Altered network homogeneity of the default mode network in patients with tension-type headache.

OBJECTIVE: Dysfunction of the default mode network (DMN) is strongly linked to pain processing, as well as onset and progression of chronic pain disorders; however, its specific role in the pathogenesis of tension-type headache (TTH) remains incompletely understood. This study aims to explore the network homogeneity (NH) of the DMN in the pathophysiological mechanisms underlying TTH. METHODS: Resting-state functional magnetic resonance imaging (rs-fMRI) and clinical headache scale assessments were conducted on 68 patients with TTH and 63 healthy controls (HCs). Independent component analysis

Mapping Review on Characteristics of Publications on Modifiable Psychosocial Factors in Headache Diseases.

PURPOSE OF REVIEW: Modifiable psychosocial risk factors are associated with and may contribute to the chronification of headache diseases. Assessing such factors is an integral part of multidimensional pain management. However, an overview of modifiable psychosocial risk factors and their role (e.g., chronification, prevalence, association, comorbidity) in migraine (MGR), tension-type headache (TTH), and cervicogenic headache (CeH) is missing. The present mapping review aimed to provide an overview of characteristics of modifiable psychosocial risk factors in MGR, TTH, and CeH. RECENT FINDING

Distribution of chronic primary and secondary pain in a patient sample with psychiatric disorders.

OBJECTIVES: Chronic pain often coexists with psychiatric disorders, yet less is known about the specific types of chronic pain conditions that may co-occur. This study aimed to classify chronic pain according to ICD-11 categories and provide an overview of the types and frequency of these conditions in a patient sample with psychiatric disorders and coexisting chronic pain. METHODS: This descriptive cross-sectional study consecutively recruited 151 patients (78.8 % females, mean age 36 years) from psychiatric outpatient units. Common psychiatric diagnoses included anxiety disorders (27.8 %),

One Molecule, Different Circuits? What Botulinum Toxin Can and Cannot Reveal About Craniocervical Comorbidity.

Botulinum neurotoxin type A (BoNT-A) is used across coexisting craniocervical conditions, and therapeutic response is often overinterpreted as evidence of a shared generator. The bruxism literature shows why. Across six controlled studies with event-level outcome measurement, reported effects on event frequency track not dose or injection field but whether the rule used to detect an event could follow the amplitude the toxin had just reduced; none yet combines a placebo arm with an amplitude-independent event definition. This targeted critical narrative review interprets representative evidenc

Characterizing insomnia and sleep quality in a cohort with tension type headache.

OBJECTIVES: Emerging literature data indicate insomnia as an risk factor for headache or migraine onset. It is generally recognized that headache and insomnia represent very common and major complaints worldwide, while finding exisit that indicate a symmetrical association, with headache being more associated with insomnia than the other way around. Therefore, in the association of headache and insomnia, other factors associated with headache or migraine probably play a role, as well. The aim of the study was to evaluate the clinical and pathogenetic relationships between insomnia and chronic

Adapting Tension-Type Headache to the International Association for the Study of Pain (IASP) Taxonomy: A Conceptual Proposal.

Tension-type headache (TTH) is a primary headache characterized by treatment effect heterogeneity, possibly reflecting variations in underlying pain mechanisms. Identifying pain phenotyping or classifying patients into subgroups based on their predominant pain mechanism may provide targeted treatment options. This paper proposes a conceptual adaptation of the International Association for the Study of Pain (IASP) clinical criteria to identify the predominant phenotype in individuals with TTH. It has been found that pain referrals elicited by trigger points (TrPs) reproduce headache symptoms in

Integrating Pharmacodynamic Data to Prioritise Candidate Migraine Genes and Pathways Using a Novel Bioinformatics Workflow.

Background/Objectives: Migraine is a neurological disorder with a heterogeneous presentation; however, compounds perceived as effective by migraine sufferers are often underrepresented in research. This study adopts a novel exploratory approach to determine whether pharmacodynamic data from such compounds can be used to prioritise candidate migraine-related genes and biological pathways for future investigation, and to generate hypotheses regarding migraine pathophysiology. Methods: A structured internet search identified 181 compounds perceived as effective for migraine, of which 148 had at l

Responsibility sharing among children with Type 1 Diabetes and their caregivers: a cross-sectional survey from North India using the Diabetes Family Responsibility Questionnaire (DFRQ).

INTRODUCTION: Reconstruction has evolved long way from primary closure to flaps. As time evolved, better understanding of vascularity of flap has led to the development of innovative reconstructive techniques. These flaps can be raised from various parts of the body for reconstruction and have shown least donor site morbidity. We use one such peroneal artery perforator flap for tongue reconstruction with advantage of thin pliable flap, minimal donor site morbidity and hidden scar. MATERIALS AND METHODS: Our patient 57yrs old lady underwent wide local excision with selective neck dissection. P

Paroxysmal migraine associated with vancomycin administration: A case report, a review of pharmacovigilance databases for similar cases and possible mechanisms.

BACKGROUND: Vancomycin is a core drug for the treatment of methicillin-resistant Staphylococcus aureus, but its neurologic adverse effects are rarely reported. This case is the first to report a possible association between intravenous vancomycin and episodic migraine. Rapid identification of drug-induced migraine can avoid unnecessary invasive testing and medication delays and has direct clinical value. OBJECTIVE: To report and document a case of episodic migraine that was highly correlated with the timing of intravenous vancomycin administration, assessing causality and its general clinical

A Curious Case of Galactorrhea in the Context of Gastroenteritis: A Case Report.

Metoclopramide is a prokinetic and antiemetic agent widely used in primary care. Its mechanism of action explains both its therapeutic effects, as well as certain adverse reactions, including hyperprolactinemia and galactorrhea. Metoclopramide-induced galactorrhea may prompt concern and lead to unnecessary diagnostic testing. This case report is about a healthy 21-year-old woman taking a combined oral contraceptive who was prescribed metoclopramide for gastroenteritis with vomiting. Forty-eight hours after starting treatment, she developed spontaneous, bilateral, multiductal, painless galactor

Extrapyramidal symptoms and effectiveness of continuous vs bolus intravenous metoclopramide: A systematic review and meta-analysis.

OBJECTIVE: Metoclopramide is widely used to treat nausea, vomiting, and headache. However, it may cause extrapyramidal symptoms (EPS) such as akathisia. Continuous intravenous (IV) infusion has been proposed as a safer alternative to bolus injection. This study aimed to compare the risk of EPS and effectiveness between continuous and bolus IV metoclopramide administration. METHODS: We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) comparing continuous vs. bolus IV administration of metoclopramide. Databases searched included CENTRAL, MEDLINE, Embase, CI
