Extreme conformal prediction: Reliable intervals for high-impact events.

BACKGROUND: Prehospital (ambulance) care can reduce morbidity and mortality from trauma. Yet, there is a dearth of effective evidence-based interventions and implementation strategies. Emergency Medical Services Traumatic Shock Care (EMS-TruShoC) is a novel bundle of five core evidence-based trauma care interventions. High-Efficiency EMS Training (HEET) is an innovative training and sensitization program conducted during clinical shifts in ambulances. We assess the feasibility of implementing EMS-TruShoC using the HEET strategy, and feasibility of assessing implementation and clinical outcomes

Direct Localization of the VIM/DRTT Using Quantitative Susceptibility Mapping in Essential Tremor: A Pilot MRI Study.

BACKGROUND AND PURPOSE: Accurate localization of the ventral intermediate nucleus (VIM) within the dentatorubrothalamic tract (DRTT) is critical for effective neurosurgical treatment of essential tremor (ET). This study evaluated the feasibility and anatomic specificity of quantitative susceptibility mapping (QSM) for direct VIM/DRTT visualization, comparing it with conventional diffusion tractography-based reconstructions. MATERIALS AND METHODS: Twenty-seven participants (10 healthy controls, 17 patients with ET) were enrolled across 2 institutions and imaged on 3T MRI systems. QSM-defined V

Skull Lesions after MR-Guided High-Intensity Focused Ultrasound: A Case Series.

Transcranial MR-guided high-intensity focused ultrasound (HIFUS) ablation is a therapeutic technique for essential tremor and tremor-dominant Parkinson disease. Skull marrow lesions may occur posttreatment and mimic metastatic disease. In this retrospective study of 10 patients receiving a delayed posttreatment MRI, 4 patients developed skull lesions. There were no lesions on immediate posttreatment MR scans obtained within 24 hours. Skull lesions on delayed scans weeks to months after HIFUS consisted of multiple heterogeneous regions, prompting 1 patient to undergo a complete metastatic work-

Brain Plasticity Induced by MR-Guided Focused Ultrasound Correlates with Tremor Improvement in Essential Tremor: A Prospective Cohort Study.

BACKGROUND AND PURPOSE: MR-guided focused ultrasound (MRgFUS) is a promising and noninvasive treatment for medication-refractory essential tremor (ET). However, the mechanism of long-term postoperative brain structural remodeling remains unclear. This study aimed to investigate the changes in gray matter volume (GMV) before and after MRgFUS, as well as to explore the relationship between GMV changes and tremor control. MATERIALS AND METHODS: Twenty-six patients with ET who underwent MRgFUS successfully were included in this study. They underwent structural MR imaging and clinical assessment b

The Utility of Olfactory Testing to Discriminate Parkinson's Disease From Diagnostic Mimics: A Systematic Review and Meta-Analysis.

BACKGROUND: Hyposmia is present in most patients with Parkinson's disease (PD), whereas olfaction is usually preserved in its diagnostic mimics. To address the limited evidence from smaller studies, we conducted a meta-analysis on the diagnostic accuracy of olfactory testing in differentiating PD from clinical look-alikes. METHODS: A systematic search was performed in PubMed and Web of Science according to the PRISMA guidelines. Studies describing results of validated smell tests in PD patients and at least one differential diagnosis were included. The risk of bias and applicability was asses

Parkinson's disease subtypes and their relationship to dementia with Lewy bodies: a comparative neuroimaging analysis based on nigrostriatal degeneration.

This study hypothesized a subtype of Parkinson's disease (PD) sharing features of nigrostriatal degeneration with dementia with Lewy bodies (DLB) to account for the difficulty of differentiating the two diseases and explored their relationship. Patients with newly diagnosed PD or DLB (n = 128 and 42, respectively) were enrolled along with patients with essential tremor as controls. Based on data from cardiac 123I-metaiodobenzylguanidine scintigraphy and 123I-FP-CIT dopamine transporter single-photon emission computed tomography, the PD group was divided into a non-advanced (PDNA) and advanced

Impact of DaTscan on the Management of Clinically Uncertain Parkinsonian Syndromes: A Retrospective Canadian Cohort Study.

BACKGROUND: Diagnosing clinically uncertain parkinsonian syndromes (CUPS) is challenging. Dopamine transporter (DaT) SPECT imaging (DaTscan) aids in differentiation, but its real-world impact on management in Canada, where it is not publicly funded, is unclear. The objective was to determine the impact of DaTscan results on clinical management for patients with CUPS in a Canadian tertiary care movement disorder service. METHODS: We conducted a retrospective chart review of 42 patients with CUPS referred for a DaTscan from a tertiary clinic in London, Ontario. DaTscan result was categorized as

Staged Versus Simultaneous Bilateral Deep Brain Stimulation: A Matched Comparison of Outcomes and Resource Utilization.

BACKGROUND AND OBJECTIVES: Deep brain stimulation (DBS) is a widely used treatment for movement disorders. While bilateral implantation is common, whether to implant electrodes in separate (staged) procedures or in a single (simultaneous) session remains debated. METHODS: In this retrospective matched cohort study, 252 patients who underwent staged bilateral DBS were propensity matched to 252 who underwent simultaneous procedures (n = 504) based on age, sex, and diagnosis. Demographic, clinical, and operative data were extracted from the electronic medical record. RESULTS: Staged patients we

Creation and clinical utility of a 3D atlas-based model for visualising brain nuclei targeted by MR-guided focused ultrasound thalamotomy for tremor.

Magnetic resonance guided focused ultrasound (MRgFUS) thalamotomy is an established treatment for tremor. MRgFUS utilises ultrasound to non-invasively thermally ablate or 'lesion' tremorgenic tissue. The success of treatment is contingent on accurate lesioning as assessed by tremor improvement and minimisation of adverse effects. However, coordinate planning and post-procedure lesion visualisation are difficult as the key targets, cannot be seen on standard clinical imaging. Thus, a computational tool is needed to aid target visualisation. A 3D atlas-based model was created using the Schaltenb

Structural basis for the subtype-selectivity of KCa2.2 channel activators.

Small-conductance (KCa2.2) and intermediate-conductance (KCa3.1) Ca2+-activated K+ channels are gated by a Ca2+-calmodulin dependent mechanism. NS309 potentiates the activity of both KCa2.2 and KCa3.1, while rimtuzalcap selectively activates KCa2.2. Rimtuzalcap has been used in clinical trials for the treatment of spinocerebellar ataxia and essential tremor. We report cryo-electron microscopy structures of NS309-bound KCa2.2 and KCa3.1, in addition to structures of rimtuzalcap-bound KCa2.2 and mutant KCa3.1_R355K. The different conformations of calmodulin and the cytoplasmic HC helices in the

From Ablation to Neuromodulation Platform: The Evolving Role of Magnetic Resonance-Guided Focused Ultrasound in Functional Neurosurgery.

Magnetic resonance-guided focused ultrasound (MRgFUS) has rapidly evolved from an experimental concept into a versatile platform in functional neurosurgery. Initially pioneered as a noninvasive thermal ablation modality for essential tremor, MRgFUS has since gained regulatory approval and demonstrated durable long-term efficacy. Its clinical applications have expanded to include Parkinson's disease, chronic pain, psychiatric disorders, and investigational use in dystonia, epilepsy, and brain tumors. Beyond lesioning, low-intensity focused ultrasound enables reversible neuromodulation and trans

Low-intensity transcranial ultrasound effects on the ventral intermediate nucleus and zona incerta in Parkinson's disease tremor.

BACKGROUND: Tremor in Parkinson's disease (PD) is a disabling symptom that often persists despite pharmacological treatment. High-intensity focused ultrasound (HIFU) targeting the ventral intermediate nucleus (VIM) alleviates Essential Tremor, but recent evidence suggests the zona incerta (ZI) may be a superior target for Parkinsonian tremor. This study compared the effects of transcranial ultrasound stimulation (TUS) to the VIM and ZI on postural and rest tremor, and examined related neural correlates using resting-state fMRI (rs-fMRI). METHODS: In this within-subject, crossover study, 19 pa

Underlying Tremor Improvement with Consistent Use of Transcutaneous Afferent Patterned Stimulation in Patients with Essential Tremor.

BACKGROUND: Transcutaneous afferent patterned stimulation (TAPS) is a non-invasive, wrist-worn neurostimulation therapy that has demonstrated acute and short-term lasting tremor reduction in patients with essential tremor (ET). However, the longer-term improvement in underlying tremor severity from consistent use of TAPS has not been fully explored. METHODS: We conducted a retrospective analysis of the multicenter PROSPECT trial, which evaluated twice-daily TAPS use over three months in patients with ET. Underlying tremor improvement was assessed by comparing pre-stimulation tremor severity a

Comparative study of the pathology in anterior versus posterior hemispheric regions of cerebellum in essential tremor and controls.

Essential tremor (ET), among the most common movement disorders, is characterized by 8-12 Hz action tremor of the upper extremities. Cognitive dysfunction is increasingly recognized. Postmortem studies of anterior cerebellar cortex, which plays a major role in motor function, have systematically identified morphologic changes centered on Purkinje cells (PCs) and adjacent neuronal connections, distinguishing ET from controls. However, the cerebellar cortex is compartmentalized into distinct functional anatomic regions, including control of cognition in posterior lobe. No systematic study of thi

Diagnostic significance of rhythmicity in postural hand tremor.

Rhythmicity is an important feature of tremor that is widely viewed as having diagnostic significance. We hypothesized that rhythmicity might be a generic function of tremor severity, reflecting greater oscillatory entrainment of motor pathways. Postural hand tremor and forearm electromyograms were recorded from 49 controls, 78 Parkinson patients, and 133 essential tremor patients. Rhythmicity was quantified in terms of approximate entropy and three measures of cycle-to-cycle frequency variability: tremor stability index, cycle-to-cycle variability, and power spectral bandwidth. Physiological

Delayed dysgeusia in five patients following MRI-guided focused ultrasound surgery targeting the ventral intermediate nucleus of the ‍thalamus for essential tremor.

Perioperative complications of MR-guided focused ultrasound therapy for Vim-targeted tremor are often reported, such as weakness and paresthesia. In this paper, five cases of dysgeusia after MR-guided focused ultrasound therapy targeting the Vim in patients with essential tremor are presented. All patients also suffered from sensory disturbances. In most cases, dysgeusia is mild, but there have been reports of patients with severe weight loss. Dysgeusia has also been reported in deep brain stimulation therapy. Dysgeusia is a complication that should be considered in the treatment of Vim-target

Current Approach to Surgical Management of Tremor Disorders.

Essential tremor (ET) is the most common cause of tremor worldwide and can become profoundly disabling in many patients, with pharmacological treatments often providing insufficient relief. Surgical interventions have emerged as effective strategies for long-term tremor control. This review summarizes the current evidence on surgical therapies, including deep brain stimulation (DBS), radiofrequency (RF) thalamotomy, magnetic resonance-guided focused ultrasound (MRgFUS), and Gamma Knife radiosurgery (GKSR) for ET and other tremor-inducing syndromes. These techniques demonstrate comparable effic

Action tremor kinematics exhibit features of stabilization dysfunction in essential tremor patients.

BACKGROUND: Action tremor that begins with voluntary movement is a common manifestation of essential tremor (ET). We investigated a novel approach to detect reach direction and tremor direction across naturalistic reaches, elucidating this relationship. METHODS: ET patients undergoing awake DBS implantation surgery performed a center-out reaching task, which elicited multi-joint, naturalistic reaching movements made to evenly spaced radial targets. This project utilized a deep-learning based markerless motion tracking system to monitor movements in 3-dimensions. For analysis, to test novel hy

Disrupted dynamic brain network and its functional topological underpinning in essential tremor.

BACKGROUND: Essential tremor (ET) is one of the most prevalent neurological diseases and is recognized as a disorder involving multiple neural network dysfunctions. Previous resting-state fMRI studies in ET ignored brain network important dynamic nature. This study aimed to investigate the alterations of dynamic functional connectivity (DFC) and its functional topology in ET. METHODS: Resting-state fMRI data were collected from 144 ET and 131 normal controls (NC). Sliding-window approach with K-means clustering algorithm was used to identify dynamic functional states and graph theory analysis
