Is Pregnancy Protective from Rebound After Fingolimod Cessation?

Multiple sclerosis (MS) is a demyelinating neuroimmunological disease that poses significant reproductive challenges for affected women during pregnancy, particularly in relation to disease-modifying therapies (DMTs). Clinical evidence suggests that while pregnancy may exert a protective effect against relapses, severe rebound disease activity can occur following the discontinuation of certain medications such as fingolimod. This case report describes a pregnant woman, age 46 years, with known MS who experienced a rebound phenomenon after abruptly stopping her DMT. She was managed acutely with

The Roles and Uses of Extracellular Vesicles in Neuroimmunologic Diseases.

Neuroimmunology is a field of neurology focused on the diagnosis and treatment of diseases where the immune system causes inflammation or injury to the nervous system. This includes diseases such as multiple sclerosis, myelin oligodendrocyte glycoprotein antibody-associated disease, neuromyelitis optica spectrum disorder, autoimmune encephalitis, autoimmune ataxia, stiff person syndrome spectrum disorder, and neuro-rheumatologic diseases such as neuropsychiatric lupus. Over the past couple decades, there have been significant advancements in the diagnostic methods for several neuroimmunologic

Proportionate and disproportionate reporting of different infectious complications of disease modifying therapy in multiple sclerosis.

INTRODUCTION: Progessive multifocal leukoencephalopathy (PML) is a complication of immunoincompetence due to disease or immunosuppressive therapy and in particular due to disease modifying therapy (DMT) in multiple sclerosis. It is not clear however, whether there is a specific vulnerability to PML or whether this is due to immunosuppression per se. The same holds for the combination of cryptococcal disease and Fingolimod. METHODS: We used the reporting odds ratio (ROR) as calculated from entries to Vigibase for a sample of drugs used for DMT (Alemtuzumab, Cladribine, Fingolimod, Dimethyl fum

Disseminated Ureaplasma urealyticum infection presenting as systemic inflammatory syndrome in ocrelizumab-treated patients with multiple sclerosis: a case series.

BACKGROUND: Ocrelizumab, an anti-CD20 monoclonal antibody, is widely used to treat multiple sclerosis (MS), inducing profound B-cell depletion and disease remission. However, it may rarely lead to opportunistic infections. We report two patients with MS on ocrelizumab who developed unique complications of anti-CD20 antibody therapy. One patient developed fulminant endophthalmitis, a novel complication that has not been previously reported. CASE PRESENTATION: A 38-year-old female of South Asian descent and a 46-year-old White female, both with MS on ocrelizumab, developed disseminated Ureaplas

Investigation of Complement Activation and Cytokine Signatures in AQP4+ NMOSD, MOGAD, and MS.

OBJECTIVE: Aquaporin-4-IgG seropositive neuromyelitis optica spectrum disorder (AQP4+ NMOSD), myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD), and multiple sclerosis (MS) are pathophysiologically distinct neuroinflammatory disorders. Complement activation is a major mechanism of injury in AQP4+ NMOSD, whereas its role in MS is complex and in MOGAD controversial. We aimed to compare complement components and cytokines between AQP4+ NMOSD, MOGAD, and MS. METHODS: CSF complement components, CSF and plasma cytokines, neurofilament light chain (NfL) and glial fibrillary aci

A 35-Year-Old Woman With Multiple Sclerosis and a New Atypical Thalamic Lesion: A Case Report From the National Multiple Sclerosis Society Case Conference Proceedings.

In multiple sclerosis (MS), new or enlarging brain lesions often suggest disease activity, but infections or tumors may mimic relapse. We report a 35-year-old woman with MS whose enlarging thalamic lesion was ultimately diagnosed as diffuse midline glioma, underscoring the need for prompt evaluation of atypical MRI findings.

Functional and Structural Imaging Features of Cognitive Profiles in Multiple Sclerosis.

BACKGROUND AND OBJECTIVES: Cognitive impairment affects 40%-70% of patients with multiple sclerosis (MS). Although recent hypothesis-driven and data-driven efforts have identified cognitive phenotypes, functional substrates underlying impairment in such phenotypes are still not clear. Our objective was to identify distinct cognitive phenotypes using clustering in a large cohort of patients with MS and characterize their associated clinical, structural MRI, and resting-state (RS) functional connectivity (FC) features. METHODS: We conducted an observational, cross-sectional study with right-han

Modulating neuroinflammation with glucagon-like peptide-1 receptor agonists: a scoping review exploring their therapeutic potential in schizophrenia.

Neuroinflammation is a key contributor to the pathophysiology and symptomatology of schizophrenia spectrum disorders. Glucagon-like peptide-1 receptor agonists (GLP-1RAs), clinically approved anti-diabetic and -obesity medication, have been associated with anti-inflammatory properties. It remains unclear whether GLP-1RA treatment can improve neuroinflammation within psychiatric disorders, including schizophrenia. This scoping review synthesizes evidence from clinical and preclinical studies on the anti-inflammatory effects of GLP-1RAs in psychiatric, neurodegenerative and neurological disease

Global epidemiological and genetic burden of rare neurological diseases in children.

We assessed the global burden of rare childhood neurological diseases from 1990 to 2021 and projected trends to 2050 using Global Burden of Disease data, alongside an analysis of the genetic burden from ClinVar. Four neurological conditions were classified as rare childhood neurological diseases: motor neuron disease, multiple sclerosis, neuroblastoma and other peripheral nervous cell tumors, and other neurological disorders. From 1990 to 2021, the global age-standardized prevalence and disability-adjusted life-year rates of rare childhood neurological diseases increased, with higher burdens a

Orally-administered beta-cyclodextrin as a novel countermeasure for perfluorooctane sulfonic acid toxicity in mice.

INTRODUCTION: Perfluoroalkyl substances (PFAS) are associated with an increased risk of several cancers, immune disorders, and neuropathology. These actions are compounded by their long half-lives. METHODS: We tested if exposure to perfluorooctane sulfonic acid (PFOS) worsens disease progression or reduces survival in experimental autoimmune encephalomyelitis (EAE), a mouse model of multiple sclerosis, and if treatment with beta-cyclodextrin (βCD, which encapsulates PFOS) mitigated PFOS effects. Adult female C57BL/6 mice were provided drinking water containing 0 (control) or 20 μg/mL PFOS for

Lymphocytosis as a biomarker for natalizumab treatment response in multiple sclerosis: The link with anti-drug antibodies.

BACKGROUND: Natalizumab is a high-efficacy therapy drug for multiple sclerosis that inhibits lymphocyte migration into the central nervous system, increasing lymphocyte counts in the peripheral blood. Anti-drug antibody responses may limit drug efficacy, but current guidance recommends anti-drug antibody testing only after evidence of 'reduced efficacy'. OBJECTIVE: To assess the lack of relative lymphocytosis as a biomarker of anti-drug antibody responses. METHODS: Lymphocyte counts and immunogenicity data from the pivotal AFFIRM clinical trial (n = 627 natalizumab vs n = 315 placebo) and re

Serum and plasma leptin levels in patients with Multiple Sclerosis: A systematic review and meta-analysis.

BACKGROUND: Increasing evidence indicates that leptin, an adipocyte-derived cytokine with strong immunomodulatory and pro-inflammatory properties, may play a role in multiple sclerosis (MS) pathogenesis through regulation of T helper cell balance and inflammatory cytokine signaling. However, findings from individual studies assessing circulating leptin levels in MS patients remain inconsistent and sometimes contradictory. This systematic review and meta-analysis aims to synthesize the available evidence and generate a more precise estimate of serum and plasma leptin levels in individuals with

Examining intersections of biographies & biologies: Diversity & outcomes of people with multiple sclerosis in Canada (iBio2Div-MS) protocol.

BACKGROUND: Although advances have been made in understanding multiple sclerosis (MS) disease etiology and developing new therapeutics, the benefits of these advances have been distributed inequitably across population groups. Persons with MS (PwMS) who are racialized, men, of lower social position, or living in rural areas experience worse outcomes for reasons yet to be fully elucidated. OBJECTIVE: We designed a prospective three-year longitudinal study to assess the individual and joint effects of contextual, biographical, and biological factors (e.g., sex, age, and genetics), on performanc
