Characteristic patterns of complement deposition in NMOSD, MOGAD, and MS.

The complement system is involved in the pathogenesis of inflammatory demyelinating diseases (IDDs) of the CNS. While complement inhibition significantly reduces the relapse rate in neuromyelitis optica spectrum disorders (NMOSDs), no clear consensus has been reached regarding the role of complement in myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) and multiple sclerosis (MS). Therefore, we examined CNS tissues from patients with NMOSD (18 autopsies and one biopsy, median age: 56 years), MOGAD (seven autopsies and 20 biopsies, median age: 34 years) and MS (24 autopsies

Natalizumab exacerbates astrocytopathy in NMOSD via blockade of endothelial VCAM1-astrocytic integrin α4 interaction.

Neuromyelitis optica spectrum disorder (NMOSD) is a rare autoimmune inflammatory disorder of the central nervous system (CNS) that shares clinical features with multiple sclerosis (MS) but typically manifests with more severe symptoms. The presence of pathogenic IgG autoantibodies targeting aquaporin-4 (AQP4) channels on astrocytes serves as a highly specific biomarker that distinguishes NMOSD from MS. Unlike MS, NMOSD is characterized by profound astrocytic destruction and exhibits a distinct response to therapies. Notably, disease-modifying therapies (DMTs) effective in MS, including nataliz

Effect of transplanted oligodendrocyte precursor cells derived from inflammatory and non-inflammatory microenvironment on remyelination in a chronic cuprizone model.

INTRODUCTION: Multiple sclerosis is a chronic demyelinating disease of the central nervous system. Transplantation of oligodendrocyte progenitor cells (OPCs) is a promising approach to enhance remyelination; however, the influence of the OPCs' microenvironmental origin on their therapeutic efficacy remains unclear. This study compared the remyelinating capacity of OPCs isolated from inflammatory (lipopolysaccharide) and non-inflammatory (cuprizone) microenvironments after transplanting into the corpus callosum and examined their effects on extracellular matrix chondroitin sulfate proteoglycans

Early-Age Cuprizone Exposure Induces Region-Specific Demyelination and Neuroinflammation in Mice.

Myelin, essential for rapid nerve conduction and axonal integrity in the central and peripheral nervous systems, is compromised in demyelinating diseases, leading to neurological deficits and progressive neurodegeneration. Although remyelination can occur, regeneration in adults is often limited, resulting in incomplete repair and impaired nerve function. In multiple sclerosis (MS), an immune-mediated demyelinating disease with diverse clinical phenotypes, progression and disability correlate with demyelination and failed remyelination, influenced by genetic and environmental factors. A well-e

Senolytic treatment induces oligodendrocyte dysfunction and demyelination in the corpus callosum.

Aging is a primary risk factor for disease progression in multiple sclerosis (MS). Because of this, treatments that can reduce the consequences of molecular aging, like senescence, have been proposed as a strategy to address disease progression. However, the effects of senolytics, a class of drugs which selectively ablate senescent cells, on the central nervous system are largely unknown. Here, we examined the effects of senolytic treatment on myelination and oligodendrocyte function in vivo using C57BL6/J mice and in vitro using primary rat oligodendrocyte cultures. Initial data showed that n

Pediatric-Onset Multiple Sclerosis Presenting With Persistent Vertigo, Ataxia, and Optic Nerve Atrophy in a 15-Year-Old Girl: A Case Report.

Pediatric-onset multiple sclerosis (POMS) is an inflammatory demyelinating disorder of the central nervous system that can present with brainstem or cerebellar symptoms and may be initially misattributed to peripheral vestibular etiologies. We report a 15-year-old girl with five days of continuous vertigo described as a sensation that "everything around me is moving," accompanied by unsteady gait and transient unilateral sensory-motor symptoms. Brain magnetic resonance imaging (MRI) with and without contrast demonstrated extensive supratentorial and infratentorial T2-weighted fluid-attenuated

Astrocyte Activation Persists after Recovery of Myelin and Motor Deficits in the Cuprizone Model: a Longitudinal PET and CNS Tissue Analysis Study.

PURPOSE: The cuprizone (CPZ) mouse model for multiple sclerosis enables researchers to investigate the underlying mechanisms of demyelination and remyelination, as well as the effect of therapeutic interventions thereon. Over the last five decades, research revealed detailed analyses of brain tissue derived from CPZ-fed animals at different times of de- and re-myelination. Yet, longitudinal analysis of the effects of a CPZ diet on locomotor performance, myelination and neuroinflammation over a two-month recovery period are still unexplored. PROCEDURES: In this study, we comprehensively examin

Controversies in neuroimmunology: Multiple sclerosis and other demyelinating disorders.

Multiple sclerosis is the most prevalent demyelinating disease affecting the central nervous system. In the past two decades, significant milestones have been achieved in this domain, including the identification of critical immunological pathways, novel biomarkers, and effective immunotherapies. Nonetheless, several areas of uncertainty and challenges persist. Globally, there has been a notable increase in multiple sclerosis cases. In Colombia, recent studies indicate a rise in prevalence from fewer than 5 cases per 100 000 inhabitants to 7.5 cases per 100 000. It is imperative for neurologis

Therapeutic updates in NMOSD and MOGAD: From present practice to future promise.

Aquaporin-4-IgG-positive neuromyelitis optica spectrum disorder (AQP4+NMOSD) and myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) are rare, antibody-mediated inflammatory disorders of the central nervous system (CNS). Although historically grouped under the umbrella of multiple sclerosis, both entities differ markedly in pathophysiology, clinical course, prognosis and therapeutic requirements. The identification of disease-specific autoantibodies has not only refined diagnostic accuracy but has also highlighted fundamental differences in therapeutic strategy, particularl

IL-4/IL-4Rα signaling activates PPARγ to promote oligodendrocyte differentiation and remyelination.

The restoration of white matter damage is essential for the functional recovery observed in demyelinating diseases such as multiple sclerosis (MS). Interleukin 4 (IL-4), a type II cytokine, has been acknowledged for its protective role in modulating microglia and immune cells, thus suppressing inflammatory processes. Emerging evidence suggests that IL-4 extends beyond its immunomodulatory function, playing a part in memory, neuronal pruning, protection, and neurodegeneration. Here, our study identified the capacity of IL-4 to stimulate myelinogenesis and remyelination in the central nervous sy

Understanding Further the Phenotypic Spectrum of Central Nervous System Inflammatory Demyelinating Disorders Using Unsupervised Clustering.

BACKGROUND: Central nervous system (CNS) inflammatory demyelinating syndromes, including multiple sclerosis (MS), aquaporin-4 antibody-positive neuromyelitis optica spectrum disorder (AQP4 + NMOSD), and myelin oligodendrocyte glycoprotein (MOG) antibody-associated disease (MOGAD), occasionally overlap. Some patients remain double-seronegative, showing atypical features that challenge current classifications. OBJECTIVE: To better characterize the phenotypic spectrum of antibody-negative atypical inflammatory demyelinating disorders (AIDD) using unsupervised clustering. METHODS: We retrospecti

Detecting early brain susceptibility changes before demyelination in cuprizone mouse model using quantitative susceptibility mapping (QSM).

Multiple sclerosis (MS) is a neurological disease that affects the central nervous system through demyelination and inflammation. Animal model, including the cuprizone (CPZ) model, provides a robust platform for studying demyelination and remyelination in MS. While conventional MRI techniques are sensitive to myelin changes, quantitative susceptibility mapping (QSM) offers additional advantages by capturing both myelin- and iron-related pathology. In this study, we performed longitudinal whole-brain multimodal magnetic resonance imaging (MRI), including T2-weighted imaging, magnetization trans

Optic chiasmal neuritis: clinical features, aetiologies, MRI patterns and prognosis in a real-world cohort.

BACKGROUND: Optic chiasmal neuritis (OCN) is considered atypical for multiple sclerosis (MS) and more frequently associated with neuromyelitis optica spectrum disorders (NMOSD). Yet the true aetiological spectrum of OCN in the antibody era remains poorly defined. METHODS: We conducted a retrospective single-centre study of consecutive patients with MRI-confirmed chiasmal enhancement between 2017 and 2025. Non-inflammatory, non-demyelinating aetiologies and previous optic neuritis were excluded. Clinical features, ophthalmological outcomes, MRI findings, and aetiologies were analysed. RESULTS

Demyelinating disorders in women: epidemiology, immunology, and clinical implications across MS, NMOSD, and MOGAD.

Multiple sclerosis (MS), neuromyelitis optica spectrum disorder (NMOSD), and myelin oligodendrocyte glycoprotein-associated disease (MOGAD) are the major types of demyelinating disorders of the central nervous system (CNS). Demyelinating disorders impact women disproportionately and frequently present during reproductive years. These conditions can cause significant neurological disability and psychosocial challenges, especially for women. Because they often present during reproductive years, clinicians frequently manage contraception, pregnancy, and the postpartum period alongside disease con
