Comparison of peribulbar anesthesia with neosaxitoxin or bupivacaine in dogs undergoing reconstructive keratoplasty.

BACKGROUND: This study compared the analgesic efficacy of peribulbar anesthesia using neosaxitoxin versus bupivacaine in dogs undergoing reconstructive keratoplasty. METHODS: Of 28 dogs, four were excluded for not meeting the inclusion criteria. The anesthetic protocol included dexmedetomidine and methadone, propofol and isoflurane. Immediately after intubation, 24 dogs were randomly assigned (12 per group) to receive peribulbar block with either neosaxitoxin (3.5µg; NeoSTX group) or 0.5% bupivacaine (0.2mL/kg, Bupi group). Intraoperatively, cardiopulmonary variables and end-tidal isoflurane

Beyond the Neck: Long-Term Motor and Non-motor Outcomes of Pallidal DBS in Craniocervical Dystonia.

Pallidal deep brain stimulation (DBS) is effective for treating cranial or craniocervical dystonia, a disorder characterized by abnormal facial expressions, impaired eyelid opening, speech and chewing difficulties, and abnormal head posture, leading to physical disability and functional impairment. However, long-term data on motor, non-motor, and quality-of-life outcomes remain limited with less robust available data than cervical dystonia. In 19 patients with craniocervical dystonia, the Unified Dystonia Rating Scale (UDRS) scores improved by 34.5% at 3-6 months, 51.0% at 1-2 years, 33.3% at

Successful Treatment of Apraxia of Eyelid Opening With Bromocriptine: A Case Report.

Apraxia of eyelid opening (AEO) is a rare neurological disorder characterized by an inability to voluntarily initiate eyelid elevation despite preserved eyelid musculature and strength. Our case involved a 71-year-old female pianist who developed AEO following a ruptured basilar artery tip aneurysm complicated by right thalamic infarction, midbrain edema, intraventricular hemorrhage, and subarachnoid hemorrhage. During inpatient rehabilitation, examination demonstrated an inability to initiate the act of opening her eyes after closure despite preserved eyelid strength and the absence of blepha

On the occurrence of mirror modes at Mercury and their comparison with magnetic holes and ion cyclotron waves.

In the context of this review, civil unrest is defined as disharmony, expressive dissatisfaction and/or disagreement between members of a community, which leads to a situation of competitive aggression that may find expression as disruption of organisation, conflicts, damage to property and injuries. Such a breakdown of harmonious relationships, which may result in property damage and human injuries that may be threatening to life, varies in magnitude from participation of a very few individuals up to the involvement of large crowds of people, which may evolve into a full-scale riot. It is the

Reduced thrombospondin-1 is associated with altered collagen-rich matrix organization in discoid lateral meniscus.

Discoid lateral meniscus (DLM) is susceptible to tearing and degeneration, but the molecular basis of its altered extracellular matrix (ECM) remains unclear. We integrated histology, electron microscopy, transcriptomics, quantitative proteomics, and donor-derived cell and spheroid models to investigate the association between thrombospondin-1 (THBS1) and matrix organization in symptomatic DLM. Compared with traumatic morphologically normal lateral meniscus (NLM), DLM tissue contained fewer cells per analyzed image (mean difference, -16.10; 95% confidence interval (CI), -23.49 to -8.71; P < 0.0

High Dose AbobotulinumtoxinA for Blepharospasm: A Case Report on the Safety and Efficacy of 500 Units of AbobotulinumtoxinA.

BACKGROUND: Blepharospasm is a focal dystonia characterized by involuntary eyelid closure that can impair vision and quality of life. Botulinum neurotoxin type A injections are the standard treatment. However, there is substantial variability in dose requirements among patients. Symptom severity ranges from mild, intermittent blinking to forceful, sustained spasms leading to functional blindness. CASE REPORT: We present two cases of blepharospasm successfully managed with unusually high doses of AbobotulinumtoxinA administered over 8 and 13 treatment cycles, respectively. DISCUSSION: Both pa

Prospective Comparative Study of Topical Tacrolimus and Sirolimus for the Treatment of Pigmentary Keratitis in Pug Dogs.

Pigmentary keratitis (PK) is a prevalent ocular surface disease in Pug dogs, yet comparative evidence on topical immunosuppressants remains limited. This prospective comparative clinical study evaluated the efficacy and safety of two agents with distinct mechanisms-tacrolimus, a calcineurin inhibitor, and sirolimus, an mTOR inhibitor-for the treatment of PK. Thirty-two Pugs (63 eyes) were randomly assigned to receive either 0.03% tacrolimus or 0.03% sirolimus three times daily for six months. Tear film quantity and quality were assessed using the Schirmer tear test, tear break-up time, and Fer

Ocular Causes for Undesired Behaviors in the Horse.

Numerous ocular diseases cause visual impairment and induce ocular pain that may be associated with undesired behaviors in horses. Horses with ocular pain, indicated by the presence of blepharospasm, blepharoedema, ocular discharge or rubbing the eye, should be rested from work to avoid worsening the ocular disease and to avoid undesired behaviors associated with pain or vision impairment. Horses with ocular disease are frequently head shy. Some stoic horses may not have obvious outward clinical signs despite significant visual impairment, and as such, undesired behaviors such as spooking, bal

[Pay attention to blepharospasm and dry eye].

Blepharospasm-related dry eye is a complex multifactorial comorbidity involving neural, ocular surface, and psychological factors, currently facing core challenges including unclear pathogenesis, ambiguous clinical assessment and diagnostic criteria, non-standardized treatment, and lack of multidisciplinary collaboration. The article proposes that a "dry eye-blepharospasm" vicious cycle may exist with significant individual differences; symptom overlap easily leads to misdiagnosis without unified diagnostic standards or objective indicators; while botulinum toxin type A (BTX-A) is effective, c

Globus pallidus internus versus subthalamic nucleus deep brain stimulation for Meige syndrome: a retrospective study on short- and long-term efficacy.

OBJECTIVE: Meige syndrome is a complex movement disorder characterized by blepharospasm and oromandibular dystonia that is often resistant to conventional pharmacological and botulinum toxin treatments. Deep brain stimulation (DBS) offers a promising alternative, but the relative efficacy of globus pallidus internus (GPi) versus subthalamic nucleus (STN) targeting remains underexplored. Therefore, the aim of this study was to evaluate and compare the short- and long-term efficacy of GPi-DBS and STN-DBS in managing Meige syndrome. METHODS: This retrospective study analyzed patients with primar

Childhood-onset ataxia with dystonia: expanding the spectrum of VWA3B-related disorders.

Hereditary cerebellar ataxias are a group of rare genetic disorders that affect coordination, balance, and speech. Childhood-onset forms can be especially severe and difficult to diagnose. The VWA3B gene, though not fully understood, plays a role in brain development and has been linked to autosomal recessive spinocerebellar ataxia type 22 (SCAR22), an adult-onset cerebellar ataxia with variable features. A single family with 3 affected adults has been described to date. We report a 1 year 9 months old boy with early-onset neuroregression and ataxia. Prominent dystonia involving the limbs and

Real-World Retrospective Safety Analysis of OnabotulinumtoxinA for the Treatment of Patients with Chronic Migraine and Concomitant Therapeutic Indications.

INTRODUCTION: Patients receiving onabotulinumtoxinA treatment for chronic migraine (CM) may have coexisting diseases warranting multi-indication use of onabotulinumtoxinA. However, data on safety and treatment patterns for concomitant treatment of CM and other diseases are limited. METHODS: SYNCHRONIZE was a phase 4, multicenter, retrospective study that explored the safety of onabotulinumtoxinA treatment for ≥ 2 therapeutic (non-aesthetic) indications within 3 months. The observation period of interest was approximately 6 months before and 24 months after receiving treatment for the second i

Clinical Presentation and Evolution of Isolated Focal and Segmental Dystonia in Adults - 20 Years' Experience from a Botulinum Toxin Clinic.

BACKGROUND AND OBJECTIVES: Isolated dystonia generally occurs due to genetic causes, and the pattern and distribution may change over time. Botulinum toxin is the first-line treatment in those with isolated focal and segmental dystonia. We aimed to describe the clinical profile and evolution of adult patients with isolated focal and segmental dystonia presenting for treatment in our botulinum neurotoxin (BoNT) clinic, and the response to treatment, over 20 years. METHODS: We retrospectively reviewed the medical records of patients with isolated focal and segmental dystonia who had at least tw

Beyond Improvement of Motor Symptoms: Central Effects of Botulinum Toxin on Anxiety and Depression in Focal Dystonia, Hemifacial Spasm, and Blepharospasm.

Cervical dystonia (CD), blepharospasm (BSP), and idiopathic hemifacial spasm (HFS) are focal hyperkinetic movement disorders with distinct underlying mechanisms. While CD and BSP involve central network dysfunctions within the basal ganglia-thalamo-cortical and cerebellar circuits, HFS primarily results from peripheral facial nerve hyperexcitability. Still, people living with all three conditions often struggle with mood issues like depression and anxiety, which can originate from both the burden of illness and changes in brain biology. We studied 61 patients (CD, n = 30; BSP, n = 9; HFS, n =
