The genetic driver of Acute Necrotizing Encephalopathy, RANBP2, regulates the inflammatory response to Influenza A virus infection.

Influenza virus infections can cause severe complications such as Acute Necrotizing Encephalopathy (ANE), which is characterised by a rapid onset of pathological inflammation following febrile infection. Heterozygous dominant mutations in the nucleoporin RANBP2/Nup358 predispose to influenza-triggered ANE1. The aim of our study was to determine whether RANBP2 plays a role in IAV-triggered inflammatory responses. We found that the depletion of RANBP2 in a human airway epithelial cell line increases IAV genomic replication by favouring the import of the viral polymerase subunits, PB1, PB2, and P

Acute Hemorrhagic Necrotizing Encephalopathy in a COVID-19 Patient With a Response to Intravenous Immunoglobulin Therapy.

BACKGROUND Acute hemorrhagic necrotizing encephalopathy (AHNE) is a rare, immune-mediated condition that follows viral infection and has a poor prognosis. Previously reported cases of AHNE have described a favorable clinical response to intravenous immunoglobulin (IVIG) therapy combined with corticosteroids. This report describes the case of a 67-year-old Bahraini man with AHNE associated with Coronavirus Disease-19 (COVID-19) with a response to IVIG therapy. CASE REPORT We report a 67-year-old Bahraini man with type 2 diabetes, hypertension, and a prior hemi-colectomy, presenting with acute f

Acute Necrotizing Encephalopathy in Children: Meta-Analysis of Observational Studies on the Efficacy of Steroid Treatment.

BACKGROUND: To determine efficacy of early steroid therapy on outcomes in children with acute necrotizing encephalopathy. METHODS: We leveraged two 2023 systematic reviews to identify eligible publications, predefined as having ≥3 patients, objective study outcomes categorized as good or poor, and described according to steroid therapy. Data were combined by (i) adding raw data from included studies (as done in the previous systematic review) and (ii) random effects meta-analysis with relative risk (RR) and 95% confidence interval (CI). Sensitivity analysis used a more consistent definition o

A pilot study of peripheral blood tsRNA expression profiling in children with influenza-associated acute necrotizing encephalopathy.

BACKGROUND: Influenza-associated acute necrotizing encephalopathy (ANE) is a fulminant pediatric neurological complication primarily driven by hypercytokinemia. Given the lack of specific biomarkers, early diagnosis remains clinically challenging. This study aimed to characterize the peripheral blood expression profiles of tRNA-derived small RNAs (tsRNAs) in pediatric ANE to identify novel non-invasive diagnostic markers and therapeutic targets. METHODS: Nine pediatric subjects (n = 3 per group) were enrolled and stratified into three cohorts: ANE (severe), uncomplicated influenza (mild), and

Acute necrotising encephalopathy of childhood secondary to influenza A infection.

A toddler boy presented with fever, rhinitis and loose stools, followed by two episodes of generalised seizures with up-rolling of eyes and posturing. Initially the case was managed as febrile seizures. Later he developed progressive encephalopathy, dystonia and persistent seizures, requiring Paediatric Intensive Care Unit admission and mechanical ventilation. Neurological examination revealed hypertonia, exaggerated reflexes and a Glasgow Coma Score of 6, without signs of meningeal irritation. Laboratory investigations showed thrombocytopenia, leucopenia and transaminitis. H1N1 influenza A wa

Familial, recurrent or genetic acute necrotizing encephalopathy of childhood: a systematic review and a case report. Focus on atypical events and on the role on immunizations.

BACKGROUND: Familial, recurrent or genetic acute necrotizing encephalopathy (F/R/G ANE) is triggered by infections in children with genetic susceptibility. We conducted a systematic review to characterize clinical-neuroradiological features, therapy and outcome of F/R/G ANE in children, and to evaluate vaccination implications. METHODS: A systematic literature review was conducted in PubMed, Scopus, and Web of Science, adhering to the PRISMA guidelines, and registered in PROSPERO. An institutional case was also described. RESULTS: Ninety-two patients were analysed (91 literature cases and 1

Variable Phenotype in Three Siblings With RAN Binding Protein 2 Variant With Potential Preventive Treatment for Recurrences.

BACKGROUND: Familial acute necrotizing encephalopathy, or acute necrotizing encephalopathy type 1 (ANE1), is a rare genetic disorder, with the majority of cases linked to a pathogenic variant in the RAN Binding Protein 2 (RANBP2) gene. Although it shares many features of the sporadic type, it shows significant phenotypic variability within the same family, with a tendency toward recurrence in many cases. CASE PRESENTATION: We report a case series of three siblings from a nonconsanguineous family, all carrying a pathogenic RANBP2 variant consistent with ANE1. Despite sharing the same genotype,

Chikungunya associated acute necrotizing encephalopathy in an adolescent: Expanding the neurological spectrum of the virus.

Acute necrotizing encephalopathy (ANE) is a rare and severe neurological disorder characterized by rapid onset encephalopathy following a viral febrile illness. A 12-year-old adolescent girl presented with fever, joint pain, and myalgia for 3 days, followed by the development of multiple convulsions and altered sensorium. Cerebrospinal fluid analysis was non-specific. Contrast-enhanced magnetic resonance imaging of the brain revealed two well-defined, bilaterally symmetrical, round-to-oval hyperintense lesions on T2W, and FLAIR sequences, representing acute necrotizing encephalopathy. Familial acute necrotizing encephalopathy temporally associated with SARS-CoV-2 infection: Two case reports and literature review.

RATIONALE: Acute necrotizing encephalopathy (ANE) is a rare but life-threatening parainfectious encephalopathy characterized by rapid neurological deterioration following viral infections. Although most cases occur sporadically, familial clustering is uncommon and suggests the involvement of genetic susceptibility factors. The emergence of coronavirus disease 2019 (COVID-19) has broadened the spectrum of viral triggers associated with ANE; however, familial COVID-19-associated ANE remains rarely reported. PATIENT CONCERNS: We retrospectively reviewed 2 siblings who developed ANE following lab

The Cases of Acute Necrotizing Encephalopathy Due to Human Herpes Virus-6.

BACKGROUND: Human herpesvirus-6 (HHV-6) is a neurotropic virus that typically remains latent in immunocompetent individuals but may cause severe neurologic complications, including encephalitis, particularly in immunocompromised hosts. Rarely, HHV-6 encephalitis can also occur in immunocompetent children, often presenting with features of acute necrotizing encephalopathy. CASES: We report 2 cases of previously healthy toddlers who presented with fever, seizures and altered mental status. Neuroimaging revealed bilateral thalamic involvement consistent with acute necrotizing encephalopathy. HHV

Co-occurrence of lung adenocarcinoma with rapidly progressive dementia and multiple cerebral microbleeds: a case report.

BACKGROUND: Various neurological diseases can cause rapidly progressive dementia, which is a clinical syndrome characterized by a rapid decline in cognitive function over a short period, typically less than 1 or 2 years. It can be caused by various neurological diseases, including neurodegenerative, inflammatory, vascular, metabolic, and neoplastic central nervous system diseases. Rapidly progressive dementia is particularly associated with Creutzfeldt-Jakob disease, but other conditions such as immune-mediated encephalitis, rapidly progressive subtypes of Alzheimer's disease, and various othe

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

A Case of Acute Hemorrhagic Necrotizing Encephalitis in the Neonatal Period: Case Report with Autopsy Findings.

Acute necrotizing encephalopathy (ANE) is a rare immune-mediated disease in children that could progress rapidly, and lead to significant morbidity or mortality. ANE's diagnostic challenges render it difficult to recognize and treat in a timely and effective manner. Although infantile-onset cases have been reported, the presentation of ANE in preterm neonates has not been described. Herein, we report a case of a preterm newborn who had a relatively stable clinical course in the first week of life, after which the neonate exhibited sudden deterioration due to progressive encephalopathy with ref

MicroRNA sequencing analysis in pediatric patients with influenza-associated acute necrotizing encephalopathy: Potential biomarkers for early diagnosis and therapy.

Acute necrotizing encephalopathy (ANE) secondary to influenza infection is characterized by fulminant neurological deterioration and a high mortality rate. The underlying mechanisms remain unclear, and specific treatments are currently lacking. Therefore, understanding the pathogenesis and identifying diagnostic and therapeutic targets for influenza-induced ANE are crucial. Peripheral blood samples were collected from two groups: influenza-infected patients without ANE (mild) and influenza infection with ANE patients (severe). Differentially expressed genes (DEG) were identified through microR
