Walking and strength outcomes of targeted hip resistance training may be predicted by both MRI and clinical measures in multiple sclerosis.

BACKGROUND: Compromised neural function contributes sensation, strength, and walking deficits in persons with multiple sclerosis (PwMS). Progressive resistance training (PRT) promotes neural adaptations, leading to favorable functional outcomes, while advances in MRI enable the evaluation of white matter tract pathology in vivo. The purpose of this work was to examine the utility of MRI with clinical measures to predict exercise responsiveness to PRT. METHODS: Twenty-eight PwMS (mean age: 49.3 and EDSS: 3.8) and 26 controls (mean age: 50) participated in 12-weeks of PRT. Primary outcomes were

Phase Image Display Settings for Paramagnetic Rim Lesion Detection in Multiple Sclerosis: A Practical Guide.

The 2024 revisions of the McDonald criteria incorporate the paramagnetic rim lesion (PRL) into the diagnostic framework of multiple sclerosis, and PRL evolution is increasingly recognized as a marker of disease progression. PRLs are detected on phase images of susceptibility-sensitive sequences, but their visibility depends critically on display settings. Current consensus recommendations provide detailed guidance on acquisition parameters yet do not address the phase-image window/level applied at the reading workstation; an inappropriate phase window can render rims invisible. We discuss prac

Genetic architecture of MRI-derived cervical spinal cord morphology reveals sensory-motor axis and systemic disease associations.

The spinal cord is critical to motor, sensory, autonomic function, and increasingly implicated in neurological disease and human health, yet its genetic architecture remains largely unexplored. We perform large-scale phenotyping of the upper cervical spinal cord (C1-C3) structure using brain magnetic resonance imaging from over 80,000 UK Biobank participants, extracting shape metrics including cross-sectional area, diameters, and eccentricity. We identify a total of 179 independent genome-wide significant variants, with cervical spinal cord morphology showing moderate to high SNP-based heritab

Mechanistic Evaluation of Psilocybin in Neurodegenerative Disorders: Relevance of 5-HT2A Receptors and Downstream Pathways.

More than fifty million individuals worldwide are experiencing neurodegenerative disorders, which include Parkinson's disease, Alzheimer's disease, amyotrophic lateral sclerosis, Huntington's disease, multiple sclerosis, and frontotemporal dementia. These disorders have a significant role in the gradual degradation of cognition and motor skills. Most current treatment approaches have little effect on changing the course of the disease and are largely symptomatic. Psilocybin, a naturally occurring tryptamine alkaloid from psilocybin mushrooms, has become a promising medicinal agent because of i

Cognitive Function and Disability in People with Multiple Sclerosis: A Systematic Review and Meta-Analysis.

BACKGROUND: Cognitive dysfunction is a common yet under-recognized component of disability in people with multiple sclerosis (PwMS). Although numerous studies have investigated the relationship between cognitive function and physical disability, the findings remain inconsistent. In this systematic review and meta-analysis, we aim to clarify and quantify the association between physical disability and cognitive performance in PwMS, while also examining potential modifiers and the clinical relevance of this relationship. METHODS: A systematic literature search in multiple databases identified 1

Reduced perivascular diffusion in multiple sclerosis is associated with aging, disease duration, and mobility impairment.

BACKGROUND: The glymphatic system supports brain homeostasis by facilitating perivascular fluid transport and metabolic waste clearance. Diffusion-tensor imaging along the perivascular space (ALPS-index) provides a non-invasive surrogate marker of glymphatic activity. The objectives of this study were to: (1) compare the ALPS-index between individuals with relapsing-remitting MS (RRMS) and non-MS controls, (2) examine the role of age in the association between ALPS-index and disease duration, and (3) examine the relationship between ALPS-index and walking speed in RRMS. METHODS: Individuals w

Efficacy of nutraceuticals as adjunctive therapies in autoimmune diseases: A systematic review of clinical trials.

INTRODUCTION: Autoimmune diseases are characterized by a dysregulation of the immune system, resulting in chronic inflammation and tissue damage. Although immunosuppressive therapies have advanced, their limited efficacy and the adverse effects associated with long-term use restrict their clinical utility, highlighting the need for alternative treatments. This study aimed to synthesize current evidence on the efficacy of nutraceutical interventions for these disorders. METHODS: A comprehensive search was conducted on PubMed, Embase, and ClinicalTrials.gov to identify studies published between

The Role of Astrocytes in the Inflammatory Response in Neurodiseases.

Astrocytes have emerged as central regulators of neuroinflammation, extending far beyond their traditional roles as structural and metabolic support cells. In neurological disorders, astrocytes undergo reactive remodeling that encompasses morphological, transcriptional, metabolic, and functional changes. Rather than fitting into a fixed A1/A2 dichotomy, reactive astrocytes exist along a disease-, region-, and stage-dependent continuum shaped by local pathology and intercellular crosstalk. This review summarizes how astrocytes contribute to neuroinflammatory processes in Alzheimer's disease, Pa

IgG N-glycosylation in autoimmune diseases.

IgG is the most abundant antibody in the human circulation. It carries a single conserved glycosylation site, Asparagine 297, in its Fc domain. This N-glycan is critical for the ability of the antibody to activate immune cells via binding to, and cross-linking Fc receptors in order to elicit functional responses. First documented in the context of rheumatoid arthritis (RA), the IgG N-glycan may undergo changes that are correlated to host health. We present a narrative in-depth comparison of changes incurred by IgG N-glycans in autoimmune diseases; RA, systemic lupus erythematosus, multiple scl

Betaine Attenuates Cuprizone-Induced Cortical Demyelination: The Role of the WNT4/mTOR Signalling Pathway in Regulating Astrogliosis and Synaptic Integrity.

Cortical grey matter pathology manifests early demyelinating diseases and is a primary driver of permanent neurological impairment. Our study investigated the neuroprotective effects of betaine against cuprizone (CPZ)-induced damage to the cerebral cortex in rats, focusing on its ability to modulate the upstream WNT4/mTOR signalling pathway. Cortical tissues were collected for biochemical assessments of oxidative and inflammatory markers, real-time PCR analysis for WNT4, GFAP, and mTOR gene expressions, histopathological examination, and immunohistochemical analysis of vimentin, S100, and syna

Diet and physical activity in multiple sclerosis: associations with quality of life and disability by disease duration.

BACKGROUND: Healthy lifestyle factors are associated with higher quality of life (QoL) and reduced disability in people with multiple sclerosis (pwMS). Whether these associations differ according to MS duration is unclear. We examined associations between diet, supplement use, wellness activities, and physical activity with QoL and ambulatory disability, and whether these associations differed between pwMS diagnosed within ≤ 5 years versus > 5 years. METHODS: Cross-sectional self-reported survey data from 1,083 pwMS were analysed. Diet, supplement use, and wellness activities were queried via

Differential Trajectories of Brain Atrophy in Multiple Sclerosis, MOGAD, and AQP4 + NMOSD: The Role of Disease Activity in MOGAD.

BACKGROUND AND OBJECTIVES: Multiple sclerosis (MS), myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD), and aquaporin-4 antibody-positive neuromyelitis optica spectrum disorder (AQP4 + NMOSD) are inflammatory demyelinating disorders with distinct mechanisms of tissue injury. MS is characterized by diffuse neurodegeneration, whereas damage in antibody-mediated diseases is considered attack related. In MOGAD, the relationship between disease activity and brain atrophy remains unclear. We aimed to compare longitudinal trajectories of global and regional brain atrophy across M

Recognizing and Differentiating Area Postrema Syndrome Across NMOSD, MOGAD, and GFAP Astrocytopathy.

Area postrema syndrome (APS)-intractable nausea, vomiting, and/or hiccups persisting ≤48 h-is a recognized presenting feature of neuromyelitis optica spectrum disorder (NMOSD) and, less commonly, of myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) and glial fibrillary acidic protein (GFAP)-astrocytopathy. APS is still frequently misattributed to primary gastrointestinal disease, delaying immunotherapy by weeks to months. For years its pathophysiology was inferred largely from lesion topography rather than demonstrated at the level of neuronal signaling. A study presented

Burden of Multiple Sclerosis in the Nordic Region, 1990-2023, and Projection to 2050: A Systematic Analysis Based on the Global Burden of Disease Study 2023.

Background Multiple sclerosis (MS) is a leading cause of non-traumatic neurological disability in young and middle-aged adults, with the Nordic region exhibiting among the highest burdens globally. However, systematic comparisons of MS burden across all five Nordic countries using standardized methodologies remain scarce. Methods Using data from the Global Burden of Disease Study 2023, we analyzed MS burden in Denmark, Finland, Iceland, Norway, and Sweden from 1990 to 2023. Core indicators included incidence, prevalence, deaths, and disability-adjusted life years (DALYs). We calculated estimat

Transorbital sonography reveals retrobulbar optic nerve neuroaxonal changes in multiple sclerosis.

BACKGROUND: The optic nerve (ON), as an extension of the central nervous system, reflects multiple sclerosis (MS) pathological processes. Transorbital sonography (TOS) is emerging as a novel technique to detect ON changes. Our aim was to investigate retrobulbar ON morphometric changes in MS and their relationship with prior history of optic neuritis. METHODS: 122 MS subjects were enrolled, 68 with and 54 without history of optic neuritis. ON diameters without and with sheath were measured at 3 mm (OND3 and ONSD3) and 5 mm (ONSD5) posterior to the optic disc; myelination index (MI=ONSD3/ONSD5)

Putative predictive power of baseline cerebrospinal fluid dipeptidyl peptidase-4 on the conversion to secondary progressive multiple sclerosis.

BACKGROUND: Dipeptidyl peptidase-4 (DPP4) has many inflammation-related functions such as T cell activation, and complement activation and is recognized as a putative pathogenic factor in multiple sclerosis (MS). Our aim was to analyze whether baseline cerebrospinal fluid (CSF) levels of DPP4 and DPP4-induced complement breakdown byproducts might predict MS progression. METHODS: Levels of DPP4, factor C3a (C3a), factor C4a (C4a), factor Ba (Ba) and soluble membrane attack complex (sMAC) were measured by ELISA in CSF obtained during the first clinical attack of 80 MS patients. The association
