Clinical features of paraneoplastic neurologic syndromes with anti-Ri antibodies: PRISMA systematic review.

BACKGROUND AND OBJECTIVES: Paraneoplastic neurological syndromes (PNSs) are a group of neurological disorders linked to malignancies. Their clinical trajectory, differential diagnoses, immune-mediated mechanisms, and outcomes remain inadequately understood. The review aims to improve our understanding of the main syndrome and its clinical progression by systematic review of studies involving patients with anti-Ri-antibody-associated paraneoplastic neurological syndrome (Ri-PNS). METHODS: The protocol adhered to the PRISMA guidelines and is registered with PROSPERO (ID: CRD420251152531). RESU

Effect of surgical obstructive sleep apnea treatment on periodic limb movements of sleep in pediatric patients.

INTRODUCTION: The relationship between obstructive sleep apnea (OSA) and periodic limb movements of sleep (PLMS) is not completely understood, especially among children. This study evaluates the effect of surgical OSA treatment on PLMS in pediatric patients. METHODS: This retrospective record review included children aged 1-18 years who underwent polysomnography (PSG) prior to and after airway surgery between 1/1/2010 and 7/31/2022. Only patients with OSA (obstructive apnea-hypopnea index (OAHI) ≥ 1.5/hour) and any pre-treatment or post-treatment PSG with PLMS elevation (PLM Index (PLMI) ≥ 5/

Neonatal developmental and epileptic encephalopathy with movement disorder and arthrogryposis: A shared phenotype across brain-expressed sodium channelopathies.

OBJECTIVE: Neonatal developmental and epileptic encephalopathy with movement disorder and arthrogryposis (NDEEMA) represents the most severe end of the gain-of-function (GOF) SCN1A disorder spectrum. Sporadic cases of congenital arthrogryposis have also been reported in individuals with SCN2A-, SCN3A-, and SCN8A-related developmental and epileptic encephalopathy. Here, we investigated whether NDEEMA occurs in other brain-expressed sodium channelopathies and characterized its features. METHODS: Individuals with the clinical phenotype of NDEEMA were identified through internal databases, an int

CCL2: A double-edged sword in neuroblastoma, with a critical role in MYCN-amplified tumors.

Neuroblastoma (NB) is the most common extracranial solid tumor in children, characterized by significant clinical heterogeneity and immune evasion. MYCN oncogene amplification is a major driver of tumor aggressiveness and poor prognosis and is inversely correlated with immune infiltration in the tumor microenvironment (TME). Emerging evidence highlights the pivotal role of the chemokine C-C motif ligand 2 (CCL2) in modulating the immune landscape of NB. CCL2 influences the recruitment of various immune cells, including invariant natural killer T (iNKT) cells, dendritic cells (DCs), monocytes,

Opsoclonus-Myoclonus-Ataxia Syndrome Related to Pembrolizumab Treatment for Invasive Ductal Carcinoma: A Case Report.

Immune checkpoint inhibitors (ICIs) such as pembrolizumab have become integral to the treatment of various metastatic malignancies. However, their use is associated with a growing spectrum of immune-related adverse events, including rare neurological complications. This report adds to the limited existing literature on ICI-associated opsoclonus-myoclonus-ataxia syndrome (OMAS) and highlights the importance of prompt recognition and multidisciplinary management of such cases. We present a case of a 67-year-old female who presented with rapid, chaotic eye movements, truncal ataxia, and full-body

Consensus roadmap on periodic leg movements during sleep: outcomes of the international PLMS taskforce.

Periodic leg movements during sleep (PLMS) are a frequently observed motor phenomenon in polysomnography, yet their biological meaning and clinical relevance are debated. Further, how these should be assessed and tabulated for optimal evaluation is uncertain. To address these gaps, an international, multidisciplinary taskforce convened a series of online meetings followed by an in person taskforce workshop held in Troina, Italy, on November 14-15, 2025, where agreement was reached through iterative discussion (qualitative; no Delphi or predefined voting thresholds). This taskforce roadmap summ

Evaluation of Selected Brain Damage Biomarkers in the Determination of Brain Damage in Dogs With Neurological Distemper.

BACKGROUND: Canine distemper is a disease with high morbidity and mortality in domestic and wild carnivores caused by Canine distemper virus (CDV). Respiratory, gastrointestinal, dermatological, ophthalmic, or neurological disorders occur in dogs with canine distemper. CDV replicates in the central nervous system and causes non-purulent encephalomyelitis. This causes damage to the brain and spinal cord. OBJECTIVES: The aim of the study was to reveal brain damage caused by the canine distemper virus in dogs using selected brain damage biomarkers and to determine which of these biomarkers are d

Cervical spondylosis with propriospinal myoclonus and vocalization: a case report.

BACKGROUND: Cervical spondylosis typically presents with neck pain, radiculopathy, or myelopathy, but rarely with spinal myoclonus. Propriospinal myoclonus (PSM)-characterized by involuntary trunk and limb jerks-is an exceedingly rare manifestation of cervical spondylosis, with limited reports of associated vocalizations. CASE PRESENTATION: A 55-year-old man presented with a 3-year history of neck pain, which progressed to dizziness, lower limb weakness, and intermittent propriospinal myoclonus manifesting as hand tremors and involuntary vocalizations. Cervical MRI and CT revealed C3/4-C5/6 d

Pistol sign: unilateral hand dystonia predominantly associated with corticobasal syndrome.

A 66-year-old right-handed woman with corticobasal syndrome (CBS) developed a distinctive unilateral hand dystonia, having previously presented with progressive cognitive and behavioural decline. An MR scan of the brain showed bifrontal cortical atrophy with subtle signal changes involving the left caudate nucleus and putamen. Electroencephalography and cerebrospinal fluid analyses were unremarkable. Over subsequent months, the patient developed worsening right-sided rigidity, limb apraxia, myoclonus and alien limb phenomena. During this progression, a characteristic abnormal hand posture beca

Lansoprazole as a possible trigger of restless legs syndrome with suspected periodic limb movement disorder in a hemodialysis patient: a case report.

Restless legs syndrome (RLS) and periodic limb movement disorder (PLMD) are common in patients undergoing hemodialysis and can significantly impair sleep and daily functioning. Several medications are known to induce or exacerbate RLS/PLMD, but proton pump inhibitors (PPIs) have not been clearly implicated. We report a hemodialysis patient with dopaminergic-refractory RLS/PLMD whose symptoms improved markedly following discontinuation of lansoprazole, suggesting a possible drug-related trigger. A 76-year-old woman with end-stage renal disease due to diabetic nephropathy began hemodialysis. Lan

Myoclonin1 haploinsufficiency in motile ciliated cells partially recapitulates epileptic features of Efhc1-deficient mice in adult age.

Mutations in the EFHC1 (EF-hand domain containing 1) gene, which encodes myoclonin1, a homolog of the Chlamydomonas axonemal protein Rib72, have been identified in patients with epilepsies including juvenile myoclonic epilepsy (JME). Myoclonin1 is expressed in choroid plexus epithelial cells during fetal development and in motile cilia of ependymal cells lining brain ventricles as well as in tracheal cilia and sperm flagella during postnatal stages. We previously reported that systemic Efhc1 deficiency in mice causes spontaneous myoclonus, increased susceptibility to the chemoconvulsant pentyl

Autonomic correlates of large muscle group movements during non-REM sleep in restless legs syndrome: a comparative analysis with periodic and non-periodic leg movements.

STUDY OBJECTIVES: Large muscle group movements during sleep (LMMS) have recently been recognized as a prevalent feature in patients with restless legs syndrome (RLS), yet their autonomic profile remains insufficiently characterized. This study aimed to compare heart rate (HR) changes associated with LMMS to those accompanying short-interval (SILMS), periodic (PLMS), and isolated leg movements (ISOLMS) during non-REM sleep in RLS. METHODS: Thirty drug-free RLS patients (20 women, mean age 57.6 ± 12.73 years) underwent full-night polysomnography. For each subject, five arousal-associated events

Transcriptomic disruption and hypoactivity in DYT-SGCE medial ganglionic eminence-patterned inhibitory neurons.

Myoclonus dystonia is a Mendelian inherited, childhood-onset dystonic disorder, caused by mutations in the autosomal dominantly inherited gene SGCE, and in which both motor and psychiatric phenotypes are observed. Results from murine and in vivo human studies suggest that dystonia is caused by disruption to neuronal networks, in particular the basal ganglia-cerebello-thalamo-cortical circuit. Work focused on the cortical component implicates disruption to neuronal excitatory-inhibitory balance as being a key contributor to the observed phenotypes. Our previous work, focused on cortical excitat

Fentanyl and Propofol Versus Fentanyl and Etomidate for the Insertion Conditions of Laryngeal Mask Airway: A Prospective Observational Study.

BACKGROUND: Laryngeal mask airway (LMA) is often used in short surgeries for its easy insertion and minimal invasiveness but successful placement needs reflex suppression and stable vitals. This study compared Fentanyl-Propofol which gives better insertion but may cause hypotension with Fentanyl-Etomidate which maintains hemodynamics but causes more myoclonus. MATERIAL AND METHODS: 60 American Society of Anesthesiologists I-II patients aged 18-60 years were included in this prospective study. Group A got Fentanyl + Propofol. Group B got Fentanyl + Etomidate. We compared attempts, time, ease,

Longitudinal Orthostatic Tremor Study (LOTS): a multi-centre, prospective cohort protocol for characterising orthostatic tremor and orthostatic myoclonus.

INTRODUCTION: Orthostatic tremor (OT) is a rare movement disorder defined by high-frequency tremor predominantly in the standing position, often causing mobility limitation and reduced quality of life. Its natural history, disease burden and treatment response remain poorly understood due to limited longitudinal evidence. The Longitudinal Orthostatic Tremor Study (LOTS) aims to characterise clinical, demographic and therapeutic trajectories in OT and orthostatic myoclonus over 5 years. METHODS AND ANALYSIS: LOTS is a multicentre prospective cohort study expanding an initial retrospectively ch
