Childhood-Onset Huntington's Disease-Like Presentation of SCA17 with Intermediate Repeats, A Case Report.

Objectives: In Spinocerebellar Ataxia type 17 (SCA17), penetrance is determined by repeat number of the CAG/CAA trinucleotide in the TATA-binding protein (TBP) gene. The intermediate range (41–48) exhibits only partial penetrance but may manifest a Huntington’s disease-like (HDL-4) syndrome with onset at age 50–60 years. We report a unique case of HDL-4 with childhood onset despite intermediate range repeats. Methods: This patient was evaluated at the University of Minnesota Movement Disorders Clinic. Standard Protocol Approvals, Registrations, and Patient Consents: Informed consent to disclos

Comprehensive description of clinical features of laryngeal dystonia and dystonic vocal tremor.

OBJECTIVE: This study aimed to characterise laryngeal dystonia (LD) phenotypes using combined classification criteria and to evaluate treatment outcomes with botulinum neurotoxin type A (BoNT-A). METHODS: The study retrospectively reviewed patient medical records from our clinic over 5 years to identify cases of LD. We classified LD subtypes (adductor, abductor, mixed) and dystonic vocal tremor (DVT) based on updated criteria. RESULTS: We analysed 75 cases, predominantly adductor LD (60%) and DVT (28%). In all, 84% of patients were females, diagnosed around 68 years of age. The DVT group had

Abrupt-onset cervico-truncal dystonia after Mycoplasma pneumoniae infection: Unmasking a possible CACNA1A-related susceptibility.

Cervico-truncal dystonia has been described in association with infectious or post-infectious processes, including Mycoplasma pneumoniae. However, the contribution of underlying genetic susceptibility in such presentations remains poorly defined. We report the extended follow-up of a 32-year-old man who developed abrupt-onset cervico-truncal dystonia three weeks after a febrile upper respiratory tract infection associated with Mycoplasma pneumoniae. Given the atypical course and lack of sustained response to immunomodulatory therapies, whole-exome sequencing was performed, followed by applicat

[Professor YIN Kejing's clinical experience in treating Meige's syndrome with four-penetrating Yangbai (GB14) combined with double-needle connection technique].

Meige's syndrome is a refractory disorder characterized mainly by dystonia of the eyelids, typically presenting as blepharospasm and difficulty in opening the eyes, and may be accompanied by involuntary facial muscle spasms, with a prolonged and recurrent course. Professor YIN Kejing believes that the main pathogenesis of Meige's syndrome is internal stirring of liver wind. Treatment should address both the root cause and manifestations, with calming the liver and extinguishing wind as the general therapeutic principle. Professor YIN innovatively proposes the concept of alternative communicati

Radiological and Clinical Effects of Exercise Intervention Therapy on Adolescent Idiopathic Scoliosis: A Meta-analysis.

Cutaneous allodynia, pain resulting from application of a non-noxious stimulus to normal skin, is a recently described symptom of migraine, with a potential role in directing optimal treatment for migraine attacks. Manifestations of cutaneous allodynia include discomfort when combing the hair, shaving, and wearing glasses, contact lenses, earrings or tight clothing. The exact mechanism by which a migraine attack is triggered is not known, but it has been theorised that, in some patients, once the attack has begun, central neurons can propagate information about the pain process without the nee

Lactylation and exercise: A molecular link to disease alleviation (Review).

INTRODUCTION: Intravenous platelet glycoprotein (GP) IIb/IIIa receptor inhibitors have a significant beneficial impact on the outcomes of patients undergoing high-risk coronary interventions and in the stabilization of patients with unstable angina pectoris refractory to conventional medical treatment. The role of long-term treatment with oral platelet GP IIb/IIIa receptor inhibitors in patients with coronary artery disease is unproven. This study examined the dose-response effect on inhibition of platelet aggregation by roxifiban (DMP754), a novel oral platelet GP IIb/IIIa receptor inhibitor,

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Fifteen-minute consultation: Management of acute dystonia exacerbation and status dystonicus.

Dystonia is a common disorder of movement and tone, characterised by sustained or intermittent muscle contractions causing abnormal movements, postures or both. Children and young people with dystonia can experience episodes of acute worsening tone, which require prompt treatment. When most severe, dystonia may become life-threatening-a state called 'status dystonicus'. This guide aims to provide a framework for how to approach the child with acutely worsening dystonia, following an 'ABCD' approach: Addressing the precipitant, Beginning supportive care, Calibrating sedation and Dystonia-specif

ATP1A3 dysfunction causes motor hyperexcitability and afterhyperpolarization loss in a dystonia model.

Mutations in the gene encoding the alpha3 Na+/K+-ATPase isoform (ATP1A3) lead to movement disorders that manifest with dystonia, a common neurological symptom with many different origins, but for which the underlying molecular mechanisms remain poorly understood. We have generated an ATP1A3 mutant mouse that displays motor impairments and a hyperexcitable motor phenotype compatible with dystonia. We show that neurons harbouring this mutation are compromised in their ability to extrude raised levels of intracellular sodium, highlighting a profound deficit in neuronal sodium homeostasis. We show

Sustained quality-of-life improvements over 10 years after subthalamic nucleus deep brain stimulation for isolated dystonia.

BACKGROUND: Bilateral deep brain stimulation (DBS) of subthalamic nucleus (STN) has demonstrated efficacy for ameliorating medication-refractory isolated dystonia. Nonetheless, the paucity of evidence regarding its long-term impact on quality-of-life (QoL) necessitates further investigation. OBJECTIVES: This study aimed to elucidate the longitudinal effects of chronic STN stimulation on QoL in patients suffering from isolated dystonia. METHODS: We enrolled 54 subjects diagnosed with isolated dystonia who underwent STN-DBS and maintained post-operative status for over 5 years. The 36-item Sho

Alternative Deep Brain Stimulation Targets in the Treatment of Isolated Dystonic Syndromes: A Multicenter Experience-Based Survey.

BACKGROUND: The globus pallidus internus (GPi) is the traditional evidence-based deep brain stimulation (DBS) target for treating dystonia. Although patients with isolated "primary" dystonia respond best to GPi-DBS, some are primary or secondary nonresponders (improvement <25%), showing variability in clinical response. OBJECTIVE: The aim was to survey current practices regarding alternative DBS targets for isolated dystonia patients with focus on nonresponders to GPi-DBS. METHODS: A 42-question survey was emailed and distributed during a DBS conference to clinicians involved in DBS for dyst

Pathogenic Variant in the 5'-Untranslated Region of GCH1 and Clinical Heterogeneity in a Chinese Family with Dopa-Responsive Dystonia.

BACKGROUND: Variants in the GCH1 gene, encoding guanosine triphosphate cyclohydrolase, are associated with dopa-responsive dystonia (DRD) and are considered risk factors for parkinson's disease. METHODS: Comprehensive neurological assessments documented motor and non-motor symptoms in a Chinese family affected by DRD. Whole-exome sequencing (WES) was employed to identify potential mutations, with key variants confirmed by Sanger sequencing and analyzed for familial co-segregation. RESULTS: The proband, a 50-year-old woman with a 10-year history of limb rigidity, abnormal posture, and a 23-ye

A multi-modal neuroimaging data release for Meige Syndrome and Facial Paralysis Research.

The sharing of multimodal magnetic resonance imaging (MRI) data is of utmost importance in the field, as it enables a deeper understanding of facial nerve-related pathologies. However, there is a significant lack of multi-modal neuroimaging databases specifically focused on these conditions, which hampers our comprehensive knowledge of the neural foundations of facial paralysis. To address this critical gap and propel advancements in this area, we have released the Multimodal Neuroimaging Dataset of Meige Syndrome, Facial Paralysis, and Healthy Controls (MND-MFHC). This dataset includes detail

Comparison of Guided and Unguided Botulinum Injections for Cervical Dystonia: EMG, Ultrasound, and Anatomic Landmarks.

BACKGROUND: Botulinum Toxin type A (BonTA) is the preferred treatment for Cervical Dystonia (CD). However, the success rate is often suboptimal. One of the reasons for treatment failure is the inaccuracy of injections. Some physicians rely on Anatomical Landmarks (AL) for injections, while others use either Ultrasound (US) or Electromyography guidance (EMGg) to improve accuracy. METHODS: This retrospective two-center study compared the therapeutic outcomes of AL-based and EMGg injections with USg injections. Demographic and clinical assessments of previous visits and current visits were recor

A Novel Large Duplication on the X Chromosome as a Cause of Familial Generalized Dystonia: A Case Report.

Chromosomal aberrations are rare but known causes of movement disorders, presenting with broad phenotypes in which dystonia may be predominant. During the investigation of such cases, chromosomal studies are not often considered as a first approach. In this article, the authors describe a family affected by a generalized form of dystonia, evolving from a focal phenotype, for which a new X chromosome large duplication was found to be the likely causative, therefore highlighting the role of such studies when facing complex movement disorders.

Emerging Deep Brain Stimulation Targets in the Cerebellum for Tremor.

Deep brain stimulation (DBS) for essential tremor is remarkably effective, leading to over 80% reduction in standardized tremor ratings. However, for certain types of tremor, such as those accompanied by ataxia or dystonia, conventional DBS targets have shown poor efficacy. Various rationales for using cerebellar DBS stimulation to treat tremor have been advanced, but the varied approaches leave many questions unanswered: which anatomic target, stimulation settings, and indications seem most promising for this emerging approach. This article reviews the clinical experience published to date an
