Clinicoradiologic profile of neuromyelitis optica spectrum disorder in a Kashmiri cohort: a prospective observational cohort study.

PURPOSE: Neuromyelitis optica spectrum disorder (NMOSD) is a rare disease that is often misdiagnosed and that can have ethnic variations. No prior study of this condition in a Kashmiri cohort has been reported, which motivated this study. METHODS: This prospective, observational cohort study included 32 patients. NMOSD was diagnosed according to the International Panel for NMO Diagnosis criteria based on serology, magnetic resonance imaging (MRI), cerebrospinal fluid (CSF), and visual evoked potential (VEP) findings. RESULTS: Among central nervous system demyelinating diseases, NMOSD constit

Infliximab-associated multifocal central nervous system demyelination mimicking multiple sclerosis in a patient with Crohn's disease: a case report.

BACKGROUND: Tumor necrosis factor-alpha (TNF-α) inhibitors are widely used for immune-mediated inflammatory diseases, including Crohn's disease. Although generally effective, TNF-α blockade has been linked to uncommon but clinically significant paradoxical immune-mediated neurologic adverse events, including inflammatory demyelinating disorders of the central nervous system (CNS). CASE PRESENTATION: A 32-year-old woman with Crohn's disease (perianal fistula) treated with infliximab for one year presented with progressive dizziness, headache, and bilateral lower-limb weakness. Neurologic exami

Proteomic Biomarker Discovery in Cerebrospinal Fluid Identifies CD74 and CD14 for Differentiating Multiple Sclerosis from Neuromyelitis Optica.

Multiple sclerosis (MS) and neuromyelitis optica (NMO) are distinct autoimmune demyelinating diseases of the central nervous system with overlapping clinical features, complicating early diagnosis. Accurate differentiation is essential to avoid inappropriate treatment and improve outcomes. We conducted a comprehensive proteomic analysis of cerebrospinal fluid (CSF) from MS and NMO patients using both conventional proteomics and a nanoparticle-based low-abundance protein enrichment strategy (LAPE). Deep CSF proteome coverage yielded 3,816 proteins. Proteomic profiling revealed shared and distin

TRIM21 autoantibodies are associated with blood-brain barrier dysfunction in a subgroup of neuromyelitis optica spectrum disorder.

We previously reported that glucose-regulated protein 78 autoantibodies (GRP78 Ab) cause breakdown of the blood-brain barrier (BBB) in neuromyelitis optica spectrum disorder (NMOSD) patients. Objective of the present study is to identify novel antibodies associated with the breakdown of the BBB in NMOSD patients negative for anti-GRP78 Ab. Human brain microvascular endothelial cells (BMECs) were incubated with purified NMOSD-IgG from 4 patients. RNA-seq, a high-content imaging, living cell-based antibody binding assay, and mass spectrometry were used to evaluate molecular changes and identify

Steroid Refractory and Plasma Exchange Partially Responsive Longitudinally Extensive Transverse Myelitis Due to Tumor Necrosis Factor-Alpha (Etanercept): A Case Report.

Background: Acute transverse myelitis (ATM) is an inflammatory disorder of the spinal cord with heterogeneous etiologies, including autoimmune, infectious, paraneoplastic, and drug-induced causes. Tumor necrosis factor-alpha (TNF-α) inhibitors have been infrequently associated with inflammatory central nervous system events, including transverse myelitis. TNF-inhibitor-associated myelitis typically presents with short-segment lesions, a normal brain MRI, and partial responsiveness to corticosteroids. Longitudinally extensive transverse myelitis (LETM) and steroid-refractory cases are uncommon.

Spatiotemporal APLNR Expression Dynamics During Oligodendroglial Remodeling of the Corpus Callosum in the Cuprizone Model.

Demyelinating disorders such as multiple sclerosis are characterized by oligodendrocyte loss and insufficient remyelination. The cuprizone model provides a well-established experimental system for studying these processes. The apelinergic system, including the apelin receptor (APLNR), has been implicated in neuroprotection and central nervous system homeostasis. However, its role in white matter demyelination and repair remains incompletely understood. This study aimed to characterize the spatial and temporal dynamics of APLNR expression in relation to oligodendrocyte lineage cells in the corp

Case Report of Combined Central and Peripheral Demyelination: Treated With Ofatumumab.

BACKGROUND AND OBJECTIVES: Combined central and peripheral demyelination (CCPD) is a rare autoimmune-mediated disorder characterized by demyelination affecting both the central nervous system and peripheral nervous system. The involvement of the CNS typically manifests as longitudinally extensive transverse myelitis and optic neuritis, whereas PNS involvement commonly presents as chronic inflammatory demyelinating polyneuropathy (CIDP). Clinically, the course of CCPD can vary from monophasic to relapsing-remitting, leading to complex and diverse symptomatology. There are few studies on the tre

GPR37 modulates remyelination following demyelinating injury.

GPR37 has been recognized as a negative regulator of oligodendrocyte myelination. However, its role in demyelination recovery remains unclear. To examine the function of GPR37 in remyelination, we compared wild-type and GPR37-deficient mice of either sex after focal (lysolecithin) or global (cuprizone) toxin-induced demyelination, as well as immune-mediated demyelination (experimental autoimmune encephalomyelitis, EAE). We found that the absence of GPR37 resulted in enhanced recovery in all three models. In focal demyelination, we observed that, while the initial lesioned area was similar in s

Anti-CD20 Therapies in Pediatric Acquired Demyelinating Syndromes: Evidence Across MS, AQP4-IgG-Positive NMOSD and MOGAD.

Pediatric-acquired demyelinating syndromes, including multiple sclerosis (MS), aquaporin-4-IgG-positive neuromyelitis optica spectrum disorder (AQP4+NMOSD), and myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) constitute distinct entities arising during critical periods of neurodevelopment. Early and effective treatment is therefore crucial to prevent long-term disability. Converging evidence supports a central role of B-cells in central nervous system autoimmunity, extending beyond antibody production to include antigen presentation, cytokine production, and T-cell modu

Tract-based χ-separation imaging differentiates multiple sclerosis from neuromyelitis optica spectrum disorder by mapping iron and myelin signatures.

BACKGROUND: Multiple sclerosis (MS) and neuromyelitis optica spectrum disorder (NMOSD) frequently present with overlapping clinical and radiological features, complicating differential diagnosis. Iron and myelin abnormalities are central to their pathophysiology, yet their distinct contributions in vivo remain poorly characterized. This study applied tract-based χ-separation imaging to evaluate susceptibility-related microstructural changes in normal-appearing white matter (NAWM) and lesions in MS and NMOSD, aiming to identify imaging patterns between these demyelinating diseases. METHODS: Ei

Intractable hiccups as a brainstem red flag: demyelinating disorders with emphasis on neuromyelitis optica spectrum disorder and area postrema syndrome.

BACKGROUND: Persistent and intractable hiccups may indicate brainstem pathology rather than benign gastrointestinal causes. Among inflammatory demyelinating disorders, neuromyelitis optica spectrum disorder (NMOSD) frequently involves the dorsal medulla, particularly the area postrema, a key structure in the hiccup reflex arc. Area postrema syndrome (APS), characterized by otherwise unexplained hiccups, nausea, or vomiting lasting ≥ 48 h, represents a core diagnostic feature of NMOSD. METHODS: A structured narrative literature review was conducted using PubMed, Scopus, and Web of Science data

Sequence Variation of Epstein-Barr Virus EBNA-1 Gene in Central Nervous System Inflammatory Demyelinating Diseases in Pakistan.

CNS inflammatory demyelinating diseases (CNS-IDDs), including multiple sclerosis and related conditions, have increasingly been associated with Epstein-Barr virus (EBV). However, data on EBV genetic diversity, particularly regarding EBNA-1 gene variants, across different CNS-IDD subtypes in South Asian populations remain limited. This study examined EBV DNA prevalence and EBNA-1 sequence variation in 40 patients with CNS-IDD and 40 healthy controls from Pakistan. EBV DNA was found in 21 of 80 samples (26%), with a significantly higher occurrence in CNS-IDD patients (16/40, 40%) compared to hea

Antibodies against MLC1 found in patients with NMOSD-like disease mediate astrocytopathy in rodent models.

The identification of autoantibodies against aquaporin-4 (AQP4) and myelin oligodendrocyte glycoprotein (MOG) has been essential in distinguishing neuromyelitis optica spectrum disorder (NMOSD) and MOG antibody-associated disease (MOGAD) from classical multiple sclerosis (MS), with important implications for treatment. However, for several patients within this disease spectrum, the target of the autoimmune response remains unknown. Here, we describe the modulator of VRAC current 1 (MLC1), a membrane protein with extracellular epitopes enriched at astrocytic end feet, as an autoantigen. Serum M
