Existential concerns and fear of recurrence or progression in people with migraine.

The current study examined the relevance and potential associations between fear of recurrence or progression (FRP) and existential concerns (i.e. death anxiety, meaninglessness, isolation, identity, and guilt) in the context of migraine. Cross-sectional survey data was obtained from 522 people with a self-reported migraine diagnosis (94% female; Mage = 42.79 years), who completed a number of online questionnaires assessing FRP, existential concerns, disability, pain, and mental health symptoms. The results revealed significant positive correlations between all five existential concern domains

Spinorphin Ameliorates Nitroglycerin-Induced Migraine-Like Responses and Trigeminovascular Neuroinflammation Potentially Through Opioid Receptor-Mediated Mechanisms.

Enhancement of endogenous opioid signaling through inhibition of enkephalin degradation has emerged as a potential strategy for pain control, yet its role in migraine remains unclear. This study investigated whether spinorphin, an endogenous inhibitor of multiple enkephalinases, attenuates trigeminovascular activation and migraine-like responses in experimental migraine. Migraine-like pain was induced in male Wistar rats by nitroglycerin. The effects of spinorphin were evaluated using periorbital and plantar von Frey tests, followed by assessment of c-Fos levels in the trigeminal nucleus cauda

Migraine comorbidity in epilepsy subtypes: Association with focal versus generalized epilepsy and seizure control-A systematic review and meta-analysis.

Migraine and epilepsy are paroxysmal neurological disorders that frequently co-occur and share common pathophysiological mechanisms. Whether comorbid migraine is associated with epilepsy subtypes (focal vs. generalized) and with seizure control remains poorly understood. This systematic review and meta-analysis aimed to evaluate these associations. This study was conducted in accordance with the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) 2020 guidelines. PubMed and Web of Science databases were searched from inception to April 1, 2026. Studies ascertaining migr

Lower fibrinogen and fibrinogen-to-albumin ratio in migraine with white matter hyperintensities: implications for microvascular vulnerability.

BACKGROUND: White matter hyperintensities (WMH) are increasingly recognized as imaging markers of migraine-related cerebral small-vessel injury. However, the biological correlates underlying WMH in migraine remain poorly defined. The relationship between WMH and the fibrinogen-to-albumin ratio (FAR), a composite hemostatic marker, has not yet been evaluated. METHODS: In this cross-sectional study, 69 patients with migraine were included after exclusion of systemic inflammatory and vascular confounders. WMH presence was assessed using brain magnetic resonance imaging (MRI). Plasma fibrinogen a

The cerebellum in migraine: a narrative review of anatomical, imaging, and clinical evidence.

Migraine is usually framed through trigeminovascular pathways and interactions among the brainstem, hypothalamus, and cortex. The cerebellum has received less attention, although human studies place it within circuits that process trigeminal nociception and vestibular signals. This narrative review examines anatomical and experimental work together with imaging, genetic, and clinical studies, giving greatest weight to direct human migraine evidence. Repeated-measures imaging suggests that cerebellar connectivity changes across the migraine cycle, but the cohorts are small. Structural findings

[Pharmacological properties and individualized selection of CGRP inhibitors for migraine prevention and acute treatment].

This article reviews the clinical and pharmacological properties of calcitonin gene-related peptide (CGRP) inhibitors for migraine prevention and acute treatment. Monoclonal antibodies (erenumab, fremanezumab, galcanezumab, eptinezumab) and small-molecule receptor antagonists (ubrogepant, rimegepant, atogepant) have shown efficacy in episodic and chronic migraine. Current evidence shows that these drug classes reduce monthly migraine days, lower the need for symptomatic medications, and improve patient functional status. They also display favorable tolerability and safety profiles compared wit

Inherited and early-life vulnerability in functional neurological disorder and functional somatic symptoms: A retrospective cohort and case-control study from a large electronic health record database.

BACKGROUND: Biological factors affecting motor, sensory, and integrative processing may predispose to functional neurological disorder (FND) and functional somatic symptoms. While early-life influences have traditionally been conceptualized within psychological frameworks, the contribution of early-life biological vulnerabilities to symptom expression remains unclear. METHODS: Using TriNetX, a large electronic health records database, we assessed proportions of inherited, congenital, perinatal, and neurodevelopmental diagnoses, capturing a range of early-life conditions. We compared 188,868 i

Headache impact and quality of life following endometriosis surgery in women: a single-center observational study.

BACKGROUND: Endometriosis and migraine are common chronic conditions in women of reproductive age that impair quality of life and frequently co-occur as comorbid conditions. Evidence regarding the effects of treatments targeting one of these conditions on the course of the other remains limited. In particular, the association between endometriosis surgery and headache burden and quality of life has not been sufficiently investigated. The aim of this study was to examine changes in headache burden and quality of life following endometriosis surgery in women with and without migraine. METHODS:

Compartment-specific redox modulation in migraine with aura: a systematic review of context-dependent mechanisms.

BACKGROUND: Preventive therapies for migraine with aura (MA) remain suboptimal, with 37-54% of patients failing to achieve a >50% reduction in migraine days despite calcitonin gene-related peptide (CGRP)-targeted therapies, while broad-spectrum antioxidants show inconsistent benefits. This gap suggests that indiscriminate reactive oxygen species (ROS) scavenging is insufficient, but emerging evidence indicates that ROS function as compartment-specific signaling molecules rather than uniform toxins. Whether this compartment-specific redox modulation occurs within the cortical spreading depressi

Targeting neuronal FKBP5 attenuates mechanical allodynia by suppressing NF-κB-mediated neuroinflammation in an NTG-induced chronic migraine mouse model.

BACKGROUND: Chronic migraine (CM) is a debilitating neurological disorder characterized by persistent trigeminovascular sensitization and neuroinflammation. While FK506-binding protein 51 (FKBP5) modulates spinal neuroinflammation in chronic pain, its specific role in trigeminovascular nociceptive processing and CM pathogenesis remains largely unknown. METHODS: We used a nitroglycerin (NTG)-induced CM mouse model. Following RNA-sequencing (RNA-seq) of total RNA isolated from trigeminal nucleus caudalis (TNC) tissue lysates, target expression was validated via reverse transcription quantitativ

Modifications to cognitive behavioural therapies for depression and anxiety in adults with neurological disorders: A qualitative analysis of intervention protocols.

Cognitive and Behavioural-based Therapies (CBT) can effectively manage symptoms of depression and anxiety. However, adults with neurological disorders (NDs) often face cognitive, physical, and social challenges that make it difficult to engage with traditional forms of these treatments. To address this, CBT protocols have been modified to better suit the needs of this population. This study analysed 41 randomized controlled trials with modified CBT-based protocols for treating symptoms of depression and/or anxiety in adults with a ND. It spanned 10 different NDs : stroke, epilepsy, migraine, P
