Impact of the TeleECHO model: Rapid identification and treatment of syphilis.

Syphilis is a growing concern globally, affecting adult men and women as well as the fetus of infected mothers. Nationally, syphilis disproportionately affects Black and Hispanic women compared with White women, and younger women are reported to experience a disproportionate burden of syphilis with the highest disparities. Syphilis can be difficult to diagnose, particularly if the health care provider is not well-versed in risk factors and signs and symptoms of the disease. TeleECHO is a telementoring model that has been employed worldwide to educate providers on a wide array of health-related

Computational drug design for neurosyphilis disease by targeting Phosphoglycerate Kinase in Treponema pallidum with enhanced binding affinity and reduced toxicity.

Neurosyphilis, a severe neurological complication of syphilitic infection caused by the gram-negative spirochete Treponema pallidum poses significant challenges in treatment due to its irregular physiology and lack of efficacy in present therapeutic strategies. Here, we report a new approach to developing drug treatment that targets the enzyme phosphoglycerate kinase (PGK), an essential component of the T. pallidum glycolytic pathway. Therefore, a ligand was designed involving common neuroprotectant elements reported from literature by a computational drug design method, to increase their bind

Severe Sexually Transmitted Infections: Neurosyphilis, Mpox, and the Tubo-ovarian Abscess.

Most sexually transmitted infections (STIs) are rarely seen by medical providers as life-threatening or as a source of severe sequelae. Human immunodeficiency virus and syphilis screening are still not universal and are affected by provider and patient misconceptions. We chose to discuss 3 STIs that can lead to significant morbidity and even mortality: neurosyphilis, due to its resurgence in the past years, Mpox due to its changing epidemiology and clinical presentation, and tubo-ovarian abscess due to its huge impact on female fertility and morbidity in general.

Neurosyphilis: A rare cause of encephalitis in a young male.

Acute neurosyphilis is a rare but easily treatable cause of subacute confusion. Its clinical presentation is varied, with reported phenotypes ranging from acute stroke-like presentations to psychosis, dementia and meningoencephalitis. Neurosyphilis is easily detectable and treatable, despite its clinical heterogeneity. We present a 45-year-old man without HIV presenting with a subacute encephalitis, with cerebrospinal fluid and blood markers strongly positive for syphilis, with clinical improvement following 2 weeks of empirical intravenous antibiotic therapy.

Ischemic stroke as a presenting feature of neurosyphilis in an immunocompetent adult and attained complete recovery following treatment: a case report and review of the literature.

BACKGROUND: Neurosyphilis typically presents with nonspecific symptoms such as behavioral changes or cognitive impairment, rather than an initial stroke. CASE PRESENTATION: This case report describes a Black African man in his thirties who experienced sudden weakness on the left side of his body. Computed tomography suggested a nucleo-capsular ischemic lesion. However, given his young age and lack of traditional stroke risk factors, further investigations were pursued. Laboratory tests revealed positive results for syphilis in both blood and cerebrospinal fluid. Additional imaging (head and n

Neurosyphilis with stroke-like manifestations: A case report.

RATIONALE: Neurosyphilis is a relatively common condition that can have stroke-like episodes and is usually seen as an ischemic lesion on magnetic resource imaging (MRI). However, we found a case of diffusion-weighted imaging (DWI)-negative neurosyphilis with stroke-like episodes. PATIENT CONCERNS: After hospitalization, MRI of the head was perfected and no new cerebral infarct lesions were seen, and DWI was negative. However, the patient's left limb weakness was significantly aggravated, and serological examination suggested a positive syphilis test. DIAGNOSES: The patient had a sudden onse

Neurosyphilis: Discussing the Diagnostic Challenges.

Neurosyphilis can be difficult to diagnose without a high index of clinical suspicion. The diagnostic process itself may be challenging, especially when there is heavy reliance on CSF findings. Empirical therapy with benzathine penicillin after positive blood serology combined with suggestive clinical findings could be lifesaving for patients.

[A case of young woman with intracranial gumma developed within 1 year after Treponema pallidum infection].

A 17-years-old woman visited the hospital due to convulsions. T2-weighted images showed high intensity areas in right temporal lobe and left frontal lobe. Enhanced T1-weighted images showed mass-like lesions on the dura mater. Based on mononuclear pleocytosis and a reactive fluorescent treponemal antibody-absorption test in the cerebrospinal fluid, neurosyphilis and intracranial gumma were diagnosed, and antibiotic therapy was initiated. After treatments, the high intensity areas improved, and she had no recurrence of symptoms or MRI images. Intracranial gumma usually develops in tertiary syph

Syphilitic gummas affecting brain and spinal cord: a case report.

Syphilitic gumma is a rare manifestation of neurosyphilis that can cause mass lesions in the central nervous system. We present an atypical case of a 53-year-old man presenting with syphilitic gummas affecting both the brain and spinal cord. The patient presented with right facial numbness, worsening back pain, gait disturbances, and lower-limb weakness. Serological tests were positive for syphilis, and cerebrospinal fluid analysis showed elevated cell count, protein concentration, and positive syphilis tests. Brain and spinal cord MRI revealed dural-based enhancing mass lesions in the right m

Hypertensive Disc Edema or Ocular Syphilis? A Case Report of the Great Masquerader.

INTRODUCTION: Ocular syphilis is a rare manifestation of syphilis caused by Treponema pallidum which can occur at any stage of infection. It most commonly presents as posterior or panuveitis but can involve various ocular structures, complicating diagnosis. CASE PRESENTATION: We describe a case of a 38-year-old female with a 7-month history of progressive blurry vision, floaters, flashes, and photophobia. Upon presentation to the emergency department, the patient's symptoms were initially attributed to a hypertensive emergency given fundoscopic examination revealing of optic disc edema. Despi

Studies on cognitive performance among older people living with HIV in eastern Europe and central Asia: a scoping review.

BACKGROUND: Despite the growth of the population of older people living with HIV (PLWH), data on cognitive disorders among older PLWH, particularly in low- and middle-income countries, are scarce. These data are especially underrepresented in the literature from eastern Europe and central Asia (EECA). OBJECTIVES: This scoping review aimed to describe the peer-reviewed literature on cognitive health among PLWH in the EECA region. ELIGIBILITY CRITERIA: We selected articles from peer-reviewed journals that reported on cognitive assessments or the prevalence and characteristics of cognitive diso

Optic Neuritis As the Initial Presentation of Syphilis.

Syphilis is transmitted via sexual contact and spreads through the lymphatic and hematologic systems. Ocular syphilis is an uncommon yet serious manifestation of syphilis that can cause significant vision impairment. This report presents a case of ocular syphilis in a 41-year-old otherwise healthy woman who presented with left-sided blurry vision and ocular discomfort. Examination confirmed decreased visual acuity and optic disc swelling in the left eye. Magnetic resonance imaging findings suggested optic neuritis. After ruling out other causes of optic neuritis and considering the presentatio

High-Dose Benzylpenicillin Treatment-Induced Febrile Neutropenia in HIV-Infected Male with Neurosyphilis: Case Report.

Background: Prevention of an irreversible sequalae in neurosyphilis patients requires immediate high-dose intravenous benzylpenicillin administered for a prolonged period of time. However, life-threatening neutropenia has been reported as one of the complications following extended usage of benzylpenicillin. Case presentation: We report a 54-year-old male patient with HIV who developed high-dose benzylpenicillin-induced febrile neutropenia during neurosyphilis treatment. The patient developed a fever of up to 39.8 °C, severe leukopenia (<1 × 109/L), and neutropenia (0.2 × 109/L). He also prese

A retrospective review of nine cases of atypical neurosyphilis.

BACKGROUND: Neurosyphilis may manifest in a form resembling autoimmune encephalitis, primarily characterized by cognitive impairment, psychiatric disorders, and seizures. Brain magnetic resonance imaging typically reveals unilateral or bilateral lesions in the mesiotemporal lobe and may also identify abnormalities in the inferior frontal lobe, parietal lobe, occipital lobe, and thalamus. Nevertheless, there are very few reports addressing this particular aspect. METHODS: We conducted a retrospective review of the medical records of inpatients diagnosed with neurosyphilis who presented with co
