Mendelian randomization applied to migraine research: Opportunities and challenges.

Observational studies have linked migraine to a wide range of risk factors and diseases, although the causality of these associations is uncertain. Mendelian randomization (MR) is an analytic paradigm that exploits germline genetic variants as natural experiments, providing a framework for causal inference from observational data. In this narrative review, we summarize the core principles and assumptions of MR and highlight methodological considerations unique to its application to migraine research. We review key MR findings across several domains of migraine research, noting areas of consist

The design and reporting of sham acupuncture and its association with the efficacy in acupuncture randomized controlled trials for migraine.

BACKGROUND: In current trials of acupuncture for migraine treatment, the heterogeneity in therapeutic outcomes has been noted, which may be closely tied to the use of diverse sham acupuncture designs. This paper aims to evaluate the reporting quality of sham acupuncture and assess the potential impact of the consistency for intervention procedure between sham and true acupuncture groups on efficacy size in randomized controlled trials (RCTs) for migraine. METHODS: RCTs with sham acupuncture as a control in migraine were searched in four English and four Chinese databases from inception to May

Heart rate variability as a predictor of migraine: Sleep-time data analysis of pre-migraine nights.

BackgroundMigraine is a complex neurological disorder characterized by recurrent, often debilitating headaches. Current evidence suggests that autonomic nervous system (ANS) alterations play a significant role in migraine pathophysiology, affecting sensory, limbic, and homeostatic processing. Heart rate variability (HRV), a well-established, noninvasive marker of ANS function, is associated with migraine severity and treatment efficacy.ObjectiveIn this study, we aimed to evaluate the use of wearable sensor technology in predicting migraine attacks by monitoring changes in the ANS during the pr

Rethinking ADHD as a neurointestinal syndrome: a gut-brain-parasite hypothesis.

Neurodevelopmental conditions such as Attention-Deficit/Hyperactivity Disorder (ADHD) are usually framed as brain-based disorders driven by genetics and neurotransmitter imbalance. At the same time, converging evidence implicates the gut-brain axis and intestinal immunity in shaping cognition and behavior. In this Hypothesis and Theory article, I propose that a subset of ADHD and related neurodivergent profiles can be usefully conceptualized as neurointestinal syndromes, emerging from co-evolutionary interactions between the gut microbiota, intestinal parasites, and host immunity. Drawing on d

Does high aura frequency in migrainous healthcare workers point out underestimated migraine aura in the general population?

INTRODUCTION: Migraine is a chronic neurovascular disease classified into two main subtypes: migraine with and without aura. Aura occurs in approximately 30 percent of migraine patients. Few studies have reported a higher frequency of aura among healthcare workers. This study aimed to investigate the frequency of aura in migraineurs among healthcare and non-healthcare workers and compare their aura characteristics. METHODS: A total of 482 migraine patients, 117 healthcare workers, and 371 non-healthcare workers were included. Demographic data, migraine, and aura characteristics were recorded

Abnormal functional activity in the cerebellar crus can distinguish patients with migraine with comorbid insomnia.

BACKGROUND: Migraine is a prevalent neurological disorder that is frequently observed in clinical practice and is commonly comorbid with insomnia. Insomnia can exacerbate and precipitate migraine attacks, with both conditions exerting a reciprocal influence on one another. The cerebellar crus is significantly associated with the pathophysiology of migraine and insomnia. The relationship between cerebellar crus functional alterations and migraine-associated insomnia remains unclear. This study utilizes resting-state functional magnetic resonance imaging (rs-fMRI) to examine functional alteratio

External trigeminal nerve stimulation for neuropsychiatric disorders: mechanisms, efficacy, and future directions.

External trigeminal nerve stimulation (eTNS) is an emerging noninvasive neuromodulation technique that modulates brain network activity implicated in multiple neuropsychiatric disorders through the application of low-intensity electrical currents to the frontal branches of the trigeminal nerve. Based on a search of major biomedical databases, we narratively synthesize clinical and mechanistic studies of eTNS across neurological and psychiatric indications. Current clinical and neurophysiological studies suggest clinically meaningful benefits of eTNS in attention-deficit/hyperactivity disorder

Practice-Based Consensus Amongst Indian Neurologists: Adopting Remote Electrical Neuromodulation for Effective Management of Migraine.

INTRODUCTION: Migraine is a major cause of disability in India, disproportionately affecting women and adolescents. Although traditional pharmacological treatments exist, side effects and poor adherence have increased interest in non-pharmacological options such as remote electrical neuromodulation (REN). However, clinical adoption of REN remains limited because of the absence of clear, India-specific guidelines. METHODS: An Indian consensus was developed through a two-phase Delphi process involving 49 neurologists. Phase 1 included a scientific committee of eight neurologists who conducted a

Extracellular Vesicles in Migraine: Biomarkers and Therapeutics.

The diagnosis and therapeutic evaluation of migraine have long relied on subjective clinical descriptions, lacking objective, stable, and quantifiable biological markers. Extracellular vesicles (EVs), which carry specific bioactive molecules, have been recognized in recent years as a potential breakthrough in research due to their role in intercellular communication, the stability of their contents, and their reflection of pathological states. This article comprehensively evaluates studies on changes in EVs and their contents in migraine patients, discusses methods for isolating and characteri

People who play games regularly are no more robust to unpleasant images compared to those who do not in preparation for brain research.

It has been suggested that flickering, striped patterns may be unpleasant to look at as they are difficult stimuli for the visual system to process, and so result in an inefficient, excessive neural response. Some people such as people who experience migraine, may be more susceptible to unpleasant stimuli, and have greater neural responses. Others, such as people who spend a lot of time playing computer games, may have more efficient visual processing and so be more resilient to these unpleasant images. We measured neural responses using steady-state visual evoked potentials (SSVEP, an EEG tec
