A previously healthy 63-year-old man presented with sudden-onset headache and left-sided clumsiness. Computed tomography revealed a linear hematoma extending from the cerebellar vermis to the left cerebellar hemisphere. Brain magnetic resonance imaging revealed an incidental thrombus occlusion of the left internal jugular vein. Based on his history of unprotected sexual intercourse, mild cognitive impairment, laboratory findings, and cerebrospinal fluid tests, he was diagnosed with meningovascular neurosyphilis. Treatment with intravenous penicillin G resulted in the resolution of both hemorrh

BACKGROUND: The rates of HIV and syphilis in Saskatchewan (SK) have been rising rapidly in recent years. The syndemic has raised concern for neurosyphilis, a complication that can occur at any stage of syphilis and is more common in people living with HIV (PLWH). Criteria published by the Public Health Agency of Canada recommends considering a lumbar puncture (LP) in patients with concomitant HIV and syphilis infection whose rapid plasma reagin (RPR) titre is ≥1:32 or whose CD4+ count is ≤350. We assessed whether this recommendation was met at 2 comparable clinical sites.
METHODS: In this ret

INTRODUCTION: Adverse prognostic events (APE) of neurosyphilis include ongoing syphilitic meningitis, meningovascular syphilis, parenchymatous neurosyphilis and death. Its complexity and rarity have the potential to result in the underestimated true burden of neurosyphilis worldwide, due to lack of recognition and under-reporting. The unmet need for a modern method of refined and targeted treatment of neurosyphilis is strengthened by the currently various distinct diagnostic criteria. The DEep learning Framework for pErsonalized prediction of Adverse prognosTic events in NeuroSyphilis study wi

Syphilitic alopecia (SA) and ocular syphilis (OS) are rare manifestations of syphilis, typically reported together in immunocompromised patients. We present a unique case of mixed-pattern, essential, SA of the scalp and OS in the form of anterior uveitis in an otherwise immunocompetent, HIV-negative, 46-year-old female. Briefly, the patient initially presented with sudden-onset scalp hair loss that was initially diagnosed as atypical alopecia areata but failed to respond to typical alopecia areata-directed therapy. Concurrently, the patient developed ocular and neurologic symptoms, and the dia

Neurosyphilis (NS) is a chronic central nervous system infection caused by Treponema pallidum. Owing to its diverse clinical manifestations and the limited sensitivity of current diagnostic methods, NS is difficult to diagnose. Understanding the molecular mechanisms of NS and identifying reliable biomarkers are essential for improving diagnostic and therapeutic strategies. This study employed Mendelian randomization (MR) analysis to explore the causal relationships among protein ratio quantitative trait loci (rQTLs), cerebrospinal fluid (CSF) metabolites, and syphilis risk at various stages. Neurosyphilis remains an important and highly morbid neurologic infection. Recent years have seen a dramatic increase in the incidence of syphilis across a variety of populations, making recognition of the many neurologic complications of this infection still relevant for modern clinicians. As manifestations of neurosyphilis are famously varied, a high index of suspicion is essential, especially in the face of atypical presentations. In this review, we discuss updates to the epidemiology, range of clinical presentations, diagnosis, and management of neurosyphilis. ANN NEUROL 2025;98:1070-1076.

This report discusses a case of diagnosing neurosyphilis in a non-classical presentation with confounding test results needing a deliberate and multidisciplinary diagnostic approach. A 38-year-old immunocompetent male presented with uveitis and a skin rash. Although serology was positive for syphilis (rapid plasma reagin 1:128), it was also positive for tuberculosis, and a dermatology consult identified the rash as psoriasis, creating a complex diagnostic picture. Based on a high suspicion of ocular syphilis, the patient received intravenous penicillin, which led to the complete resolution of

The study presents the results of a 2-year research project on the therapeutic practices in psychiatry at the University Clinic of Vienna under the leadership of Hans Hoff from 1951 to 1969. Hoff took over as Head of the Department of Psychiatry and Neurology shortly after his return to Vienna from whence he had had to flee after Austria's "Anschluss" (annexation) to Nazi Germany in 1938 because of his Jewish background. The focus of the investigation was on the extent, methods and justifications for the application of four highly invasive somatic treatments, which were developed during the in

Ocular syphilis is a rare and easily preventable disease that can progress quickly to permanent blindness. Symptoms can mimic other ocular pathologies and further delay diagnosis. The incidence of syphilis continues to rise each year and represents significant morbidity in high-risk communities. In this case, the patient presented with unilateral blindness, uveitis, and a maculopapular rash on the lower extremities. Initially, she was found to have a corneal abrasion, leading to a missed diagnosis of ocular syphilis and delayed treatment. This case brings awareness to ocular syphilis as a key

OBJECTIVES: Neurosyphilis is a serious complication of Treponema pallidum infection. Some patients experience relapse or serofast response despite standard intravenous penicillin G. While adjunctive benzathine penicillin is recommended in some guidelines, its optimal duration and effectiveness remain unclear. This study aimed to identify predictors of serological response and evaluate the effect of prolonged therapy in HIV-negative neurosyphilis.
METHODS: We retrospectively analyzed 246 HIV-negative neurosyphilis patients treated at three hospitals from 2011 to 2024. Patients received either

Ocular manifestations of syphilis can occur at any stage of the disease and present with a wide range of clinical features. If left untreated, they carry a high risk of permanent vision loss. Although syphilis mainly affects individuals with human immunodeficiency virus (HIV) positive and other immunocompromised individuals, it can also present in immunocompetent individuals. A 61-year-old immunocompetent male presented with blurry vision and associated central vision loss in the left eye. Initial ophthalmologic evaluation revealed bilateral optic-disc swelling and serological tests positive f
