BACKGROUND: Cortical tremor (CT), a rhythmic variant of cortical myoclonus (CM), is the hallmark of benign adult familial myoclonus epilepsy (BAFME), though the underlying mechanism of rhythmicity remains unproven. This study aimed to reveal the cortical rhythmic activity of CT using induced activity analysis for somatosensory evoked potentials (SEP).
METHODS: We investigated 46 SEP datasets from 23 patients (11 with BAFME, 12 with other CM) and 35 SEPs from 18 healthy controls. SEPs were recorded by 1.1-Hz stimuli at a sampling rate of 10,000 Hz. A short-time Fourier transform was applied to

Lafora disease is a fatal neurodegenerative disorder caused by loss-of-function mutations in the EPM2A or EPM2B genes, which encode laforin and malin, respectively. These mutations lead to the accumulation of intracellular inclusions of abnormal glycogen, known as Lafora bodies, the hallmark of the disease. Symptoms typically begin in early adolescence with seizures and rapidly progress to cognitive and motor decline, ultimately resulting in dementia and death within a decade of onset. Disruption of Epm2a or Epm2b in mice causes neuronal degeneration and Lafora body accumulation in the brain a

Drug trial design is at an inflection point in epilepsy and neurological disorders. We are moving from conventional parallel-group randomized controlled trials (RCTs) to other designs, such as n-of-1 trials. Historically, inclusion criteria for antiseizure medicine (ASM) RCTs have stipulated a minimum number of seizures in the baseline period to judge ASM efficacy. For Dravet syndrome, RCTs have demanded 4 or more seizures/month in the baseline period. However, only 25% of children have sufficiently frequent seizures to qualify for RCTs. Trial outcomes should be tailored to the disease and lon

BACKGROUND: Cenobamate (CNB) has demonstrated remarkable efficacy in the treatment of drug-resistant focal epilepsy (FE). However, its role in other epilepsy types - such as drug-resistant generalized epilepsies (GEs), combined generalized and focal epilepsies (CGFEs), and developmental and epileptic encephalopathies (DEEs) - remains poorly explored. This article assesses the current evidence of CNB efficacy in these often complex and challenging patient populations.
METHODS: A case report is presented detailing a 22-year-old male with drug-resistant epilepsy with myoclonic-atonic seizures (E

Neurodevelopmental disorders can have long-lasting effects, causing not only early pediatric symptoms but also a range of neurological issues throughout adulthood. West syndrome is a severe neurodevelopmental disorder marked by infantile spasms, an early symptom that typically subsides with age. However, many patients progress to other seizure forms, known as seizure evolution, which is closely linked to poor long-term outcomes. Despite its clinical significance, the neurobiological mechanisms behind seizure evolution in West syndrome remain poorly understood. Recent genetic studies have consi

BackgroundSubacute sclerosing panencephalitis is typically characterized by myoclonic jerks, cognitive decline, movement disorders, and periodic complexes on electroencephalography (EEG). Although myoclonus is a hallmark feature, other seizure types including generalized/focal seizures are less commonly described in subacute sclerosing panencephalitis. We aimed to study seizure frequency, types, spectrum of epilepsy syndromes, and atypical EEG findings among children with subacute sclerosing panencephalitis.Materials and MethodsA retrospective chart review of 100 children (aged 1-18 years) dia

BACKGROUND: Dysregulated cellular metabolism is one of the major causes of colorectal cancer (CRC), including mitochondrial fission. Therefore, this study focuses on the specific regulatory mechanisms of mitochondrial dysfunction on CRC, which will provide theoretical guidance for CRC in the future.
METHODS: The Cancer Genome Atlas (TCGA)-CRC dataset, GSE103479 dataset and 40 mitochondrial fission-related genes (MFRGs) were downloaded in this study. The differentially expressed genes (DEGs) were analyzed in TCGA-CRC samples. Using MFRGs scores as traits, key module genes associated with its s

OBJECTIVE: Mutations in the cyclin-dependent kinase-like 5 gene (CDKL5) are associated with a wide spectrum of clinical presentations. Early-onset epileptic encephalopathy (EOEE) is the most recognized phenotype. Here we describe phenotypic features in eight Tunisian patients with CDKL5-related encephalopathy.
METHODS: We included all cases with clinical features consistent with CDKL5-related encephalopathy: infantile epileptic spasm, acquired microcephaly, movement disorders and visual impairment. We collected data about seizure types, electroencephalogram, magnetic resonance imaging, and me

OBJECTIVE: In this study, we present a novel biomimetic deep learning network for epileptic spasms and seizure prediction and compare its performance with state-of-the-art conventional machine learning models.
METHODS: Our proposed model incorporates modular Volterra kernel convolutional networks and bidirectional recurrent networks in combination with the phase amplitude cross-frequency coupling features derived from scalp EEG. They are applied to the standard CHB-MIT dataset containing focal epilepsy episodes as well as two other datasets from the Montefiore Medical Center and the Universit

OBJECTIVE: The objective of the study was to propose a candidate animal model of absence status epilepticus induced by specific alpha-2a adrenergic receptor (α2AR) activation. We also aim to investigate the responsiveness of this model to classical anti-status or anti-absence medications.
METHODS: An α2AR agonist, dexmedetomidine (DEX), was injected intracerebroventricularly into adult rats with genetic absence epilepsy, and their electroencephalography (EEG) was recorded. The total duration, number, and mean duration of each spike-and-wave discharges (SWDs) were evaluated. The blocks of abse

OBJECTIVE: Within the spectrum of developmental and epileptic encephalopathy (DEE), there are a group of infants with features that are distinct from the well-recognized syndromes of early infantile developmental and epileptic encephalopathy (EIDEE), infantile epileptic spasm syndrome (IESS), and Lennox-Gastaut syndrome (LGS). We refer to this condition as late infantile epileptic encephalopathy (LIEE). Our objective was to highlight the characteristics of this group by analyzing patients who exhibit prototypical features.
METHODS: From July 2022 to May 2023, we searched for LIEE features in

During development of the sensory cortex, the ascending innervation from deep to upper layers provides a temporary scaffold for the construction of other circuits that remain at adulthood. Whether an alteration in this sequence leads to brain dysfunction in neuro-developmental diseases remains unknown. Using functional approaches in a genetic model of Absence Epilepsy (GAERS), we investigated in barrel cortex, the site of seizure initiation, the maturation of excitatory and inhibitory innervations onto layer 2/3 pyramidal neurons and cell organization into neuronal assemblies. We found that co

BACKGROUND: Research has shown gene ATN1 to be associated with the nuclear receptor signaling. Its mutations in an evolutionarily conserved histidine-rich motif may cause CHEDDA, short for congenital hypotonia, epilepsy, developmental delay and digital anomalies, a recently identified neurodevelopmental syndrome that could evolve into developmental and epileptic encephalopathy (DEE). Up to date, there have been reported less than 20 cases, whose clinical features and treatment are worth in-depth exploring.
METHODS: The clinical characteristics and genetic data of an infant with CHEDDA and fur
