Transverse myelitis is one of the most serious neurological complications occurring in the course of systemic lupus erythematosus. We describe two lupus patients, with transverse myelitis, one of whom had associated optic neuritis. In both, magnetic resonance imaging of the spinal cord showed an abnormal signal. In one case a good response to steroid and immunosuppressive therapy was observed; the other case failed to improve despite the therapy applied.

Eighteen adult patients presenting with acute transverse myelitis (ATM) were evaluated using magnetic resonance imaging. Only 7 had abnormal scans showing an area of increased signal intensity within the cord solely on T2 weighted images; T1 weighted images were normal. The MRI abnormality did not correlate with the cause of the transverse myelitis, the extent of maximum neurological deficit, or the prognosis. A scan performed more than 5 days after the onset of disease was most likely to be positive. Even though the prognostic value of MRI in ATM may be limited, it remains a valuable techniqu

We report the clinical features and laboratory-instrumental findings in two patients with acute monophasic demyelinating disease with focal clinical expression: one case of incomplete cervical transverse myelitis and one of focal supratentorial encephalitis. We describe the clinical course and evolution of the neuroradiological findings (CT and MRI) and discuss their pathological meaning and diagnostic value.

The case of a patient with pathologic Stage 1A Hodgkin disease is reported in whom subacute paraneoplastic myelopathy developed before treatment. Myelography, computed tomography, magnetic resonance imaging, and lumbar puncture examination showed no evidence of central nervous system involvement. The patient was treated with combination chemotherapy (to avoid spinal cord exposure to radiation) without neurologic improvement. The patient's neurologic condition responded well to intrathecal dexamethasone.

MR imaging was performed on a patient with syphilitic myelitis. T2-weighted images showed high intensity areas in the thoracic spinal cord. On T1-weighted images after gadolinium-DTPA injection, heterogeneous enhancement was observed in the superficial portion of the spinal cord. The intramedullary high intensity areas on T2-weighted images disappeared after antibiotic therapy.

A 54-year-old man received insertion of an acupuncture needle into the region extending from the posterior neck to the back on two occasions for the treatment of shoulder stiffness. Two weeks after the second acupuncture, he developed fever, dysarthria and mictionary disturbance, finally reaching the condition of tetraplegia. He was immediately admitted to an emergency room in our hospital, and was diagnosed as sepsis with DIC, ARDS, heart failure, renal failure, liver failure, and myelitis. After one month, he recovered with transverse myelopathy as a residual deficit. Neurological findings s

In a man aged 28 clinical picture, myelography and computerized myelotomography demonstrated epidural haematoma at the level of Th1-Th2 vertebrae with associated arachnoidal cyst. Owing to initially erroneous diagnosis of transverse myelitis the haematoma was diagnosed only after 7 days and removed surgically with simultaneous evacuation of the cyst. The therapeutic result was minimal.

This is a report of a 20-year-old man with typical features of Devic disease (neuromyelitis optica) associated with demyelinating peripheral neuropathy. A sural nerve biopsy showed markedly decreased myelinated fibers of a large diameter. Teased fiber preparations showed segmental remyelination in 50% of examined fibers, as well as a few demyelination. Demyelination of bilateral optic nerves, spinal cord, and peripheral nerves at the same time suggests a possibility of common pathogenetic mechanisms in both the central and peripheral nervous systems.

Acute myelopathies (acute transverse myelopathies) can be divided into two major groups. Vascular myelopathies are caused by aortic disease, surgery, or intrinsic vascular disease of cord vessels. Inflammatory/demyelinative myelopathies include lupus erythematosus, various viral infections, and primary demyelinating disease. Criteria for diagnostic decisions and recent data from magnetic resonance imaging are reviewed.

We describe a patient with systemic lupus erythematosus (SLE) who presented with acute transverse myelitis. Magnetic resonance imaging (MRI) of the cervical spine demonstrated increased signal intensity and diffuse edema in the cervical cord. The patient had low titer of IgG antiphospholipid antibodies and hypocomplementemia. Cerebrospinal fluid analysis showed pleocytosis, increased protein and decreased glucose. Clinical improvement of the neurologic impairment was noted after high dose corticosteroids and intravenous bolus cyclophosphamide. A repeat MRI 12 days after the treatment was norma

Acute transverse myelitis (ATM) is a well recognized clinical entity, though its etiology remains obscure. Only a few reports of magnetic resonance imaging of ATM appear in the literature. These reports describe conflicting findings with respect to the signal intensity of the spinal cord on long repetition time (TR) sequences. The purpose of this study is to present our experience with five cases of ATM in which long TR sequences demonstrated abnormal increase in signal intensity of the cord. Magnetic resonance imaging also demonstrated extension of abnormal cord signal intensity over at least

Sixteen cerebrospinal fluid (CSF) specimens serially obtained during long-term follow-up of two patients with Devic's neuromyelitis optica (DNO) were compared with 65 CSF samples from patients with multiple sclerosis (MS). By statistical analysis, the CSF profile in DNO was found to differ from that observed in MS, mainly showing pleocytosis, blood-brain barrier damage, and absence of persistent immunoglobulin G synthesis within the central nervous system. Oligoclonal bands, detected with isoelectric focusing, were present in CSF of 92% of the patients with MS, and in three CSF specimens from

Sixty-two consecutive patients with clinically definite multiple sclerosis (MS) were classified into 2 subgroups: group A, consisting of 16 patients who had shown acute transverse myelopathy (ATM) during the course of illness; and group B, 46 patients without ATM. The clinical features of these 2 groups were analysed prospectively for certain periods, and some significant differences were found. There was (1) later onset, (2) less frequent occurrence of brain stem, cerebellar and cerebral symptoms, (3) more frequent and severe involvement of the optic nerve, (4) a smaller proportion of patient

Acute transverse myelitis (ATM) is a disorder of the spinal region of the central nervous system. In the present case, the clinical course showed ATM associated with mycoplasma pneumoniae (MP) and little recovery; the girl was left with a flaccid paraplegia, and thus differed from other cases in the literature which all reported complete or near-complete recovery.

Brainstem auditory evoked potentials (BAEP) were studied on 38 optic myelitis (OM) and 16 of myelopathy with definite abnormal VEP. Forty-two normal subjects were studied for comparison. The results showed that BAEP were abnormal in 42% of the OM and 38% of the latter myelopathy groups. Among the abnormal BAEP 82% showed V wave abnormality, 32% prolongation in I-III or III-V interpeak latency or absence of III wave, and 59% with unilateral lesions. These findings would imply that the lesions were mostly on the white matter and small and localized in character which was compatible with the path

Myelopathy is a well recognised but infrequent neurological manifestation of systemic lupus erythematosus (SLE). The case of a 27 year old woman with SLE of seven years' duration who developed a spastic paraparesis during her second pregnancy is reported. Magnetic resonance imaging did not show any intrinsic abnormality of the spinal cord. Anticardiolipin antibody was weakly positive and C4 was low. The patient responded dramatically to steroids.
