Neuromyelitis optica with unusual vascular changes.

A case of neuromyelitis optica had strikingly increased numbers of thickened, hyalinized small blood vessels, which were associated with cystic degeneration in the cervical spinal cord and the optic chiasm. These findings support the contention that neuromyelitis optica is a clinical syndrome rather than a distinct nosologic entity.

Transverse myelitis from intraarterial penicillin.

A child was given intramuscular benzathine penicillin and experienced manifestations of sudden, irreversible transection of the spinal cord in the lower thoracic region. The biopsy supported an intravascular injection with occlusion of the spinal vasculature as the etiological mechanism. A review of similar cases reveals a recurring pattern--intramuscular injection with standard techniques and sites into a small muscle mass without evident blood return followed by rapid progression of paralysis. The problem seems to turn upon an inability to recognize the inadvertent intraarterial injection.

[Our experiences of PISCES (percutaneously inserted spinal cord electrical stimulation) in SMON and other neurologic disorders].

Percutaneously inserted spinal cord electrical stimulation (PISCES) was carried out in eleven intractable pain cases and in one spastic paraplegic case. The causes of intractable pain constitute subacute myelo-optic neuropathy (SMON) 6 cases, cerebrovascular disease 2 cases, multiple sclerosis (MS) 1 case, Charcot-Marie-Tooth (CMT) 1 case and transverse myelitis (TM) 1 case. The cause of spastic paraplegia was due to the ossification of posterior longitudinal ligament (OPLL). A trial stimulation was performed about two weeks before planning a permanent implantation of PISCES system. For the tr

[Acute neurologic complications of rubella].

Two cases of rubella with neurological complications are reported. In one case an extensive atypical polyradiculoneuritis was associated with a late albumino-cytologic dissociation. The other presented as a transverse myelitis with meningitis and radiculitis. In neither case was an history of contagion or recent vaccination elicited, and no eruption appeared in either patient. Clinical onset was sudden, with an acute meningitis followed several days later by neurological deficits. The course was marked by successive phases of early rapid deterioration, stabilization and slow regression over se

Immunoreactive myelin proteolipid, protein-like activity in cerebrospinal fluid and serum of neurologically impaired patients.

Two hundred seventy-eight samples of cerebrospinal fluid and 273 samples of sera from a variety of neurological patients were examined by radioimmunoassay for myelin proteolipid protein-like reactivity as an index of structural damage to myelin. Groups of patients with active central nervous system damage could be distinguished easily, although there was a small percentage of false positive reactions. The reactivity was most marked in the demyelinative, encephalitic, and stroke categories. A radioimmunoassay for proteolipid protein of proper sensitivity and specificity may prove to be a useful

Absence of evoked potential abnormalities in acute transverse myelopathy.

We studied 12 consecutive patients with inflammatory acute transverse myelopathy (ATM) and no prior history of neurologic disease. All had normal pattern shift visual and brainstem auditory evoked potentials. All of nine patients tested also had normal median nerve short-latency somatosensory evoked potentials. None developed new neurologic signs in 18 months' mean follow-up. In contrast, 72% of 50 patients with definite, probable, or possible multiple sclerosis (MS) had abnormalities in at least one evoked potential test. When defined as a virtually or completely transverse inflammatory lesio

[Prognostic value of cortical evoked potentials in spinal-spinal cord pathology: study of 1 clinical case].

The clinical use of S.E.R. (Somesthesic Evoked Responses) from the lower limbs has peculiar prognostic value in cases of spinal pathology with clinical signs of transection, where other neuroradiological investigations are not helpful. However, in the present case of acute trasverse myelitis, persistence of an evoked response from the lower limbs was never followed by signs of recovery below the lesional level, while autopsy showed a complete degeneration of the descending spinal pathways with a good preservation of the posterior roots. The limits of this technique appear connected to the fact

Acute transverse myelopathy complicating tetanus.

A 12-year-old boy presented with typical features of short incubation tetanus. Spastic paraparesis with a mid-thoracic sensory level was present from the time of the initial examination. X-rays of the spine and lumbar myelogram were normal. The paraparesis improved rapidly and within a month the patient was free of neurological deficit. The case is possibly a rare example of tetanus involving the central nervous system other than the anterior horn cells.

[Epidural spinal electrostimulation (ESES) in patients with chronic pain and central motor disturbances (author's transl)].

Epidural spinal electrostimulation (ESES), as method in the treatment of patients with chronic pain or severe central motor disturbances, especially spastic paresis of spinal origin and bladder dysfunction, is indicated when conservative measures prove ineffectual and before surgical intervention is considered. The biochemical and innervation processes which are brought about by ESES are discussed, as well as the literature on the efficacy and possible complications of the method. Twenty cases were subjected to a test stimulation and in twelve of these the stimulation system was implanted. Spi

Sensory nerve stimulation and evoked cerebral potentials.

Cerebral potentials were evoked by stimulating sensory fibers of the median, ulnar, radial, musculocutaneous, sural, superficial peroneal, and saphenous nerves. The sensory nerve action potential was simultaneously recorded from these nerves, allowing comparison of peripheral and central sensory conduction times. Somatosensory potentials (SEPs) elicited by sensory nerve stimulation are of particular value in evaluating proximal segments of peripheral as well as central somatosensory pathways.

[Ileal autoplasty of a sclerotic urinary bladder in the presence of a traumatic spinal cord lesion].

The authors elaborated an operation--enterocystoplasty combined with direct and transcutaneous electric stimulation of the artificial urinary bladder in its sclerosis (contracted urinary bladder) in patients with affection of the spinal cord. The results of 21 operations with follow-up periods of 9 years are discussed. Three patients died. The results in the remaining patients were good: the act of urination was restored, the urge to urinate reappeared, and the ability to hold the urine was restored. The restoration of the act of urination was conducive to preservation of the renal function an

Dectection of spinal cord lesion using skin electrode recordings of spinal evoked potential.

The multi-channel recording of somatosensory spinal evoked potential (SSEP) was carried out on 30 patients with various neurological disorders. In some cases, SSEP changes corresponded well with the lesioned level. In general, the lesions of thoraco-cervical level were difficult to detect, because the appearance rate of SSEP peaks are reduced over the thoraco-cervical spine even in normal controls. In cases with spinal type of MS, SSEP revealed marked changes and subclinical sensory demyelinating lesions were detectable. Chronological SSEP recording in patients with acute transverse myelitis r

Spinal cord involvement in systemic lupus erythematosus.

A review is made of the literature on central nervous system involvement in systemic lupus erythematosus (CNS-SLE). Particular attention is focused on lupus myelopathy. Referring to two well-documented personal cases, the authors describe the characteristic multisystem involvement, protean clinical pattern, serum and cerebrospinal fluid changes, histoimmunologic findings, response to corticosteroids and immunosuppressive agents and some prognostic features.

Uncommon neurological presentations of tuberculoma.

The neurological manifestations and autopsy findings of 3 cases of tuberculoma have been described. A cerebello-pontine angle tuberculoma presented with a long history of trigeminal neuralgia. Intramedallary tuberculoma of the spinal cord presented with transverse myelitis, history of one complete remission and a normal myelogram. A tuberculoma of the medulla led to, apart from other features, severe orthostatic hypotension. Tuberculous meningitis, as a terminal process, developed in the first two cases and the third patient died from a vasomotor collapse on suddenly standing up from bed.

Schistosomiasis of the spinal cord.

Schistosomiasis of the spinal cord is a rare presentation of a disease involving over 200 million people. A patient from an endemic area presenting as a transverse myelitis or a spinal cord tumor, and with eosinophilia, should alert the physician to the possibility of schistosomiasis of the spinal cord. Diagnosis is based on finding the characteristic eggs in the stool or urine, or if necessary, by rectal, bladder or liver biopsies. Myelography is performed to determine if decompressive laminectomy is necessary.
