Spinal Cord and Nerve Root Involvement in Histiocytic Neoplasms: A Retrospective Observational Cohort Study.

BACKGROUND AND OBJECTIVES: Histiocytic neoplasms, including Erdheim-Chester disease (ECD), Langerhans cell histiocytosis (LCH), and Rosai-Dorfman-Destombes disease (RDD), have frequent nervous system involvement. The aim of this study was to characterize the neurologic, radiologic, and laboratory features of spinal cord and nerve root involvement in histiocytic neoplasms. METHODS: We evaluated patients with biopsy-proven ECD, LCH, or RDD seen at a tertiary referral center between January 1996 and December 2024. Patients were included if spinal MRI demonstrated spinal cord or nerve root involv

Epigenetic mechanisms in neuroinflammatory disease of the brain: Multiple sclerosis.

Multiple sclerosis (MS) is a leading cause of lifelong incurable progressive disability in young adults that results in fatigue and accumulation of physical and cognitive disability. MS is characterized by autoimmune destruction of the myelin and subsequent neuroaxonal loss. The disease is triggered by environmental factors in genetically predisposed individuals. In this chapter, we review the key findings from epigenetic studies, with a focus on the most studied modification, that is, DNA methylation. Utilization of causal analytic approaches in large cohorts provides evidence that DNA methyl

IRF7 signalling in tanycytes controls brain-body-immune response across the ovarian cycle.

Sex-specific differences in metabolic and immune function are shaped by endocrine-immune interactions, yet the complexity of this relationship remains poorly understood. Here, we identify an immune cell-like state of tanycytes, an ependymoglial cell type lining the third ventricle that mediates brain-body communication1,2 in both mice and humans. Using single-cell transcriptomics, we provide a high-resolution cellular map of the mouse and human mediobasal hypothalamus with cross-species comparison of tanycytes. We capture dynamic sex-, oestrous cycle- and high-fat diet-dependent transcriptomic

Targeting Ferroptosis in Neurodegenerative Diseases: Therapeutic Potential and Mechanistic Insights of Traditional Chinese Medicine.

Neurodegenerative diseases (NDs), including Alzheimer's disease, Parkinson's disease, Huntington's disease, amyotrophic lateral sclerosis, and multiple sclerosis, are characterized by progressive and irreversible neuronal loss, whereas current treatments remain largely symptomatic. Ferroptosis is an iron-dependent form of regulated cell death whose execution is defined by iron-enabled phospholipid peroxidation and failure of cellular ferroptosis-defense systems. Mitochondrial metabolism can modify ferroptosis susceptibility, but its contribution varies with cell type and initiating stimulus. C

Gut Microbiota-Derived Indole-3-Acetaldehyde Contributes to the Disease-Alleviating Effects of Kurarinone in Experimental Autoimmune Encephalomyelitis.

Multiple sclerosis (MS) is a central nervous system autoimmune disease primarily driven by Th1 and Th17 cells, whose onset and progression are closely associated with gut microbiota. Kurarinone (KU), an active component from Radix Sophora Flavescens, exhibits anti-inflammatory, immunoregulatory and gut microbiota-modulating properties. However, whether KU alleviates MS severity via a gut microbiota-mediated mechanism remains unclear. Here, we demonstrate that KU ameliorates experimental autoimmune encephalomyelitis (EAE)-an animal model of MS-by modulating the gut microbiota, particularly via

Assessing Body Image in Multiple Sclerosis: Adaptation and Validation of the Modified Body Image Scale.

BACKGROUND: Multiple sclerosis (MS) is frequently associated with physical, cognitive, and emotional changes, including functional loss, which may significantly affect body image (BI) perception. However, a BI questionnaire specifically adapted for use with people with MS (PwMS) is currently lacking. This study aims to adapt and validate the original Italian version of the Body Image Scale (BIS) for the MS population and to examine its psychometric properties. METHODS: A three-phase adaptation process was conducted, involving a panel of MS experts and PwMS to ensure coherence with MS clinical

From synapse to sheath: how OPCs couple neural activity to adaptive myelination and behavioral plasticity.

Oligodendrocyte precursor cells (OPCs) are abundant progenitor cells in the central nervous system. Unlike mature oligodendrocytes, OPCs form functional synapses with neurons and respond to both glutamatergic and GABAergic signals. Glutamate activates calcium-permeable AMPA receptors on OPCs, generating local Ca2+ signals that regulate myelin sheath formation and stabilization. In parallel, GABAergic signaling modulates OPC proliferation and differentiation through both calcium-dependent and transcriptional mechanisms. Activity-dependent myelination driven by OPCs contributes directly to neura

Prevalence of Self-Reported Pelvic-Floor-Related Symptoms Among Female Patients with Multiple Sclerosis.

Introduction: Multiple sclerosis (MS) is more prevalent among females worldwide. The Expanded Disability Status Scale (EDSS) assesses MS progression, including disability related to pelvic floor dysfunction (PFD), such as bowel and bladder dysfunction. However, data on PFD symptoms in patients with MS remain limited. Objective: To evaluate the prevalence and impact of self- reported pelvic-floor-related symptoms among women with MS in Saudi Arabia and their effects on quality of life (QOL). Methods: A cross-sectional study was conducted among 112 female patients with MS (EDSS ≤ 7, age ≥ 18 yea

Gluten-Free and Gluten-Restricted Diets in Multiple Sclerosis: A Systematic Review of Disease Activity and Patient-Reported Outcomes.

Background: Dietary modification is widely used by people with multiple sclerosis (MS), and gluten avoidance is a common self-directed strategy. Biological plausibility has been proposed through gut-barrier and immune mechanisms, but whether gluten restriction improves clinical outcomes or objective disease activity remains uncertain. Objective: To systematically identify, appraise, and synthesise evidence on gluten-free and gluten-restricted diets in MS, with particular attention to whether observed effects can be attributed specifically to gluten. Methods: The protocol was registered with PR

Neuroimaging in neuro-ophthalmologic emergencies with acute visual loss.

Neuro-ophthalmologic emergencies presenting with acute visual loss pose significant diagnostic and therapeutic challenges, with potential for irreversible vision loss if not promptly recognized. A comprehensive approach that integrates clinical evaluation with advanced imaging modalities is essential for timely diagnosis, prognostic stratification, and management. Magnetic resonance imaging remains the gold standard for demyelinating optic neuritis, enabling differentiation among multiple sclerosis, neuromyelitis optica spectrum disorder, and myelin oligodendrocyte glycoprotein antibody-associ

Impact of multi-echo ICA modeling decisions on motor-task fMRI analysis.

Multi-echo independent component analysis (ME-ICA) has been demonstrated to improve sensitivity and reliability of task functional magnetic resonance imaging (fMRI) data and, in particular, motor-task data with inherent task-correlated head motion. However, previous work has shown that an overly aggressive ME-ICA denoising approach may unintentionally remove task-related signal, while a more conservative approach may not effectively mitigate noise. While the effects of varied implementations of ME-ICA on signal and noise characteristics have been tested thoroughly in breath-hold data, the effe

The efficacy of mechanical spinal manipulation in the standing position in patients with non-specific low back pain: a randomised placebo-controlled trial.

INTRODUCTION: Non-specific low back pain (NSLBP) is not attributable to a known cause and is a leading contributor to the global disease burden. The effect of specific spinal manipulation in patients with NSLBP using the vertical adjustment position standing (VAPS) technique described by Hartman remains unclear. This technique is used to address spinal restrictions and improve mobility. The main aim of the study was to examine the immediate and medium-term effects of VAPS on pain (VAS), mobility of the lumbar spine (MSt), and pain-related disability (ODI) in patients with non-specific low back

From scavenger to victim: mechanism and treatment of microglial ferroptosis in white matter injury disease.

Failure of myelin repair is a shared pathological feature of white matter injury (WMI)-associated diseases, including Alzheimer's disease (AD), vascular dementia (VaD), and multiple sclerosis (MS), but the underlying mechanisms remain incompletely understood. Microglia, the resident innate immune cells of the central nervous system (CNS), contribute to white matter homeostasis by clearing myelin debris and supporting remyelination. Within the iron- and lipid-rich white matter microenvironment, sustained uptake of myelin and oligodendroglial debris may impose phagocytic and metabolic stress on
