Long-term outcomes of alemtuzumab in relapsing-remitting multiple sclerosis: a real-world cohort with particular focus on treatment sequencing after natalizumab.

BACKGROUND: The role of alemtuzumab (ALEM) after natalizumab (NTZ) discontinuation in relapsing-remitting multiple sclerosis (RRMS) remains to be better defined in real-world settings. METHODS: We conducted a retrospective, longitudinal, single-centre cohort study including RRMS patients treated with ALEM. Clinical and MRI evaluations were performed every 6 months. Annualized relapse rate (ARR), and radiological activity were assessed longitudinally. NEDA-3 was evaluated both as an annual interval-specific outcome and as sustained NEDA-3 using Kaplan-Meier analysis. RESULTS: Eighty-one patie

Survivor bias and disease progression in observational cohorts in multiple sclerosis.

BACKGROUND: Long-term studies on multiple sclerosis progression often face dropout, possibly among those with more aggressive disease, biasing the cohort. OBJECTIVES: We assessed potential survivorship bias in a long-term observational multiple sclerosis cohort. METHODS: Three hundred sixty-one patients with multiple sclerosis were followed for 10 years, with clinical assessments, blood biomarkers and magnetic resonance imaging at baseline, follow-up 1 and follow-up 2. Progression was defined as an Expanded Disability Status Scale increase. Dropout was analysed and predicted; estimated Expan

Effects of Exercise Training on the Metabolic Cost of Walking in Health and Disease: A Systematic Review and Meta-analysis.

BACKGROUND: The metabolic cost of walking (Cw) increases with heathy aging and in neurological conditions. No comprehensive assessment has been performed yet to determine whether chronic exercise training and rehabilitation could improve Cw. OBJECTIVE: The purpose of this study was to determine whether the effects of chronic exercise interventions on Cw differ by type, dose, and intensity in healthy adults and individuals with neurological conditions (multiple sclerosis, Parkinson's disease, and stroke). DESIGN: Systematic review and robust variance estimation meta-analysis with meta-regress

Utility of Blood Neutrophil to Diagnose Bacterial Infections under Satralizumab Treatment for Aquaporin-4 Antibody-positive Neuromyelitis Optica Spectrum Disorder: A Report of Two Cases.

Satralizumab is an anti-interleukin-6 receptor biologic that may mask bacterial infections. We report two patients with neuromyelitis optica spectrum disorder who developed afebrile bacterial infections with normal C-reactive protein levels during satralizumab treatment. During non-infectious periods under satralizumab treatment, the neutrophil count and neutrophil-to-lymphocyte ratio decreased, whereas the neutrophil change ratio, defined as the ratio of present to previous neutrophil counts, did not. At the time of infection, these variables were elevated compared to the intra-individual con

Anti-CD20 Drugs and Natalizumab Confer Equal Protection Against Progression Independent of Relapse Activity (PIRA) in People With Multiple Sclerosis.

BACKGROUND: Highly effective (HE) disease-modifying therapies (DMTs) reduce relapse-associated worsening (RAW) and progression independent of relapse activity (PIRA). Natalizumab (NTZ) and anti-CD20 agents show similar efficacy on relapse suppression, but long-term comparative data on PIRA are limited. We aimed to compare PIRA accrual in relapsing-remitting MS (RRMS) patients treated with NTZ and anti-CD20 drugs over a long follow-up. METHODS: We included RRMS patients followed at our centre and recorded in the Italian MS and Related Disorders Register with at least 5 years of continuous trea

Case Report: CD20/BCMA bispecific CAR-T therapy for progressive multiple sclerosis.

Refractory secondary progressive multiple sclerosis (SPMS) remains an area of unmet need, particularly once disability continues to accumulate despite multiple disease-modifying therapies. Two SPMS patients are reported with C-CAR168 treatment, a bispecific CD20/BCMA CAR-T product, achieving 12- and 6-month drug-free remission. Patient 001 (EDSS 4.0→3.0) showed improvement of oculomotor dysfunction, ataxia, and cognitive impairment. Patient 002 (EDSS 6.5→6.0) demonstrated halted lower-limb progression and relieved bowel/bladder dysfunction. C-CAR168 infiltrated into the cerebrospinal fluid (CS

Effects of exercise and physical activity on circulating inflammaging- and immunosenescence-related biomarkers in older adults and adults with chronic diseases: an umbrella review.

BACKGROUND: Chronic low-grade inflammation and immunosenescence accompany ageing and chronic disease. Exercise and physical activity are proposed to modify circulating inflammatory biomarkers, but review-level evidence is dispersed across populations and conditions. METHODS: We searched PubMed, Web of Science, Embase, Cochrane Library, Scopus, and SPORTDiscus through 1 March 2026 for systematic reviews, meta-analyses, and overviews on exercise or physical activity and circulating inflammaging- or immunosenescence-related biomarkers in older adults and adults with chronic diseases. We reported

Cerebrospinal fluid amino acid alterations are associated with oxidative stress and neuronal injury markers in multiple sclerosis.

Altered glutamatergic neurotransmission and oxidative stress are implicated in the pathophysiology of multiple sclerosis (MS), yet their interplay remains incompletely understood. This study investigated cerebrospinal fluid (CSF) levels of neuroactive amino acids (aspartate, glutamate, glycine, γ-aminobutyric acid) in relation to markers of lipid peroxidation, antioxidant capacity, and neuronal injury. Amino acids were quantified using a validated high-performance liquid chromatography method with fluorescence detection. Aspartate levels were significantly elevated in MS compared with other ne

Single-cell Transcriptomics in Autoimmune Diseases: Mechanistic Insights, Cross-disease Convergence, and Translational Challenges in Rheumatoid Arthritis, Systemic Lupus Erythematosus, Type 1 Diabetes, and Multiple Sclerosis.

Autoimmune diseases are characterized by loss of immune tolerance and substantial heterogeneity across patients, tissues, and disease stages. Although bulk transcriptomic studies have identified important inflammatory pathways, they cannot directly resolve rare, transitional, and tissue-contextualized cell states that may be relevant to disease mechanisms and therapeutic response. Single-cell RNA sequencing (scRNA-seq) enables transcriptomic profiling at cellular resolution, while complementary multimodal, receptor-repertoire, epigenomic, and spatial approaches can link cellular states to prot

Management of pregnancy in patients with Graves' disease secondary to alemtuzumab: a case series and literature review.

INTRODUCTION: Alemtuzumab treatment for multiple sclerosis frequently induces Graves' disease (GD), characterized by common coexistence of stimulating and blocking anti-thyrotropin receptor autoantibodies (TRAb), causing fluctuations of thyroid function. Alemtuzumab-induced TRAb-positive hypothyroidism is also common. These conditions are challenging to manage during pregnancy, considering the imperative necessity to maintain euthyroidism to minimize risks for both pregnancy and fetal/neonatal outcomes. METHODS: We reviewed the clinical course and therapeutic management of four pregnant women

Glucosamine and selenium differentially regulate glutathione, lipid peroxidation, and mitochondrial function during glutamate stress in retinal neural cells.

Glutamate can disrupt cystine-dependent glutathione metabolism and promote oxidative and mitochondrial dysfunction in neural cells. We integrated human multiple-sclerosis optic-chiasm transcriptomics (GSE100297), glutamate-treated primary-neuron RNA sequencing (GSE255041), R28 differential-expression summaries, a normal human optic-nerve single-nucleus atlas (GSE236566), and R28 retinal precursor-cell experiments to characterize glucosamine (GlcN) and selenium responses. Human tissue showed enriched unfolded-protein-response, oxidative-stress, apoptosis, and ferroptosis programs, whereas gluta

Multiple Sclerosis During Pregnancy and the Postpartum Period: Diagnostic and Multidisciplinary Challenges in Two Cases.

BACKGROUND: Pregnancy may complicate the recognition and evaluation of new neurological symptoms suggestive of a central nervous system demyelinating disease. Diagnostic uncertainty may arise when clinical progression, neuroimaging findings, and supportive investigations are incomplete or inconclusive. CASE PRESENTATION: We report two pregnant women with progressive focal neurological symptoms and radiological findings suggestive of a demyelinating disease. Case 1 involved a 27-year-old woman with a history of autoimmune spondyloarthritis who developed dizziness, gait instability, asymmetric

Development and validation of a social vulnerability index for use in Canadians living with multiple sclerosis.

BACKGROUND: Social factors influence the health outcomes of individuals with multiple sclerosis (MS). Social vulnerability indices (SVIs) can identify populations at risk of poor outcomes and inform targeted interventions, but no SVI has been developed for people with MS to date. OBJECTIVE: We developed a SVI, which aggregates several measures of social vulnerability into a composite, and assessed its performance. METHODS: We conducted a national, cross-sectional survey of people with MS in Canada from June 2025-March 2026. We selected social vulnerability factors across demographic, socioec

Autoimmune hepatitis and primary biliary cholangitis are among a subset of autoimmune diseases defined by profound co-prevalence and genetic risk association with the IL12 pathway.

BACKGROUND AND AIMS: Distinct autoimmune diseases can share symptoms and autoantibodies, but the molecular pathways underlying this shared etiology remain poorly understood. We assessed genetic support for IL12 signaling across 27 diseases. APPROACH AND RESULTS: We identified a rare variant upstream of IL12A with large risk-increasing effects on autoimmune hepatitis [AIH; per-allele odds ratio (OR)=2.98, p=3×10-9], primary biliary cholangitis (PBC; OR=2.80, p=2×10-8), systemic sclerosis (SSc; OR=2.64, p=2×10-11), Sjogren's disease (SjD; OR=1.94, p=10-11), systemic lupus erythematosus (SLE; OR

Degenerative cervical myelopathy: early detection and the potential for preventing disability.

Degenerative cervical myelopathy (DCM), the most common non-traumatic spinal cord injury, is a prevalent, progressive, and highly disabling neurological disease, causing heterogeneous impairment ranging from sensory loss and impaired hand dexterity to gait, balance, and autonomic dysfunction. Disability often persists despite surgical decompression, and untreated disease can progress to irreversible neurological injury, including tetraplegia. Current clinical management is largely reactive, with diagnosis and intervention typically only after neurological deterioration has become clinically ap

Temperature extremes and autoimmune diseases.

Temperature extremes linked to climate change are becoming more frequent and more intense. This has significant implications for human health, including conditions driven by immune dysregulation. There is growing evidence that external thermal stress, particularly heat, may influence both the development and clinical course of several autoimmune conditions. Heat and cold can alter immune cell function through effects on cell signalling, cytokine production, barrier integrity and stress response within tissues. These mechanisms provide a strong biological basis for the observed associations bet

Blood Glial Fibrillary Acidic Protein in MRI-Negative Attack-Like Episodes of Neuromyelitis Optica Spectrum Disorder.

OBJECTIVES: Magnetic resonance imaging (MRI) is widely used to support relapse diagnosis in neuromyelitis optica spectrum disorder (NMOSD), yet some patients present with attack-like symptoms without corresponding lesions. We assessed serum glial fibrillary acidic protein (sGFAP) levels in MRI-negative attack-like episodes. METHODS: We retrospectively analyzed aquaporin-4-IgG-positive NMOSD patients presenting with relapse-like symptoms but without corresponding new MRI lesions. sGFAP was measured from stored serum collected at presentation and converted to demographic-adjusted z-scores.
