Protein Misfolding Enteropathy Predicts and Prognosticates Neurodegenerative Disease Years Before Symptom Onset.

BACKGROUND & AIMS: Neurodegenerative disorders are characterized by progressive, irreversible neuronal loss that often advances silently for years before symptoms appear. Disease-modifying therapies are generally less effective once symptoms emerge, as substantial neuronal damage has already occurred. Consequently, there is an urgent need for accessible biomarkers that can predict disease well in advance and serve as reliable target-engagement measures in prevention trials. METHODS: We analysed archival gastrointestinal (GI) biopsies from 196 individuals with unexplained GI symptoms and 13-15

Cardiometabolic diseases and risk of early-onset dementia: a population-based case-control study in Northern Italy.

BACKGROUND AND AIMS: Early-onset dementia (EOD), defined by symptom onset before 65 years, has an uncertain etiology. Cardiovascular and metabolic diseases have been suggested to increase risk for EOD, but evidence remains limited and inconsistent. We investigated the extent to which history of cardiometabolic conditions is associated with EOD, and the extent to which the associations vary by sex, age at onset, and dementia subtype. METHODS AND RESULTS: We conducted a population-based case-control study in Modena, Northern Italy, including 334 EOD cases diagnosed from 1999 to 2021, and 1991 c

Analysis of the Association Between Psychosocial Stressors and Depression Among Family Caregivers of Individuals With Young-Onset Dementia and Late-Onset Dementia: A Cross-Sectional Study.

BACKGROUND: Previous research on caregiver depression has mainly focused on caregivers of late-onset dementia (LOD), whereas studies involving young-onset dementia (YOD) remain limited. This study aimed to examine the associations between psychosocial stressors and depression among YOD and LOD caregivers in Japan. METHODS: A total of 1798 questionnaires were distributed to family caregivers affiliated with 214 organisations, including dementia family associations and care facilities. Depressive symptoms were assessed using the Center for Epidemiologic Studies Depression Scale (CES-D), and the

Variants in the proteasome regulator PSMF1 cause a phenotypic spectrum from parkinsonism to perinatal lethality.

Dissecting biological pathways highlighted by Mendelian gene discovery has provided critical insights into the pathogenesis of Parkinson's disease (PD) and neurodegeneration. This approach ultimately catalyzes the identification of potential biomarkers and therapeutic targets. Here we identify PSMF1 as a gene implicated in parkinsonism and childhood neurodegeneration. We find that biallelic PSMF1 missense and loss-of-function variants co-segregate with phenotypes from early-onset PD to perinatal lethality with neurological manifestations across 18 pedigrees with 25 affected subjects, showing c

Genome-wide association study of copy number variations in Parkinson's disease.

We investigated the role of copy number variations (CNVs) in Parkinson's disease (PD) using genotyping data from 10,815 patients (2731 early-onset PD, EOPD) and 8901 controls from the COURAGE-PD consortium. CNVs were analyzed using a sliding window genome-wide association and burden approach. No genome-wide significant CNVs were detected in the overall cohort, but a robust deletion spanning exons 2-6 of PRKN was identified in EOPD cases, validated by MLPA, and replicated in the GP2 dataset (23,089 cases, 18,824 controls). CNV burden was significantly enriched in PD-related genes, primarily dri

Genotype and Age at Onset Drive Vermis Atrophy in CACNA1A- and GAA-FGF14-related Ataxias.

CACNA1A- and GAA-FGF14-related channelopathies are among the most frequent genetic etiologies of cerebellar ataxia. They display overlapping features consisting of both chronic and episodic neurological symptoms and usually show only a mild cerebellar atrophy in imaging studies, with prevalent involvement of the vermis. To evaluate cerebellar volumetry as a biomarker for distinguishing between these disorders and for understanding their neuroanatomical correlates. We applied a deep learning method (CerebNet) for the lobular segmentation and volumetry assessment of the cerebellum on 3.0 Tesla M

Lifestyle intervention and cognitive outcomes in Down syndrome: a horizon 21 European Down syndrome consortium scoping review.

BACKGROUND: Life expectancy for individuals with Down syndrome (DS) has significantly increased, primarily due to medical advances. While DS is considered a genetically determined form of Alzheimer’s disease (DS-AD), with neuropathological markers evident by age 40, the onset of clinical dementia varies. Modifiable risk factors are thought to contribute meaningfully to dementia risk in the general population. Advances in intervention studies in the general population suggest cognitive decline can be reduced through multimodal lifestyle interventions, however no large-scale multimodal studies h

Tau oligomerization induces nuclear lamina invagination and chromatin remodeling in Alzheimer's disease.

The aggregation of the microtubule-associated protein tau into oligomeric complexes is strongly correlated with the onset and progression of neurodegeneration in Alzheimer's disease (AD). Increasing evidence implicates nuclear membrane disruption in AD and related tauopathies; however, whether this is a cause or consequence of neurodegeneration remains unresolved. Here, we show that nuclear lamina disruption emerges at the early Braak stages, coinciding with the initial formation of pathological tau aggregates in post-mortem AD brain tissue. Using the tauopathy mouse model (P301S PS19), we dem

Plasma biomarker trajectories across Alzheimer's clinical onset: a 17-year prospective study.

BACKGROUND: Long-term trajectories of plasma biomarkers relative to clinical symptom onset in Alzheimer’s disease (AD) remain scarce. METHODS: We analyzed 17-year longitudinal data from 195 initially cognitively unimpaired participants in the Biomarkers of Cognitive Decline Among Normal Individuals (BIOCARD) study. Plasma biomarker trajectories were modeled as a function of years from estimated mild cognitive impairment (MCI) symptom onset using generalized additive mixed models. RESULTS: In participants who progressed to amyloid-positive MCI, plasma p-tau181/Aβ42 and GFAP diverged from stable

Mechanistic research on the vestibular-hippocampal pathway in neurodegenerative diseases: an integrative perspective from molecular to behavioral levels.

This paper systematically reviews the pivotal role and bidirectional regulatory mechanisms of the Vestibular-hippocampal pathway in the onset and progression of neurodegenerative diseases (such as Alzheimer's disease), focusing on the common comorbidity of vestibular dysfunction and cognitive decline. Evidence spanning molecular to behavioral levels indicates that vestibular signal loss can induce hippocampal atrophy and spatial memory impairment through neuroinflammation, impaired synaptic plasticity, and disrupted theta rhythms. Conversely, hippocampal degeneration further impairs vestibular

Research progress in neuroimaging of sporadic early-onset Alzheimer's disease.

Early-onset Alzheimer's disease (EOAD) is defined as Alzheimer's disease (AD) with an age at onset younger than 65 years, accounting for approximately 5% of all AD cases. More than 90% of EOAD cases do not carry autosomal dominant pathogenic mutations. Although its prevalence is lower than that of late-onset Alzheimer's disease (LOAD), EOAD follows a more aggressive clinical course. A subset of EOAD patients present with non-amnestic variant phenotypes, including logopenic variant of primary progressive aphasia (lvPPA), frontal variant Alzheimer's disease (fvAD), posterior cortical atrophy (PC

Distinct Multimodal Imaging Correlates of Depression in Middle-Aged Adults With and Without a Family History of Alzheimer's Disease.

BACKGROUND: Depression is associated with risk for late-onset Alzheimer's disease (LOAD), but its underlying pathogenesis in at-risk individuals remains unclear. We examined multimodal imaging correlates of depressive symptoms in cognitively normal middle-aged offspring of patients with LOAD (O-LOAD) compared with control individuals without LOAD history up to a 4 th degree of kinship (HC). METHODS: Participants (n=58; 52±3 years; 74% female) underwent assessment with the Beck Depression Inventory-II (BDI), structural MRI, resting-state fMRI, FDG-PET, and PiB-PET. Resting-state fMRI data were

The forgotten population: Early-onset Alzheimer's in people with Down syndrome.

In recent decades, Alzheimer's disease has increasingly been discussed through narratives of prevention, resilience, and delayed onset. For individuals with Down syndrome, however, Alzheimer's disease is not a distant possibility, but an almost predictable consequence of increased longevity. The substantial rise in life expectancy in this population represents one of the greatest achievements of modern medicine. At the same time, it has exposed a profound biological vulnerability, as most individuals with Down syndrome will develop Alzheimer's disease-related neuropathology decades earlier tha

Mitochondrial-nuclear crosstalk: A central axis in Alzheimer's disease.

Alzheimer's disease (AD) is a progressive neurodegenerative disorder traditionally defined by amyloid-β plaques and tau tangles. However, growing evidence indicates that deeper disruptions in cellular homeostasis contribute to disease onset and progression. Among these, impaired communication between mitochondria and the nucleus has emerged as a central yet underrecognized pathological feature. Mitochondrial-nuclear (mito-nuclear) crosstalk regulates energy metabolism, stress responses, and survival pathways, making it a critical determinant of brain aging and AD vulnerability. Despite its imp

Alzheimer's disease-like brain pattern biomarker: capturing risks and predicting disease onset.

Preventing Alzheimer's disease (AD) requires early-warning biomarkers. We developed a Regional Vulnerability Index (RVI) that quantifies individual brain similarity to AD patients' expected brain deficit patterns. We calculated regional effect sizes to establish brain deficit patterns in amyloid-positive AD cases compared to amyloid-negative healthy controls. RVI-AD was calculated as a linear index of individual similarity to this established brain pattern in AD. We demonstrated RVI-AD elevation associated with risk factors in 335 participants (mean age: 49 ± 13 years) in the Amish Connectome

Women's reproductive life patterns, intrinsic capacity, and the risk of all-cause and cause-specific dementia: a prospective cohort study.

BACKGROUND: Few studies comprehensively examined women's life-course reproductive patterns and the risk of dementia. This study aims to examine the association between women's reproductive life sequence and dementia, and to explore the potential role of intrinsic capacity (IC) on such association. METHODS: This study used data of 153,909 women who were post-menopause and free of dementia at baseline from the UK Biobank. We conducted sequence analysis and cluster analysis to identify women's life-course reproductive sequence and its potential patterns based on self-reported single reproductive

Oropharyngeal dysphagia and amyloid beta pathology in the TgF344-AD rat model of Alzheimer's disease.

INTRODUCTION: Dysphagia is a major consequence of Alzheimer's disease (AD) that is understudied and undertreated. Neuropathology in AD occurs early in the disease progression, but little is known about pathologies underlying functional swallowing changes; this knowledge gap is a barrier to developing effective treatment. We hypothesized that an established AD rat model (TgF344-AD) would demonstrate significant deficits in oromotor/swallowing function versus Wild Type (WT) with corresponding amyloid beta pathology in brain structures critical to swallowing. METHODS: Nine male TgF344-AD and 6 W

[Alzheimer's disease: Diagnostic criteria and their evolution].

Studies of diseases associated with cognitive impairment have been undertaken since ancient times, at the dawn of medical science. Initially, the main focus was on the connection between higher cortical function disorders and age-related changes. This approach has been used for many centuries. In the 19th century, as neurology, anatomy, morphology, and other disciplines developed, it became clear that dementia is associated with specific changes in brain structure. At the beginning of the 20th century, Alzheimer's disease was recognized as a distinct nosological entity, with descriptions of it

[Myoclonus in Alzheimer's disease].

Neurodegenerative diseases are often presented with myoclonus, sudden, short-term, involuntary muscle contractions («flinching»). According to the literature, myoclonus in Alzheimer's disease (AD) can occur up to 40-50% in cases of disease with early onset and in atypical forms. Importantly, epileptic myoclonus may indicate a risk of seizures with loss of consciousness, convulsions, and epileptiform activity, which may lead to more rapid progression of cognitive deficits. Diagnosis of myoclonus is essential for timely treatment and improving the quality of life of AD patients. The paper descri
