EFNS guideline on the drug treatment of migraine - report of an EFNS task force.

Migraine is one of the most frequent disabling neurological conditions with a major impact on the patients' quality of life. To give evidence-based or expert recommendations for the different drug treatment procedures of the different migraine syndromes based on a literature search and an consensus in an expert panel. All available medical reference systems were screened for all kinds of clinical studies on migraine with and without aura and on migraine-like syndromes. The findings in these studies were evaluated according to the recommendations of the EFNS resulting in level A,B, or C recomme

DHE in the pharmacotherapy of migraine: potential for a larger role.

Despite a large array of currently marketed, frequently effective drugs for the acute treatment of migraine headache, comprising various classes and formulations, predictably reliable treatment for most headache types is often lacking. Dihydroergotamine mesylate (DHE) is a comparatively safe and effective therapy for migraine headache that could potentially be used for a broader range of headache types than occurs at present. The features of DHE supporting this assertion include (1) effectiveness in terminating severe, long-lasting headaches, (2) rapid onset of action, (3) very low rates of he

Successful treatment of status migrainosus after electroconvulsive therapy with dihydroergotamine.

We describe a case of status migrainosus precipitated by electroconvulsive therapy (ECT) that was refractory to treatment with triptan medications but which resolved with dihydroergotamine (DHE). We briefly review some of the receptor pharmacology possibly related to this observation.

[Therapy of severe migraine attacks: practical tips].

Severe migraine attacks are treated with triptans. The forms of administration of the active substances depend on the drug profile. A standard therapy does not exist. The rate of side effects is low. Triptans are contraindicated for patients with coronary, cerebral and peripheral vascular diseases. Under no circumstances are they to be combined with ergot alkaloids. Triptans are all the more effective, the earlier they are taken. For determining the optimal time point for taking the medication, the so-called triptan threshold is helpful. Triptans are not suitable for treating status migrainosu

Clopidogrel reduces migraine with aura after transcatheter closure of persistent foramen ovale and atrial septal defects.

OBJECTIVE: To report the clinical events leading to alteration of an anticoagulation regimen for patients undergoing transcatheter closure of an atrial shunt and how this affected migraine symptoms after the closure procedure. METHOD: Audit of a change of anticoagulant regimen. RESULTS: In the first few weeks after a closure procedure migraine frequency and severity increased despite treatment with aspirin for six months in 71 patients. Severe attacks of migraine with aura, including status migrainosus, in the first few weeks after transcatheter closure were terminated by addition of clopido

Estradiol supplementation modulates neuroendocrine response to M-chlorophenylpiperazine in menstrual status migrainosus triggered by oral contraception-free interval.

BACKGROUND: Migraine triggered by oral contraception (OC)-free interval is very common and may be extremely severe, long-lasting and poorly responsive to analgesics (status migrainosus). The serotoninergic (5-HT) system is crucially involved in pain threshold and it is sensitive to estradiol (E2). Therefore, we aimed to assess neuroendocrine correlates of OC status migrainosus in response to the direct central 5-HT agonist meta-chlorophenylpiperazine (m-CPP) and to test the effect of transdermal E2 supplementation of the OC-free interval. METHODS: Clinical investigative protocol, single-blind

[Office Treatment of Migraine in 2004].

The current headache classification differentiates between migraine with and migraine without an aura. Each form is characterized by typical pain and concomitant symptoms. In the case of status migrainosus the additional administration of a corticosteroid has proven useful. To cut short mild attacks of migraine, analgesics together with an antiemetic are employed; for severe attacks, triptans are needed. The aim of migraine prophylaxis is to prevent the occurrence of medication-induced headache. It is indicated when at least 6 migraine attacks a month occur. The recommendation to apply prophyl

Headaches in pregnancy.

Migraine and TTH are primary headache disorders that occur commonly during pregnancy. Migraine sometimes occurs for the first time with pregnancy. The majority of migraineurs improve while pregnant; however, migraine often recurs post partum. Some disorders that produce, headache, such as stroke, cerebral venous thrombosis, eclampsia, and SAH, occur more frequently during pregnancy. Diagnostic testing serves to exclude organic causes of headache, to confirm the diagnosis, and to establish a baseline before treatment. If neurodiagnostic testing is indicated, the study that provides the most inf

Neuroendocrine response to the serotonin agonist M-chlorophenylpiperazine in women with menstrual status migrainosus.

To assess the neuroendocrine correlates of menstrual status migrainosus (MSM) and menstrual migraine (MM), we evaluated the prolactin (PRL) and cortisol responses to the direct central serotoninergic (5-HT) agonist meta-chlorophenylpiperazine (m-CPP) administered orally (0.5 mg/kg) during the follicular (FP: +6, +8) and luteal phases (LP: -4, -6) of the same menstrual cycle. Managing intractable migraine with naratriptan.

BACKGROUND: Intractable migraine presents a significant treatment challenge to both patient and physician. Most attacks are treatable or self-limiting, but occasionally they may continue for extended periods regardless of treatment. OBJECTIVE: To determine the efficacy of naratriptan 2.5 mg twice daily for the treatment of intractable migraine. METHODS: We reviewed 24 patients treated with naratriptan twice daily for an intractable migraine attack. Patients were permitted to take prophylactic medication if such treatment had been effective in the past. RESULTS: Nineteen patients (79%) impro

[Management of intractable migraine in adults].

The management of intractable migraine is not yet standardised. The first point in the emergency department is to eliminate severe cephalalgic non-migrainous disease, then to confirm the diagnosis of migraine. The second point is to determine trigger factors responsible for the refractory migraine--principally inadequate therapy, such as too low a dosage, inadequate treatment compared with intensity, and delayed treatment. Examples of inadequate classical treatments are presented for the following four main oral therapies: a nonsteroidal anti-inflammatory drug (NSAID), analgesics, ergot deriva

A fresh look at migraine therapy. New treatments promise improved management.

Successful management of migraine headaches involves identifying and avoiding headache triggers and using appropriate abortive therapy once a headache is recognized. Pain relief measures include over-the-counter analgesics, parenteral NSAID therapy when needed, and use of antiemetics and cold packs. Narcotic analgesics are best used only as a "last resort" measure. Prophylactic therapy should be considered for patients who have more than two acute migraine attacks each month or whose daily activities are seriously compromised by headaches. For the patient in whom status migrainosus threatens w

Treatment of status migrainosus.

Migraine episodes that persist for at least 3 days despite treatment are termed status migrainosus. Traditionally, these long-lasting migraine episodes were treated with brief in-patient hospitalisations for iv. medication. The full duration of disability associated with these episodes includes both the several days of migraine and the several days of hospital stay. The development of medications that specifically target the mechanism of migraine, such as dihydroergotamine and the triptans, has reduced the number of headache episodes that persist after initial treatment or fail to respond to s

The first world report of botulinum A toxin injection for status migrainosus.

The author reports the first ever documented publication in the world concerning the use of botulinum toxin A (BTX-A) injection for status migrainosus. A 58-year old man had been suffering from migraine without aura for 20 years. This last attack (a very severe throbbing headache) started over the left side of his head and he had tried several medications (paracetamol, aspirin, ergotamine, mefenamic acid, and diazepam) during the attack to no vail. Physical examination revealed an acutely ill patient with an agonizing pain condition. General and neurological examinations were normal. BTX-A sol

MIGRAINE AND PREGNANCY.

Migraine is an episodic headache disorder that occurs in four percent of children, six percent of men, and 18 percent of women. Most women with migraine improve during pregnancy. Some women have their first attack during pregnancy. Migraine can recur postpartum; it can also begin at that time. Despite their drug use, migraineurs do not differ from nonmigraineurs in their incidence of miscarriages, toxemia, congenital anomalies or stillbirths. Drugs are commonly used during pregnancy and, although medication use should be limited, it is not absolutely contraindicated. Most drugs are not teratog
