Temporal Lobe Epilepsy Associated With Glutamic Acid Decarboxylase Antibodies: Defining a Distinct Epilepsy Syndrome.

BACKGROUND AND OBJECTIVES: Epilepsy associated with glutamic acid decarboxylase antibodies (GAD-abs) often presents as temporal lobe epilepsy (TLE). However, detailed data on this condition are lacking. The aim of this study was to demonstrate that TLE with GAD-abs (GAD-TLE) is distinct from other forms of TLE and to describe the subgroup of patients with epilepsy and GAD-abs but no evidence of TLE. METHODS: Inpatients from 2011 to 2022 with high serum GAD-abs (≥1:80, confirmed by a cell-based assay and typical staining pattern on a tissue-based assay) were included. The control group compris

Diffusion MRI biomarkers for predicting treatment outcomes in infantile epileptic spasms syndrome with non-lesional MRI.

BACKGROUND: Infantile epileptic spasms syndrome (IESS) is a devastating developmental epileptic encephalopathy (DEE) and patients exhibit diffuse white matter alterations and structural remodeling. However, the correlation between these structural changes and brain network properties, or their effect on the efficacy of treatment outcomes in MRI non-lesional IESS patients is not clear. METHOD: This retrospective study was conducted on IESS patients using fixel-based analysis (FBA) of diffusion MRI and graph theory analysis of structural connectivity, involving 26 non-lesional IESS patients age

Comparative effectiveness of second-line treatments for epileptic spasms.

OBJECTIVE: This study was undertaken to evaluate the response to second treatments for infantile epileptic spasms syndrome (IESS). METHODS: Infants aged 2-24 months with IESS were prospectively enrolled in the National Infantile Spasms Cohort study at 21 pediatric epilepsy centers in the United States from 2012 to 2018. We analyzed data from infants who initially received standard treatment (hormonal therapy [adrenocorticotropic hormone, high-dose prednisolone] or vigabatrin), had continued or recurring epileptic spasms, and received a second treatment. We excluded those with tuberous scleros

Cognitive Orientation to daily Occupational Performance Approach with children with Dravet syndrome using a telerehabilitation format: A single subject study.

INTRODUCTION: Dravet syndrome (DS) is a severe form of epilepsy that affects a child's cognitive, language, behavioral, and physical abilities, leading to reduced adaptive functioning. Despite its impact, there is limited evidence supporting effective interventions for these children, particularly in addressing their occupational performance. OBJECTIVE: To explore the effectiveness of the Cognitive Orientation to Daily Occupational Performance (CO-OP) approach in a telerehabilitation format for children with DS. METHODS: A single-subject design with multiple baselines across goals was used w

Emerging Insights into the Pathogenic Mechanisms of Dravet Syndrome.

Dravet syndrome (DS) is a rare monogenic developmental and epileptic encephalopathy (DEE) most frequently caused by SCN1A gene mutations that lead to Nav1.1 sodium channel haploinsufficiency. DS is now recognized as a multisystem disorder with widespread developmental consequences that go beyond epilepsy. Indeed, children with DS often exhibit cognitive impairment, motor and speech delays, and neuropsychiatric comorbidities such as anxiety, attention deficits, and autistic traits. Sudden unexpected death in epilepsy (SUDEP) is also frequent. This complex phenotype underscores the need for mult

Epilepsy phenotype and developmental outcome in girls with mosaicism in X-linked neurodevelopmental disorders.

CDKL5 deficiency disorder (CDD) and SMC1A-related epilepsy are X-linked neurodevelopmental disorders characterized by early-onset, mostly intractable epilepsy and severe intellectual disability. While recent advances in next-generation sequencing improved diagnostic yields, detecting low-frequency mosaicism remains challenging. We retrospectively analyzed the epilepsy and developmental phenotype of four female patients with mosaic CDKL5 or SMC1A variants. Clinical data, including seizure types, electroencephalographic findings, neurodevelopmental milestones, and genetic analyses, were reviewed

Cannabidiol attenuates epileptic phenotype and increases survival in a mouse model of developmental and epileptic encephalopathy type 1.

OBJECTIVE: Developmental and epileptic encephalopathy type 1 (DEE1) is a rare drug-resistant pediatric epilepsy caused by trinucleotide repeat expansions in the X-linked ARX gene, leading to elongation of the first polyalanine tract. It presents with early onset tonic seizures or spasms, developmental and cognition delay, and high risk of premature mortality. We evaluated the therapeutic potential of highly purified cannabidiol (CBD) in Arx(GCG)7/Y mice, a genetic DEE1 model that replicates key features of the human condition. METHODS: Arx(GCG)7/Y mice received daily intraperitoneal CBD (100

Electroencephalogram (EEG) Spike Metrics Discriminate Impending Epileptic Spasms From Other Seizures in Children With Tuberous Sclerosis Complex: A Pilot Study.

In data from a multicenter prospective observational study, we assessed whether interictal epileptiform discharge metrics in the pre-seizure onset surveillance scalp electroencephalograms (EEGs) in children with tuberous sclerosis complex could predict seizure outcomes, specifically epileptic spasms. In 16 children with eligible EEG data (7 with epileptic spasms and 9 with other seizure types) and 16 controls, 2 spike metrics were calculated through automated detection followed by expert review: (1) spike rate (spikes per minute) and (2) number of unique spike foci. In patients who developed s

Assessing the impact of Dravet syndrome on caregivers' quality of life and perceived burden in Poland.

BACKGROUND: Dravet syndrome (DS) is a rare, severe, genetically-based epilepsy that begins in infancy. Due to the multiplicity, severity, and treatment-refractory seizures, DS patients are at high risk of status epilepticus and premature death. They also experience many other symptoms and comorbidities affecting their life. This study aimed to assess the impact of DS on caregivers' quality of life and perceived burden using Polish versions of standardized tools - the WHO Quality of Life-BREF and the Zarit Burden Interview. METHODS: We conducted an anonymized, self-administered, web-based surv

LIMP-2 deficiency-associated glycolipid abnormalities in mice.

Glucocerebrosidase (GCase) catalyzes the lysosomal degradation of glucosylceramide (GlcCer). GCase deficiency results in Gaucher disease (GD), a lysosomal storage disorder with characteristic hepatosplenomegaly. Transport of GCase to lysosomes is mediated by the lysosomal integral membrane protein type 2 (LIMP-2). Deficiency of LIMP-2 leads to reduced cellular GCase levels and manifests as Action Myoclonic Renal Failure Syndrome (AMRF). We investigated the cause for the markedly different symptomatology of GD and AMRF. In tissues of Limp2 -/- mice no prominent abnormalities in lysosomal enzyme

Genetic Epilepsies: Clinical pearls for early career epileptologists.

This review offers a practical look at the most relevant genetic epilepsies that an early-career epileptologist needs to navigate clinical practice, starting with a general overview of molecular genetic mechanisms, then moving to diagnostic testing rationale. The review emphasizes the importance of appropriate pre-testing clinical phenotyping and systematic genetic counseling and provides advice to optimize diagnostic yield and accurate interpretations of findings. Special attention is given to developmental and epileptic encephalopathies, particularly Dravet Syndrome and X-linked epilepsies.

TUBA1A-related tubulinopathy associated with the infantile epileptic spasms syndrome and atypical absence seizures.

Patients with TUBA1A pathogenic variants may present with complex brain malformation, intellectual disability, and epilepsy. The epilepsy phenotype is varied, ranging from mild to severe, with epileptic spasms and focal seizures being the most common seizure types. We report on an infant with a TUBA1A variant presenting with an evolving epileptic encephalopathy. Initially, this patient presented with infantile epileptic spasms syndrome, with the eventual appearance of atypical absence seizures before the age of 1 year. We discuss the initial diagnostic uncertainty that was raised by the findin

Generation of a human induced pluripotent stem cell line (CIBIOi007-A) from a Lafora disease patient.

An induced pluripotent stem cell (iPSC) line was generated from peripheral blood mononuclear cells (PBMCs) of a 24-year-old male patient affected by Lafora disease. The patient is homozygous for the c.721C>T, p.(Arg241*) nonsense variant in the EPM2A gene, which codes for the laforin protein. The generated iPSC line possesses a normal karyotype, expresses pluripotency markers, and can differentiate into derivatives of the three germ layers. It is configured as a useful tool for both disease modeling and therapy development.

Vigabatrin Versus Adrenocorticotropic Hormone After High-Dose Prednisolone Failure for the Treatment of Epileptic Spasms.

Infantile epileptic spasms syndrome carries high morbidity and mortality compared with other childhood epilepsy syndromes. High-dose prednisolone is considered the first-line option by many neurologists, with efficacy comparable to adrenocorticotropic hormone (ACTH).  The second-line therapy is debated if prednisolone fails to induce remission. The objective of this study was to evaluate the efficacy of adrenocorticotropic hormone versus vigabatrin as second-line antiseizure medication in patients with infantile epileptic spasms syndrome after high-dose prednisolone failure.
