Long-term neuropsychiatric outcomes in offspring of mothers with multiple sclerosis and neuromyelitis optica spectrum disorder: A nationwide cohort study.

BACKGROUND: Multiple sclerosis (MS) and neuromyelitis optica spectrum disorder (NMOSD) predominantly affect women of childbearing age, yet the impact of maternal disease status on long-term offspring outcomes remains unclear. METHODS: Using the Korean National Health Insurance Service database (2005-2019), we conducted a nationwide cohort study. Live-born offspring of mothers diagnosed with MS or NMOSD before childbirth were identified and matched 1:4 to controls using propensity scores. Neuropsychiatric outcomes were defined using ICD-10 codes. RESULTS: Among 3,524,987 live births, 257 were

Thalamic quantitative susceptibility mapping and susceptibility source separation: a critical narrative review of evidence and interpretation.

Quantitative susceptibility mapping (QSM) measures net tissue magnetic susceptibility, which can conceal coexisting contributions of opposite sign. This is relevant to the thalamus, where nuclei differ in cellular composition, myelinated fibers, and susceptibility contrast. This narrative review evaluates what QSM and magnetic susceptibility source separation, including χ-separation, currently establish about thalamic alterations in aging and neurological disease. The synthesis prioritizes human thalamic evidence and distinguishes it from methodological validation, findings outside the thalamu

Associations between month/season of birth and health outcomes: an umbrella review of systematic reviews and meta-analyses.

BACKGROUND: The relationship between month or season of birth and long-term health outcomes remains an important but underexplored topic in public health. Evidence suggests that birth timing may influence disease susceptibility and broader health trajectories, reflecting interactions between biological and ecological factors. Clarifying these associations is essential for advancing prevention and a holistic view of health shaped by temporal exposures. METHODS: We conducted an umbrella review of systematic reviews and meta-analyses indexed in PubMed, Embase, and Web of Science up to 2 July 202

Fatigue across physical and mental health conditions: results from a multicenter study at the German Center for Mental Health.

BACKGROUND: Fatigue is a frequent and disabling symptom across many physical and mental health conditions. Despite similar phenomenology, research has largely focused on single conditions using heterogeneous assessments, limiting cross-condition comparability. This study examines fatigue-related symptom burden across diverse clinical conditions within a shared framework. METHODS: In this multicenter cross-sectional study, adult outpatients (18-65 years) were recruited at five German university hospitals into seven cohorts: chronic primary pain, multiple sclerosis, cancer, chronic renal failur

Mechanisms underlying exercise-induced pain modulation in people with multiple sclerosis - a scoping review.

BACKGROUND AND STUDY AIM: Multiple Sclerosis (MS) is a prevalent chronic neuroinflammatory disease, characterized by demyelination, inflammation and axonal degeneration. Up to 40% of people with MS (pwMS) experience central neuropathic pain (CNP). Despite numerous debilitating consequences, CNP is often underdiagnosed and undertreated, with limited pharmacological options available. However, exercise is a promising non-pharmacological intervention in the management of pain. This scoping review therefore aims to (a) outline the pathophysiological mechanisms of CNP in MS, (b) summarize the neuro

Elevated atherogenic index of plasma in multiple sclerosis: association with metabolic profile in a cross-sectional study.

OBJECTIVE: Multiple sclerosis (MS) is a chronic neuroimmunological disorder characterized by inflammation, demyelination, and neuroaxonal loss within the central nervous system. Recent evidence suggests that MS is not only a neuroimmune disease but also associated with systemic metabolic alterations. Furthermore, certain disease-modifying therapies (DMTs) used in MS may influence lipid metabolism, leading to changes in serum cholesterol levels. The atherogenic index of plasma (AIP), calculated based on the triglyceride-to-high density lipoprotein (HDL) cholesterol ratio, is a sensitive biomark

Alignment with sex-typical brain architecture: Links to mood pathology, sex hormones and transcriptomics of neurodegenerative conditions.

Understanding the mechanisms that underpin the pervasive sex bias in risk for specific psychiatric and neurodegenerative conditions could substantially advance efforts promoting personalized medicine. As a first step towards addressing this issue, we applied imaging transcriptomics and multivariate analysis techniques to data from an adolescent sample enriched in anxiety/depression cases, a child sample enriched in attention deficit hyperactivity disorder (ADHD) cases, and adult patients with treatment resistant major depressive disorder (MDD). We tested whether, across sexes, alignment with f

A Human Genome-wide CRISPR-Cas9 Screen Reveals Host Factors That Support Epstein-Barr Virus B cell infection.

UNLABELLED: Epstein-Barr virus (EBV) infects 90% of the population worldwide and is associated with multiple malignancies, including Burkitt, Hodgkin and post-transplant lymphomas, nasopharyngeal and gastric carcinoma. EBV also triggers autoimmune diseases, including multiple sclerosis and systemic lupus erythematosus. While host factors important for EBV B-cell entry have been defined, including CD21/CR2, MHC class II and R9AP, much remains to be learned about host factors that support post-entry steps of EBV infection. To gain insight, we conducted a human genome-wide CRISPR-Cas9 screen. In

Ocular safety signals of sphingosine-1-phosphate receptor modulators in postmarketing surveillance.

BACKGROUND: Sphingosine-1-phosphate (S1P) receptor modulators are used for immune-mediated diseases, and macular oedema is an established ocular adverse effect. However, ocular-focused, class-wide postmarketing reporting across all five approved agents remains incompletely characterized. METHODS: We analyzed U.S. Food and Drug Administration Adverse Event Reporting System (FAERS) data from 2004 Q1 through 2026 Q1 using report-level deduplication, approval-aligned windows, and primary-suspect exposure. Ocular events were assessed using a curated set of 52 Preferred Term (PT) strings. Sensitivi

Comparator Event Rates, Baseline Activity, and Operational Selection Pressure in Relapsing Multiple Sclerosis Trials: A Meta-Epidemiological Analysis.

BACKGROUND: Reference-arm annualized relapse rates (ARRs) in relapsing multiple sclerosis trials have declined. We examined whether baseline disease activity and the treatment environment during recruitment changed alongside this trend. METHODS: We analyzed 15 treated-reference arms from nine Phase 3 programs. ARR was related to recruitment era, prior-year relapse activity, MRI markers, and an Operational Selection Pressure Index (OSPI) combining recruitment geography with time since ocrelizumab approval. Associations were descriptive; disability outcomes were audited separately. RESULTS: Re
