Lafora disease (LD, OMIM#254780) is a rare, devastating form of progressive myoclonus epilepsy characterized by the accumulation of aberrant insoluble forms of glycogen (polyglucosans) in the brain and peripheral tissues. Recently, we reported that neuroinflammation could be considered a second hallmark of LD in mouse models, since brains from LD mice present high levels of proinflammatory cytokines and chemokines, produced by reactive astrocytes and microglia. We also reported that the TNF and IL6/Jak2 signaling pathways were the main contributors to this neuroinflammation, and that there was

OBJECTIVE: Tinnitus is the perception of sound without an external source. More than 50 million people in the United States have reported experiencing tinnitus, resulting in an estimated prevalence of 10% to 15% in adults. The focus of this guideline is on tinnitus that is both bothersome and persistent (lasting 6 months or longer), which often negatively affects the patient's quality of life. The target audience for the guideline is any clinician, including allied health professionals, involved in managing patients with tinnitus. The target patient population is limited to adults (18 years an

Stiff-person syndrome (SPS) is an rare autoimmune neurological disorder that is associated with progressive axial and proximal muscle rigidity, stimulus-sensitive painful spasms, and significant functional disability. The disorder is commonly associated with high-titer antibodies against glutamic acid decarboxylase-65 (GAD65), leading to impaired gamma-aminobutyric acid (GABA)-mediated inhibitory neurotransmission, resulting in persistent motor neuron hyperexcitability. Despite the conventional classification of SPS as a motor disorder, the condition commonly occurs with neuropsychiatric sympt

Progressive encephalomyelitis with rigidity and myoclonus (PERM) represents the most severe subtype of stiff-person spectrum disorders and is exceedingly rare in children. We report a 14-year-old female patient who presented with chronic joint pain and axial rigidity as the initial manifestations, with a disease course of two years, during which she had been diagnosed with juvenile idiopathic arthritis but lacked typical peripheral arthritis signs and showed poor response to therapy. The patient exhibited paroxysmal axial rigidity, autonomic dysfunction, and sleep disturbances. Although typica

Palatal myoclonus is a clinical manifestation caused by lesions involving the Guillain-Mollaret triangle (dentato-rubro-olivary pathway) and is often associated with hypertrophic olivary degeneration. Reported causes of palatal myoclonus include infectious, inflammatory, autoimmune, demyelinating, neoplastic, vascular, and drug-related disorders. We report a case of drug-resistant tuberculous meningitis presenting with palatal myoclonus and describe the patient's clinical course and outcome.

OBJECTIVE: Progressive encephalomyelitis with rigidity and myoclonus (PERM) is a rare autoimmune neurologic disorder associated with antibodies against the glycine receptor (GlyR). Neuropathologic descriptions are limited. We here report clinical and autopsy findings of a patient with GlyR antibody-associated PERM.
METHODS: Comprehensive postmortem neuropathologic examination was performed using routine histology, immunohistochemistry, and immunofluorescence across neocortical, subcortical, brainstem, cerebellar, and spinal cord regions. Antibodies included GAD65/67, GlyR, IBA1, CD163, GFAP,

Lance-Adams syndrome (LAS) is a rare condition described as chronic intentional myoclonus following prolonged hypoxia. With limited cases of the condition, no standardised diagnostic criteria, and features overlapping acute myoclonus status epilepticus (MSE), diagnosis and management are challenging. Our case is a man in his 20s, who was admitted to the intensive care unit following two in-hospital cardiac arrests. He consequently developed persistent generalised myoclonic seizures despite multiple antiepileptics and sedation. CT, MRI and electroencephalogram (EEG) ruled out a diagnosis of MSE,

BACKGROUND: Reliable clinical phenotyping is crucial for diagnosis, treatment, and research in hyperkinetic movement disorders, yet its interrater reliability remains uncertain.
OBJECTIVES: To quantify interrater agreement in clinical phenotyping of hyperkinetic movement disorders among experts in a large, phenotypically diverse cohort.
METHODS: Within the NEMO project, 161 patients with hyperkinetic movement disorders underwent standardized video assessments. Each patient was independently classified by three international movement disorders experts in a structured, stepwise procedure. Expe

BACKGROUND: Absence of cortical somatosensory evoked potentials (SSEPs) in comatose survivors of cardiac arrest is a proven predictor of poor neurological outcome, yet this test is not widely available or used. The correlation between absent cortical SSEPs and other clinical, electrophysiologic, and electroencephalographic (EEG) markers in this population remains incompletely defined.
METHODS: We conducted a retrospective cohort study of comatose adult patients after cardiac arrest who underwent SSEPs and brain MRI, collecting clinical, neurologic, MRI, and EEG data. We classified MRI finding

BACKGROUND AND AIMS: The use of supraglottic airway devices (SADs) in paediatrics has considerably increased. Focusing haemodynamics at anaesthetic induction, the study evaluated change in mean arterial pressure (MAP) amongst propofol (P), etomidate (E), and etomidate-propofol combination(etofol, EP) groups at the attainment of bispectral index (BIS) 50 for SAD insertion.
METHODS: A total of 132 patients of either gender between 6 and 10 years of age were randomly allocated as groups P, E, and EP with n = 44/group. Induced with a group-specific anaesthetic agent at 50 mL/h infusion, SAD was i

BACKGROUND: Cryptococcal meningoencephalitis is a serious opportunistic infection, classically associated with advanced immunosuppression. The increasing use of immunomodulatory drugs, such as leflunomide, has broadened the at-risk population.
CASE REPORT: A 66-year-old woman with rheumatoid arthritis, on long-term leflunomide therapy, presented with progressive cognitive decline, behavioral changes, and myoclonus over a six-week period. The initial presentation mimicked rapidly progressive dementia. Cerebrospinal fluid analysis showed elevated opening pressure, hypoglycorrhachia, and hyperpr

BACKGROUND: Patients with chronic rhinosinusitis with nasal polyps (CRSwNP) and comorbid asthma have more severe disease and are difficult to treat. However, the molecular endotypes associated with CRSwNP with comorbid asthma (CRSwNP + AS) are not clear. This study aimed to investigate the characteristics of type 2 inflammation and the molecular signatures associated with CRSwNP + AS.
METHODS: A total of 195 subjects; including 65 CRSwNP + AS patients, 99 CRSwNP-alone patients, and 31 healthy control subjects; were enrolled in the study. Nasal tissues from patients with CRSwNP + AS, CRSwNP-al

A 78-year-old woman developed a coarse tremor in her left hand that had persisted for 2 months. 3 years before, she had undergone surgery for lung adenocarcinoma, followed by chemotherapy. On examination, there was a postural and action tremor (3-4 Hz) in the left upper limb, identified as a Holmes tremor, with oculopalatal myoclonus. MR scan of the brain showed bilateral hypertrophic olivary degeneration with hyperintensity of the middle cerebellar peduncles. CT scan of the head showed a calcified lesion in the pontine tegmentum, suggesting disruption of the Guillain-Mollaret triangle, which

STUDY OBJECTIVES: Respiratory event-related leg movements (RRLMs) in obstructive sleep apnea may reflect event severity and predict adverse outcomes. This study examines the association between RRLM frequency and mortality.
METHODS: A sample of 571 men (median [IQR] age: 76.0 [72.0-81.0] years; BMI: 28.0 [25.0-30.0] kg/m2) with an apnea-hypopnea index (AHI, ≥3 per cent desaturation) ≥10 events/h was selected based on available incident data and annotations of RRLMs, from the Osteoporotic Fractures in Men Studies . RRLMs were scored if they occurred within ±5 s of respiratory event termination
