INTRODUCTION: Longitudinally extensive transverse myelitis (LETM) is classically associated with aquaporin-4 antibody-mediated neuromyelitis optica spectrum disorder and myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD). The "H-sign," reflecting gray matter involvement, is considered suggestive of MOGAD. However, infectious etiologies, including neurosyphilis, may rarely mimic autoimmune inflammatory myelopathies radiologically.
CASE REPORT: A 50-year-old man presented with 6 months of erectile dysfunction and urinary retention, followed by progressive gait instability.

UNLABELLED: Electrical stimulation has emerged as a promising strategy for promoting cardiogenic differentiation of mesenchymal stem cells; however, the available evidence remains highly heterogeneous across stimulation parameters, experimental platforms, and outcome measures. This systematic review synthesized the available in vitro evidence regarding the effects of electrical stimulation on MSC cardiogenic differentiation, with particular emphasis on stimulation parameters, experimental platforms and biological outcomes related to early cardiogenic commitment and structural maturation. Neurosyphilis is a rare but reversible cause of ischemic stroke. We report a 41-year-old male with no cardiovascular risk factors initially diagnosed with Bell's palsy for left facial weakness. Four days later, he developed left-sided hemiparesis and dysarthria. Neuroimaging confirmed a right nucleocapsular ischemic stroke. Cerebrospinal fluid analysis revealed pleocytosis (39 cells/µL), elevated protein (191.9 mg/dL), and a reactive VDRL, confirming meningovascular neurosyphilis. Treatment with 14 days of intravenous Penicillin G led to complete clinical recovery after six months. This case h

National case-based surveillance data show that reported neurologic manifestations of syphilis increased during 2019-2022 among persons with early and late-stage syphilis. Neurologic manifestations occurred across demographic groups and among those with and without human immunodeficiency virus (HIV), highlighting the importance of evaluating for neurologic signs and symptoms in all persons with syphilis.

Neurosyphilis, a severe and often neglected complication of Treponema pallidum infection, poses a significant global public health threat, particularly impacting marginalized populations such as men who have sex with men and people with human immunodeficiency virus. Despite global efforts to control syphilis, neurosyphilis is increasingly prevalent, with documented surges of >700% in some regions. Its role in driving cognitive decline often misdiagnosed as psychiatric or neurodegenerative disorders exemplifies "neglected" dimension of diseases. This narrative review synthesizes recent advances

Neurosyphilis remains a diagnostic challenge, particularly when it presents with cranial neuropathies that mimic more common conditions such as Bell's palsy, underscoring the need for heightened clinical awareness amid the global resurgence of syphilis, especially among men who have sex with men (MSM). We report the case of a 40-year-old, previously healthy man who presented with acute-onset left peripheral facial nerve palsy, initially treated as Bell's palsy with corticosteroids; however, subtle neurological findings and limited improvement prompted further evaluation. Serologic testing reve

INTRODUCTION: Syphilis, caused by Treponema pallidum, the "great imitator," has re-emerged as a public health concern. It presents with diverse manifestations, ranging from asymptomatic meningitis and psychiatric changes to meningovascular syphilis presenting as ischemic stroke. We aim to report a case of an elderly male with cerebral neurosyphilis mimicking an acute frontal lobe infarction, emphasizing the importance of considering syphilis in patients with atypical risk factors or clinical presentations.
CASE PRESENTATION: A 77-year-old male presented with subacute personality changes and e

Neurosyphilis (NS) remains a diagnostic challenge due to its diverse clinical manifestations and its ability to mimic other neurological disorders. Despite the global decline in syphilis incidence during the antibiotic era, a recent resurgence has led to an increase in atypical and meningovascular presentations. Early diagnosis is critical, as timely treatment can prevent irreversible neurological damage. We report the case of a 38-year-old immunocompetent male who presented with acute vertigo, unsteady gait, and subsequent confusion. Initial imaging and laboratory investigations were unremark

Syphilis is an infection caused by Treponema pallidum. Without treatment, neurosyphilis may develop and lead to complications such as Charcot arthropathy (CA). First described by Jean-Martin Charcot, this condition was initially recognized as a manifestation of syphilitic infection. With the advent of antibiotics, the incidence of syphilis declined; however, it has since re-emerged in regions including Europe, North America, and China. In recent years, there has been a relative paucity of literature on syphilitic CA, with most reports involving the hip or knee. This article describes a rare pr

Simultaneous diagnosis of myasthenia gravis (MG) and early asymptomatic neurosyphilis is extremely rare. There have only been two cases described in the literature. To the best of our knowledge, this was the first case of MG resolving with concurrent treatment of neurosyphilis. Our patient was a 67-year-old male who presented to the hospital with a diffuse rash, bilateral ptosis, bilateral facial weakness, and bulbar symptoms (dysarthria, difficulty with mastication, and difficulty drinking fluids). After testing positive for syphilis in serum and cerebrospinal fluid (CSF), the patient was di

BACKGROUND: Neurosyphilis is an infectious disease of the central nervous system (CNS) caused by Treponema pallidum, which presents with a wide variety of symptoms depending on the affected CNS structures. We describe two cases of neurosyphilis with ocular motor nerve palsy (OMNP) defined as palsy involving cranial nerves (CNs) III, IV, and/or VI, and provide a systemic review of the literature on OMNP associated with neurosyphilis. METHODS: We describe two patients who presented with unilateral palsy of CN III caused by neurosyphilis. We performed a systematic literature search according to P

Cerebral syphilitic gummata are rare granulomatous lesions associated with tertiary syphilis, typically developing several years after the initial infection. Early-stage syphilis with cerebral gummata and accompanying severe neurological symptoms is rare. Failure to recognize the lesions or delays in diagnosis may lead to marked and potentially irreversible central nervous system injury. In the present study, the case of a 60-year-old man with a 6-month history of syphilis who presented with progressive right-sided limb weakness and dysarthria is reported. BACKGROUND: The global incidence of syphilis has been rising, and atypical clinical presentations are increasingly recognized. Human immunodeficiency virus (HIV)-induced immune changes can alter syphilis progression and complicate diagnosis. Otosyphilis is a rare but potentially reversible cause of sensorineural hearing loss (SNHL). This study investigates otologic involvement in patients co-infected with syphilis and HIV.
METHODS: Medical records of patients diagnosed with both syphilis and HIV infection at Etlik City Hospital between January 2023 and June 2025 were retrospectively reviewed.

INTRODUCTION: Neurosyphilis, caused by Treponema pallidum, is a complex neurological condition arising from untreated syphilis and can present with diverse manifestations, including Parkinsonism-like motor dysfunction. Its pathophysiology involves Blood-Brain Barrier (BBB) disruption, dopaminergic neuronal loss, and neuroinflammatory cascades that resemble mechanisms seen in idiopathic Parkinson's Disease (PD).
METHODS: A literature review from PubMed, ScienceDirect, Scopus, and Google Scholar (2014- 2025) explored studies on neurosyphilis, dopaminergic degeneration, BBB dysfunction, neuroinf

Tabetic arthropathy is a destructive neurogenic joint disease that has become rare due to the early treatment of syphilis with penicillin G. It results from neurosyphilis caused by Treponema pallidum. The diagnosis is suspected in patients presenting with severe joint destruction disproportionate to the absence or minimal intensity of pain and is confirmed by positive syphilitic serological tests. We report the case of a 62-year-old patient with a history of high-risk sexual behavior and a remote episode of cervical lymphadenopathy treated 40 years ago. He presented with chronic painless monoa

We report the case of a 36-year-old male who presented with an acute onset of headache, vomiting, and feverish sensation with a history of prior unsafe sexual activity. VDRL and TPHA were reactive in both the blood and cerebrospinal fluid. A diagnosis of neurosyphilis was thus made, and we successfully treated the patient with ceftriaxone and doxycycline. With the increasing global trend of syphilis, the involvement of the central nervous system, neurosyphilis, is an increasing concern too. Due to the vague nonspecific symptoms and variable timing of presentation, it presents as a diagnostic c

Neurosyphilis is caused by the invasion of Treponema pallidum into the central nervous system (CNS), which is characterized with complex and varied clinical manifestations and lacks early symptom specificity. It usually occurs when syphilis is untreated or poorly treated and can occur in any stage of syphilis, potentially causing severe neurological damage. Otosyphilis, characterized by auditory-vestibular dysfunction, may manifest independently without cerebrospinal fluid (CSF) abnormalities, or coexist with neurosyphilis, as a part of the neurological symptoms of neurosyphilis, and is partic

Neurosyphilis is a central nervous system infection caused by Treponema pallidum and can occur at any stage of syphilis, presenting with diverse neurological and psychiatric manifestations. Although once considered a disease of the past, the recent global resurgence of syphilis has renewed the clinical relevance of neurosyphilis. Early manifestations include meningitis, cranial neuropathies, and meningovascular disease, whereas late-stage disease is characterized by parenchymal involvement, such as general paresis and tabes dorsalis. Diagnosis requires clinical suspicion and is based on combin
