A Novel Mouse Model Unveils Protein Deficiency in Truncated CDKL5 Mutations.

Mutations in the cyclin-dependent kinase-like 5 gene (CDKL5) cause a severe neurodevelopmental disorder, yet the impact of truncating mutations remains unclear. Here, we introduce the Cdkl5492stop mouse model, mimicking C-terminal truncating mutations in patients. 492stop/Y mice exhibit altered dendritic spine morphology and spontaneous seizure-like behaviors, alongside other behavioral deficits. After creating cell lines with various Cdkl5 truncating mutations, we found that these mutations are regulated by the nonsense-mediated RNA decay pathway. Most truncating mutations result in CDKL5 pro

Long-term safety and effectiveness of fenfluramine in children and adults with Dravet syndrome.

OBJECTIVE: We analyzed the long-term safety and effectiveness of fenfluramine (FFA) in patients with Dravet syndrome (DS) in an open-label extension (OLE) study after participating in randomized controlled trials (RCTs) or commencing FFA de novo as adults. METHODS: Patients with DS who participated in one of three RCTs or were 19 to 35 years of age and started FFA de novo were included. Key endpoints were: incidence of treatment-emergent adverse events (TEAEs) in the safety population, and median percentage change in monthly convulsive seizure frequency (MCSF) from the RCT baseline to end of

Relationship between the time course of Burden of Amplitudes and Epileptiform Discharges scores and relapse in children with infantile epileptic spasms syndrome.

OBJECTIVE: For patients with infantile epileptic spasms syndrome (IESS) who have achieved remission of epileptic spasms (ES), indicators of how well the electroencephalographic (EEG) state should be maintained during follow-up are not available. We hypothesized that the time course of the Burden of Amplitudes and Epileptiform Discharges (BASED) score after ES remission is associated with ES relapse. This study aimed to investigate the association between ES relapse and BASED scores at the time of initial ES remission and during the subsequent follow-up period. METHODS: We collected clinical a

Variants in ATP6V0C are associated with Dravet-like developmental and epileptic encephalopathy.

OBJECTIVE: Dravet syndrome (DS) is a developmental and epileptic encephalopathy. Diagnosis is clinical, but ~90% of patients have pathogenic variants in SCN1A. ATP6V0C has recently been proposed as a novel candidate gene for epilepsy, with or without developmental delay. Here we describe two adult patients with a clinical diagnosis of DS associated with ATP6V0C variants. METHODS: Patients with developmental and epileptic encephalopathies were evaluated by physicians who are experts in DS, and their clinical diagnosis was correlated with genetic findings. A subgroup of those patients with DS b

In silico screening to search for selective sodium channel blockers: When size matters.

Dravet Syndrome is a severe childhood drug-resistant epilepsy. The predominant etiology of this condition is related to de novo mutations within the SCN1A gene, which codes for the alpha subunit of the NaV1.1 sodium channels. This dysfunction leads to hypoexcitability of GABAergic interneurons. In turn, the loss of electrical excitability in GABAergic interneurons leads to an imbalance of excitation over inhibition in many neural circuits. Notably, exacerbation of symptoms is observed when non-selective sodium channel blockers are administered to patients with Dravet. Recent studies in animal

Autobiographical memory impairment in genetic generalized epilepsies: neurocognitive and pathophysiological determinants.

The neuropsychological breakdowns of autobiographical memory (AM) in adults suffering from genetic generalized epilepsy (GGE) are far from being understood and largely neglected. We aimed at identifying AM impairments in GGE by analyzing neurocognitive deficits in illness-related variables possibly affecting AM.Patients with GGE were compared to healthy controls (HCs), through semistructured interviews on AM, as well as neuropsychological measures to identify potential determinants of AM impairment.A single GGE group was formed by including patients with juvenile myoclonic epilepsy (JME), juven

Antiseizure potential of the triple T-type calcium channel blocker ACT-709478 (apinocaltamide) in rodent models.

OBJECTIVE: T-type Ca2+ channels in the brain contribute to the generation of spike-and-wave discharges (SWDs). Absence seizures are defined by the occurrence of SWDs and are associated with impairment of consciousness. ACT-709478 is a selective and highly potent blocker of the low-voltage-activated T-type Ca2+ channels, Cav3.1, Cav3.2, and Cav3.3, with no relevant activity on other targets. Our aim was to investigate the efficacy of ACT-709478 in reducing absence-like and other types of seizures in rodents. METHODS: We studied the effect of oral ACT-709478 on SWDs in two models of absence-lik

Decreased homovanillic acid and 5-hydroxyindoleacetic acid levels in the cerebrospinal fluid of patients with Dravet syndrome with parkinsonism.

Dravet syndrome (DS) is an early onset, developmental, and epileptic encephalopathy characterized by drug-resistant seizures and multiple comorbidities. It has been reported that in adulthood, it may be accompanied by parkinsonism, but the pathogenesis of this condition remains unclear. We performed dopamine transporter single-photon emission computed tomography (DAT SPECT) and measured monoamine metabolite levels in the cerebrospinal fluid (CSF) in two adult patients with DS who developed parkinsonism around the age of 30 years. DAT SPECT showed no abnormalities in either patient, whereas CSF

Discovery and Evaluation of Active Site-Directed, Potent, and Selective Sulfophenyl Acetic Amide-Based Inhibitors for the Laforin Phosphatase.

Lafora disease is a rare and fatal progressive myoclonus epilepsy characterized by the accumulation of insoluble glycogen deposits in the brain and peripheral tissues. Mutations in the gene encoding the glycogen phosphatase laforin result in Lafora disease. Currently, there are no laforin-specific chemical probes, limiting our understanding of the roles of laforin in glycogen metabolism and other cellular processes. Here, we identified sulfophenyl acetic amide (SPAA), as a novel nonhydrolyzable phosphotyrosine mimetic for laforin inhibition. Using fragment-based and scaffold-hopping strategies

Exploring neurodevelopment in CDKL5 deficiency disorder: Current insights and future directions.

CDKL5 Deficiency Disorder (CDD) is a rare and severe neurodevelopmental condition marked by profound developmental delays, early-onset epilepsy, and significant impairments in motor and communication skills. The outcomes in CDD are shaped by various factors, including early-onset epilepsy and environmental influences. Genotype-phenotype correlations reveal that specific CDKL5 mutations impact developmental milestones, although considerable variability persists. Recent advancements have introduced novel antiseizure medications and emerging treatments such as gene therapy and targeted molecular

Drug-drug interaction between anti-seizure medications in Dravet syndrome and Lennox-Gastaut syndrome.

INTRODUCTION: Dravet syndrome (DS) and Lennox-Gastaut syndrome (LGS) are rare, severe epileptic encephalopathies requiring complex, individualized treatment due to drug-resistant seizures, non-seizure outcomes, and comorbidities. Polytherapy is an inevitable aspect of managing these conditions, making the management of drug-drug interactions (DDIs) crucial for optimizing efficacy, minimizing toxicity, and addressing broader patient needs. AREAS COVERED: This review discusses current and emerging pharmacological therapies for seizures in DS and LGS. We explore documented and theoretical DDIs b
