delayed because of its many different clinical manifestations and causes. Updates to the definition and classification of dystonia can assist clinicians in improving its recognition and timely diagnosis. Dystonia is characterised by sustained or intermittent abnormal movements, postures, or both. Patterned phenomenology, alleviating manoeuvres, worsening by voluntary movement, and overflow to adjacent muscles are all clinical clues of the condition. Additional non-motor manifestations are part of the clinical spectrum. In parallel to the broad heterogeneity o

The classificatory ambiguity of task-specific tremor.

Task-specific tremor is a clinically heterogeneous phenotype that remains difficult to accommodate within current tremor classification. Although defined by its occurrence during a specific motor task, the boundaries with position-specific and isometric tremors remain uncertain. Additional classificatory complexity arises when a task-specific tremor phenotype is accompanied by neurological signs that remain subtle, questionable, or uncertain, such as possible dystonic posturing, rest tremor, mild bradykinesia, or response to alleviating maneuvers, and do not clearly support classification as a

Signal-based differentiation of essential and dystonic tremor.

BACKGROUND: There is substantial overlap in the perceived clinical presentation of essential tremor (ET) and dystonic tremor (DT). As both etiologies require different treatment approaches, establishing the correct clinical diagnosis remains key. Massive time series feature extraction and machine learning allows the examination of oscillating biological signals in an objective manner. OBJECTIVE: This study compares the spectrum of tremor characteristics in ET and DT patients across several academic centers to establish a generalizable differentiation. METHODS: 278 accelerometer recordings fr

Metabolic and Volumetric Alterations in the Basal Ganglia and the Cerebellum in Dopa-Responsive Dystonia in Symptomatic and Asymptomatic GCH1 Mutation Carriers.

BACKGROUND: Dopa-responsive dystonia is caused by pathogenic variants in the GCH1 gene. Although its clinical features and reduced penetrance are known, in vivo metabolic and structural alterations in symptomatic (sMC) and asymptomatic mutation carriers (aMC) remain poorly understood. OBJECTIVES: The aims were to characterize the volumetric and neurometabolic brain changes in GCH1 mutation carriers (MC) and explore their relationship with clinical severity. METHODS: We studied 20 sMCs, 5 aMCs, and 25 mutation-free healthy controls (HC) using volumetric magnetic resonance imaging (MRI) combin

Prospective multi-phase observational study evaluating local field potentials to guide deep brain stimulation programming in dystonia at a UK Tertiary Neurosciences Centre (LFP-DYT): a protocol.

INTRODUCTION: Deep brain stimulation (DBS) for dystonia is effective but programming optimisation can take months. Local field potentials (LFPs) recorded by the Medtronic Percept device may provide biomarkers to guide stimulation. This study will prospectively evaluate whether chronic LFP profiles correlate with clinical outcomes and can inform DBS programming strategies. METHODS AND ANALYSIS: LFP-DYT is a single-centre, multi-phase observational study at Newcastle upon Tyne National Health Service (NHS) Foundation Trust. An internal pilot (Cohort 1) will refine recording workflows, followed

Preferential age-related striatal volume preservation in cervical dystonia.

BACKGROUND AND PURPOSE: Previous research has shown inconsistent striatal volumetric differences in isolated, idiopathic dystonia. In healthy populations, progressive striatal volume loss occurs with advancing age. We investigated age-related striatal volume in participants with adult-onset cervical dystonia (CD) and other isolated, idiopathic focal dystonias. METHODS: High-resolution structural MRI was analyzed in 184 participants (92 focal dystonia: 33 CD, 18 laryngeal, 18 blepharospasm, 10 embouchure, 13 upper-limb; 92 healthy controls). Subcortical volumes (caudate, putamen, hippocampus,

Skin intraneural phosphorylated α-synuclein is a highly specific biomarker for early Parkinson's disease.

The early diagnosis of Parkinson's disease (PD) represents a challenge, and novel accurate biomarkers are therefore needed urgently. Detection of phosphorylated α-synuclein (p-α-syn) in skin nerve fibres has shown promise as such a marker. However, its accuracy for the identification of PD among patients with early signs of parkinsonism has not been explored thoroughly. In this blinded, multicentre, prospective and follow-up study, 151 patients diagnosed with early-stage parkinsonism (<18 months duration) were enrolled at three tertiary movement disorder centres. Clinical scales were performed

Clinicopathological characterisation of the first Australian family with the p.G51D SNCA mutation.

PURPOSE: Parkinson's disease is one of the most common neurodegenerative disorders worldwide. We describe the first Australian family with the p.G51D glycine aspartate mutation in the alpha synuclein (SNCA) gene causing early onset Parkinson's disease and dementia. METHODS: The proband was referred to our Artemis Project - a community study of young onset dementia. The proband had an extrapyramidal syndrome with onset at age 38 and at 41 developed cognitive difficulties, which progressed to dementia and was shown to have extensive Lewy body pathology and the p.G51D SNCA gene mutation.
