CV2/CRMP5 antibodies are specifically associated with parenchymal neurosyphilis: a retrospective cohort study.

BACKGROUND: Neurosyphilis can trigger neuro-specific immune responses, but evidence remains scarce. This study investigates the prevalence and clinical significance of neural antibodies in neurosyphilis, with a focus on CV2/CRMP5 antibodies. METHODS: We retrospectively analyzed 1149 patients with syphilitic neurological symptoms. Of 247 diagnosed with neurosyphilis, 215 met inclusion criteria. Using CBA and line blot, 97 patients suspected of autoimmune encephalitis were tested for neural antibodies, with positives confirmed by in-house CBA. Complete medical records were collected. Homology b

Serum Toluidine Red Unheated Serum Test (TRUST) titer and cerebrospinal fluid parameters as predictors of neurosyphilis in HIV-negative people with late latent syphilis.

BackgroundThe diagnosis of neurosyphilis is challenging due to the limited sensitivity of cerebrospinal fluid Venereal Disease Research Laboratory (VDRL) test. Late latent syphilis is defined as positive treponemal serology without clinical manifestations and untreated infection for more than 1 year. Symptomatic or asymptomatic neurosyphilis is diagnosed based on cerebrospinal fluid findings and clinical symptoms. This cross-sectional study investigated serum Toluidine Red Unheated Serum Test (TRUST) titer and cerebrospinal fluid inflammatory markers as predictors of neurosyphilis in HIV-negat

Alterations of cognitive function and hippocampal metabolism in asymptomatic neurosyphilis patients: based on 1H-magnetic resonance spectroscopy.

OBJECTIVES: Neurosyphilis has been found to be associated with cognitive decline. However, as a common type of neurosyphilis, the change of cognitive function in asymptomatic neurosyphilis (ANS) has received little attention. This study aims to investigate the cognitive function and cellular metabolism changes in bilateral hippocampus in ANS. METHODS: Twenty-four ANS, 20 syphilis patients and 23 control individuals who were admitted in Hangzhou Third people's Hospital from June 2021 to July 2025 were selected. 1H-MRS (magnetic resonance spectroscopy) was used to detect the cellular metabolism

High Titers, Hidden Disease: Asymptomatic Neurosyphilis in Secondary Syphilis - A Case Series from Western Maharashtra.

Syphilis can involve the central nervous system at any stage; however, routine cerebrospinal fluid (CSF) examination is rarely performed in neurologically asymptomatic patients with secondary syphilis. We describe a case series of eight male patients with secondary syphilis and high serum venereal disease research laboratory (VDRL) titers (≥1:16) evaluated at a tertiary teaching hospital in western Maharashtra, India (2024-2025). Five patients were human immunodeficiency virus (HIV)-positive and on antiretroviral therapy. None had neurological symptoms or signs; however, all had CSF abnormalit

Cytokines in cerebrospinal fluid combined with machine learning improve the diagnostic accuracy and predict the progression of neurosyphilis.

BACKGROUND: Syphilis is a sexually transmitted disease (STD) caused by Treponema pallidum (TP) which could progress to neurosyphilis affecting the nervous system. However, the pathogenesis of neurosyphilis remain unknown. In addition, current diagnosis of neurosyphilis is mainly based on serological tests and cerebrospinal fluid (CSF) analysis, with limitations in terms of sensitivity and specificity. METHODS: From April 2021 to April 2023, 129 patients with syphilis were enrolled and divided into the non-neurosyphilis group (common syphilis, CS, n=19), asymptomatic neurosyphilis group (AN, n

Simultaneous Diagnosis of Myasthenia Gravis and Neurosyphilis, With Symptoms Improving After Neurosyphilis Treatment: A Case Report.

Simultaneous diagnosis of myasthenia gravis (MG) and early asymptomatic neurosyphilis is extremely rare. There have only been two cases described in the literature. To the best of our knowledge, this was the first case of MG resolving with concurrent treatment of neurosyphilis. Our patient was a 67-year-old male who presented to the hospital with a diffuse rash, bilateral ptosis, bilateral facial weakness, and bulbar symptoms (dysarthria, difficulty with mastication, and difficulty drinking fluids).  After testing positive for syphilis in serum and cerebrospinal fluid (CSF), the patient was di

A case report of nitrous oxide-induced subacute combined degeneration complicated with neurosyphilis in a 23-year-old female.

BACKGROUND: Recreational nitrous oxide (N₂O) abuse has become a common health issue in young adults, which can induce subacute combined degeneration (SCD) of the spinal cord. Neurosyphilis has highly similar clinical and imaging manifestations with SCD. The coexistence of N₂O-induced SCD and asymptomatic neurosyphilis is extremely rare and easily overlooked. We herein report such a case to improve clinical diagnosis. CASE PRESENTATION: A 23-year-old female presented with 3-month progressive limb numbness and gait instability, with a 6-month history of N₂O inhalation. Spinal MRI showed typical

Immune-inflammatory-nutritional dysfunction in HIV-negative patients with asymptomatic neurosyphilis: a retrospective comparative study.

OBJECTIVES: The host's immune function, inflammatory response, and nutritional status are closely related to the occurrence and development of neurosyphilis (NS). Asymptomatic neurosyphilis (ANS), as a completely curable type of NS, has received limited study on these fields. This study explores the immune function, inflammatory response, and nutritional status levels in patients with ANS, as well as their changes after treatment, aiming to provide reference for the early diagnosis and treatment of ANS. METHODS: This study retrospectively included 32 healthy controls (HCs), 27 cases of ANS, a

Risk factors for neurosyphilis in HIV patients: A retrospective cohort study.

INTRODUCTION: Syphilis is a highly prevalent sexually transmitted infection worldwide. Patients living with Human Immunodeficiency Virus (HIV) have a higher risk of developing neurosyphilis. Actual guidelines advise to proceed with lumbar puncture only if neurologic symptoms are present. However, asymptomatic neurosyphilis patients are not rare in the HIV population and other risk factors should be defined to guide screening. METHODS: We performed a retrospective cohort to evaluate risk factors related to neurosyphilis in HIV patients. Adults with HIV infection and laboratory confirmed syphil

Neurosyphilis.

The incidence of syphilis has increased world-wide over the last ∼25 years, and this increase has been accompanied by a resurgence of neurosyphilis. This review sets the stage for understanding neurosyphilis by briefly summarizing the clinical and laboratory findings in uncomplicated syphilis. It then addresses the epidemiology, clinical manifestations and treatment of neurosyphilis, beginning with information from the pre-penicillin era and extending to modern studies. Ocular and otosyphilis are included as their treatment is the same as for neurosyphilis. Importantly, the review focuses on a

Untargeted metabolomics in a prospective cross-sectional observational study reveals differences in plasma and cerebrospinal fluid between asymptomatic neurosyphilis and serofast syphilis.

Asymptomatic neurosyphilis (ANS), a diagnostically elusive complication of Treponema pallidum infection, necessitates invasive cerebrospinal fluid (CSF) analysis for detection. This study investigated metabolic differences between patients with ANS and those with serofast syphilis. Using untargeted metabolomics, we analyzed plasma and CSF from 15 patients with ANS, 15 patients with serofast syphilis, and 16 healthy controls via liquid chromatography-mass spectrometry. Univariate statistical analyses identified differential metabolites, followed by pathway enrichment through Kyoto encyclopedia

Establishment of non-invasive diagnostic prediction model for HIV-positive asymptomatic neurosyphilis patients.

BACKGROUND: Although guidelines strongly recommended universal screening for neurosyphilis (NS) in people living with HIV (PLWH), the invasiveness of lumbar puncture (LP) made comprehensive screening difficult, particularly among asymptomatic individuals, increasing the risk of missed diagnoses. Because of the common transmission routes, the co-infection rate of PLWH and syphilis was extremely high. Establishing effective diagnostic and predictive models for NS was essential to improve diagnostic accuracy and timeliness. This study aimed to develop a non-invasive method for detecting asymptoma

Asymptomatic Neurosyphilis Presenting With Bilateral Pulmonary Nodules: A Case Report.

Globally, syphilis cases have been increasing and pulmonary lesions have been reported. However, asymptomatic neurosyphilis has not been documented. This condition is often unrecognised and presents with symptoms similar to those of other infectious and non-infectious lung diseases. In syphilitic pneumonia, defined as a secondary form of syphilis, intramuscular injection of benzylpenicillin (24 million units) remains the gold standard for treatment, with a favourable prognosis. This report presents for the first time a case of asymptomatic neurosyphilis with multiple pulmonary nodules and butt

Response to Ceftriaxone in Asymptomatic Neurosyphilis Refractory to Doxycycline and Benzathine Penicillin.

An asymptomatic male in his mid-30s presented with a positive Venereal Disease Research Laboratory (VDRL) test report. He was investigated and detected to be reactive for human immunodeficiency virus (HIV)-1 antibodies. A lumbar puncture revealed cerebrospinal fluid (CSF) VDRL to be reactive at a titer of 1:160 which led to a diagnosis of asymptomatic neurosyphilis. The unavailability of first-line antibiotics necessitated the search for alternative regimens. The patient was administered oral doxycycline 200 mg twice daily for 28 days along with intramuscular benzathine penicillin 2.4 million

Neurosyphilis is characterized by a compartmentalized and robust neuroimmune response but not by neuronal injury.

BACKGROUND: Neurosyphilis is increasing in prevalence but its pathophysiology remains incompletely understood. This study assessed for CNS-specific immune responses during neurosyphilis compared to syphilis without neurosyphilis and compared these immune profiles to those observed in other neuroinflammatory diseases. METHODS: Participants with syphilis were categorized as having neurosyphilis if their cerebrospinal fluid (CSF)-venereal disease research laboratory (VDRL) test was reactive and as having syphilis without neurosyphilis if they had a non-reactive CSF-VDRL test and a white blood ce
