BACKGROUND AND OBJECTIVE: Cefepime-induced neurotoxicity (CIN) is a recognized side effect of excessive cefepime exposure and high plasma concentrations in adults. However, the relationship between cefepime concentrations and neurotoxic symptoms in paediatric patients is unknown. The objective of this paper is to explore the relationship between cefepime pharmacokinetics and CIN in a paediatric intensive care unit (PICU) cohort.
PATIENTS AND METHODS: PICU patients with cefepime concentrations measured for a previous PK study were chosen for this cohort. CIN symptoms and related testing were e

PURPOSE: This study aimed to identify risk factors for epilepsy severity in institutionalized adults with profound intellectual and multiple disabilities.
METHODS: We analyzed patients aged 20 years or older who were institutionalized in two long-term care facilities because of profound intellectual and multiple disabilities from childhood. We examined the relationship between epilepsy severity and possible risk factors, such as patients' clinical characteristics (e.g., cause and level of disability), brain imaging and electroencephalography findings, and antiseizure drug treatment. Epilepsy

BACKGROUND: Mixed movement disorder due to pathogenic variants in the ADCY5 gene (MxMD-ADCY5) is a rare condition characterized predominantly by paroxysmal involuntary choreiform movements involving the limbs, face, and neck, often accompanied by dystonia and myoclonus. Speech disturbances, delayed motor development, cognitive impairment, axial hypotonia, and episodic exacerbations of dyskinesia are also common features. Currently, treatment strategies remain limited; however, several studies indicate a beneficial effect of caffeine in the management of dyskinesia.
OBJECTIVE: This clinical re

Hypoxic-ischemic brain injury (HIBI) can occur after cardiac arrest, asphyxiation, carbon monoxide poisoning, and diffuse brain injury. While it is a major cause of mortality and morbidity, there is no consensus on its management. Therefore, I performed a literature review to analyze HIBI pathophysiology, clinical progression, imaging, treatment, and prognosis with a goal to deepen our understanding of best treatment and prognostication options. A search was performed in PubMed, Scopus, Google scholar, EMBASE, and Crossref. The pathophysiology of HIBI consists of the primary insult due to cess

INTRODUCTION: Progressive Myoclonus Epilepsy (PME) is a rare and complex group of inherited neurodegenerative disorders characterized by progressively worsening myoclonus, cognitive impairment, tonic-clonic seizures and ataxia. The clinical features and genetic underpinnings of PME are diverse, with approximately 80% of individuals now able to receive a molecular diagnosis. This review outlines the clinical phenotypes, genotypes, and management strategies for PME. Literature search for publications on PME in the preceding 20 years, with emphasis for the past one decade, performed using Medline

Transplantation is an effective therapeutic option to improve quality of life in patients with severe methylmalonic acidemia (MMA). However, data regarding neurological complications following transplantation remain limited. A retrospective, single-center study was conducted at Necker Hospital (France), including MMA patients who underwent kidney (KT), liver (LT), or liver-kidney transplantation (LKT) between 2007 and 2022. Our aim was to evaluate acute neurological complications occurring after transplantation, focusing on clinical features, laboratory parameters and neuroimaging. Sensorineur

This review explores the therapeutic potential of the stable gastric pentadecapeptide BPC 157 in addressing electrolyte imbalances, specifically hyperkalemia, hypokalemia, hypermagnesemia, and hyperlithiemia. In hyperkalemia, BPC 157 demonstrated a comprehensive counteractive effect against KCl overdose (intraperitoneally, intragastrically, and in vitro), effectively mitigating symptoms such as muscular weakness, hypertension, sphincter dysfunction, arrhythmias, and lethality. It also counteracted the adverse effects of succinylcholine and magnesium overdose, including systemic muscle paralysi

BACKGROUND: An increasing number of dogs are presented with suspected canine cognitive dysfunction (CCD), and a subset also exhibits myoclonus.
OBJECTIVES: Because CCD shares multiple pathological and pathophysiological features with Alzheimer's disease in humans, and myoclonus has been linked to neurodegenerative disorders in people, the aim of the study was to describe myoclonus in dogs with clinical signs of CCD.
MATERIAL AND METHODS: An anonymous online survey for owners of geriatric dogs (over 7 years of age), consisting of 46 questions, was conducted. The survey included items on signa

INTRODUCTION: PIGK-related glycosylphosphatidylinositol (GPI) biosynthesis disorder is an extremely rare neurodevelopmental condition, with only 12 cases described to date. It is caused by biallelic mutations in the PIGK gene, which encodes a catalytic subunit of the GPI transamidase complex. This enzyme facilitates the attachment of GPI anchors to proteins crucial for cellular signaling and development. Eight of the 12 described cases were reported to have seizures, but the electroclinical characteristics are not well defined.
CASE PRESENTATION: We report a 15-month-old female who presented

PURPOSE: Etomidate often induces adverse effects such as myoclonus during anesthesia induction, thereby increasing perioperative risks for patients to some extent. Oliceridine is a novel opioid with fewer opioid-related adverse reactions. This study aims to investigate the effect of oliceridine pretreatment on etomidate-induced myoclonus.
PATIENTS AND METHODS: This study is a prospective, randomized, double-blind, controlled study. Patients scheduled for elective surgery under general anesthesia were selected and randomly divided into the oliceridine group (Group O) and the normal saline grou

BACKGROUND: Orthostatic tremor (OT) and orthostatic myoclonus (OM) are rare weight-bearing hyperkinetic disorders defined electrophysiologically but often overlap clinically. Prior studies were limited to small series with little assessment of comorbidities, functional outcomes, or treatments. The Longitudinal Orthostatic Tremor Study (LOTS) was initiated to address these gaps through multidimensional phenotyping.
METHODS: Baseline data from 58 consecutively identified patients with OT or OM at a tertiary neurology center in India were analyzed. Clinical evaluation, surface electromyography (

BACKGROUND: Opioid-induced neurotoxicity (OIN) is a significant complication in palliative care, presenting with symptoms of hypersomnolence, hallucinations, cognitive dysfunction (including delirium), myoclonus and hyperalgesia. Due to limited research studies, a consensus on recognising these clinical manifestations is yet to be reached, complicating both early diagnosis and management.
OBJECTIVES: This rapid review aims to synthesise current evidence on the prevalence, clinical manifestations, risk factors and management strategies of OIN in the palliative care setting to enhance clinical

LRRK2 (encoding leucine-rich repeat kinase 2) variants are the most common genetic cause of Parkinson's disease (PD). Lowering LRRK2 levels and/or inhibiting LRRK2 activity may modify PD-associated neuropathology. BIIB094 (ION859), an antisense oligonucleotide, targets LRRK2 mRNA for degradation. REASON was a first-in-human randomized phase 1 study investigating the safety, tolerability, pharmacokinetics and pharmacodynamics of intrathecal BIIB094 in patients with PD. In part A, 40 participants received single doses of BIIB094 10-150 mg or placebo. In part B, 42 participants, stratified by LRR

BACKGROUND: Canine distemper has a high mortality rate in the acute phase, and survivors typically suffer irreversible, progressive neurological sequelae, due to demyelinating leukoencephalitis, under current treatment standards. Mesenchymal stromal cells (MSCs) are found in the adult organism and represent a promising alternative for treating central nervous system disorders by secreting trophic and immunomodulatory factors that reduce inflammation, protect neurons, and promote regeneration. The therapeutic action of MSCs is largely mediated by the secretion of trophic and immunomodulatory fa

BACKGROUND: Tranexamic acid (TXA) is widely used as an antifibrinolytic agent to reduce bleeding in cardiac and extracorporeal circulation settings. Neurological adverse effects, including seizures and myoclonus, are rare and not well documented in patients supported by extracorporeal membrane oxygenation (ECMO).
CASE SUMMARY: We report the case of a 52-year-old male with ST-elevation myocardial infarction and severe left ventricular dysfunction, who was transferred to our intensive care unit department under veno-arterial (VA)-ECMO support and continuous renal replacement therapy (CRRT). To
