Expanding the Genetic Landscape of ATXN2 Variants: Insights From a Biallelic Trinucleotide Repeat Expansion in an Acadian Family.

BACKGROUND AND OBJECTIVES: Spinocerebellar ataxias are a diverse group of autosomal dominant cerebellar ataxias. SCA2 is a complex ataxia with various extracerebellar symptoms, including parkinsonism, dystonia, hyporeflexia, and cognitive impairment. ATXN2 is a modulator of neurologic disease: Expansions of at least 37 CAG (glutamine) repeats are pathogenic for SCA2, while expansions in the intermediate range (30-32) convey risk for the development of neurodegenerative disorders including Parkinson disease and amyotrophic lateral sclerosis. Homozygous variants are exceedingly rare. This study

The impact of paediatric periodic limb movement disorder and obstructive sleep apnoea syndrome on sleep quality and daytime symptoms.

PURPOSE: There is little available literature comparing paediatric periodic limb movement disorder (PLMD) and paediatric obstructive sleep apnoea syndrome (OSA). This retrospective study aimed to examine the impact of PLMD, OSA and OSA with an elevated periodic limb movement index (PLMI) on sleep quality and daytime symptoms. METHODS: This retrospective study examined 132 children who had been diagnosed with OSA (with or without elevated PLMI) and PLMD. All participants underwent overnight polysomnography. Patients were divided into three groups, patients with OSA with low PLMI, patients with

Survival Following a Tricyclic Antidepressant Overdose Presenting With Serotonin Syndrome-Like Symptoms and Critical Illness Polyneuropathy: A Case Report.

We report the case of a patient who ingested a lethal dose of amitriptyline, presented with serotonin syndrome-like neurological symptoms on arrival, and subsequently developed suspected critical illness polyneuropathy (CIP) during the clinical course. A 54-year-old man with a history of depression intentionally ingested approximately 2,250 mg of amitriptyline in a single dose. On arrival, he exhibited altered consciousness, hyperthermia, metabolic acidosis, and QRS prolongation, necessitating the immediate initiation of mechanical ventilation and intravascular cooling. Despite sedation, myocl

Basal Ganglia Intraparenchymal Cyst Revealed by a Movement Disorder.

Neuroepithelial cysts are rare, benign intracranial lesions most often detected incidentally due to their typically silent clinical course. We report the case of a 12-year-old right-handed girl who presented with progressive weakness of the left upper limb accompanied by involuntary movements of the left hemibody, including myoclonus and dystonia. Brain magnetic resonance imaging (MRI) demonstrated a well-defined intraparenchymal cystic lesion within the right basal ganglia, with cerebrospinal fluid-like signal characteristics and no contrast enhancement, radiologically suggestive of a neuroep

Case Report and Literature Review of Mycoplasma Pneumoniae Associated Opsoclonus-Myoclonus-Ataxia Syndrome.

Mycoplasma pneumoniae (MP) is a common respiratory pathogen, but in addition to respiratory tract infection, it can also involve the nervous system, cardiovascular and so on. Nervous system is the most common extrapulmonary complication of MP infection, with a total incidence of about 0.1%. Opsoclonus-myoclonus-ataxia syndrome (OMAS) associated with MP sensation has been reported, but the number of OMAS has been few, and there are few reports on the clinical characteristics and treatment response of the disease. Since the animal model of extrapulmonary manifestations caused by MP has not been

Clinical Heterogeneity in Orthostatic Movement Disorders: Insights from Latent Class Analysis and Disease Burden Scoring.

BACKGROUND: Orthostatic tremor (OT) and orthostatic myoclonus (OM) are rare hyperkinetic disorders characterized by unsteadiness during stance. Substantial clinical overlap limits the prognostic and therapeutic value of categorical diagnostic labels. OBJECTIVES: To assess whether a multidomain disease burden framework provides more meaningful stratification than traditional OT/OM diagnoses and to identify phenotypic subgroups. METHODS: We conducted a cross-sectional analysis of 58 patients with OT/OM who underwent multidomain assessment. A composite Disease Burden Score (DBS) was derived fro

A phenomenological approach to spastic movement disorders: an international expert panel consensus.

BACKGROUND AND OBJECTIVES: Hyperkinetic movement disorders, characterized by complex and overlapping motor patterns, present significant challenges in classification and treatment. The inconsistency in definitions and descriptors complicates both research and clinical communication. This study aims to provide an overview of current terminology and definitions related to spasticity and other hyperkinetic movement disorders associated with central nervous system lesions. We propose a unified terminology and classification system for spastic movement disorders (SMD). METHODS: In this scoping rev

Reversible Isolated Opsoclonus in a Patient With Gastric Cancer and Antibodies to Glutamic Acid Decarboxylase and Myelin Oligodendrocyte Glycoprotein.

Opsoclonus and myoclonus are known paraneoplastic manifestations of malignancy. The most common antibodies found are anti-neuronal antibodies (Hu, Ri, and Ma). The most common affiliated malignancies are small-cell lung cancers, breast cancers, or gynecological malignancies. Gastric cancer is uncommon. Other antibodies have been cited, but are rare. Isolated opsoclonus without significant myoclonus is also rare. While both glutamic acid decarboxylase (GAD) and myelin oligodendrocyte glycoprotein (MOG) antibodies have individually been reported to be associated with opsoclonus, reports of a com

High-amplitude short infraslow activity (SISA) with burst suppression: A potential marker of ictogenicity in critical care patients.

OBJECTIVE: Infraslow activity (ISA) <.1 Hz is supposed to represent epileptogenicity; however, its activity is rarely recorded in clinical practice. Therefore, we have focused on the previously reported short infraslow activity (SISA) with a time constant (TC) of 2 s, always accompanied by burst suppression in acute anoxic encephalopathy (AnE). As the association between SISA and seizures has not been evaluated, this study aimed to elucidate the ictogenicity of SISA. METHODS: Electroencephalographies (EEGs) were evaluated in 16 consecutive comatose patients who showed burst suppression in 115

Periodic limb movements during sedation and general anesthesia in elderly patients: a prospective observational study.

Dexmedetomidine sedation in elderly patients is associated with cognitive benefits due to its biomimetic non-rapid eye movement (NREM) sleep–like state. However, this state may predispose patients to periodic limb movements (PLMs), which can cause unintended procedural risk during sedation. We aimed to determine the incidence of PLMs during dexmedetomidine-based multimodal sedation for transcatheter aortic valve implantation (TAVI) and explore associations with the need for conversion to general anesthesia. In this prospective observational study, 35 consecutive patients (mean age 81 ± 6 years

A New Case of a Neurodevelopmental Disorder and Myoclonic Dystonia Associated with the C.1404del Variant of the ATP5F1A Gene.

ATP synthase (ATPase) is a mitochondrial enzyme responsible for the majority of ATP production. Disease phenotypes associated with mutations in ATPase subunits are extremely rare. To date, only four types of ATPase subunit mutations have been described in detail, with heterogeneous clinical manifestations ranging from hypotonia to epilepsy, movement disorders, developmental delay, and intellectual disability. In this work, in the form of a case report, we present a rare case of a patient with ATPase mutation and knowledge of the clinical symptoms associated with ATPase mutations has a positive

Myoclonus in Pediatric Metabolic Diseases: Clinical Spectrum, Mechanisms, and Treatable Causes-A Systematic Review.

Background: Myoclonus, a sudden brief shock-like involuntary movement, represents a common yet under-recognized manifestation across many inherited metabolic disorders. Although its occurrence has been reported in case series and small cohorts, the overall spectrum, pathophysiological mechanisms, and therapeutic relevance of metabolic myoclonus have not been systematically summarized. Methods: A systematic search of PubMed was conducted for English-language publications from 2014 to 2025 using predefined MeSH terms related to myoclonus, movement disorders, and inborn errors of metabolism. Post-traumatic Hypertrophic Olivary Degeneration with Palatal Myoclonus-A Case Report.

This is a case report of a 7-year-old boy with a medical history of a motor vehicle accident (MVA) 2 years previously. Two years post-MVA, he developed quivering and twitching of the right side of his lip, palate, and neck. Multiple antiseizure medications were attempted with no effect. However, carbamazepine was started with marked improvement. The most recent brain magnetic resonance imaging showed findings suggestive of hypertrophic olivary degeneration (HOD). Electroencephalography captured the abnormal facial twitching with audible palatal clicking, but was not associated with electrograp

Periodic limb movements in sleep and stroke risk: A systematic review and Meta-Analysis.

BACKGROUND: Periodic limb movements in sleep (PLMS) have been associated with cardiovascular morbidity, but their relationship with stroke remains incompletely understood. This systematic review and meta-analysis evaluated the association between PLMS and stroke risk. METHODS: We systematically searched MEDLINE, EMBASE, and Cochrane Library databases through March 2025 for observational studies and clinical trials examining PLMS and stroke. Two independent investigators selected studies, extracted data, and assessed quality using the Newcastle-Ottawa Scale. Risk ratios were pooled using random

A dual manifestation of GAD-antibody spectrum disorder: a case of progressive encephalomyelitis with rigidity, myoclonus and autoimmune epilepsy with mesial temporal sclerosis.

BACKGROUND: Progressive encephalomyelitis with rigidity and myoclonus (PERM) is a rare neurological disorder within the spectrum of stiff-person syndrome (SPS) and epilepsy, and is part of the group of GAD (glutamic acid decarboxylase) antibody-associated disorders. PERM diagnosis is often challenging due to its heterogeneous presentation, and treatment remains difficult, as clear therapeutic guidelines for this condition have yet to be established. To the best of our knowledge, no prior reports have documented the simultaneous occurrence of PERM and anti-GAD–positive autoimmune epilepsy assoc

Transcriptomic and proteomic insights into progressive myoclonus epilepsy type 1.

Progressive myoclonus epilepsy type 1 (EPM1) is a rare neurodegenerative disease caused by partial loss of function of cystatin B (CSTB), a cysteine protease inhibitor with known neuroprotective roles. The disease mechanisms remain largely unsolved, and no treatments are available to control the debilitating myoclonus in EPM1. We investigated the impact of CSTB loss on transcriptome and proteome in three regions of CSTB-deficient (Cstb-/-) mouse brain - the cerebellum, cerebral cortex and hippocampus - during disease progression, providing comprehensive insights into the molecular changes and
