Interictal epileptiform discharges as the most significant confounding factor underlying memory impairment in focal epilepsy.

BACKGROUND: Cognitive disturbances are highly prevalent in focal epilepsy and are often attributed to multifactorial causes, though the specific contributions of various predictors remain poorly understood. This study aimed to identify and prioritize factors contributing to memory deficits in temporal lobe epilepsy, with a particular emphasis on interictal epileptiform discharges (IED). METHODS: In a cohort of 48 patients with temporal lobe epilepsy undergoing presurgical evaluation with invasive foramen ovale electrodes, we examined offline memory performance alongside language and attention

Clinical course and management challenges in Lafora disease: a narrative analysis in an Apulian cohort.

BACKGROUND: Lafora disease (LD) is an ultra-rare, autosomal recessive neurodegenerative disorder characterized by the accumulation of Lafora bodies in the brain, leading to drug-resistant epilepsy, myoclonus, progressive dementia, and cerebellar dysfunction. This retrospective study describes the clinical course and management challenges of LD in a cohort of patients from the Apulia region of Southern Italy, where the disease prevalence appears to be higher than in other populations. METHODS: We retrospectively analyzed clinical, electroencephalographic, and management data from six unrelated

CDKL5 regulates the initiation of retrograde axonal transport through CLIP170-dynactin complex formation.

Cyclin-dependent kinase-like 5 (CDKL5) is a serine-threonine kinase implicated in regulating microtubule (MT) dynamics. Mutations in CDKL5 are associated with a rare neurodevelopmental disease called CDKL5 deficiency disorder (CDD), which is characterized by early-onset seizures and intellectual disabilities. Microtubule (MT)-related functions of CDKL5 are in part correlated with its interaction with MT-associated proteins, such as CAP-Gly domain-containing linker protein 1 [CLIP1; also known as cytoplasmic linker protein 170 alpha-2 (CLIP170)]. CLIP170 is a MT plus-end tracking protein that,

Content Validation of the Communication Inventory Disability-Observer Reported (CID-OR).

CDKL5 deficiency disorder is a rare and severe developmental and epileptic encephalopathy that has profound effects on communication. It is essential that communication be measured accurately for upcoming gene therapy trials. The Communication Inventory Disability-Observer Reported (CID-OR) was developed from a framework of communication derived from parent/caregiver interview data (n = 23), in consultation with disability and communication experts, and after reviewing concepts in existing measures. In this study, parents and caregivers (n = 21) took part in 1-hour semistructured think-aloud o

Safety and efficacy of Igk-TATk-CDKL5 gene therapy in mosaic CDKL5 deficiency.

CDKL5 Deficiency Disorder (CDD) is a severe neurodevelopmental disorder caused by mutations in the X-linked CDKL5 gene, resulting in early-onset seizures, developmental delays, and cognitive and sensorimotor impairments. While emerging therapies show promise, substantial challenges remain in developing a cure for CDD. In our prior work, we developed an innovative gene therapy strategy based on an Igk-TATk-CDKL5 fusion protein, which enhances brain distribution of the therapeutic protein, significantly improving treatment efficacy in a Cdkl5 knockout male mouse model. However, CDKL5 dosage sens

Parent/caregiver perspectives of meaningful improvement in functional domains for people with CDKL5 deficiency disorder: a mixed-methods study.

PURPOSE: CDKL5 deficiency disorder (CDD) is a rare developmental and epileptic encephalopathy. Greater understanding of the smallest meaningful improvements for individuals with CDD in clinical trials and practice is needed for a person-centred approach to treatment efficacy. This study explored how parent/caregivers of people with CDD understood meaningful improvements and described change for priority functional domains including communication, gross motor, fine motor, feeding. METHODS: This study included an in-person workshop and a convergent mixed-methods online survey. Parent/caregivers

Excitatory cortical neurons from CDKL5 deficiency disorder patient-derived organoids show early hyperexcitability not identified in neurogenin2 induced neurons.

CDKL5 deficiency disorder (CDD) is a rare developmental and epileptic encephalopathy resulting from variants in cyclin-dependent kinase-like 5 (CDKL5) that lead to impaired kinase activity or loss of function. CDD is one of the most common genetic etiologies identified in epilepsy cohorts. To study how CDKL5 variants impact human neuronal activity, gene expression and morphology, CDD patient-derived induced pluripotent stem cells and their isogenic controls were differentiated into excitatory neurons using either an NGN2 induction protocol or a guided cortical organoid differentiation. Long-term seizure outcomes and the likelihood of antiseizure medication withdrawal in patients with juvenile absence epilepsy: A 10-year follow-up study.

BACKGROUND AND OBJECTIVE: Juvenile absence epilepsy is a common idiopathic generalized epilepsy syndrome that often requires long-term antiseizure medications (ASMs). However, it remains unclear whether and when such long-term medication is necessary. In this study, we aimed to explore the long-term seizure outcomes and the feasibility of ASMs withdrawal in patients with JAE. Additionally, we aimed to investigate the evolution of EEG patterns over time. METHODS: We conducted a retrospective study on patients diagnosed with JAE according to the 2022 ILAE at Xijing Hospital between March 2014 a

Epilepsy surgery for seizure control in individuals with Dravet syndrome: is it a safe and practical option?

INTRODUCTION: Dravet syndrome is a severe developmental and epileptic encephalopathy presenting in childhood with drug resistant seizures and multiple comorbidities including developmental and cognitive delays. While medical therapy has continued to advance, leading to improved seizure control, outcomes such as seizure freedom remain elusive. As the underlying cause of Dravet syndrome is a genetic channelopathy, epilepsy surgery is rarely considered. AREAS COVERED: The authors weigh up the potential benefits of surgical therapy when seizure reduction, but not seizure freedom, is expected, dis

The challenge of ultra-rarity: Dual diagnosis of Lafora disease and developmental encephalopathies linked to TRIO and SHANK3 pathogenic variants.

We report two cases of dual genetic diagnoses involving Lafora disease (LD) and co-occurring neurodevelopmental disorders caused by pathogenic variants in TRIO and SHANK3, respectively. LD is an ultra-rare, autosomal recessive, severe form of progressive myoclonus epilepsy affecting previously healthy children or adolescents. In both patients, the presence of developmental delay, intellectual disability, and behavioral abnormalities was consistent with a primary genetic disorder-TRIO-related neurodevelopmental disorder in one, and Phelan-McDermid syndrome in the other. However, the onset of ep
