Multifocal continuous theta burst stimulation in idiopathic generalized epilepsy: a cohort study.

OBJECTIVE: Idiopathic generalized epilepsy (IGE) is a disorder of the cortico-subcortical networks. Continuous theta burst stimulation (cTBS) can target these networks to provide an innovative, noninvasive neuromodulation treatment. This study evaluated the safety and efficacy of multifocal cTBS in IGE. METHODS: This single-arm, open-label trial recruited adults with IGE. Following a four-week screening period, participants received one daily cTBS session for six days. Each session involved delivering three cTBS trains (600 pulses) at 80% of resting motor threshold over the vertex. The primar

Idiopathic generalized epilepsy: biases and inconsistencies.

OBJECTIVES: Distinguishing Idiopathic Generalized Epilepsy (IGE) from focal epilepsy (FE) can be challenging when IGE cases present with asymmetric clinical or electroencephalographic (i.e., atypical) features. We aimed to identify factors that lead to discordance in IGE diagnoses among epileptologists. METHODS: 41 patients with IGE and 6 patients with FE or mixed focal and generalized epilepsy followed at the NYU Comprehensive Epilepsy Center for ≥5 years were identified. IGE cases included typical (n = 18) and atypical (n = 23) presentations. Anonymized summaries of patients at initial pres

Retinal neuronal layers in children with idiopathic generalized epilepsy: effects of epilepsy and long-term valproate use.

OBJECTIVE: Idiopathic generalized epilepsies (IGEs) and long-term antiseizure treatment may differentially affect retinal neuronal structures. This study aimed to evaluate optical coherence tomography (OCT)-derived retinal and optic nerve head (ONH) parameters in adolescents with IGEs, in order to explore retinal alterations associated with epilepsy and valproic acid (VPA) exposure. METHODS: This cross-sectional study included three groups: patients with IGEs receiving VPA monotherapy for at least one year (PwE+VPA, n = 35), newly diagnosed patients with IGEs who had not yet initiated antisei

Multicontrast MR-Guided Diffusion Model for Ultra-Low-Dose Brain PET Denoising in Temporal Lobe Epilepsy.

Positron Emission Tomography (PET) is a critical imaging modality in nuclear medicine but requires radioactive tracer administration, which increases radiation exposure risks. While recent studies have investigated MR-guided low-dose PET denoising, they neglect two critical factors: the synergistic roles of multicontrast MR images and disease-specific denoising requirements. In this work, we propose a diffusion model that integrates T1-weighted, T2 fluid attenuated inversion recovery (T2 FLAIR), and hippocampal-optimized (T2 HIPPO) MR sequences to achieve ultra-low-dose PET denoising tailored

Utilization of Induced Pluripotent Stem Cell-Derived Neurons to Investigate the Splice-Modification Efficacy of Splice-Switching Drug Candidates.

Modification of RNA splicing represents a promising therapeutic strategy for treating genetic diseases. Several splice-switching drugs have already been approved, notably for spinal muscular atrophy and specific genotypes of Duchenne muscular dystrophy. While peripheral blood mononuclear cells and fibroblasts are commonly used to evaluate the efficacy of these drug candidates in patient-derived samples, such approaches may lead to misleading conclusions due to the cell type-specific nature of RNA splicing.Induced pluripotent stem cells (iPSCs), which can be differentiated into various somatic

Effective connectivity of face and fear processing in juvenile myoclonic epilepsy.

OBJECTIVE: Individuals with Juvenile Myoclonic Epilepsy (JME) exhibit impaired facial emotion perception, especially regarding fear. However, the underlying neural mechanisms remain to be fully understood. Particularly, the connectivity between emotion-related regions, such as the amygdala, and visual face-processing areas, specifically the Fusiform Face Area (FFA), appears to be relevant for typical facial fear processing. We therefore explored the effective connectivity within the FFA-amygdala network in JME. METHODS: Fourteen individuals diagnosed with JME and 14 healthy controls matched f

GABAergic dynamics in thalamocortical circuits: Insights into absence seizures and hyperinhibition.

Over the last decade, experimental and clinical evidence has shaped the excitation/inhibition imbalance doctrine, which serves as our contemporary understanding of the mechanisms underlying various epilepsies. In addition, emerging findings have highlighted the peculiar role of γ-aminobutyric acid (GABA), the brain's main inhibitory neurotransmitter, as an underappreciated factor in hyperexcitation disorders such as epilepsy. Intriguingly, the unique neuroanatomy and synaptic physiology of thalamocortical (TC) circuits have made this axis a distinctive field of study. In this review, we focus

A systematic review of highly purified cannabidiol in developmental and epileptic encephalopathies and complex treatment-resistant epilepsies: Changes in seizure frequency and adverse events.

OBJECTIVE: The efficacy and safety of a highly purified plant-derived cannabidiol (CBD) oral solution (Epidiolex® [US]/Epidyolex® [EU], EPX) have been established for the treatment of seizures in patients with Lennox-Gastaut syndrome, Dravet syndrome, or tuberous sclerosis complex. These conditions involve diverse etiologies, suggesting EPX may have broad utility across different seizure types. This systematic literature review (SLR) evaluated studies reporting CBD effectiveness and tolerability in patients with other developmental and epileptic encephalopathies (DEEs) and complex treatment-re

Efficacy of Dravet Syndrome Treatments in a Subset of Individuals with 2q24.3 Deletion: A-5 Patient Case Series.

Dravet syndrome (DS) is a developmental and epileptic encephalopathy often resulting from haploinsufficiency of the voltage-gated sodium channel (VGSC) gene SCN1A located on chromosome 2q24.3. Although single-nucleotide changes account for the majority of cases, rare cases are due to 2q24.3 microdeletions involving SCN1A. The 2q24.3 region surrounding SCN1A contains a cluster of VGSC genes including SCN2A, SCN3A, SCN7A, and SCN9A. Prior publications reported larger 2q24.3 deletions affecting multiple VGSC genes being associated with a more severe phenotype. Consensus recommendations for epilep

Cognitive Orientation to daily Occupational Performance Approach using telerehabilitation with parents to improve performance issues in children/adolescents with Dravet Syndrome.

INTRODUCTION: The Cognitive Orientation to Occupational Performance (CO-OP) Approach, a problem-solving intervention process, has been shown to improve activity performance in children with Dravet Syndrome (DS). However, some individuals with DS have cognitive, language and/or behavioral deficits that preclude their ability to participate in the problem-solving process. To date, the CO-OP Approach has never been applied to parents with the goal of enhancing children/adolescent's performance. OBJECTIVE: Explore the feasibility of a telerehabilitation-based CO-OP Approach with parents of indivi

Identification of cognitive phenotypes in temporal lobe epilepsy and genetic generalized epilepsy using robotic assessment.

BACKGROUND: Cognitive dysfunction is common in people with epilepsy (PWE). Although expectations exist for deficits based on diagnosis, phenotypic variation of cognitive deficits is observed within epilepsy subtypes. Classification by severity and type of cognitive deficits has been shown across multiple studies of people with temporal lobe epilepsy (TLE). In this study, we replicate these findings using robotic assessment in TLE and apply the same method in genetic generalized epilepsy (GGE) to uncover potential cognitive phenotypes. METHOD: Participants with TLE and GGE were recruited to pa

The effect of sensory deprivation and mutant gene dosage on seizures and sustained attention in a mouse model of absence epilepsy.

OBJECTIVES: Attention-deficit/hyperactivity disorder (ADHD) is a frequent comorbidity in epilepsy, particularly absence epilepsy, but the mechanistic links between these conditions remain poorly understood. We evaluated attention and seizure susceptibility in a genetically tractable mouse model carrying heterozygous and homozygous mutations in Cacng2, a gene encoding an AMPA receptor-associated membrane protein implicated in both epilepsy and deficits in sustained attention. METHODS: Mice underwent sustained attention testing using the 5-Choice Serial Reaction Time Task (5-CSRTT) and electroe

Isolated absence epilepsy associated with a de novo FBXW7 missense variant in the F-box domain.

The FBXW7 gene encodes a substrate-recognition component of the Skp1-Cul1-F-box (SCF) E3 ubiquitin ligase complex, which targets key regulatory proteins for proteasomal degradation. Recently, loss-of-function FBXW7 variants have been associated with a novel neurodevelopmental disorder characterized by heterogeneous clinical features. Most reported pathogenic variants cluster within the WD40 domains, while variants in other regions, such as the F-box domain, remain poorly characterized. In this study, we performed trio-exome sequencing on a 3-year-old girl with Early-Onset Childhood Absence Epi

KCC2 activation during postnatal development alleviates long-term deficits in CDKL5-deficient mice.

Cyclin-dependent kinase-like 5 (CDKL5) deficiency disorder (CDD) is a severe developmental and epileptic encephalopathy characterized by early onset drug-resistant seizures and later cognitive and social impairments. Existing therapies primarily involve antiseizure medications, which have sedative side effects and lack effective treatments for behavioral impairments. Potassium chloride cotransporter (KCC2) activity is regulated by phosphorylation and is a crucial component of the GABAergic inhibitory system. However, KCC2 dysfunction in CDD remains poorly understood. Here, to investigate poten

Lafora disease gene therapy: EPM2A but not EPM2B overexpression results in Lafora body formation.

Lafora disease (LD) is a fatal teenage-onset neurodegenerative epilepsy caused by loss-of-function mutations of the genes encoding the laforin phosphatase-malin E3 ubiquitin ligase complex. This complex regulates glycogen structural integrity, through yet unclear mechanisms. Deficiencies of the complex lead few glycogen molecules at a time to misshape, precipitate and cumulate into toxic Lafora bodies (LBs) that drive LD. We developed an intrathecal AAV9-based gene therapy for malin-deficient LD and obtained preclinical efficacy without toxicity. A similar gene therapy for laforin-deficient LD
