Brain functional network topology and connectivity in primary blepharospasm.

BACKGROUND: The pathophysiology of primary blepharospasm (BSP) remains incompletely understood. This study aimed to characterize whole-brain functional network topology in treatment-naive BSP patients. METHODS: Thirty-nine treatment-naive BSP patients and 39 matched healthy controls (HCs) underwent resting-state fMRI. Graph theoretical analysis was applied to assess global and nodal network metrics. Network-Based Statistics (NBS) identified subnetworks with altered functional connectivity (FC). Correlations between network metrics and clinical variables [Jankovic Rating Scale (JRS), illness d

Abnormal inter-hemispheric functional cooperation in blepharospasm.

BACKGROUND: Blepharospasm, characterized by involuntary contractions of the orbicularis oculi muscles, significantly impairs the quality of life. Its pathophysiology remains unclear. Inter-hemispheric cooperation is a prominent feature of the human brain. This study utilizes resting-state functional magnetic resonance imaging (rs-fMRI) to explore inter-hemispheric functional cooperation in blepharospasm patients by examining connectivity between functionally homotopic voxels (CFH), aiming to identify neural disruptions associated with the disorder. METHODS: We recruited 30 patients with bleph

Combined ophthalmic therapy and equine placental extract supplementation to treat corneal perforation in geriatric cats: A case report in the absence of surgical intervention.

BACKGROUND: Environmental changes affecting pet animals, along with an increase in age-related conditions and diseases among pets, have become increasingly evident. Despite this trend, there remains a paucity of research on therapeutic approaches for geriatric companion animals. CASE DESCRIPTION: The case report describes a 19-year-old spayed female cat that was diagnosed with chronic renal failure and recurrent corneal disease. Surgical intervention was not pursued at the owner's request, leaving medical management as the only viable option. The patient's corneal condition deteriorated, culm

High-Density Glasses in Blepharospasm: Influence of Migraine and Early Morning Effects.

Photophobia and somatosensory disorders in patients with benign essential blepharospasm (BEB) are caused by the accumulation of light input to the brain, resulting in persistent eye-opening difficulties. Consecutive patients with photophobia and photosensitivity due to BEB were recruited. The patients were asked to participate in a trial using high-density glasses, with only 1.5% visible light transmittance, produced by Tokai Optical Co. Ltd. (HD glass®) (Aichi, Japan). Patients were instructed to wear glasses 2-3 times per day for 20 min or more in a dimly lit room every day for at least two

Intra- and Postoperative Botulinum Toxin Injection in Postsurgical Pain Management: A Literature Review.

Postoperative pain remains a significant challenge in surgical services, which necessitates improving analgesic strategies to enhance patient outcomes. Botulinum neurotoxin (BoNT), which was primarily approved for the treatment of strabismus and blepharospasm, has demonstrated a promising impact on pain reduction through mechanisms such as neurotransmitter inhibition, receptor modulation, glial activity suppression, and interactions with opioidergic and GABAergic systems. A number of studies have investigated BoNT's impact on postsurgical pain. However, there is a lack of evaluation of its eff

Efficacy and safety of combination of frankincense and botulinum toxin in the treatment of blepharospasm: a protocol for a single-centre, open-label, randomized, controlled trial.

INTRODUCTION: Blepharospasm (BSP) is a functionally disabling disease with a marked impact on the quality of life of patients. Botulinum toxin (BoNT) injections have been recommended as first-line therapy for BSP. However, the clinical benefits of BoNT are temporary with only about 8-10 weeks duration of benefit in most patients with BSP. Per previous case reports, regular use of topical frankincense essential oil (FEO) can achieve significant symptom relief and can decrease the frequency of BoNT injections. This trial will explore whether BoNT combined with regular application of FEO can impr

Treatment of Blepharospasm with Methylphenidate: A Telemedicine Study.

BACKGROUND: The treatment of choice for blepharospasm and related craniofacial dystonias involves local injection of botulinum toxins. However, some individuals report incomplete or short-duration responses, so additional therapies are sometimes needed. OBJECTIVE: To assess the potential value of methylphenidate as an add-on therapy in this population. METHODS: Six individuals with craniofacial dystonia were given trials of methylphenidate. Videos were collected via a telemedicine platform before and after treatment, and four clinicians applied five different rating scales to score severity

Meige syndrome, a cranio-cervical dystonia with a unique clinical phenotype.

BACKGROUND: The eponym "Meige syndrome" and the term "cranio-cervical dystonia" have been used inter-changeably in the literature and recently the validity of this eponym has been debated. OBJECTIVES: To study the uniqueness of the term "Meige syndrome", from a large cohort of patients with cranio-cervical dystonia. METHODS: We describe the 15years follow-up of a large cohort of 50 patients with Meige syndrome, seen in a neurology movement disorder clinic and in a multidisciplinary neuro-ear-nose-throat (ENT) clinic. Following the original description of Henry Meige (1866-1940), we have incl

A nomogram based on coagulation markers for predicting meige syndrome risk.

BACKGROUND: Meige syndrome (MS) is a rare adult-onset cranial dystonia associated with complex neuropathological mechanisms. Recent studies have shown that abnormal coagulation plays a vital role in the pathological and physiological mechanisms of neurological disease and injury. However, the association between coagulation markers and MS remains unclear. METHODS: Data of 493 patients with MS and 684 healthy controls (HCs) were recruited from the Department of Clinical Laboratory of the Third People's Hospital of Henan Province. Differences in coagulation markers were compared between differe

Comparative Analysis of Progression Milestones Across Parkinson's Disease Clinical, Pathological, and Data-driven Subtypes: A 10-year follow-up.

Parkinson's disease (PD) heterogeneity complicates disease-modification clinical trial design and lead to ambiguous results, necessitating robust subtyping frameworks to identify rapid progressors. This study compared progression milestones across previously defined clinical (tremor-dominant [TD], postural instability/gait difficulty [PIGD], indeterminate), pathological (brain-first, body-first), and data-driven (diffuse malignant [DM], intermediate [IM], mild motor-predominant [MMP]) PD subtypes. Using data from the Parkinson's Progression Markers Initiative (PPMI), we analyzed milestone atta

Development of a Patient-Centered Outcome Tool for Blepharospasm: A Stepwise Modified Delphi Study.

Blepharospasm (BSP) is characterized by excessive orbicularis oculi muscle activity leading to abnormal blinking and involuntary eyelid closure. Botulinum neurotoxin (BoNT) injections are the main treatment for BSP, but they only partially and transiently relieve symptoms, leading to a waxing and waning therapeutic response. A patient-centered outcome (PCO) tool that measures BSP symptoms in a simple and efficient way could inform the development of better treatments. Using a stepwise modified Delphi approach, potential PCO items were first identified using the Dystonia Coalition Database with

Parkinson's disease mild cognitive impairment with MRI evidence of cholinergic nucleus 4 degeneration: A new subtype?

BACKGROUND: Subtyping Parkinson's disease with mild cognitive impairment (PD-MCI) into clinically and pathologically homogeneous groups could improve clinical trials and enable personalized treatments. Cholinergic nucleus 4 (Ch4) degeneration has been linked to cognitive decline in PD, suggesting a distinct PD-MCI subtype with worse prognosis. OBJECTIVE: To determine whether PD-MCI patients with MRI evidence of Ch4 degeneration exhibit a different clinical profile and cognitive trajectory than those without Ch4 degeneration. METHODS: We analyzed baseline MRI scans from 162 PD-MCI participant
