Neurophysiological basis of functional movement disorders: A transcranial magnetic stimulation study.

BACKGROUND: The neurophysiological abnormality in patients with functional movement disorders (FMDs) is not clear. We aimed to study the neurophysiological abnormality in these patients using transcranial magnetic stimulation (TMS). METHODS: We enrolled 30 patients with FMD and 31 healthy controls (HC) in the study. Basic demographic and clinical details were recorded. The severity of motor symptoms was evaluated using the Psychogenic (Functional) Movement Disorder (PMD) Scale. The hospital Anxiety and depression rating scale (HADS) was used for assessing anxiety and depression. The motor cor

DHDDS variants in epilepsy and movement disorders: Two novel mutations and an updated literature review.

INTRODUCTION: Recent evidence suggests that dehydrodolichol diphosphate synthase (DHDDS, MIM*608,172) gene variation can cause neurological diseases. However, the complete clinical and genetic spectrum of DHDDS-related disorders has not been fully elucidated. This study aims to systematically describe the clinical and genetic features of five patients with molecularly confirmed DHDDS variants and to provide an updated review of previously reported cases. METHODS: We retrospectively enrolled patients with DHDDS gene variants in the Epilepsy Center of Beijing Tiantan Hospital, Capital Medical U

Neuronal intermediate filament neurologic autoimmunity in a patient with Merkel cell carcinoma receiving pembrolizumab.

BACKGROUND: Neuronal intermediate filament (NIF) autoimmunity is an uncommon paraneoplastic neurologic syndrome that has increasingly been recognized in association with immune checkpoint inhibitor (ICI) therapy. Merkel cell carcinoma is among the malignancies most frequently associated with NIF antibodies, although reported cases remain limited. CASE PRESENTATION: A 55-year-old female with stage IIIA Merkel cell carcinoma developed vertigo, gait instability, dysarthria, and persistent right hemifacial and cervical spasms after receiving second dose of pembrolizumab for recurrent disease. Simultaneous hyperkinetic movement disorders phenotyping: a cross-cohort pediatric transfer study using routine videos, markerless pose estimation and a tabular foundation model.

BACKGROUND: Accurate recognition of movement disorders (MDs) phenomenology remains a demanding task in pediatric practice, compounded by mixed and evolving motor presentations, inter-examiner expertise variability, and the scarcity of structured digital tools validated across age groups. A child with combined MDs may receive different phenomenological diagnoses from different examiners, with direct consequences for etiological guidance, treatment planning, neuromodulation candidacy, and longitudinal monitoring. Video-based examination is already central to expert practice, but its systematic a

A Small Dose With Large Movements: Suspected Gabapentin-Associated Mixed Hyperkinetic Movement Disorder in End-Stage Renal Disease: A Case Report.

Gabapentin is eliminated almost entirely by the kidneys, and markedly reduced clearance in end-stage renal disease (ESRD) can predispose patients to neurotoxicity. Myoclonus is the most frequently reported gabapentin-associated movement disorder, whereas choreiform and other dyskinetic phenotypes are less common. We present a 77-year-old Hispanic man with ESRD, hypertension, diabetes mellitus with neuropathic pain, and macrocytic anemia who presented to the emergency department for generalized weakness, not for an abnormal movement complaint. Collateral history subsequently established approxi

Case Report: Lithium-induced bradycardia triggered by cecal volvulus: a case of obstruction-associated drug accumulation.

BACKGROUND: Lithium toxicity can rarely manifest as cardiac arrhythmias, often initially presenting as bradycardia. Bowel obstruction is a rare cause of acute lithium toxicity. We present a case of lithium-induced symptomatic bradycardia induced by cecal volvulus. CASE REPORT: An 84-year-old female with a history of bipolar 1 disorder on chronic lithium therapy was admitted for symptomatic bradycardia and abdominal pain. She also presented with myoclonus, polydipsia, and encephalopathy. Laboratory evaluation revealed lithium toxicity in the setting of renal impairment, and abdominal imaging f

Abdominal Tics in Children and Adolescents: A Phenomenology-First Diagnostic Framework and Differential Diagnosis.

AIM: Abdominal tics are a less commonly described axial or tonic tic phenotype in children and adolescents with Tourette syndrome and chronic tic disorders. Their abdominal location and symptom language may create diagnostic uncertainty with gastrointestinal, epileptic, myoclonic, respiratory or functional presentations. METHODS: This clinically focused narrative synthesis identified relevant literature through targeted searches of PubMed/MEDLINE, Scopus and Web of Science from database inception through March 2026, supplemented by a targeted PubMed/MEDLINE and citation-chasing update during

Neuromelanin-sensitive magnetic resonance imaging in the diagnosis and assessment of Parkinson's disease.

Parkinson's disease (PD) is a progressive neurodegenerative disorder characterized primarily by the progressive loss of dopaminergic neurons in the substantia nigra pars compacta (SNpc); noradrenergic neurons in the locus coeruleus (LC) are also frequently affected. Because clinical motor symptoms usually emerge only after substantial neurodegeneration has occurred, there is a compelling need for noninvasive imaging biomarkers capable of detecting early pathological changes. Neuromelanin-sensitive magnetic resonance imaging (NM-MRI) generates neuromelanin-related image contrast in the SNpc and

Sleep-state transition instability with recurrent multisystem activation: A case-based phenotypic framework.

The wake-to-sleep transition is a physiologically vulnerable period during which benign and pathologic motor phenomena may emerge. Current sleep nosology recognizes isolated hypnic jerks, propriospinal myoclonus at sleep onset, sleep-related rhythmic movement disorder, and other minor sleep-related motor activities, but these entities may not fully explain patients with persistent, repetitive motor activation recurring across successive attempts to fall asleep. We describe eight representative cases characterized by recurrent motor events centered on sleep onset or early sleep-state transition

Extensive Parenchymal Brain Calcification in a Patient With Autosomal Dominant Polycystic Kidney Disease and Stage 5 Chronic Kidney Disease.

Extensive parenchymal brain calcification is uncommon in chronic kidney disease (CKD): intracranial vascular calcification affects up to 10% of uremic patients, but widespread Fahr-type parenchymal deposits are rare and, where reported, have occurred in patients with a coexisting hypoparathyroid or parathyroid-hormone (PTH)-resistant state. We describe a man in his 50s with autosomal dominant polycystic kidney disease (ADPKD) and Stage 5 CKD who presented with word-finding difficulty, gait imbalance, and myoclonus and was found to have extensive symmetric calcification of the basal ganglia, th

Is Unilateral Focused Ultrasound Thalamotomy as Effective and Safe as Unilateral Deep Brain Stimulation for Essential Tremor? - A critical evaluation.

BACKGROUND: Focused ultrasound (FUS) thalamotomy has become a common surgical treatment for essential tremor (ET), in some settings partially replacing deep brain stimulation (DBS). However, no randomized comparative studies have been performed. A review directly comparing unilateral FUS thalamotomy and unilateral DBS is therefore warranted. SUMMARY: A systematic literature search identified studies of thalamotomy FUS or thalamic DBS for ET. Eligible studies reported mean values for predefined items of the Clinical Rating Scale for Tremor (CRST) at 6-12 months follow-up. Studies with long-ter

Postoperative Changes in Line-Drawing Trajectories After Vim Thalamotomy for Essential Tremor: An Exploratory Analysis of Functional Correlates.

INTRODUCTION: Ventral intermediate nucleus (Vim) thalamotomy effectively suppresses essential tremor (ET); however, improvements in tremor severity do not necessarily translate into enhanced upper-limb dexterity or quality of life (QOL). In this study, we aimed to characterize postoperative changes in line-drawing trajectories and explore their functional correlates following Vim thalamotomy. METHODS: Thirty patients with ET who underwent unilateral Vim thalamotomy and 13 healthy controls (HC) were included. The ET group was assessed preoperatively and at 1 week and 1 month postoperatively us

Empagliflozin in Lafora disease: A first-in-human 6-month pilot study with electroclinical and FDG-PET findings.

OBJECTIVE: Lafora disease (LD) is a rare, fatal neurodegenerative epilepsy with no approved treatments. Empagliflozin, a sodium-glucose cotransporter-2 (SGLT2) inhibitor, has shown neuroprotective actions in preclinical models through modulation of glycogen metabolism, induction of a pseudo-ketogenic state, improved mitochondrial efficiency, and reduction of excitatory neurotransmission. Based on this rationale, we conducted a first-in-human pilot study of empagliflozin in two genetically confirmed LD patients. METHODS: Two patients with genetically confirmed LD received empagliflozin 10 mg/d

Peripheral Nerve Stimulation for Essential Tremor: a Systematic Review and Meta-Analysis of Effectiveness and Safety.

BACKGROUND: Essential tremor (ET) is the most prevalent movement disorder worldwide. There are limited current pharmacological therapies, and their side effects often preclude adequate doses. Surgical therapies, such as Deep Brain Stimulation (DBS) and Focused Ultrasound (FUS), are reserved for patients with tremor that debilitates activities of daily living (ADLs). Peripheral nerve stimulation (PNS), including transcutaneous afferent patterned stimulation (TAPS) and transcutaneous peripheral nerve stimulation (TPNS), has emerged as a promising non-invasive neuromodulatory approach for many pa

EMG-Driven Phase-Locked Spinal Cord Stimulation Physiologically Modulates Essential Tremor.

Essential tremor (ET) is the most prevalent movement disorder, yet existing pharmacological and surgical treatments often lack efficacy or involve significant risks. Phase-locked neuromodulation offers a targeted approach to disrupt pathological oscillations, but its implementation through non-invasive spinal cord stimulation (tSCS) has remained largely unexplored. In this study, we present a novel closed-loop tSCS framework for ET based on real-time tremor phase estimation from surface electromyography (EMG). The system implements a Fourier Linear Combiner (FLC) algorithm to track the demodul

An Adolescent With Neurosyphilis.

Neurosyphilis is a central nervous system (CNS) infection that has become exceedingly rare in the postantibiotic era, especially in children and HIV-negative persons. It can develop during any stage of an untreated syphilis infection and is known as "the great imitator" for its highly variable clinical presentation with inconsistent lab and imaging findings. We describe the case of a previously healthy 17-year-old boy who presented with 2 weeks of right thumb and middle finger myoclonus followed by right upper extremity focal seizures with secondary generalization.
