OBJECTIVES: This study was conducted to (1) consider the causes of neck pain in tension-type headache (TTH) by investigating how the patients with TTH, idiopathic neck pain (INP), and healthy controls group together based on the presence of cervical musculoskeletal dysfunction; and (2) determine any differences in TTH features, pain sensitivity, tenderness scores, and active trigger points between clusters.
BACKGROUND: Neck pain is common in TTH, yet its source is still unclear. Pain sensitivity, tenderness, and active trigger points are also found in individuals with TTH. Understanding the c

BACKGROUND: Primary headache disorders, particularly migraine, are closely linked to sleep disturbances. However, the relationship between headache phenotypes, sleep quality, and chronotype in large populations remains to be fully elucidated. This study aimed to investigate sleep quality and chronotype across different primary headache types compared to headache-free individuals.
METHODS: This nationwide, cross-sectional study included 5,311 Polish adults recruited via an online research panel. Headaches were classified using the HARDSHIP questionnaire according to ICHD-3 criteria. Sleep qual

BACKGROUND: Headaches cause high disease burden, and are often under-treated. Adequate treatment requires time and expertise, which can be limited in general practices. With the aim to improve the quality and accessibility of care, an unique nurse-led, digital transmural care pathway has been developed in which treatment is provided by nurse practitioners specialized in headache care. It is essential to understand patients' perspectives on the novice care pathway to uncover opportunities for enhancement.
OBJECTIVE: To explore patients' experiences with the nurse-led transmural headache care p

BACKGROUND: Cerebral venous thrombosis (CVT) predominantly affects young adults and frequently presents with headache. Although functional outcomes are generally favorable, many patients report persistent or recurrent headache long after the acute event. This systematic review aimed to assess the prevalence, clinical characteristics, associated factors, and management of long-term headache following CVT.
MAIN BODY: A systematic search of PubMed and Embase (2000-2025) identified studies reporting original data on long-term headache after CVT in adult populations. Case series including fewer th

BACKGROUND: In our earlier report of headache prevalence among children (6-11 years) and adolescents (12-17) in Zambia, we defined undifferentiated headache (UdH) conventionally as mild headache lasting < 1 h. However, we recognised diagnostic uncertainties, which also occurred in our similar studies elsewhere in sub-Saharan Africa (Ethiopia and Benin). Here we use a modified definition of UdH, making new estimates of prevalence accordingly, and estimates based on these of headache-attributed burden. The study was part of the global schools-based programme conducted by the Global Campaign agai

Chronic overlapping pain conditions (COPCs) refer to a set of chronic pain disorders that frequently co-occur and may involve partially overlapping mechanisms. The U.S. National Institutes of Health currently recognizes ten COPCs: fibromyalgia, painful temporomandibular disorders, chronic low back pain, chronic migraine headache, chronic tension-type headache, irritable bowel syndrome, endometriosis, interstitial cystitis/bladder pain syndrome, vulvodynia, and myalgic encephalomyelitis/chronic fatigue syndrome. When multiple COPCs coexist, they are associated with a disproportionate multimorbi

BACKGROUND: Although tension-type headache is the most prevalent primary headache disorder, affecting nearly 26-30% of the population, migraine affects approximately 14-15% globally and is associated with a considerably higher burden of disability. Migraine typically presents as recurrent, pulsatile pain accompanied by nausea, photophobia, and phonophobia. Despite long-term use of conventional treatment, chronic cases often remain refractory. In Unani medicine, this condition corresponds to the melancholic type of migraine (Shaqīqa Sawdāwiyya), caused by derangement of black bile (Sawda'). Thi

This case-control study assessed symptom severity and prevalence of obsessive-compulsive disorder (OCD), attention-deficit/hyperactivity disorder (ADHD), and autism spectrum disorder (ASD) in children attending a headache clinic compared to controls. We included 343 patients (5-19 years) and 130 controls (6-19 years; no prior headache diagnosis/medical contact for headache) between October 2018 and January 2024. Controls were divided into CH+ (experiencing headaches) and CH- (headache-free). Severity was assessed using scores from questionnaires, and prevalence was defined by prior diagnosis o

Headache is a common neurologic problem affecting many members of the population, including athletes. In athletes, headache is most likely a primary disorder, such as tension-type headache or migraine; however, athletes can be at a higher risk of secondary causes, such as posttraumatic headache. Thus, based on the headache history, clinical examination, and presence of headache red flags, a focused diagnostic evaluation is recommended, ranging from blood tests to neuroimaging, depending on the headache characteristics. Regardless of the primary or secondary headache disorder diagnosis, treatme

BACKGROUND Forward head posture in older adults is associated with cervical-thoracic misalignment, chronic tension-type headache, and impaired respiratory mechanics. This study investigated the synergistic effects of combining cervical stabilization exercises with an AI-driven remote respiratory rehabilitation program. MATERIAL AND METHODS This randomized controlled trial enrolled 50 older adults with chronic neck pain and tension-type headache, who were randomly assigned to an experimental group (n=25) or a control group (n=25). After a 12% attrition rate, data from 44 participants (22 per gr

BackgroundGlutathione (GSH) and glutamate (Glu) have been individually implicated in migraine pathophysiology, but their ratio as a marker of oxidative-excitatory balance in the thalamus-a key pain-processing hub-remains unexplored. This study investigated thalamic GSH/Glu ratios across primary headache subtypes and evaluated their diagnostic utility.MethodsThis cross-sectional study included 131 participants: 36 healthy controls (HC), 17 migraine with aura (MwA), 46 migraine without aura (MwoA), and 32 tension-type headache (TTH). INTRODUCTION: Migraine and tension-type headache (TTH) are leading causes of neurological disability globally. While their macro-trends are documented, the nonlinear drivers and the regulatory role of socio-economic development across G20 countries remain under-explored.
METHODS: Based on Global Burden of Disease (GBD) 2023 data, the headache burden across G20 members was quantified. Developmental trajectories were identified by integrating the Socio-demographic Index (SDI). Crucially, a comparative analysis was conducted between a decomposition analysis model and an explainable machine learn

Many children present with chronic primary headache (CPH), yet classification within the International Classification of Diseases (ICD-11) is difficult, potentially impeding treatment pathways. The ICD-11 includes adult criteria, but it lacks coding notes addressing pediatric neurodevelopmental differences, and it stipulates inconsistent time criteria. The current study thus proposes optimized diagnostic criteria for CPH, chronic tension-type headache (TTH), and chronic migraine to better fit the observed symptoms among pediatric patients with CPH. Symptoms of N = 231 pediatric patients presen

BACKGROUND: Tension-type headache (TTH) is the most common primary headache disorder and chronic tension-type headache (CTTH) is difficult to treat. Despite this clinical importance, the pathophysiology especially of CTTH remains unclear. Therefore, the aim of this study was to search for microstructural changes in CTTH to further evaluate the pathogenesis of CTTH.
METHODS: Nine female patients suffering from CTTH and ten healthy controls matched by age and gender were included. All participants underwent a magnetic resonance imaging (MRI) scan with a diffusion tensor imaging (DTI) sequence.

BACKGROUND: Tension-type headache (TTH) is the most common primary headache, yet accompanying sleep disturbances have not been quantitatively synthesized.
METHODS: PubMed, Embase, Web of Science, and the Cochrane Library were searched from inception to January 3, 2026, for observational studies reporting sleep outcomes in adults with TTH. The primary outcome was the prevalence of poor sleep quality assessed with the Pittsburgh sleep quality index (PSQI). Pooled estimates were calculated using random-effects models. Study quality was assessed with Joanna Briggs Institute checklists.

Individuals with persistent pain have disproportionately high cigarette smoking prevalence and poorer cessation outcomes. There has been a shift from examining single pain diagnoses to a focus on co-occurring pain disorders referred to as chronic overlapping pain conditions (COPCs). COPCs represent a cluster of 10 disorders (e.g., irritable bowel syndrome [IBS], fibromyalgia, chronic low back pain, chronic tension-type headache) characterized by central sensitization. Yet, the relevance of COPC burden on smoking-related processes and internalizing psychopathology remains unexplored. The purpos
