PURPOSE: Aberrant plastic changes among adult-generated hippocampal dentate granule cells are hypothesized to contribute to the development of temporal lobe epilepsy. Changes include formation of basal dendrites projecting into the dentate hilus. Innervation of these processes by granule cell mossy fiber axons leads to the creation of recurrent excitatory circuits within the dentate. The destabilizing effect of these recurrent circuits may contribute to hyperexcitability and seizures. Although basal dendrites have been identified in status epilepticus models of epilepsy associated with increas

PURPOSE: The reasons why the mortality of patients with epilepsy is significantly increased, even many years after seizure onset, are not fully understood. The aim of this study was to compare the distribution of the causes of death (COD) in an epilepsy population with that in the general population and with previous findings in other epilepsy populations. In addition, we investigated the chronological relationship between the onset of epilepsy and the onset of the diseases leading to death.
METHODS: The COD for patients who were registered with a diagnosis of epilepsy at Stavanger University

BACKGROUND: Frontal Lobe Epilepsy (FLE) is the second most frequent type of partial epilepsy and its onset is generally in childhood. Though cognitive and behavioural impairments have been described as co-morbid disorders in epilepsy, their extent in FLE, particularly in children, remains unknown.
AIMS: In this study, we assess cognitive skills and behaviour in a cohort of paediatric FLE patients.
METHODS: We measured the performance of 71 children with cryptogenic FLE on intelligence tests, neuropsychological tests, and behavioural questionnaires. Age-dependent normative values were used fo

INTRODUCTION: Dysfunction of immunoglobulins (Igs) has been detected in association with antiepileptic drugs (AEDs) and in newly diagnosed epilepsy patients. The aim of this study was to evaluate the effect of clinical features and the current or past use of AEDs on serum Ig concentrations in a well-examined group of patients with refractory epilepsy.
PATIENTS AND METHODS: Using a nephelometric method, concentrations of IgA, IgG and IgM were analyzed in the sera of 257 patients with refractory epilepsy, 15 patients with controlled epilepsy and 584 healthy control subjects.
RESULTS: A low IgA

Tuberous sclerosis complex (TSC) is an autosomal dominant syndrome with a variable neurocognitive phenotype. Recently, different intelligence profiles were observed for distinct mutation types and locations, suggesting that individuals with missense mutations represent a subgroup with milder neurocognitive outcomes. We applied these recent insights to the analysis of the epilepsy phenotype in a large cohort of patients with TSC. Associations between genotype and a history of epilepsy and/or infantile spasms (IS) were explored retrospectively, using data from 478 TSC patients from the databases

Prognosis describes the trajectory and long-term outcome of a condition. Most studies indicate a better prognosis in idiopathic generalized epilepsy (IGE) in comparison with other epilepsy syndromes. Studies looking at the long-term outcome of different IGE syndromes are relatively scant. Childhood absence epilepsy appears to have a higher rate of remission compared to juvenile absence epilepsy. In absence epilepsies, development of myoclonus and generalized tonic-clonic seizures predicts lower likelihood of remission. Although most patients with juvenile myoclonic epilepsy (JME) achieve remis

Benign myoclonic epilepsy in infancy (BMEI) is a rare syndrome included among idiopathic generalized epilepsies (IGE) and syndromes with age-related onset. Recently, it has been shown that a few patients with BMEI later had other epilepsy types mainly IGE but never childhood absence epilepsy (CAE). We report a patient who at 11 months of age showed isolated myoclonic jerks occurring several times a day. The ictal video-EEG and polygraphic recording revealed generalized discharge of spike-wave (SW) lasting 1-2s associated with isolated bilateral synchronous jerk involving mainly the upper limbs

INTRODUCTION: Learning disorders are very frequent among children with epilepsy. The etiology is multifactorial, being affected by the type of epileptic syndrome, the cause of epilepsy, a high frequency of epileptic seizures, a previous history of status epilepticus, the age of onset of epilepsy, the antiepileptic treatment being selected, and the role of interictal epileptiform discharges. Several studies have sought to analyze to what extent cognitive impairment can be attributed to interictal epileptiform discharges among the other epilepsy factors.
AIM: To review the existing evidence on

INTRODUCTION: Epilepsy is a frequent CNS manifestation of mitochondrial disorders (MIDs). At present, patients with MID-related epilepsy are largely treated in the same way as any other epilepsy sufferer. The problem with this approach is that some antiepileptic drugs (AEDs) are mitochondrial toxic and care is, therefore, needed when administering these AEDs to patients with MIDs.
AREAS COVERED: This review summarizes and discusses the mitochondrial toxicity, tolerability and beneficial effects of AED in patients with MIDs. The literature for this article was retrieved through PubMed using th

The present study investigated a possible correlation between prenatal stress and the onset of infantile spasms. A total of 120 infants (60 cases, 30 positive controls, and 30 negative controls) went through routine etiologic screening. The Pregnant Woman Life Event Scale was used to investigate and evaluate the degree of prenatal stress of the mothers in the three infant groups. Etiologic analyses indicated no statistical difference between the infantile spasms group and the other epilepsy control group. There was a significant difference in the degree of prenatal stress among mothers of the

Using the Child Behavior Checklist, behavior of 51 children with cryptogenic localization-related epilepsy was studied. According to parent report, children with cryptogenic localization-related epilepsy scored in the clinical range on the subscales "internalizing behavior," ''total behavior,'' and "attentional problems.'' No relation between the epilepsy factors seizure frequency, age at onset, duration of epilepsy or the number of antiepileptic drugs, and the subscales of the Child Behavior Checklist was found. Only for seizure type a relationship was found. Although in the normal range, the

Treatment with antiepileptic drugs (AED) increases the chance of seizure control in patients with epilepsy. This control is best achieved with AED monotherapy in the early stages of the disease. In up to 28% of the remaining patients, complete seizure control is obtained with AED combination therapy. In our own retrospective studies, levetiracetam proved to be the most effective drug in add-on treatment of focal epilepsies. Seizure control for at least 6 months was seen in 22.8% of patients, and reduction of seizure frequency by more than 50% was achieved in 58.8%. One must however consider th

INTRODUCTION: "Acute repetitive seizures" (ARS) is a term to describe a condition manifest by multiple seizures occurring over a relatively brief period of time -generally 24 hours- in patients with epilepsy. There is limited information regarding the epidemiology of ARS in the general population.
METHODS: We performed a historical cohort study using data from the United Kingdom General Practice Research Database (GPRD) to identify all incident and prevalent cases of active epilepsy in 2005. From among this group, we identified individuals at risk for ARS. This included those with "catastroph

BACKGROUND: The impairment of the pontine reticular formation (PRF) has recently been revealed to be histopathologically connected with focal-cortical seizure induced generalized convulsive status epilepticus. To elucidate whether the impairment of the PRF is a general phenomenon during status epilepticus, the focal-cortical 4-aminopyridine (4-AP) application was compared with other epilepsy models. The presence of "dark" neurons in the PRF was investigated by the sensitive silver method of Gallyas in rats sacrificed at 3 h after focal 4-AP crystal or systemic 4-AP, pilocarpine, or kainic acid

Some ambiguous symptoms may delay or lead to an erroneous diagnosis. We present a case of pure sleep, generalized tonic seizures in a patient with concomitant sleep apnea syndrome. The prolonged apneic periods with tonic muscle contracture lasting minutes and occurring exclusively at night with ensuing confusional state posed diagnostic difficulties because of the negative EEG at the beginning of the workup and the absence of other epilepsy symptoms (i.e. clonic phase, tongue biting, enuresis, seizures while awake). Numerous apneas on polysomnography led to the diagnosis of sleep apnea syndrom

PURPOSE: To evaluate health care resource utilization (HRU) in active epilepsy.
METHODS: Thomson-Reuters insurance databases included 14 million persons in 2005-2007. We extracted information for individuals with insurance claims suggestive of epilepsy. Using iterative expert classification, we sorted patients by type of epilepsy. For each type we calculated prevalence and HRU. A distance analysis identified closely similar types, and a principal components analysis revealed dimensions of variation in HRU.
RESULTS: The prevalence of active epilepsy was 3.4 per 1,000. Most common diagnoses am
