A potential biomarker for the efficacy of complement inhibitors in myasthenia gravis.

BACKGROUND: In acetylcholine receptor (AChR) antibody-positive myasthenia gravis (MG), complement activation is a major mechanism underlying impaired neuromuscular transmission and complement inhibitors are used in clinical practice. In the absence of biomarkers predicting responsiveness to complement inhibitors, this study aimed to identify novel predictors. METHODS: Serum samples from 14 patients with AChR antibody-positive MG and 10 healthy controls (HCs) were incubated with AChR-expressing HEK293 cells. AChR binding IgG and membrane attack complex (MAC) deposition were quantified by flow

Perceptual, patient-reported, and acoustic voice characteristics in individuals with multiple sclerosis.

PURPOSE: Voice impairment is increasingly recognised as a relevant but often underexplored feature of multiple sclerosis (MS). The present study aimed to investigate vocal characteristics in individuals with MS using a combined subjective, perceptual, and acoustic assessment approach. METHODS: A total of 91 participants were included, comprising 48 individuals with MS and 43 healthy controls. Vocal function was assessed using the Voice Handicap Index (VHI), the GRBAS perceptual scale, and acoustic analysis of sustained phonation, including fundamental frequency (fo), jitter, shimmer, and harm

Myrtenol mitigates seizure activity and oxidative imbalance in a mouse model of PTZ-induced seizure.

Pentylenetetrazole (PTZ)-induced seizures are strongly associated with oxidative imbalance, and the monoterpenoid alcohol myrtenol has been proposed to exert antioxidant and neuroprotective effects. This study investigated whether myrtenol attenuates PTZ-evoked seizure activity and oxidative disturbances in the mouse hippocampus and cerebral cortex. Sixty male mice were assigned to six groups: control, PTZ-only, three myrtenol pretreatment doses (25, 50, 100 mg/kg), and diazepam. Seizure susceptibility was evaluated by measuring latencies to minimal clonic and generalized tonic-clonic seizures

Left dorsomedial prefrontal cortex hyperactivation reflects early compensation for emerging social cognition difficulties in multiple sclerosis.

BACKGROUND: Social cognition (SC) has emerged as a relevant neurocognitive domain in people with Multiple Sclerosis (pwMS), given its impact on everyday functioning. However, the neural mechanisms supporting SC performance in pwMS remain underexplored, highlighting the need for ecologically valid assessment paradigms capable of capturing both behavioral and neural aspects of social-cognitive functioning. In this study, we addressed this gap by investigating the clinical application of an fMRI-adapted version of the Edinburgh Social Cognition Test (ESCoT) to explore the neural correlates of SC

Multi-scale Gaussian product-of-experts for missing sequence multiple sclerosis lesion segmentation.

OBJECTIVE: To develop and evaluate a unified framework for multiple sclerosis (MS) white matter lesion segmentation that operates robustly under clinically prevalent missing sequence magnetic resonance imaging conditions. METHODS: We propose a multi-scale Gaussian product-of-experts framework (MG-PoE) integrated into nnU-Net, where sequence-specific inputs are independently processed through a shared encoder and parameterized as Gaussian experts before combination through precision-weighted product-of-experts (PoE) fusion at each encoder scale. The unified fusion rule handles different sequen

How do neurorehabilitation patients perceive their fundamental care needs? Findings from a qualitative study.

BACKGROUND: The Fundamentals of Care (FoC) framework provides a structured model to support the delivery of person-centered nursing care by integrating physical, psychosocial, and relational dimensions. Although the framework has been widely applied in acute care, little is known about its relevance in long-term neurorehabilitation settings, where patients often face complex, prolonged, and multidimensional care needs. OBJECTIVE: To explore how individuals with neurological disorders perceive their fundamental care needs during inpatient rehabilitation. DESIGN: A qualitative secondary analys

Multiple sclerosis: Advances in diagnosis, biomarkers, and disease management.

Multiple sclerosis (MS) care has advanced rapidly across diagnosis, biomarkers, and disease-modifying therapy. The 2024 revision of the McDonald criteria enables earlier and more biologically grounded diagnosis through the cerebrospinal fluid (CSF) kappa free light chain (KFLC) index, the central vein sign (CVS), and paramagnetic rim lesions (PRLs), and applies uniform criteria across adults and children. Our understanding of disease subtypes is also evolving. In parallel, fluid biomarkers-particularly serum neurofilament light chain (NfL) and glial fibrillary acidic protein (GFAP)-together wi

Artificial intelligence in central nervous system autoimmune diseases: from clinical decision support to mechanistic discovery and translation.

Central nervous system (CNS) autoimmune diseases, including multiple sclerosis (MS), neuromyelitis optica spectrum disorder (NMOSD), autoimmune encephalitis (AE), and myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD), remain difficult to diagnose, stratify, and treat because clinical phenotypes overlap, biomarkers are imperfect, and the disease course requires longitudinal observation. Artificial intelligence (AI) is increasingly being evaluated to integrate clinical, imaging, immunological, and molecular data for decision-making in neuroimmunology. Clinical applications

Retinal Ganglion Cell Quantification for Evaluating Neurodegeneration in Mouse Models of Multiple Sclerosis.

Quantification of retinal ganglion cell (RGC) loss provides a sensitive and reproducible measure of neurodegeneration in mouse models of neurological  disease and injury. In experimental autoimmune encephalomyelitis (EAE), a widely used mouse model of multiple sclerosis, inflammation of the optic nerve can lead to optic neuritis, axonal injury, and progressive RGC loss. Therefore, quantification of RGC survival provides a valuable readout of inflammation-mediated neurodegeneration and can be used to evaluate disease severity and neuroprotective interventions. Here, we describe a protocol for r

Pleiotrophin: a multifunctional regulator in neurodevelopment and neuropsychiatric diseases.

Pleiotrophin (PTN) is a heparin-binding growth factor that plays a pivotal role in neurodevelopment, neuroplasticity, and neuroinflammation. PTN signals through a multi-receptor network involving protein tyrosine phosphatase receptor type ζ (PTPRZ1), syndecans (particularly syndecan-3), integrins, nucleolin, and neuropilin-1. Anaplastic lymphoma kinase (ALK) has also been functionally linked to PTN signaling, although whether PTN directly binds and activates it remains debated. This review comprehensively summarizes recent advances in understanding the structure, distribution, and multifunctio

TIGIT+ B cells: emerging immunoregulatory players in autoimmunity and transplantation tolerance with therapeutic potential.

T cell immunoreceptor with Ig and ITIM domains (TIGIT) is an inhibitory immune checkpoint receptor classically studied in T cells and natural killer (NK) cells. Recent evidence demonstrates that TIGIT is also expressed on distinct B cell subsets exhibiting potent immunoregulatory properties. TIGIT+ B cells are proposed to contribute to immune homeostasis through anti-inflammatory cytokine secretion, modulation of antigen-presenting cell function, regulation of T cell differentiation, and bidirectional crosstalk with regulatory T cells and myeloid populations. The strongest direct evidence for

Finding the right words in connected speech: An examination of word-finding difficulties in persons with multiple sclerosis.

Self-reported word-finding difficulties are common in people with multiple sclerosis (PwMS). A lack of suitable methods for assessing these sometimes-subtle word-finding difficulties in PwMS has been highlighted. The aim of this study was to investigate how word-finding difficulties can be detected and how they manifest themselves in connected speech in PwMS compared to non-brain-damaged (NBD) controls. Twenty-seven PwMS with self-reported anomia and 23 NBD controls performed a video-retelling task which was evaluated by (i) counting the number and types of indicators of word-finding difficult
