[Diffuse large B-cell lymphoma with central pontine myelinolysis without rapid changes in serum sodium levels].

Central pontine myelinolysis (CPM) typically arises from the rapid correction of hyponatremia. A literature search showed that CPM secondary to malignant lymphoma is rare, with only 11 reported cases. An 80-year-old woman presented with anorexia as her primary symptom and was subsequently diagnosed with diffuse large B-cell lymphoma (DLBCL) following an axillary lymph node biopsy. Shortly after treatment initiation, she experienced convulsions and altered consciousness. Brain MRI revealed a symmetrical lesion in the pons, but gadolinium enhancement was not observed and cerebrospinal fluid cyto

Primary Sjogren's Disease Coexisting With Myasthenia Gravis: Two Distinct Autoimmune Diseases Successfully Treated With a Rituximab-Based Induction Regimen.

Primary Sjögren's syndrome (pSS) and myasthenia gravis (MG) are autoimmune diseases and are rarely reported in coexistence. MG is a chronic autoimmune neuromuscular disease where antibodies bind to acetylcholine receptors or to functionally related molecules in the postsynaptic membrane, weakening the skeletal muscles and causing diplopia, ptosis, and difficulty in breathing and swallowing. In this report, we discuss the case of a 43-year-old female patient who presented with dry eyes, weight loss, fatigue, ptosis, dysarthria, and quadriparesis, which ultimately led to the diagnosis of pSS and

Tacrolimus-Related Neurotoxicity of the Pons in Children: Review of the Literature and a Case Report.

INTRODUCTION: Tacrolimus is a potent immunosuppressive agent effective in preventing solid organ transplant rejection. It is widely used following allogeneic liver, kidney, heart, and bone marrow transplantation. Tacrolimus-related neurotoxicity, which can present in up to one-third of patients, manifests with a broad clinical spectrum. Neuroradiological features are classically reported as bilateral and symmetrical lesions involving the parietal and occipital lobes, similar to posterior reversible encephalopathy syndrome. Tacrolimus-related toxicity can also affect other parts of the brain, i

A Rare Case of Osmotic Demyelination Syndrome in Advanced Pancreatic Adenocarcinoma Without Hyponatremia.

Osmotic demyelination syndrome (ODS) is a rare neurological disease involving myelin loss in the central nervous system and is traditionally linked with abrupt correction of hyponatremia. In the absence of hyponatremia, ODS can occur if there are other risk factors such as malignancy, malnutrition, and metabolic disturbances. We describe a 52-year-old male with advanced pancreatic adenocarcinoma and liver metastases who presented with confusion, falls, bilateral leg oedema, hyperglycemia and hypokalemia. Initial imaging was negative for intracranial pathology but positive for pancreatic and he

Appropriate Correction of Hyponatremia in a Patient With Psychogenic Polydipsia: A Case Report.

Hyponatremia, a common electrolyte imbalance characterized by low serum sodium values, can range from mild, asymptomatic cases to life-threatening conditions. Complex etiologies, such as psychiatric disorders, may complicate the presentation and treatment modalities utilized. Additionally, treatment strategies must be personalized, as rapid correction of hyponatremia can lead to severe complications such as osmotic demyelination syndrome (ODS). This case report examines a 69-year-old male with a history of schizophrenia and chronic psychogenic polydipsia who presented with severe hyponatremia,

Neurologic and psychiatric disorders following correction of profound hyponatremia - A cohort study.

BACKGROUND: Rapid correction of profound hyponatremia (serum sodium <125 mmol/L) has been associated with osmotic demyelination syndrome (ODS) but this has been challenged. Potential associations with other neurological and psychiatric diseases have not been studied. METHODS: The basis of this study was the Stockholm Sodium Cohort, a laboratory data repository covering inhabitants of the Stockholm Region who had a sodium concentration analyzed (2005-2018, n = 1632,249). Patients admitted with profound hyponatremia for the first time during the study period where sodium correction rates during

Osmotic demyelination syndrome: A pediatric case report and a systematic literature review.

Osmotic Demyelination Syndrome (ODS) is a life-threatening neurologic disorder including two distinct types of demyelination: Central Pontine Myelinolysis (CPM) and Extrapontine Myelinolysis (EPM). While it's clear that the fast correction of low sodium levels is a key precipitating factor, the underlying pathophysiological mechanisms that cause ODS are still being explored. The present paper describes a case of an eleven-year-old male patient diagnosed with ODS based on typical imaging findings. This case report sounds like it could be significant by providing a thorough overview of the patho

Osmotic Demyelination Syndrome Following Rapid Correction of Hyponatremia in a Young Woman: A Case Report and Review of Literature.

Osmotic demyelination syndrome (ODS) is a rare and devastating neurological condition linked with the rapid correction of serum hyponatremia. We present a case report of a young female patient who developed ODS following an aggressive correction of low serum sodium levels. ODS is characterized by demyelination in the central and extrapontine regions of the brain, resulting in disastrous outcomes. The pathophysiology involves disruption of the blood-brain barrier (BBB) due to a sudden rise in serum sodium, which leads to astrocyte dysfunction secondary to osmotic shift, leading to inflammation,

Central Pontine Myelinolysis in a Patient with Alcohol Use Disorder without Hyponatremia: A Case Report.

BACKGROUND Central pontine myelinolysis (CPM) is an osmotic demyelination syndrome most commonly observed in patients with chronic hyponatremia who undergo rapid serum sodium correction. Risk factors for CPM include malnutrition, hypokalemia, advanced liver disease, hyperemesis gravidarum, and alcohol use disorder. In this case report, we present an unusual case of CPM in a 30-year-old man with alcohol use disorder who did not have hyponatremia during hospitalization and had no history of chronic hyponatremia. Osmotic Demyelination Syndrome in Hyperosmolar Hyperglycemic State With Contrasting Initial Sodium Levels: A Report of Two Cases.

Osmotic demyelination syndrome (ODS) is typically associated with rapid correction of severe hyponatremia but is a rare complication of hyperosmolar hyperglycemic state (HHS). We describe two cases of ODS developing in HHS with contrasting initial sodium levels: one with profound hyponatremia (Na 112 mmol/L, corrected 167 mmol/L) and the other with hypernatremia (Na 149 mmol/L, corrected 177 mmol/L). Both patients exhibited severe hyperglycemia (2,398 and 1,277 mg/dL, respectively) and marked hyperosmolality (387 and 421 mOsm/kg) and were managed with cautious correction rates (1.2 and 1.0 mOs

Central pontine myelinolysis caused by hyperglycemia: A case report and mechanistic review.

This report highlights a rare case of a man in his 50s with central pontine myelinolysis who initially presented with dysphagia and dysarthria. The patient also exhibited unsteady gait, dizziness, and weakness in all extremities. Imaging studies revealed a typical "bat-wing" lesion at the base of the pons. The patient was diagnosed with central pontine myelinolysis secondary to a diabetic hyperglycemic hyperosmolar state complicated with hypernatremia. Following insulin therapy and rehydration, his symptoms were resolved within 1 week, while the imaging-detected lesions persisted. In most case

Central pontine myelinolysis in diffuse large B-cell lymphoma, a case series and literature review.

INTRODUCTION: Diffuse large B-cell lymphoma is the most common type of non-Hodgkin lymphoma in Saudi Arabia. Osmotic demyelination syndrome (ODS) is divided into central pontine myelinolysis (CPM) and extrapontine myelinolysis. The occurrence of osmotic demyelination syndrome is classically described in the setting of rapid correction of hyponatremia. Clinical Manifestations are variable, ranging from asymptomatic cases to quadriparesis and locked-in syndrome in the severe form. METHOD: We reported a total of six cases with diffuse large B-cell lymphoma (DLBCL) that developed central pontine m

An Unusual Case of Extrapontine Myelinolysis in a Post-partum Woman with Hypernatremia.

Hypernatremia is a condition that can lead to severe neurological symptoms, including flaccid paralysis, cognitive impairment, encephalopathy, and coma. Osmotic demyelination syndrome (ODS), which encompasses central pontine myelinolysis and extrapontine myelinolysis (EPM), is a known complication of the rapid correction of sodium levels. Limited data exist on the occurrence of ODS in post-partum women, particularly those with hypernatremia. Here, we present a case of a post-partum lady who presented in an unconscious and agitated state with hypernatremia and was found to have imaging characte

Osmotic demyelination syndrome with normal serum sodium in an alcohol-dependent patient: a case report.

Osmotic demyelination syndrome (ODS), which embraces central pontine myelinolysis and extrapontine myelinolysis, is a well-known but uncommon disorder of the central nervous system. The disease primarily occurs after rapid correction of severe hyponatremia. However, excessive drinking is one of the rare etiologies of ODS. Cases of alcohol-related ODS without documented hyponatremia are rarely reported, and optimal therapeutic approaches remain unclear. We report a patient in his 30's with a history of heavy drinking who presented with unsteady gait and limb tremor as the main clinical manifest

Development of central pontine myelinolysis from hepatic injury and liver transplantation: A narrative review.

Controlling serum osmolarity levels is a homeostatic mechanism that allows the body to manage sudden changes in water and electrolyte concentrations. Rapid correction of hyponatremia is an example of how such an imbalance might be addressed; however, this may give rise to serious consequences. Among these is a neurological condition called central pontine myelinolysis (CPM), which often develops from a swift correction of hyponatremia. Patients who present with a liver injury (LI), such as cirrhosis and alcoholic liver disease, may suffer from hyponatremia, thus making them susceptible to CPM.

Chronic hyperglycemia-related central pontine myelinolysis: A case report and a literature review.

INTRODUCTION: Central pontine myelinolysis (CPM) is a rare condition that has been reported to occur in association with rapid correction of hyponatremia. Chronic hyperglycemia is an extremely rare cause of CPM with no other metabolic disturbances. Our aim was to describe the case of a patient who presented with CPM associated with chronic hyperglycemia. OBSERVATION: We report the case of a 55-year-old male with a personal history of diabetes mellitus who presented, over two months, gait disorders, slurred speech, and dysphagia. All laboratory tests were within normal limits except for glycat

A Rare Fatal Case of Hemophagocytic Lymphohistiocytosis With Central Pontine Myelinolysis in the Setting of Epstein-Barr Virus Reactivation.

Hemophagocytic lymphohistiocytosis (HLH) is a rare hyperinflammatory disorder associated with infections, malignancies, autoimmune conditions, and inflammatory states. HLH is characterized by unregulated cytokine release and immune activation, leading to widespread tissue damage. In this case, we describe a male in his 30s who presented to the emergency department with concerns about sepsis after being diagnosed with mastoiditis a week earlier. Further investigation revealed persistent fevers, pancytopenia, hepatosplenomegaly, hyperferritinemia, and hypertriglyceridemia, fulfilling the diagnos
