Glutamate (Glu), although the main excitatory neurotransmitter (NT) of the central nervous system (CNS), evidence recognizes its role in influencing immune cell function and activation, including cytokine production, cell proliferation, migration, and survival. Importantly, dysregulation of Glu homeostasis has been implicated in the pathogenesis of Alzheimer's disease (AD), a neurodegenerative disorder characterized by progressive cognitive decline, neuronal loss and neuroinflammation. This review synthesizes current evidence on the biochemistry of the glutamatergic synapse, as well as the exp

INTRODUCTION: Alzheimer's Disease and Alzheimer's Disease Related Dementias (AD/ADRD) are among the costliest diseases worldwide, with most expenses borne by persons living with dementia (PLWD) and their care partners. For lesbian, gay, bisexual, transgender, queer, and other sexual and gender minorities (LGBTQ+), who often have lower incomes and less savings at baseline, dementia care costs can be financially devastating. Despite rising healthcare costs, no current measure captures dementia-related financial hardship. This study explored how LGBTQ+ and non-LGBTQ+ care dyads experienced AD/ADR

Natural killer T (NKT) cells are innate-like T lymphocytes that express natural killer (NK) cell receptors (NKRs), which recognize glycolipid antigens presented by the MHC class I-like protein CD1d, linking innate and adaptive immunity. NKT cells include two main subsets, type I and type II, which differ in TCR usage and antigen specificity. In addition, NKT-like cells, which express NKRs but are CD1d-independent, have attracted growing interest in neuroimmunology. These NKR-expressing T cells play important roles in either promoting or suppressing pathological conditions within the brain. Thi

Purine metabolism dysregulation has been implicated in Alzheimer's disease (AD), but its temporal dynamics and mechanistic links to core pathologies remain unclear. This translational study integrated a longitudinal preclinical AD mouse model and clinical cohort investigations to define the role of purine metabolism in AD progression. In APPswe/PSEN1dE9 (APP/PS1) mice, serum-untargeted metabolomics identified purine metabolism as the most notably altered pathway, with metabolite levels (e.g., adenine, adenosine) shifting dynamically across disease stages alongside increasing amyloid-β (Aβ) bur

In recent years, the notion of Tourettic obsessive-compulsive disorder (Tourettic OCD), viewed as a distinctive endophenotype of Gilles de la Tourette syndrome, has garnered increasing attention. Despite this growing interest, clinical observations of affected patients remain sparse in the literature. This article presents the clinical case of a teenager diagnosed with Gilles de la Tourette syndrome, whose condition evolved to domination of obsessive-compulsive and behavioral disorders over the previously dominant multiple motor and vocal tics. The question of the nosological independence of T

BACKGROUND: In addition to the well-established neurogenic niche of the ventricular-subventricular zone (V-SVZ) of the lateral ventricles, neural stem/progenitor cells (NPCs) have been identified in the periventricular regions (PVRs) throughout the adult mouse ventricular system. The potential role of these NPCs in neuroplasticity or neurorestoration, however, remains largely unexplored.
OBJECTIVE: Given that vascular endothelial growth factor A (VEGF-A) has been shown to promote stem cell proliferation in the V-SVZ, this study aimed to investigate VEGF-A effects on NPC proliferation in mid-

Brain inflammation is a key feature of frontotemporal dementia (FTD). Positron emission tomography (PET) of the 18 kDa translocator protein (TSPO) is widely used as an in vivo proxy for neuroinflammation, but whether the elevated signal reflects microglial, astrocytic or vascular pathology remains controversial, and this has not previously been resolved in FTD using matched ante mortem imaging and post-mortem tissue. We paired ante mortem [11C]PK11195 TSPO-PET with quantitative post-mortem neuropathology in 10 individuals with FTD (5 FTLD-tau, 5 FTLD-TDP) and 5 age- and sex-matched controls, c

BACKGROUND: Alzheimer's disease (AD) polygenic risk scores (PRS) reflect cumulative genome-wide genetic risk but lack cell-type specificity and functional context.
METHODS: We examined the relationships between cell-type-specific ADPRS, domain-specific cognitive performance, and AD or amnestic mild cognitive impairment (aMCI) prevalence among elders. Secondary analyses evaluated additive and multiplicative interactions between ADPRS and adherence to Mediterranean diet (MedDiet), a known AD modifiable risk factor. Cross-sectional analyses were performed in a community cohort of 1,172 older ind

Parkinson's disease is increasingly recognized as a disorder that extends beyond neurons. In this issue of JEM, Stevenson et al. (https://doi.org/10.1084/jem.20251245) highlight the functional consequence of α-synuclein aggregates at the brain border, uncovering their active role in reshaping lymphatic integrity and the neuroimmune macroenvironment.

Neuromelanin-sensitive magnetic resonance imaging (NM-MRI) enables in vivo assessment of locus coeruleus (LC) integrity, but findings in Parkinson's disease (PD) vary across cohorts and methods. We searched PubMed/MEDLINE, Embase, Web of Science Core Collection and Scopus from inception to 5 September 2026 and pooled independent PD-healthy control (HC) comparisons using random-effects models. Sixty-five reports were included, of which 17 contributed to at least one main pooled outcome. The primary LC-signal analysis included 14 independent cohorts (1,078 participants with PD and 543 HCs) and s

Alzheimer's Disease (AD) is marked by the accumulation of amyloid-beta (Aβ) plaques, neurofibrillary tangles, and neuroinflammation. Cannabidiol (CBD), the major non-psychoactive compound of Cannabis, has demonstrated significant pharmacological potential for the treatment of AD. CBD is known to act on various receptors, including cannabinoid receptor 1 (CB1R) and 2 (CB2R), and peroxisome proliferator activated receptor γ (PPARγ). However, the precise changes in receptor expression in AD, as well as following CBD treatment, remain unclear and inconsistent in literature. To address this, we per

Parkinson's disease (PD) is characterized by progressive nigrostriatal dopaminergic degeneration and a dysregulated neuroinflammatory environment. Here, we investigated whether intranigral delivery of a microRNA‑204‑5p mimic (miR‑204-5p) at the time of lesion modulates dopaminergic‑dependent rotational asymmetry, glial morphology, and striatal medium spiny neurons (MSNs) recruitment in a unilateral 6‑OHDA mouse model. Adult C57BL/6 mice received a unilateral striatal 6‑OHDA injection and simultaneous intranigral infusion of a synthetic miR‑204‑5p mimic or vehicle, and apomorphine‑induced rotat

Spinocerebellar ataxia type 27B (SCA27B) is a recently described autosomal dominant ataxia caused by a pathogenic GAA repeat expansion in the FGF14 gene. The frequency and the distribution of FGF14 repeat alleles remain largely unexplored in underrepresented populations, particularly in Latin America. To investigate the frequency of SCA27B and characterize the allelic distribution of the GAA-FGF14 microsatellite locus in a Peruvian cohort of individuals with ataxia of unknown molecular etiology. We analysed a cohort of affected individuals evaluated at a referral center for neurogenetic disord

BACKGROUND: Whether donepezil exerts long-term efficacy in mitigating cognitive decline, preserving brain structure, and restoring impaired structural covariance networks in patients with mild cognitive impairment due to Alzheimer's disease (AD-MCI) remains controversial.
OBJECTIVE: This study aimed to characterize the longitudinal dynamic alterations in cognition, brain morphology, and structural covariance network topology in AD-MCI patients receiving continuous 36-month donepezil treatment, thereby elucidating the long-term modulatory effects of donepezil on AD progression.
METHODS: Cross

Alzheimer's disease (AD) is an irreversible and chronic neurodegenerative disease, posing a growing global health burden, necessitating the development of multitarget-directed ligands (MTDLs) to combat its complex, multifactorial pathology. In this research, a new series of β-naphthol-based dithiocarbamate derivatives (5a-5g) were designed, synthesized using a rational drug design approach, and their structures were elucidated using FTIR, UV, and NMR spectra analysis. Reportedly, compound 5d showed characteristic C-N, C = S, and C-S vibrations in FTIR, while distinct aromatic and piperidinyl p

The abnormal aggregation of amyloid-beta (Aβ) peptides is a core driver of Alzheimer's disease (AD). Developing central nervous system (CNS)-penetrant small-molecule Aβ inhibitors offers a complementary strategy to overcome the high costs and severe side effects of antibodies. Lanosterol has demonstrated excellent potential in inhibiting abnormal protein aggregation. However, poor physicochemical and pharmacokinetic profiles strictly hinder its direct therapeutic application. Herein, rational modifications on lanosterol were conducted to modulate the hydrophilic-lipophilic balance around the 3

Neurological disorders represent one of the leading causes of disability and mortality worldwide, and their prevalence is expected to increase substantially with population aging. Despite major advances in neuroscience, effective disease-modifying therapies remain unavailable for most chronic disorders of the central nervous system (CNS), highlighting the need for novel therapeutic strategies. Enzymes are attractive drug targets because they regulate fundamental processes including neurotransmission, signal transduction, energy metabolism, protein homeostasis, epigenetic regulation, lipid meta

Alzheimer's disease (AD) is multifactorial, prompting multi-target therapy. A series of donepezil-based derivatives were designed, synthesized, and evaluated as dual inhibitors of human butyrylcholinesterase (hBuChE) and fatty acid amide hydrolase (hFAAH). Among them, lead G15 displayed potent and balanced inhibition with IC50 values of 2.44 μM and 0.17 μM, respectively, and favorable selectivity over carboxylesterases CES1/2 (SI 14.4 and 8.4). G15 not only acted as a time-dependent, mixed-type hFAAH inhibitor and a pseudo-irreversible hBuChE inhibitor, but also exhibited good drug-like proper

IMPORTANCE: Walking speed has been shown to be associated with mortality and is conventionally obtained from short walking tests in clinical settings. Free-living walking parameters such as walking cadence can be derived from accelerometers.
OBJECTIVE: To examine whether walking parameters measured in clinical and free-living settings are associated with comparable discriminative ability for all-cause mortality risk.
DESIGN, SETTING, AND PARTICIPANTS: This population-based cohort study, the Whitehall II accelerometer substudy in the UK, included a baseline from January 2012 to December 2013

There is little causal evidence on factors that can protect individuals against Alzheimer's Disease and Related Dementias (ADRD) risk. We study the causal effect of education on ADRD, exploiting a regression discontinuity generated by a compulsory schooling reform. ADRD was ascertained based on medical history, hospital records, and death registries, addressing concerns about selective sample attrition. We find that education reduces incidence of ADRD. Using molecular genetic data, we show that the reform weakened the relationship between genetics and ADRD incidence, implying genetic risk of d
