Isolated absence epilepsy associated with a de novo FBXW7 missense variant in the F-box domain.

The FBXW7 gene encodes a substrate-recognition component of the Skp1-Cul1-F-box (SCF) E3 ubiquitin ligase complex, which targets key regulatory proteins for proteasomal degradation. Recently, loss-of-function FBXW7 variants have been associated with a novel neurodevelopmental disorder characterized by heterogeneous clinical features. Most reported pathogenic variants cluster within the WD40 domains, while variants in other regions, such as the F-box domain, remain poorly characterized. In this study, we performed trio-exome sequencing on a 3-year-old girl with Early-Onset Childhood Absence Epi

KCC2 activation during postnatal development alleviates long-term deficits in CDKL5-deficient mice.

Cyclin-dependent kinase-like 5 (CDKL5) deficiency disorder (CDD) is a severe developmental and epileptic encephalopathy characterized by early onset drug-resistant seizures and later cognitive and social impairments. Existing therapies primarily involve antiseizure medications, which have sedative side effects and lack effective treatments for behavioral impairments. Potassium chloride cotransporter (KCC2) activity is regulated by phosphorylation and is a crucial component of the GABAergic inhibitory system. However, KCC2 dysfunction in CDD remains poorly understood. Here, to investigate poten

Lafora disease gene therapy: EPM2A but not EPM2B overexpression results in Lafora body formation.

Lafora disease (LD) is a fatal teenage-onset neurodegenerative epilepsy caused by loss-of-function mutations of the genes encoding the laforin phosphatase-malin E3 ubiquitin ligase complex. This complex regulates glycogen structural integrity, through yet unclear mechanisms. Deficiencies of the complex lead few glycogen molecules at a time to misshape, precipitate and cumulate into toxic Lafora bodies (LBs) that drive LD. We developed an intrathecal AAV9-based gene therapy for malin-deficient LD and obtained preclinical efficacy without toxicity. A similar gene therapy for laforin-deficient LD

RNA Toxicity and Interacting RNA-Binding Protein NOVA2 of (UUUCA)exp RNA Foci in Familial Cortical Myoclonic Tremor with Epilepsy.

BACKGROUND: Familial cortical myoclonic tremor with epilepsy (FCMTE) is an autosomal dominant neurological disease characterized by cortical myoclonic tremor and epileptic seizures. The proposed pathogenic (TTTCA) pentanucleotide repeat expansion (exp) insertion, flanking the polymorphic (TTTTA)exp, has been reported in seven distinct FCMTE causative genes/loci, and a repeat motif-specific phenotype correlation is claimed. However, the pathogenic mechanism of FCMTE is still poorly understood. METHODS: We investigated how the (TTTCA)exp insertion causes the disease, mainly employing the FCMTE1

Structural targeting of a familial variant of neuroserpin (Ser52Arg) by epigallocatechin gallate binding that reduces polymerization and enhances the tissue plasminogen activator inhibition.

Neuroserpin (NS) inactivation of tissue plasminogen activator (tPA) in brain reduces neurotoxicity. However several familial point mutations in its gene are linked with a neurodegenerative disease termed Familial Encephalopathy with Neuroserpin Inclusion Bodies (FENIB) that results in epilepsy/dementia. Variable expression level of the NS has been linked to several other neurological pathologies like Alzheimer's, glaucoma and ischemic stroke. A familial variant of NS that leads to mortality, Ser52Arg (S52R) was made using site directed mutagenesis. A circular dichroism study indicated close co

Epm2bP71A and Epm2bD148N knock-in mouse models of Lafora disease exhibit distinct and pronounced neurological alterations.

Lafora disease is a rare, autosomal recessive neurodegenerative disorder characterized by the progressive accumulation of abnormal, insoluble, and hyperphosphorylated forms of glycogen, known as Lafora bodies, in the brain and other tissues. The disease typically manifests during early adolescence with myoclonus, seizures, and rapidly progressive cognitive decline, ultimately leading to severe neurological deterioration and death within a decade of onset. Mutations in the EPM2A or EPM2B genes, which encode the proteins laforin and malin-key regulators of glycogen metabolism-are the underlying

Base editing restores CDKL5 expression and rescues neuronal deficits in a patient-derived model of CDKL5 deficiency disorder.

Cyclin-dependent kinase like 5 (CDKL5) deficiency disorder (CDD) is a rare monogenic neurodevelopmental disorder caused by pathogenic mutations in the CDKL5 gene, with approximately 50% of reported variants being point mutations. Base editing presents a promising therapeutic strategy to correct such mutations, restore endogenous CDKL5 expression, and pave the way for novel treatments for CDD. To assess the therapeutic potential of base editing for CDD, we applied adenine base editing (ABE) to correct a CDKL5-R550* (c.1648 C > T) mutation in induced pluripotent stem cells (iPSCs) derived from a

Soticlestat for drug-resistant epilepsy: Current evidence and clinical perspectives.

Epilepsy, particularly drug-resistant epilepsy, remains a significant clinical challenge, with many patients failing to achieve seizure control through conventional antiseizure medications. This unmet need has directed research toward alternative therapeutic targets, including neurosteroid modulation and novel molecular pathways. Soticlestat, a selective inhibitor of cholesterol 24-hydroxylase, is a mechanistically distinct investigational approach to epilepsy treatment by modulating mechanisms beyond those targeted by traditional antiseizure medication. Early-phase clinical trials and preclin

Utilization of Stiripentol in Children With Developmental and Epileptic Encephalopathies: Considerations for Clinical Practice.

Developmental and epileptic encephalopathies (DEEs) are a group of pediatric seizure syndromes, accompanied by developmental delay or regression and cognitive, psychiatric, and/or behavioral impairment. Seizures can be frequent and are typically refractory to treatment with antiseizure medications. In addition to the high burden of disease endured by patients, DEEs are also associated with negative psychosocial impacts and reduced quality of life for parents and caregivers. Stiripentol is an antiseizure medication approved for the treatment of seizures associated with Dravet syndrome in childr

Long-read genome sequencing increases diagnostic yield in a short-read sequencing unsolved developmental epileptic encephalopathy (DEE) cohort.

Developmental epileptic encephalopathy (DEE) comprises neurodevelopmental disorders with early-onset seizures and developmental impairment. Despite >900 implicated genes, many patients remain undiagnosed after short-read sequencing (SRS). We assessed long-read genome sequencing (LR-GS) in 38 previously unsolved infantile-onset DEE probands (10 singletons, 28 trios). Variant detection included single nucleotide variants (SNVs), structural variants, copy number variants and short tandem repeats in established repeat expansion disease genes. LR-GS identified candidate variants in 8 out of 38 prob

Novel Smartphone-Based Screening Tool for Absence Seizures in Pediatric Settings: A Comparative Study With Video EEG.

BACKGROUND: Staring spells, a common chief complaint in pediatrics, lead to both over-referral and under-referral to neurologists, creating significant challenges in clinical care. Children with nonepileptic staring episodes are frequently referred unnecessarily to neurology, while children with Childhood Absence Epilepsy are commonly misdiagnosed with disorders such as attention-deficit/hyperactivity disorder (ADHD). Hyperventilation (HV) has long been used as a neurobehavioral tool to provoke and diagnose absence seizures in children with suspected Childhood Absence Epilepsy. We evaluated th

5-(Benzofuran-2-yl)-3-(2-chloro-4-fluorobenzyl)-1,3,4-oxadiazol-2(3H)-one (GM-90663) Alleviates Dravet Syndrome via Inhibiting Monoamine Oxidase Activity.

Dravet syndrome (DS) is a severe, catastrophic childhood epilepsy predominantly caused by loss-of-function mutations in the SCN1A gene, which encodes the voltage-gated sodium channel Nav1.1. In this study, we evaluated the therapeutic potential of 5-(Benzofuran-2-yl)-3-(2-chloro-4-fluorobenzyl)-1,3,4-oxadiazol-2(3H)-one (GM-90663), a novel small molecule designed to address the complex pathophysiology of DS. In vivo adenine base editing ameliorates Dravet syndrome phenotypes in a mouse model.

Dravet syndrome (DS) is a severe neurodevelopmental disorder characterized by drug-resistant epilepsy, temperature-sensitive seizures, cognitive impairment, and a high incidence of sudden unexpected death in epilepsy (SUDEP). DS is caused by loss-of-function variants in SCN1A, which encodes the α subunit of the voltage-gated sodium channel (Nav1.1). Current approved treatments manage symptoms of DS but do not correct the root cause of the disease. Here, we describe the use of an adenine base editor (ABE) to directly correct SCN1AR613X, a recurrent variant found in patients with DS. We identifi
