Pilot Study of a Device to Induce the Hanger Reflex in Patients with Cervical Dystonia.

The hanger reflex (HR) is an involuntary head rotation that occurs in response to a clothes hanger encircling the head and compressing the unilateral fronto-temporal area. Here, we developed an elliptical device to induce the HR and examined its utility for the treatment of cervical dystonia (CD). The study included 19 patients with rotational-type CD. The device was applied to each subject's head for at least 30 min/day for 3 months. Severity scores on part 1 of the Toronto Western Spasmodic Torticollis Rating Scale were evaluated at baseline and after the 3-month trial. Mean scores without a

New Rhizotomy Procedure for Primary Spasmodic Torticollis.

OBJECTIVE: Spasmodic torticollis (ST) is an idiopathic neurologic disorder affecting the muscles of the neck. Surgery is a preferred treatment, when conservative treatments or Botulinum neurotoxin injections fail to relieve the symptoms. Our objective here is to report the outcome of a new surgical method for treating ST patients in our department. METHODS: The new procedure consists of rhizotomy of the spinal accessory nerve (SAN) and C1-C2 nerve roots, coagulation of the distal end of SAN (Group A). The results of this procedure were compared with a group of patients who underwent only rhiz

GABAA Receptor Availability Changes Underlie Symptoms in Isolated Cervical Dystonia.

GABAA receptor availability changes within sensorimotor regions have been reported in some isolated forms of dystonia. Whether similar abnormalities underlie symptoms in cervical dystonia is not known. In the present study, a total of 15 cervical dystonia patients and 15 age- and sex-matched controls underwent 11C-flumazenil PET/CT scanning. The density of available GABAA receptors was estimated using a Simplified Reference Tissue Model 2. Group differences were evaluated using a two-sample T-test, and correlations with dystonia severity, as measured by the Toronto Western Spasmodic Torticolli

Botulinum Toxin Type A Injection for Cervical Dystonia in Adults with Dyskinetic Cerebral Palsy.

We aimed to evaluate the efficacy and safety of injecting botulinum toxin A (BoNT-A) into the neck muscles to treat cervical dystonia (CD) in patients with dyskinetic cerebral palsy (CP). This was a randomized, double-blinded, placebo-controlled trial with cross-over design. We prospectively enrolled adults with dyskinetic CP who were over 20 years old and had been clinically diagnosed with CD for more than one year. The primary outcome measure was the change in Toronto Western Spasmodic Torticollis Rating Scale (TWSTRS) at four weeks from the baseline TWSTRS. Seventeen patients were initially

Neuropsychological consequences of pallidal deep brain stimulation altering brain networks.

The purpose of this study was to evaluate postoperative changes in the neuropsychological function of cervical dystonia patients who had undergone deep brain stimulation (DBS) of the internal segment of the globus pallidus (GPi) and to investigate how DBS affects neuropsychological function by altering the neural networks of the brain. In 12 cervical dystonia patients, the Toronto Western Spasmodic Torticollis Rating Scale (TWSTRS) was used to measure the preoperative and postoperative status of cervical dystonia, and the Seoul Neuropsychological Screening Battery was used to gather neuropsych

Both Deep Brain Stimulation and Thalamotomy in a 13-Year-Old Patient with Primary Dystonia.

BACKGROUND: Primary dystonia is a neurologic disease with characteristics of abnormal, involuntary twisting and turning movements, which greatly affect quality of life of patients. Treatments for dystonia consist of oral medications, botulinum neurotoxin injections, physical therapy, and surgery. For medication-refractory dystonia, surgery, especially deep brain stimulation (DBS), is the optimal option. CASE DESCRIPTION: The patient was a 13-year-old boy suffering from extremely severe primary dystonia, with a Burke-Fahn-Marsden Dystonia Rating Scale-motor score of 118 and a Toronto Western S

The effects of a relaxation program featuring aquatic therapy and autogenic training among people with cervical dystonia (a pilot study).

Classic physical interventions for cervical dystonia (CD) have focused on treating motor components or, on motor components and relaxation programs. However, no CD treatment study has focused on a relaxation program alone. We developed a pilot study to assess whether a therapy completely based on a relaxation program could improve the physical and mental symptomatologies of patients with CD. Fifteen persons were included in the experimental group, which received individual sessions of aquatic (Watsu) therapy (WT) and autogenic training (AT). In addition, 12 persons were included in passive con

Isolated focal dystonia phenotypes are associated with distinct patterns of altered microstructure.

OBJECTIVE: Isolated adult-onset focal dystonia is considered a network disorder with disturbances to the motor basal ganglia and cerebellar circuits playing a pathophysiological role, but why specific body regions become affected remains unknown. We aimed to use diffusion tensor imaging to determine if the two most common phenotypes of focal dystonia are associated with distinguishing microstructural changes affecting the motor network. METHODS: Fifteen blepharospasm patients, 20 cervical dystonia patients, and 30 age- and sex-matched healthy controls were recruited. Maps of fractional anisot

Acupuncture as Adjuvant Therapy for the Management of Cervical Dystonia.

Objectives: There are no curative treatments for cervical dystonia (CD), therefore conventional management is aimed at pain relief and muscle relaxation. Many patients with CD use complementary and integrative medicine interventions to manage symptoms, yet there are limited data on the use of acupuncture for CD. The aim of the current study was to determine the feasibility, safety, and efficacy of adjuvant acupuncture. Materials and Methods: A pilot open-label study was conducted on acupuncture treatments as add-on therapy to botulinum-toxin injection sessions (3 months apart) in 5 subjects wi

[Hardware- software complex for the differential diagnosis and evaluation of the effictiveness of treatment of extrapyramidal diseases].

The hardware-software system of differential diagnosis and evaluation of the efficacy of treatment of a number of extrapyramidal diseases (Parkinson's disease, essential tremor, idiopathic cervical dystonia) is based on the methodology of special processing of the electrical activity of muscles (EMG). The system defines the diagnosis for each investigated muscle, and determine various forms of extrapyramidal diseases including early preclinical ones. The system allows viewing the dynamics of changes in the patient's state, both in numerical characteristics and in the graphic interpretation, an

[Professor FANG Jianqiao's clinical experience in the treatment of primary cervical dystonia].

Professor FANG Jianqiao's clinical experience in the treatment of primary cervical dystonia based on the syndrome differentiation of TCM was explored preliminarily. Based on the disease identification of western medicine and the syndrome differentiation of TCM, in combination with the differentiations of meridians and collaterals of acupuncture, Professor FANG proposes the three-dimensional system of diagnosis and treatment of acupuncture, named "disease differentiation, TCM syndrome differentiation and meridian differentiation". Regarding the diagnosis and treatment of primary cervical dyston

Integration of Osteopathic Manual Treatments in Management of Cervical Dystonia with Tremor: A Case Series.

BACKGROUND: Cervical dystonia, also known as spasmodic torticollis, is a chronic disorder in which patients exhibit involuntary repetitive contractions of neck muscles resulting in abnormal postures or movements. Occasionally, there is also a dystonic head tremor. The underlying mechanisms for cervical dystonia and dystonic tremor are not clear, and treatments are limited. CASE REPORT: In the present cases, two females with head tremor starting in adolescence developed worsening symptoms of cervical dystonia with dystonic tremor in their 60s. On osteopathic physical examination, both had a ve

A pilot trial of square biphasic pulse deep brain stimulation for dystonia: The BIP dystonia study.

BACKGROUND: Dystonia often has inconsistent benefits and requires more energy-demanding DBS settings. Studies suggest that squared biphasic pulses could provide significant clinical benefit; however, dystonia patients have not been explored. OBJECTIVES: To assess safety and tolerability of square biphasic DBS in dystonia patients. METHODS: This study included primary generalized or cervical dystonia patients with bilateral GPi DBS. Square biphasic pulses were implemented and patients were assessed at baseline, immediately postwashout, post-30-minute washout, 1 hour post- and 2 hours postinit

How Many Dystonias? Clinical Evidence.

Literary reports on dystonia date back to post-Medieval times. Medical reports are instead more recent. We review here the early descriptions and the historical establishment of a consensus on the clinical phenomenology and the diagnostic features of dystonia syndromes. Lumping and splitting exercises have characterized this area of knowledge, and it remains largely unclear how many dystonia types we are to count. This review describes the history leading to recognize that focal dystonia syndromes are a coherent clinical set encompassing cranial dystonia (including blepharospasm), oromandibula

Levodopa response differs in Parkinson's motor subtypes: A task-based effective connectivity study.

Parkinson's disease (PD) is a circuit-level disorder with clinically-determined motor subtypes. Despite evidence suggesting each subtype may have different pathophysiology, few neuroimaging studies have examined levodopa-induced differences in neural activation between tremor dominant (TD) and postural instability/gait difficulty (PIGD) subtype patients during a motor task. The goal of this functional MRI (fMRI) study was to examine task-induced activation and connectivity in the cortico-striatal-thalamo-cortical motor circuit in healthy controls, TD patients, and PIGD patients before and afte
