Prolonged ictal amnesia with transient focal abnormalities on magnetic resonance imaging.

A previously healthy woman had a prolonged amnestic state caused by complex partial status epilepticus with bilateral mesiotemporal lobe involvement confirmed by EEG with nasopharyngeal electrodes. A magnetic resonance imaging (MRI) scan obtained shortly after recovery from the amnesia showed reversible focal abnormalities consisting of increased signal intensity on T2-weighted scan in the mesiotemporal lobe.

Mapping of cerebral energy metabolism in rats with genetic generalized nonconvulsive epilepsy.

The quantitative 2-[14C]deoxyglucose autoradiographic method was applied to measure local cerebral metabolic rates of glucose (LCMRglc) in a model of genetic petit-mal-like seizures in a strain of Wistar rats. During the experimental period, epileptic rats exhibited synchronous spike-and-wave discharges, whereas the EEG pattern of control animals was normal. Overall, LCMRglc was consistently higher in epileptic rats than in the non-epileptic controls. The increase in LCMRglc was widespread and concerned all cerebral functional systems studied, whether they exhibit spike-and-wave discharges (ne

Triphasic waves and spike wave stupor.

Triphasic waves are usually thought of as indicating a metabolic encephalopathy. Recent investigations have added nonmetabolic etiologies to the differential diagnosis of triphasic waves. Seizures are not generally thought of as associated with triphasic waves. Similarities in the appearance of records with encephalopathies and continuous triphasic waves and those of some patients with the Lennox-Gastaut syndrome have been noted. We presented a case which suggests that the presence of TW in a patient with a metabolic encephalopathy might suggest petit mal status epilepticus.

[Chlormethiazole in the treatment of complex partial epileptic status in childhood].

The authors report on the results of clinical investigations, treatment and follow-up of a 9-year old boy with complex partial status epilepticus (CPSE) which occurred after the first onset of partial secondary generalized epileptic seizures. Electroencephalografic recordings during status epilepticus showed a diffuse, generalized, high-voltage delta dysfunction and bilateral epileptiform activity, with local maximum over the posterior right temporal parietal regions. Parenteral administration of diazepam, phenytoin and phenobarbitone as choice anticonvulsant drugs, failed to stop CPSE in the

Complex partial status epilepticus simulating an ischemic insult: case report.

A patient presenting complex partial status epilepticus (CPSE) had a clinical history and neurological picture on admission that mimicked a cerebrovascular insult. On admission she was confused and totally unresponsive to verbal stimuli. EEG showed high voltage paroxysmal activity on the left hemisphere, prominent on the temporoccipital leads and tending to spread to the opposite regions. Intravenous diazepam led to resolution of the clinical and EEG picture within 24 hours, only a medium voltage slow wave focus being present on the left temporal regions. CPSE should be considered whenever a p

Complex partial status epilepticus of extratemporal origin: report of a case.

A 28-year-old woman with no history of seizure was 7 months pregnant when she developed a prolonged complex partial status epilepticus (CPSE) organized in recurrent complex partial seizures of occipital origin, which was ascertained by the presence of elementary visual hallucinations and nystagmus heralding the attacks. EEG demonstrated recurrent seizures starting from the right occipital area. This especially refractory case of CPSE resolved after treatment with antiepileptic drugs and termination of pregnancy by cesarean section.

Complex partial status epilepticus provoked by ingestion of alcohol: a case report.

A 27-year-old man had a 5-year history of a peculiar state characterized by clouded consciousness provoked by ingestion of alcohol. The episode was identified electroclinically as nonconvulsive status epilepticus and was activated by ingestion of alcohol. Single-photon emission computed tomography (SPECT) examination during the nonconvulsive status epilepticus showed increased regional cerebral perfusion in the right frontal region. This finding, along with the EEG expressions, supports a cortical origin of the nonconvulsive status epilepticus and a diagnosis of complex partial status epilepti

Periodic febrile confusion as a presentation of complex partial status epilepticus.

A 75-year-old woman was evaluated for recurrent episodes of fever she experienced periodically every 4-5 weeks over the last 12 months, lasting 2-3 days each. The fever was associated with continuous complex partial seizures, paralleled the seizure activity and returned to normal after the seizures had ceased. The ictal EEG recordings showed rhythmic bitemporal 3-4 Hz activity; the interictal recordings showed a spike and wave discharge over the right fronto-temporal region. Carbamazepine effectively controlled both the seizures and the fever; the latter was presumed to be an inherent manifest

Electroclinical correlation examined by simultaneous VTR-EEG monitoring in a patient with complex partial status epilepticus.

We discussed the electroclinical correlation of complex partial status epilepticus (CPSE) followed by simultaneous VTR-EEG monitoring. An eight-year-old boy had repetitive seizures lasting for 30 to 90 minutes in addition to ordinary seizures with typical oral automatism of a few minutes duration. This case fulfilled the criteria for diagnosis of CPSE by Treiman et al. (1983), and was classified as the discontinuous type of CPSE described by Gastaut et al. (1975). The clinical symptoms were characterized by a repeated occurrence of psychomotor arrest and automatic phases. The EEG showed an alt

Complex partial status epilepticus: report of one case.

Complex partial status epilepticus (CPSE) has rarely been reported in children. We experienced a 5-year-old girl having had an abrupt onset of complex partial seizure with a fluctuating state of consciousness and aphasia. Electroencephalogram revealed repetitive epileptiform discharges originating from bilateral temporal and parieto-occipital regions over the background of diffuse continuous slow activity. Computerized tomography of the brain showed mild atrophy without focal lesions. All the other studies including bacteriology, virology, toxicology and metabolic screening were unremarkable.

Complex partial status epilepticus associated with cyclosporin A therapy.

Significant neurotoxicity including seizures, encephalopathy and coma may complicate the use of cyclosporin A (CyA). Two patients are described, receiving CyA, who presented with abnormal behaviour, stupor, focal motor activity and were shown to be in complex partial status epilepticus (CPSE). Abnormalities of behaviour and/or stupor in patients receiving CyA may be a manifestation of CPSE. Patients receiving CyA who develop an encephalopathy should have electroencephalography performed at the time of the abnormal behaviour.

Complex partial status epilepticus provoked by "crack" cocaine.

A 53-year-old woman with a history of chronic abuse of "crack" cocaine developed isolated complex partial status epilepticus after 3 days of frequent cocaine use. The electroencephalogram demonstrated focal epileptiform activity. Follow-up for 1 year during which she reported continuous abstinence from cocaine and anti-convulsants revealed no recurrence of seizures.

Anticonvulsant activity of the diaryltriazine, LY81067: studies using electroencephalographic recording and positron emission tomography.

It is reported that LY81067, a new diaryltriazine, possesses anticonvulsant properties against grand mal status epilepticus induced by intravenous administration of picrotoxin binding site ligands (Ro 5-4864 and pentylenetetrazole) in the baboon. Intravenous administration of LY81067 during the seizures blocked grand mal type electroencephalographic (EEG) paroxysmal discharges and led to a long electrical silence, progressively replaced by spike-and-wave discharges of low frequency (2 c/sec). A transient blocking effect was also observed when LY81067 was injected during grand mal status epilep

Status epilepticus.

Status epilepticus generally is considered to be a medical emergency requiring immediate intervention to prevent permanent injury to the brain. Tonic-clonic status epilepticus is a very frightening event; the inexperienced parents invariably believe that their child is dying. Emergent management is imperative for convulsive tonic-clonic (grand mal) status epilepticus, but there are nonconvulsive types of status epilepticus in which the problem is more one of correct diagnosis than emergent management. This article reviews aspects of status epilepticus, particularly the classification and treat

Marfan's syndrome, recurrent complex partial status epilepticus and myoclonus: a case report.

A 26 year-old mentally retarded woman suffering from Marfan's syndrome and epileptic seizures presented weekly recurrent complex partial status epilepticus for two years, and myoclonus at rest. A marked increase of the myoclonic jerks and transient signs of cerebellar dysfunction followed each occurrence of the status epilepticus. Myoclonus could be interpreted as: 1) a permanent neurologic consequence of primary CNS disorders or secondary to disorders of the connective tissue, 2) a persistent post-ictal sequela sustained by recurrence of complex partial status, or 3) an additional consequence
