Mechanoresponsive reprogramming of tumor-associated macrophages during cancer progression.

IMPORTANCE: Nocturnal sleep disruption develops in Alzheimer disease (AD) owing to the derangement of the sleep-wake cycle regulation pathways. Orexin contributes to the regulation of the sleep-wake cycle by increasing arousal levels and maintaining wakefulness. OBJECTIVES: To study cerebrospinal fluid levels of orexin in patients with AD, to evaluate the relationship of orexin cerebrospinal fluid levels with the degree of dementia and the cerebrospinal fluid AD biomarkers (tau proteins and β-amyloid 1-42), and to analyze potentially related sleep architecture changes measured by polysomnogra

Diabetes, Alzheimer's Disease Risk Factors, and the Cafeteria Diet: A Comprehensive Review.

Alzheimer's disease (AD) is a progressive neurodegenerative disorder with multifaceted risk factors, including diet and metabolic dysfunction. The rising prevalence of AD and diabetes has drawn attention to their shared pathophysiological mechanisms. The "cafeteria diet," characterized by high-fat, high-sugar, and energy-dense foods, has emerged as a significant contributor to metabolic dysfunctions, including obesity and insulin resistance, which are risk factors for both diabetes and neurodegenerative diseases. This study explores the effects of the cafeteria diet on cognitive impairment, AD

Ginkgolide as a Promising Multi-Target Therapeutic for Alzheimer's Disease: Targeting ApoE4 and Beyond.

INTRODUCTION: The progressive neurodegenerative disease known as Alzheimer's disease (AD) is typified by neuroinflammation, amyloid-beta buildup, and cognitive impairment. Current pharmacological treatments merely alleviate symptoms, despite extensive research, which underscores the need for innovative, multi-target medicines. Since apolipoprotein E4 (ApoE4) is a significant genetic risk factor linked to the development of AD, it is a potentially effective treatment target. With their neuroprotective qualities, natural substances like Ginkgolide may help treat some diseases. This study investi

Targeting JNK3: An In-silico Approach to Uncover Potential Therapeutics for Alzheimer's Disease.

INTRODUCTION: JNK3 is a specific isoform of c-Jun N-terminal kinase, mainly found in the brain, and is highly sensitive to stress-associated signals in the central nervous system. It has been reported that JNK3 plays a crucial role in neurite formation and cognition. During pathological states such as Alzheimer's disease, cerebral ischemia, Traumatic brain injury (TBI), Parkinson's disease, and epilepsy, it is found to be in a hyperactivated form. Hyperphosphorylation of amyloid precursor protein (APP) and tau leads to toxic Aβ42 and neurofibrillary tangles. Excess Aβ activates JNK3 signaling,

Estimating the time course of biomarker changes in Alzheimer's disease.

Recent advancements in biomarkers have transformed Alzheimer's disease (AD) diagnosis from being purely symptom-based to include biological criteria. With new treatments targeting the core biology of Alzheimer's disease, understanding the timeline of biological changes is crucial as the disease progresses over decades. Longitudinal data from amyloid-beta (Aβ) PET and cognitive tests [Mini-Mental State Examination (MMSE) and Alzheimer's Disease Assessment Scale-Cognitive Subscale (ADAS-cog)] from the Alzheimer's Disease Neuroimaging Initiative (n = 1448) and BioFINDER (n = 2088) were used to st

β-Amyloid PET Signal Reduction in Prior ARIA-E Regions after Antiamyloid Therapy for Alzheimer Disease.

BACKGROUND AND PURPOSE: The relationship between regional brain edema caused by antiamyloid monoclonal antibodies (ARIA-E) and the degree of regional beta-amyloid (Aβ) PET signal reduction is unknown. We aimed at determining this relationship. MATERIALS AND METHODS: In patients with moderate or severe ARIA-E, we quantified changes in Aβ PET signal before and after ARIA-E resolution, comparing regions affected by ARIA-E with unaffected regions. RESULTS: In 4 of 5 patients treated with lecanemab or donanemab and who had moderate or severe ARIA-E, Aβ PET signal decreased significantly more in r

Effect of protective mutation on structure and dynamics of APOE: a molecular dynamics simulation study.

Apolipoprotein E (APOE) plays a significant role in determining the risk of Alzheimer's disease (AD). Three mutations-APOE3-R136S, APOE3-V236E, and APOE4-R251G-have been reported to reduce the risk of AD. Unveiling the molecular mechanism behind this reduction could lay a foundation for developing therapeutics for AD. To shed light on this subject, we investigate the mutation-induced variation in structural and dynamic properties of APOE3-R136S, APOE3-V236E, and APOE4-R251G in explicit solvent using molecular dynamics simulations. The APOE2, APOE3, and APOE4 were used as the reference. The ana

Free-water imaging predicts cognitive decline and clinical progression in cognitively normal older adults.

The main objective of this study was to investigate the role of cortical free-water (cFW) imaging as a sensitive biomarker for tau-related microstructural injury and cognitive decline in preclinical Alzheimer's disease (AD). We aimed to determine whether cFW levels, derived from diffusion MRI, could predict cognitive decline and clinical progression independent of amyloid burden and other traditional imaging biomarkers. We analyzed data from 276 cognitively normal older adults from the Harvard Aging Brain Study (HABS). Participants underwent diffusion MRI, amyloid and tau PET imaging, and neur

Synergistic integration of clinical and multi-omics data for early MCI diagnosis using an attention-based graph fusion network.

BACKGROUND: Mild cognitive impairment (MCI), a precursor to Alzheimer's disease (AD), requires precise early diagnosis. Single-omics approaches often miss disease complexity, motivating integrative and interpretable solutions. NEW METHOD: We present the Attention-based Multimodal Graph Fusion Network (A-MGFN), which integrates clinical, genomic, epigenomic, and transcriptomic data via biologically curated features - Clinico-Genetic Risk Score (CGRS), Curated Epigenomic Signature (CES), and Differential Expression Signature (DES). Each modality is encoded by a modality-specific graph convoluti

In Vitro and In Vivo Evaluation of Small-Molecule Disassemblers of Pathological Tau Fibrils.

Aggregation of the microtubule-binding protein tau is the histopathological hallmark of Alzheimer's disease (AD) and other neurodegenerative diseases, which are collectively known as tauopathies. Tau aggregation in AD patients is correlated with neuron loss, brain atrophy, and cognitive decline, and pro-aggregation tau mutations are sufficient to cause neurodegeneration and dementia in humans and tauopathy model mice. Thus, reversing tau aggregation is a potential therapeutic avenue for AD. In a previous study, we discovered CNS-11, a small molecule that disaggregates AD patient brain-extracte

Current and Emerging Therapeutic Approaches for Vascular Cognitive Impairment and Dementia.

Cardiovascular risk factors contribute to the majority of dementia cases, with about 20% directly attributable to vascular cognitive impairment and dementia (VCID). VCID treatment developments have been slow compared with Alzheimer's disease (AD), which now has several FDA-approved symptom- and disease-modifying agents. In the second part of this JACC Seminar Series, advances and new perspectives on the management and prevention of VCID are reviewed. There is reasonable evidence that cognitive enhancers (donepezil, galantamine, and memantine) modestly improve cognition in vascular dementia (Va

Voxelwise correlation of neighbors as a hypothesis driven framework for characterizing white matter lesion heterogeneity.

White matter hyperintensities (WMH) on T2-weighted brain magnetic resonance imaging (MRI) are common in aging and associated with small vessel cerebrovascular disease. Standard segmentation methods treat these lesions as uniform binary entities, fundamentally reducing WMH signal by flattening a complex spectrum of tissue damage into a single label. Most WMH methods threshold voxel intensities to estimate lesion volume, missing richer characterization achievable by combining fluid-attenuated inversion recovery (FLAIR) with diffusion MRI. We introduce Voxel-wise Correlation of Neighbors (VCON),

Elovanoids: linking nutrition to neuroprotection.

PURPOSE OF REVIEW: Elovanoids are homeostatic lipid mediators derived from the very long-chain n-3 polyunsaturated fatty acids, which are in turn derived from docosahexaenoic acid (DHA). The aim of this review is to summarize the latest research on these lipid mediators. RECENT FINDINGS: Elovanoids beneficially modulate thioredoxin reductase 1, a key component in the activation of the cellular glutathione antioxidant system. Elovanoids prevent oligomeric amyloid-beta-induced senescence and inflammaging in retinal pigment epithelium and other cells. Rod cells' ability to use DHA to produce elo

Elevated fecal silver, lithium, and platinum in cognitive impairment: A pilot exploration of microbiota-metal interactions.

BACKGROUND: Gut microbiota (GMB) and metal exposure have both been implicated in cognitive impairment (CI), including amnestic mild cognitive impairment (aMCI) and Alzheimer's disease (AD). However, studies integrating these areas remain scarce. OBJECTIVE: This pilot study aimed to investigate whether exposure to metals modulates the relationship between GMB composition and clinical outcomes in individuals with CI. METHODS: Stool samples were collected from aMCI (n = 12), AD (n = 18), and cognitively healthy controls (HC, n = 30). Participants were categorized into CI (n = 30) and HC (n = 30

Cell type- and neuronal differentiation-dependent MME 5'UTR splice variants with distinct translational outputs in human cells.

Neprilysin (NEP) is a major enzyme that degrades amyloid β-peptide (Aβ) in the brain. Deficient NEP activity causes Aβ to accumulate, leading to amyloid pathology in Alzheimer's disease. Transcripts from the MME gene encoding NEP comprise seven splice variants (v1, v1bis, v2a, v2b and v3-5) with different structures in the 5'-untranslated region. We analyzed the expression levels of MME variants in neuronal and non-neuronal cells using real-time PCR and investigated their translational outputs using cells co-transfected with cDNAs of each MME variant and green fluorescent protein. v1 and v1bis

Metabolism fluctuation coupling can track the progression of dementia and describe MST1 gene-related pathology.

BACKGROUND: Cognitive resilience refers to an individual's capacity to cope with brain aging and pathology and to delay cognitive decline, whereas existing techniques such as functional magnetic resonance imaging capture only macroscopic features without linking them to neurophysiological mechanisms. Recent studies have shown that overexpression of the MST1 gene exacerbates Alzheimer's disease phenotypes by affecting neuronal activity and metabolism; however, its association with cognitive trajectories and imaging biomarkers remains to be further investigated. METHODS: Multimodal imaging data
