Impaired Dynamic Cerebral Autoregulation Across the REM Sleep Behavior Disorder Spectrum: Evidence From Isolated REM Sleep Without Atonia to Parkinson's Disease With RBD.

OBJECTIVES: Rapid eye movement (REM) sleep behavior disorder (RBD) is a parasomnia characterized by REM sleep without atonia (RSWA) and dream-enactment behaviors and is recognized as a prodromal manifestation of α-synucleinopathies. Isolated RSWA (iRSWA) has been proposed as a potential early stage within the RBD spectrum, although its clinical and physiological significance remains incompletely understood. This study aimed to investigate alterations in sleep architecture, phasic muscle activity, and dynamic cerebral autoregulation (dCA) across the spectrum from iRSWA to isolated RBD (iRBD) an

Correlation Between Cognitive Function and Serum Irisin Levels in Parkinson's Disease Patients.

INTRODUCTION: Cognitive impairment is a common non-motor symptom of Parkinson's disease (PD) that markedly reduces patients' quality of life. Given the clinical heterogeneity of PD-related cognitive impairment (PDCI), further clarification of clinical and circulating biological factors associated with cognitive status may help improve our understanding of cognitive dysfunction in PD. METHODS: In this cross-sectional study, 243 patients with PD and 90 age- and sex-matched healthy controls (HCs) were enrolled. Cognitive function was evaluated using the Montreal Cognitive Assessment (MoCA), and

Association Between Exercise and Brain-Derived Neurotrophic Factor (BDNF) Levels in Diabetes Mellitus: A Systematic Review and Meta-Analysis.

BACKGROUND: Diabetes mellitus (DM) is a metabolic disorder characterized by a consistently elevated blood sugar level. DM poses a major risk factor for many neurodegenerative diseases, causing a decline in cognitive function. BDNF (brain-derived neurotrophic factor) is a neurotrophin in the central and peripheral nervous systems that plays a crucial role in the development of brain circuits. Hence, disturbance in levels of BDNF is linked to the occurrence of neurodegenerative diseases and cognitive decline in diabetic patients. Physical exercise has emerged as a nonpharmacological intervention

Substrate Mimicry by Inhibitors of the Mycobacterium tuberculosis 20S Proteasome and Their Activity against Mycobacteria In Vitro and in Macrophages.

The remarkable and problematic capacity of Mycobacterium tuberculosis to persist in the host is dependent on a functional 20S proteasome that is structurally and functionally analogous to its mammalian counterpart. It has been proposed that this enzyme is critical for the bacterium's adaptive response to a variety of stressors in the host. Here, we report the structure of a rationally designed and species-selective inhibitor of the Mtb 20S proteasome in complex with the enzyme. The inhibitor was inspired by the syringolin natural products-covalent proteasome inhibitors that mimic a substrate.

Association Between Plasma N-Terminal Tau, Cognitive Decline, and Neurodegeneration in the ADNI Cohort.

BACKGROUND AND OBJECTIVES: Blood-based biomarkers offer a cost-effective, minimally invasive alternative to neuroimaging and CSF biomarkers. Newly developed fluid biomarkers, including the N-terminal tau fragment (NT1), show promise for identifying individuals at risk for Alzheimer disease (AD) and for monitoring disease progression. Here, we determine whether NT1 levels are associated with key clinical and biomarker measures in a large, well-characterized cohort. METHODS: Participants from the Alzheimer's Disease Neuroimaging Initiative (ADNI) cohort with available plasma NT1, β-amyloid (Aβ)

18β-Glycyrrhetinic Acid-Loaded ROS-Responsive Liposomes Enhance the Neuroprotective Efficacy against Cerebral Ischemic Stroke in Mice via a Microenvironment-Responsive Strategy.

18β-Glycyrrhetinic acid (GA) exhibits excellent anti-inflammatory and antioxidant activities, and has attracted considerable attention in the field of cerebral ischemia-reperfusion (I/R) injury therapy. However, it is plagued by problems of low bioavailability and dose-dependent toxicity. As a drug delivery platform for the treatment of ischemic stroke, microenvironment-responsive liposomes can cross the impaired blood-brain barrier, efficiently deliver drugs to the lesion sites, and improve therapeutic efficacy. Herein, to address the challenges posed by the highly oxidative-stress microenvir

Beyond inherited repeat length: somatic expansions in Huntington disease.

Huntington disease (HD) is a rare, genetic, autosomal dominant neurodegenerative disease, caused by an unstable CAG repeat expansion in the HTT gene. Over the past decade, somatic instability has become a major focus of investigation. Large genome-wide association studies together with work on post-mortem tissue, animal and cellular models, have brought this mechanism to the forefront, linking it to age at onset, disease severity and progression, reshaping our understanding of HD as a dynamic DNA-driven process over the lifetime, and highlighting it as a potentially actionable target. However,

When brain networks crash: connectivity, slow-wave activity and neural replay in Alzheimer's disease.

Brain connectivity disruption is a hallmark of neurodegenerative disorders, including Alzheimer's disease (AD), yet how network breakdown gives rise to cognitive deficits remains unclear. Here, we review the evidence bearing on three interconnected processes that have been proposed to link molecular pathology to cognitive decline. First, we summarize evidence that misfolded proteins propagate along brain connectivity pathways, making the connectome the biological substrate for disease progression. Second, we discuss the divergent trajectory reported across the AD continuum, an initial compensa

AlphaFold-driven structure-based drug discovery: Capabilities, benchmarks and limitations for medicinal chemistry.

Structure-based drug discovery (SBDD) has become an indispensable tool of medicinal chemistry due to the possibility of rational drug design based on the three-dimensional structure of the biological target. Nevertheless, the use of SBDD technology has been restricted due to the scarcity of known experimentally solved structures of the proteins that are the targets of drug action, especially those which are difficult to characterize, poorly understood and associated with diseases. Artificial Intelligence (AI)-based protein structure prediction technologies, such as AlphaFold, have substantiall

Integration of hydrogen peroxide sensing and detoxification by the C. elegans nervous system.

Hydrogen peroxide (H2O2) is the most common reactive chemical threat organisms face. Here, we map the neural circuit that drives chemotactic escape from environmental H2O2 in the nematode C. elegans. Twenty-four neuron classes with sensory endings at the mouth and nose respond to H2O2, with dynamics encoding stimulus intensity and exposure history. Sensing begins when H2O2 oxidizes the peroxidatic and resolving cysteines of the cytosolic peroxiredoxin PRDX-2, which relays this oxidative signal to cysteines on the LITE-1 and GUR-3 ion channels, triggering calcium influx in sensory neurons that

Rubicon modulates neuroimmune responses following traumatic brain injury.

Traumatic brain injury (TBI) elicits robust neuroinflammation and oxidative stress, coupled with an acute inhibition of macro-autophagy (autophagy) in neurons and microglia. Rubicon (Rubcn), a Beclin1 interacting protein that suppresses autophagy and mediates LC3-associated phagocytosis and endocytosis (LAP/LANDO), influences inflammatory signaling in metabolic, neurodegenerative, and inflammaging diseases; yet its role in acquired brain injury has not been examined. Using a controlled cortical impact model, we investigated the role of Rubicon in acute neuroinflammatory responses following inj

Coexistence of anhedonia and sensation-seeking with context-dependent gut dysbiosis in a prodromal transgenic mouse model of Parkinson's disease.

Accumulating evidence indicates that gut microbiota plays a role in the progression of Parkinson's disease (PD). Microbiota-based treatments for prodromal PD, which is accompanied by a wide range of neuropsychiatric symptoms, are being considered a potential early intervention but are still under-investigated. This study examined the association between symptoms of affective and motivational impairments and gut dysbiosis in a transgenic mouse model (line G2-3) that overexpresses A53T mutant α-synuclein, a key protein involved in the disease pathology. By introducing biologically meaningful var

SOFEx: A Self-Organized Feature Extraction Approach for Detecting Mild Cognitive Impairment and Alzheimer's Disease using EEG Datasets.

Early differentiation of mild cognitive impairment (MCI) from Alzheimer's disease (AD) remains difficult because diagnosis integrates cognitive, imaging, laboratory, and neurological evidence. EEG-based models may provide complementary information, but fixed descriptors can miss signal-specific structure. SOFEx was developed as an adaptive and interpretable EEG feature-engineering framework. The analysis used 1,905 EEG segments from 451 recordings of 168 participants (59 AD, seven MCI, and 102 controls). SOFEx combined self-organized rose pattern extraction, fold-wise feature selection, channe

Real-world safety and efficacy of lecanemab in the treatment of Alzheimer's disease: a systematic review and meta-analysis.

BACKGROUND: Despite the approval of lecanemab for Alzheimer's disease (AD), evidence regarding its performance outside controlled trial settings remains scarce, leaving its real-world safety and clinical utility largely unknown. METHODS: We searched PubMed, Embase, and Web of Science (through October 1, 2026) for real-world studies of lecanemab in AD. Outcomes included ARIA, discontinuation, infusion-related reactions, serious adverse events, MMSE, MoCA, CDR-SB, and amyloid PET. Random-effects models with REML and Hartung-Knapp adjustment were used. Subgroup analyses were stratified by region

Family and community care for older persons with Parkinson's disease in Kenya: a qualitative, ethnographic study.

BACKGROUND: Informal family care remains central to care for older adults in Kenya, yet evidence hints towards an 'erosion' of large, multi-generational family care networks and a conditionality on reciprocity across many African countries. The increasing incidence and prevalence of chronic conditions associated with later life have made care needs more complex and resource intensive. Parkinson's is one such example of a progressive condition characterised by a host of motor and non-motor symptoms, which results in increasing disability over time and, therefore, care requirements and resources

Whole-transcriptome sequencing reveals Alzheimer's disease-associated ceRNA regulatory networks and downstream pathways.

OBJECTIVE: Alzheimer's disease (AD) molecular mechanisms remain unclear, and effective therapies are lacking. This study employed whole-transcriptome sequencing to investigate the expression profiles of circRNAs, lncRNAs, miRNAs, and mRNAs in the hippocampus of APP/PS1 transgenic mice, aiming to identify ceRNA regulatory networks and clarify their roles in AD pathogenesis. METHODS: Differentially expressed RNAs were integrated into ceRNA networks, and programmed cell death-related pathways were analyzed using GO and KEGG. The computationally predicted circRNA_13083/miR-298-5p/CD14 axis was pr

Cross-species molecular stratification enables T cell receptor diagnosis of Parkinson's disease.

BACKGROUND: Developing peripheral blood-based diagnostic models for idiopathic Parkinson's disease (iPD), particularly those leveraging the T-cell receptor (TCR) repertoire, has long been considered infeasible because patient-derived TCRs appear to lack convergent sequence motifs. We reasoned that this apparent absence of shared TCR features likely reflects both insufficient sample sizes and unaccounted immune heterogeneity within the iPD population. METHODS: We reconstructed TCR repertoires from the two largest iPD cohorts currently available, anchoring them with three mechanistically distin

Preclinical models of Wilson's disease: From pathogenesis to clinical translation.

Wilson's disease (WD) is an autosomal recessive disorder of copper homeostasis caused by pathogenic ATP7B variants, leading predominantly to hepatic and neurological injury. Extensive heterogeneity in genotype, age at onset, clinical presentation, and treatment response complicates both diagnosis and therapeutic development, highlighting the need for reliable experimental models that reproduce distinct features of WD pathogenesis. This review critically evaluates animal models, conventional cell systems, induced pluripotent stem cell models, and organoids of WD, with an emphasis on the phenoty
