Seronegative Neuromyelitis Optica Spectrum Disorder in a Six-Year-Old Patient: A Case Report.

Neuromyelitis optica spectrum disorder (NMOSD) is a rare demyelinating disease of the central nervous system (CNS) in children. Its main clinical manifestations are optic neuritis and myelitis. In most cases, it is associated with suggestive magnetic resonance imaging (MRI) and positive serology (AQP4-IgG). We report a six-year-old girl with acute asymmetric quadriparesis, bilateral decreased visual acuity, and sudden loss of sphincter control. Although AQP4-IgG testing was negative, MRI demonstrated findings consistent with NMOSD. Consequently, she received high-dose methylprednisolone, intra

[Diagnostic and therapeutic challenges of neuromyelitis optica in Côte d'Ivoire: a pediatric case report].

INTRODUCTION: Neuromyelitis optica (NMO) is an inflammatory and demyelinating disease of the central nervous system (CNS) that typically progresses in episodes. The average age of onset is around 40 years old, though there are also pediatric forms. We present our first pediatric case and discuss its unique features. CLINICAL OBSERVATION: An 8-year-old child was hospitalized in June 2023 due to motor deficits in all four limbs. In November 2021, she presented with a binocular decrease in visual acuity, followed by partial recovery. Eleven months later, she experienced a second, more severe epi

Cerebrovascular risk factors impact on brain atrophy and DTI-ALPS decrease in neuromyelitis optica spectrum disorder.

BACKGROUND: Neuromyelitis optica spectrum disorder (NMOSD) is an autoimmune astrocytopathy where disability is largely relapse-driven. Cerebrovascular risk factors (cVRFs) may exacerbate astrocytic and small-vessel injury. Therefore, we investigated their impact on MRI damage and disability in NMOSD. METHODS: Twenty-eight aquaporin 4 immunoglobulin G-positive NMOSD patients and 56 age- and sex-matched healthy controls (HC) underwent 3T brain and cervical MRI, cVRF and neurological assessment, including the Expanded Disability Status Scale (EDSS). Brain T2-hyperintense white matter lesion volu

Recurrent Myelin Oligodendrocyte Glycoprotein Antibody-Positive Aseptic Meningitis at the Identical Site in a Child: Leptomeningeal Enhancement Detected by Contrast-Enhanced FLAIR Imaging.

Myelin oligodendrocyte glycoprotein (MOG) antibody-associated disease (MOGAD) is an acquired demyelinating syndrome of the central nervous system mediated by MOG antibodies. Clinical phenotypes include optic neuritis, transverse myelitis, and acute disseminated encephalomyelitis. MOG antibody-associated meningitis (MOGAM) is a form of aseptic meningitis characterized by the presence of serum MOG antibodies. Parenchymal lesions are typically absent in MOGAM; however, some cases may progress to MOGAD. Leptomeningeal lesions are rarely detected on contrast-enhanced T1-weighted magnetic resonance

Seronegative demyelinating disease within the neuromyelitis optica/MOG antibody-associated disease spectrum following nivolumab and ipilimumab: A case report and literature review.

Immune checkpoint inhibitors (ICIs) are increasingly used in the treatment of advanced malignancies and have been associated with a broad spectrum of immune-related adverse events (irAEs). We present the case of a 65-year-old woman who developed a series of neurological complications, including autoimmune encephalitis, myelitis, and optic neuritis, following treatment with nivolumab and ipilimumab for non-small-cell lung cancer. The clinical and radiological findings supported a diagnosis within the seronegative neuromyelitis optica spectrum disorder/myelin oligodendrocyte glycoprotein antibod

Longitudinal Spinal Cord Atrophy in Patients With Neuromyelitis Optica Spectrum Disorder and Its Association With Rituximab Treatment.

BACKGROUND AND OBJECTIVES: Longitudinally extensive transverse myelitis (LETM) is a core feature of neuromyelitis optica spectrum disorder (NMOSD), which leads to spinal cord atrophy. We aim to determine annual spinal cord atrophy rates and influencing factors in NMOSD. METHODS: Spinal cord structural metrics, including cross-sectional area (CSA), anteroposterior diameter (AP), and right-left diameter (RL), were calculated using the Spinal Cord Toolbox from brain 3D sagittal T1-weighted images covering the C1-C3 levels. Normative references were constructed based on healthy participants and u

The role of neuroimmunology subspecialty in disease management of patients with neuromyelitis optica spectrum disorder.

BACKGROUND: Neuromyelitis optica spectrum disorder (NMOSD) is a severe autoimmune inflammatory disease of the central nervous system (CNS) that requires specialized management. Although the role of subspecialty management has been established in various neurological conditions, the impact of neuroimmunology subspecialty (NIS) management on NMOSD patient outcomes remains inadequately explored. METHODS: A retrospective analysis was conducted on 188 NMOSD patients, stratifying them into groups based on their management pathway: initial visit in NIS, initial visit in non-neuroimmunology subspecia

Integrated Blood Inflammatory Ratios and Cerebrospinal Fluid Blood‒Brain Barrier Dysfunction Predict Relapse Risk in Neuromyelitis Optica Spectrum Disorder.

BACKGROUND: Relapse is the primary driver of irreversible disability accumulation in neuromyelitis optica spectrum disorder (NMOSD). Although aquaporin-4 immunoglobulin G (AQP4-IgG) is central to disease diagnosis and pathogenesis, reliable tools for individualized relapse risk stratification remain limited. Given emerging evidence that blood-brain barrier (BBB) dysfunction and systemic immune activation play key and interconnected roles in NMOSD pathophysiology, we aimed to develop and internally validate an integrated prognostic model incorporating these dimensions to predict relapse risk.

CXCL9 associates with experimental neuromyelitis optica spectrum disorder following adoptive transfer of Tfh and Th17 cells.

BACKGROUND: This study investigates the pathogenic contributions of aquaporin-4 (AQP4)-specific follicular helper T (Tfh) and T helper 17 (Th17) cells in neuromyelitis optica spectrum disorder (NMOSD), utilizing newly established murine models based on adoptive transfer of antigen-specific T-cell populations. METHODS: AQP4-knockout mice were immunized with the AQP4-derived peptide to generate AQP4-reactive Tfh and Th17 cells. These cells were subsequently isolated and adoptively transferred into wild-type recipient mice. At disease peak-defined by consistent neurological deficits-spinal cord

A Case of Longitudinal Extensive Transversal Myelitis in Patient With Neuromyelitis Optica Spectrum Disorder and Systemic Lupus Erythematosus.

Neuromyelitis optica spectrum disorder (NMOSD) is a rare autoimmune, demyelinating disorder of the central nervous system. Clinical manifestations include optic neuritis, longitudinally extensive transverse myelitis (LETM) involving three or more vertebral segments, and, in most cases, seropositivity for anti-aquaporin-4 antibodies (AQP4-IgG). Although longitudinally extensive transverse myelitis (LETM) is a hallmark feature of NMOSD, it is not pathognomonic and may occur in a variety of infectious, metabolic, neoplastic, and autoimmune conditions, including systemic lupus erythematosus (SLE).

Comparative immunopathology and clinical features of MOGAD and AQP4-IgG NMOSD in children: A scoping review.

Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is a distinct immune-mediated demyelinating central nervous system disorder, frequently presenting as optic neuritis in children and adolescents. Conversely, aquaporin-4 antibody-positive neuromyelitis optica spectrum disorder (AQP4-NMOSD) is an astrocytopathy characterized by more severe relapses and greater residual visual impairment. This scoping review, conducted according to PRISMA-ScR and JBI methodology, mapped evidence from 23 studies identified across PubMed and Scopus over the last decade. The objective was to ex

Cell-type-specific Mendelian randomization analysis of brain single-nucleus eQTLs and neuromyelitis optica spectrum disorder risk.

BACKGROUND: Neuromyelitis optica spectrum disorder (NMOSD) is a rare central nervous system autoimmune disease characterized by aquaporin-4 antibody-mediated astrocyte injury. Although genetic risk variants in the major histocompatibility complex (MHC) region have been consistently implicated in NMOSD susceptibility, the cellular context in which these genetic risk factors operate in the brain remains uncharacterized. We aimed to identify cell-type-specific genetic associations between brain gene expression and NMOSD risk through a systematic Mendelian randomization screen. METHODS: Cis-eQTL
