Penicillin or Ceftriaxone for Neurosyphilis in People With HIV: A Retrospective, Propensity Score-Matched Multicenter Study in Mexico.

BACKGROUND: Neurosyphilis, ocular syphilis, and otosyphilis occur more frequently among people with HIV-1 and are associated with higher morbidity and treatment failure. Intravenous penicillin G remains the standard therapy but requires hospitalization or prolonged intravenous access, creating barriers that are amplified in resource-limited settings facing recurrent penicillin shortages. Ceftriaxone is a potential alternative; however, comparative data in people with HIV are limited. METHODS: We conducted a multicenter retrospective cohort study of adults with HIV at 5 public hospitals and 2

Two cases of early neurosyphilis with ocular involvement: The need for specialist consultations in patients with early syphilis.

BACKGROUND: Syphilis is a systemic infectious disease caused by Treponema pallidum subspecies pallidum. The incidence of syphilis is increasing in Europe and, similarly, in Poland and was 9.9 cases per 100,000 inhabitants in European Union countries in 2023. The central nervous system (CNS) involvement may occur at any stage of untreated syphilis. OBJECTIVE: To demonstrate the need for specialist consultations in patients with early syphilis. MATERIAL AND METHODS: We present two, epidemiologically linked, HIV-negative cases of secondary syphilis with CNS and, in particular, ocular involvemen

Ischemic cerebrovascular accident in an HIV patient: Neurosyphilis and antiphospholipid syndrome.

HIV infection is a risk factor for the development of cardiovascular diseases including ischemic strokes, for which there are different multiple etiologies in these patients. We present the case of a 27-year-old man with AIDS, with an ischemic stroke in the left middle cerebral artery territory. Laboratory studies confirmed the diagnosis of neurosyphilis but also the existence of antiphospholipid antibodies. Seven months after the treatment of neurosyphilis, anticoagulation and antiretroviral therapy, the persistence of antiphospholipid antibodies was confirmed. The diagnostic approach to a va

Progressive exacerbation of neurological symptoms in a patient with neurosyphilis following T9-T10 intradural space-occupying lesion resection: a case report.

BACKGROUND: Neurosyphilis is a disease of the central nervous system (CNS) caused by Treponema pallidum (syphilis spirochete), which usually presents with a variety of nonspecific symptoms and is easily misdiagnosed. Although Treponema pallidum can remain latent after treatment, reactivation may occur in the presence of a compromised or immunosuppressed immune system. This article reports a case of reactivation of neurosyphilis after spinal surgery, with the aim of exploring the possible role of immunosuppression in the process. CASE PRESENTATION: The patient was a 75-year-old female admitted

Diagnosis and Treatment of Neurosyphilis Complicated by Implicit Clinician Bias and Unconscious Stigma: A Case Report and Literature Review.

Syphilis, a condition known as "the great imitator," is a sexually transmitted infection caused by the bacterium Treponema pallidum that can present with a myriad of symptoms, including psychiatric and neurological changes. Severe complications of syphilis are rare nowadays due to the widespread availability of antibiotics. Nonetheless, the incidence has been increasing over the past few years. We describe the case of a middle-aged man who presented with neuropsychiatric symptoms that were initially thought to be secondary to substance use and who was eventually diagnosed with neurosyphilis. A

The use of non-mydriatic ocular fundus imaging in the emergency department for detection of ocular syphilis presenting as presumed "papilledema": a case series.

BACKGROUND: Ocular syphilis and neurosyphilis can present as seemingly "isolated" bilateral optic disc edema mimicking papilledema from intracranial hypertension. As a result, patients are often sent to the emergency department (ED) for "papilledema work-up," which can lead to delayed or missed ocular syphilis diagnosis since serologic testing for syphilis is not routinely performed in patients with presumed "papilledema." Our study describes eight patients initially referred to our ED for "papilledema" workup who were ultimately diagnosed with ocular syphilis based on characteristic findings

Stage-Specific Reduction of Serum IL-32 in Secondary Syphilis and Associations with Lipid Metabolism Parameters.

OBJECTIVES: Interleukin-32 (IL-32) is a pro-inflammatory cytokine implicated in the pathogenesis of multiple inflammatory diseases, but its expression pattern and clinical significance in syphilis have not been fully elucidated. This study aimed to determine serum IL-32 levels in different stages of syphilis and evaluate its potential value in clinical staging and diagnosis. METHODS: A total of 166 patients with syphilis (secondary syphilis, late latent syphilis, sero-resistant syphilis, and neurosyphilis) and 30 healthy controls were enrolled. Serum IL-32 concentrations were detected using e

Functional network analysis in hepatolithiasis: identifying novel therapeutic targets through whole-exome sequencing.

BACKGROUND: Hepatolithiasis (HL) is a prevalent condition in hepatobiliary surgery, often complicated by hepatatrophia. This study aimed to identify gene mutations in HL specimens with hepatatrophia and construct a mutation landscape using whole-exome sequencing (WES). METHODS: Genetic variants and copy number variations (CNVs) were compared between HL (n = 10) and normal control (NC, n = 10) specimens, encompassing 20 participants in total, using WES data. Hub genes within HL specimens were identified via the GeneCards database. The mutation frequencies and interaction patterns of these hub

Immune-inflammatory-nutritional dysfunction in HIV-negative patients with asymptomatic neurosyphilis: a retrospective comparative study.

OBJECTIVES: The host's immune function, inflammatory response, and nutritional status are closely related to the occurrence and development of neurosyphilis (NS). Asymptomatic neurosyphilis (ANS), as a completely curable type of NS, has received limited study on these fields. This study explores the immune function, inflammatory response, and nutritional status levels in patients with ANS, as well as their changes after treatment, aiming to provide reference for the early diagnosis and treatment of ANS. METHODS: This study retrospectively included 32 healthy controls (HCs), 27 cases of ANS, a

Implementation and Innovation in Syphilis Diagnostics: Insights from Global Health Systems, Molecular Testing, and Self-Testing Strategies.

BACKGROUND: Syphilis remains a persistent global public health problem. Given that low-cost diagnostics and highly effective therapies have been available for decades, the current epidemic is driven by structural weaknesses in health systems, stigma, absence of political will and failures to integrate new technologies into routine care. METHODS: At the Shanghai 2025 Syphilis Meeting, experts highlighted the evolving landscape of syphilis diagnostics. The presentations offered an overview of our changing understanding of seronegative primary disease, reinfection patterns, and asymptomatic muco

Technological innovations and breakthrough pathways in syphilis diagnosis: addressing global resurgence and the journey toward elimination.

The global resurgence of syphilis, characterized by a precipitous rise in incidence and the persistence of congenital transmission, has necessitated a critical re-evaluation of traditional diagnostic paradigms. This review synthesizes recent technological innovations in the detection of Treponema pallidum, spanning from molecular precision to decentralized point-of-care (POC) accessibility. This review synthesizes the paradigm shift from traditional darkfield microscopy (DFM) toward advanced nucleic acid amplification tests (NAATs). By evaluating innovations such as CRISPR-based assays and tra

From seropositivity to neurosyphilis: clinical spectrum and diagnostic classification in hospitalized neurology patients: a 16-year retrospective study.

BACKGROUND: Neurosyphilis remains a diagnostic challenge because of its heterogeneous neurological manifestations and the absence of a universally accepted diagnostic gold standard. Diagnostic categories such as confirmed and probable neurosyphilis may help stratify syphilis-seropositive patients according to clinical and cerebrospinal fluid findings. This study aimed to evaluate syphilis seropositivity among hospitalized neurology patients and to compare clinical, radiological, and discharge neurological characteristics across diagnostic groups. METHODS: This single-center retrospective obse

Papulonecrotic tuberculid mimicking lues maligna in an immunocompromised male.

Papulonecrotic tuberculid (PNT) is a rare hypersensitivity reaction to Mycobacterium tuberculosis, typically seen in immunocompetent individuals but occasionally reported in human immunodeficiency virus (HIV)-positive patients. We describe the case of a 30-year-old HIV-positive male with high-risk sexual behavior presenting with crusted papulopustular lesions and severe headache. Cerebrospinal fluid and venereal disease research laboratory (VDRL) were positive for VDRL and treponema pallidum hemagglutination confirming neurosyphilis, while skin biopsy revealed PNT. This case underscores the di

BACKGROUND Infectious arthritis is an infection of the joint and joint cavities, with Staphylococcus aureus being the most common causative agent. Most cases of infectious arthritis are caused by a single pathogen, with polymicrobial infections being comparatively rare. This article describes a patient with a knee infection caused by Treponema pallidum, Brucella spp., and possibly co-infected with Mycobacterium tuberculosis and Escherichia coli (E. coli). The aim of this paper is to discuss the characteristics of atypical joint diseases caused by multi-pathogenic infections and to provide a re
