Pathofysiological and clinical correlates of sleep in primary headache disorders: A narrative review.

Sleep and headache are strongly interconnected, this narrative review gives an overview of the current pathophysiological and clinical correlates of sleep in primary headache disorders. Circuits across the brainstem and hypothalamus that are involved in the regulation of sleep and wake overlap with known headache and pain pathways, offering a possible pathophysiological basis for this relationship. Furthermore, growing evidence supports a key role for orexin (hypocretin), with dysregulated signalling and widespread orexin receptor expression in the trigeminocervical complex and trigeminal gang

Bridging the gap in headache care: a qualitative needs assessment for primary care education in Indonesia.

INTRODUCTION: Headache is a common complaint encountered in primary care. In Indonesia, however, only migraine and tension-type headache are expected to be fully managed by general practitioners (GPs). Effective management requires a comprehensive understanding of headache diagnosis, treatment, and appropriate referral in primary care settings. Educational interventions for GPs are therefore essential to address existing unmet needs. This study aims to conduct a needs assessment to identify barriers in headache management in primary care and to determine gaps that can inform the development of

Endocannabinoid Modulation in Headache: Mechanisms, Models, and Translational Therapies.

Headache disorders, including migraine, tension-type headache, trigeminal autonomic cephalalgias, post-traumatic headache and medication overuse headache, represent a major global health burden and remain difficult to treat despite therapeutic advances. The endocannabinoid system (ECS) has emerged as a key regulator of neural, vascular, and immune processes central to headache pathophysiology. Through coordinated actions of CB1 and CB2 receptors, the endogenous ligands anandamide (AEA) and 2-arachidonoylglycerol (2-AG), and their metabolic enzymes, the ECS modulates trigeminovascular activity,

Active trigger points are common but not universal with tension-type headache: Pain features rather than musculoskeletal function may explain why.

BACKGROUND: Active trigger points (ATrPs) are common and considered a peripheral nociceptive source in tension-type headache (TTH). Understanding differences in features between individuals with TTH who have few or more ATrPs is needed. OBJECTIVES: i) To determine sub-groups by the number of ATrPs present in TTH; ii) to compare headache and participant characteristics, psychological and behavioural features, number of painful areas, quantitative sensory testing (QST), tenderness scores, cervical musculoskeletal function and symptoms on cervical testing between subgroups. METHODS: This second

Reversible cerebral vasoconstriction syndrome in psychiatric settings: Context-dependent diagnostic bias and consultation-liaison psychiatry practice.

BACKGROUND: Reversible cerebral vasoconstriction syndrome (RCVS) is a potentially life-threatening neurological condition characterized by thunderclap headaches and multifocal cerebral arterial vasoconstriction resolving within 3 months. Context-dependent diagnostic bias in psychiatric settings may delay recognition of life-threatening conditions including RCVS. CASE PRESENTATION: A 52-year-old Japanese woman with bipolar II disorder stable on lithium developed recurrent thunderclap headaches 9 days after mild COVID-19 infection. Headaches were consistently triggered by hot showers, reached m
