Real-world outcomes of responsive neurostimulation in patients with Lennox-Gastaut syndrome: A multicenter retrospective study.

OBJECTIVE: Lennox-Gastaut syndrome (LGS) is a severe developmental and epileptic encephalopathy (DEE) characterized by multiple seizure types and high resistance to antiseizure medication (ASM), often necessitating nonpharmacologic therapies, including neuromodulation. Responsive neurostimulation (RNS), a closed-loop brain-responsive neuromodulation device, has emerged as an adjunctive treatment option for drug-resistant epilepsy (DRE), but evidence on its safety and effectiveness in LGS remains limited. This study examines real-world outcomes of RNS therapy in patients with LGS-associated DRE

Responsive Neurostimulation for Treatment of SCN1A-Associated Developmental and Epileptic Encephalopathies.

BACKGROUND: Dravet syndrome and other SCN1A-associated epilepsies are developmental and epileptic encephalopathies with high seizure burden and frequent status epilepticus despite modern antiseizure therapies. Evidence for responsive neurostimulation (RNS) in SCN1A-spectrum epilepsy remains limited. METHODS: We performed a multicenter retrospective clinical observation of six individuals with genetically confirmed SCN1A-spectrum epilepsy treated with RNS (five Dravet syndrome, one with genetic epilepsy with febrile seizures plus). RESULTS: Five patients underwent network-targeted bilateral t

Ganaxolone, an approved therapy for CDKL5-deficiency disorder, is an inhibitor of PTP1B.

CDKL5-deficiency disorder or CDD, which results in intellectual disability, speech, and motor deficits, and seizures that can start as early as 6 weeks after birth, is caused by de novo mutations in the CDKL5 gene. In early 2022, the FDA approved ganaxolone (Commercial name: Ztalmy) for the treatment of seizures in CDD patients 2 years and older. Ganaxolone has been reported to act as a GABAA receptor agonist that helps reduce neuronal excitability; however, based on its chemical structure, we hypothesized that ganaxolone may also act as a PTP1B inhibitor. We observed that ganaxalone was able

Epileptic and developmental encephalopathy secondary to inversion-duplication of chromosome 15: Description of epilepsy characteristics and therapeutic outcomes.

OBJECTIVE: Inversion-duplication syndrome of chromosome 15 (invdup15) is a chromosomal disorder characterized by an inverted duplication of 15q11.2-q13.1. Epilepsy is highly prevalent (60%-80%), drug-resistant, and may progress to developmental epileptic encephalopathies (DEEs) such as Lennox-Gastaut syndrome (LGS). Despite its impact, epilepsy and optimal treatments remain poorly described. METHODS: A retrospective multicenter study across 37 Spanish hospitals analyzed 54 patients, divided into two genetic subgroups: invdup15q and intdup15q. Clinical and genetic data were reviewed, focusing

Cannabidiol Use in Developmental and Epileptic Encephalopathies: A Syndrome- and Age-Stratified Systematic Review and Meta-analysis.

BACKGROUND: Developmental and epileptic encephalopathies (DEEs) are severe, drug-resistant epilepsies associated with major developmental, cognitive, and behavioral burden. Although pharmaceutical-grade cannabidiol (CBD) has shown efficacy in Dravet syndrome (DS) and Lennox-Gastaut syndrome (LGS), evidence across the broader DEE spectrum remains fragmented. OBJECTIVE: The aim of this systematic review was to quantify seizure and safety outcomes with pharmaceutical-grade CBD in DEE and explore whether effectiveness differs by syndrome type, age band, dose, clobazam co-medication, or follow-up

Burst-Suppression EEG in Early Infantile Developmental and Epileptic Encephalopathies: Phenotype, Genotype, and Outcome.

BACKGROUND AND OBJECTIVES: Developmental and epileptic encephalopathies (DEEs) with early burst-suppression EEG (EIDEE-BS) are among the most severe neonatal epileptic syndromes, typically presenting in the first months of life with refractory seizures and profound neurodevelopmental impairment. Although variants in the KCNQ2, STXBP1, and SCN2A genes are recognized as major causes, the full genetic spectrum remains uncertain. We aimed to delineate the electroclinical characteristics, genetic etiologies, and long-term outcomes in a large MRI-negative EIDEE-BS cohort. METHODS: We retrospectivel

Fenfluramine in developmental and epileptic encephalopathies: An age- and syndrome-stratified systematic review and meta-analysis.

PURPOSE: Developmental and epileptic encephalopathies (DEEs) are heterogeneous developmental encephalopathies in which ongoing epileptic activity contributes to worsening of pre-existing cognitive, behavioral, and motor impairment. Fenfluramine evidence is strongest in Dravet syndrome (DS), but limited in Lennox-Gastaut syndrome (LGS), other DEEs, and adults. We evaluated efficacy by syndrome and age, considering follow-up for durability. METHODS: We conducted a systematic review and meta-analysis (PROSPERO CRD420251161054). Compensatory rearrangement of parvalbumin interneuron voltage-gated sodium channel subunits in a mouse model of Dravet syndrome.

Heterozygous loss-of-function variants in the gene SCN1A, which encodes the voltage-gated sodium channel (VGSC) pore-forming (α) subunit NaV1.1, lead to a spectrum of neurological disease, including Dravet syndrome. NaV1.1 is prominently expressed at the proximal portion of the axon initial segment (AIS) of fast-spiking γ-aminobutyric acidergic parvalbumin-expressing inhibitory interneurons (PV+INs). In Dravet syndrome (Scn1a+/- haploinsufficient) mice, action potential firing is impaired in PV+INs during postnatal development; however, in mature animals, PV+IN fast-firing frequency has recove

Smartphone videos for infantile epileptic spasms triaging and assessment (VISTA study): Impact of education and standardized clinical history on diagnostic accuracy.

OBJECTIVE: Diagnostic and treatment delays in infantile epileptic spasms syndrome (IESS) increase the risk of poor neurodevelopmental outcomes. Early clinical recognition of IESS is essential, especially in regions lacking expedited access to electroencephalograms (EEG). This study aimed to determine clinicians' accuracy at recognizing infantile epileptic spasms (ES) based on smartphone videos, and the impact of brief IESS education on accuracy, diagnostic confidence, and willingness to treat without EEG. METHODS: This multicenter prospective cohort study took place over seven sessions global

SUDEP and mortality in developmental and epileptic encephalopathies: A meta-analysis of randomized clinical trials and extension studies.

OBJECTIVE: Developmental and epileptic encephalopathies (DEEs) are associated with high premature mortality and increased risk of sudden unexpected death in epilepsy (SUDEP). However, epidemiological data remain limited, particularly for specific syndromes such as Dravet syndrome (DS), Lennox-Gastaut syndrome (LGS), and infantile epileptic spasms syndrome (IESS). To address this gap, we conducted a Bayesian meta-analysis of randomized controlled trials (RCTs) and corresponding long-term open-label extension (OLE) studies. METHODS: All-cause mortality and SUDEP incidence rates were estimated u

Vigabatrin therapy for infantile epileptic spasms syndrome with periventricular leukomalacia.

BACKGROUND: Infantile epileptic spasm syndrome (IESS) and periventricular leukomalacia (PVL) cause developmental regression and poor neurological outcomes. The primary treatments for IESS include adrenocorticotropin and vigabatrin; however, no study has yet evaluated the efficacy of vigabatrin for patients with both IESS and PVL. Therefore, we investigated the efficacy and safety of vigabatrin in patients with both disorders. METHODS: We retrospectively enrolled patients with IESS and PVL who were treated with vigabatrin. We performed ophthalmological examinations and electroretinography (ERG

Rapid hippocampal kindling of mice as a model of sudden unexpected death in temporal lobe epilepsy.

Sudden unexpected death in epilepsy (SUDEP) is the leading cause of seizure-related death in people with epilepsy. Yet, its mechanisms remain poorly understood. While most epilepsy syndromes have no known genetic component, animal SUDEP models are often of rare monogenetic mutations. Here, we present a novel non-genetic mouse model of SUDEP, relevant to temporal lobe epilepsy, the most common seizure disorder. Our accelerated CA1 hippocampal kindling paradigm, referred to as 'rapid kindling', reliably produces a spectrum of seizure phenotypes with progressive severity, from no seizure behavior

cGAS-mediated type I IFN signaling contributes to disease progression in drug-refractory epilepsy.

Epilepsy is a prevalent neurological disease, with one-third of individuals becoming nonresponsive to antiepileptic drugs and developing drug-refractory epilepsy (DRE). Here we identify activation of cyclic GMP-AMP synthase (cGAS), a double-stranded DNA sensor that induces type I interferon (IFN) signaling, in human DRE brain tissue. Microglia from individuals with DRE exhibit a robust type I IFN signature and the activation of upstream cGAS-STING signaling. Further, in mouse models of Dravet syndrome, a genetic form of DRE, we similarly detect activation of the cGAS pathway. We show that micr

Parvalbumin interneuron activation rescues both seizures and impaired social novelty in digenic absence epilepsy mice.

Childhood absence epilepsy (CAE) is a common genetic epilepsy with a frequently complex polygenic etiology, for which a genetically tractable animal model is lacking. Current anti-seizure medications fail to address the significant neurocognitive and social comorbidities of CAE, highlighting a critical unmet need for comprehensive therapies and a better understanding of the underlying mechanisms. To address this, we developed a digenic mouse model (Cacng2stargazer/+;Cacna1a+/-, hereafter DiGstg+Ca1A) that mimics human polygenicity. The double mutant mice exhibit both absence seizures and alter

Beyond seizures in Dravet and Lennox-Gastaut syndromes - An Italian Expert Consensus on Non-Seizure issues and the role of fenfluramine.

BACKGROUND: Dravet syndrome (DS) and Lennox-Gastaut syndrome (LGS) are rare developmental and epileptic encephalopathies (DEEs) in which disability and family burden are driven by seizures and non-seizure issues (NSIs), including cognition, behavior, sleep, communication, motor function, and adaptive abilities. OBJECTIVE: To develop practical consensus statements on NSIs in DS and LGS, integrating NSI evaluation within a DEE framework and appraising the perceived role of fenfluramine (FFA). METHODS: An Italian panel of 12 epilepsy specialists spanning pediatric and adult practice developed s
